철결핍성 빈혈 환자에서 한약 투여에 따른 혈청철, 철포합능 및 적혈구 지표의 변화 (Change of RBC Indices with Serum Iron, TIBC Level in Iron Deficiency Anemia Patients Who Treated Oriental Medicine)

  • Anemia are often detected among many patients who are treated in oriental traditional medicine hospitals in Korea through blood tests which are taken to diagnose the patients' general state, and most of them are diagnosed to have iron deficiency anemia. This study amis the effect of herbal medicine on the RBC count, hemoglobin, hematocrits and indices of red blood cells such as mean corpuscular volume(MCV), mean corpuscular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC), serum iron, and total iron binding capacity(TIBC) among patients who were treated in oriental medical hospital and diagnosed to have anemia through blood tests other than main symptoms. The subjects of this study were 29 patients(male 7, female 22, mean age: 56.24${\pm}$19.11years) who were chosen ones, they has below hemoglobin 12 gm/dL, serum iron 70 IU/L, MCV 80 fL on laboratory screening test when they first visits our hospital. As a result, the volume of red blood cell(RBC), hematocrit(HCT), and hemoglobin(Hb) in peripheral blood increased from $348.27{\pm}8.95{\times}10^5$ /UL, 28.82${\pm}$0.56%, 9.24${\pm}$0.22 g/dL to $375.62{\pm}9.11{\times}10^5$ /UL, 31.36${\pm}$0.68%, 10.31${\pm}$0.25 g/dL comparing before and after oriental medicine treatment. After the treatment, MCH, MCHC, and MCV also increased from 26.98${\pm}$0.84 Pg, 32.62${\pm}$0.45 g/dl, 79.26${\pm}$0.17 fL to 27.74${\pm}$0.74 Pg, 32.84${\pm}$0.33 g/dl, 84.15${\pm}$1.67 fL. There was statistically significant increase in MCH, MCV(P<0.05); however, there was no significant change in MCHC(P>0.05). The amount of iron in blood increased from 32.96${\pm}$4.20 ug/dL to 67.22${\pm}$7.18 ug/dL, which showed a statistically significant increase(P<0.05). On the other hand, TIBC decreased from 325.08${\pm}$13.97 ug/dL to 315.61${\pm}$18.79 ug/dL, which was not a statistically significant change(P>0.05). Like this, all the patients showed favorable results in anemia based on the measurements of indices of red blood cells except MCHC. According to the classification of herbal medicines, in strengthening energy group(補氣), replenishing blood group(補血), and replenishing both energy and blood group(補氣血), the volume of RBC, HCT, and hemoglobin showed significant increases. Furthermore, in herbal medicines strengthening blood(補血) and replenishing both energy and blood group(補氣血), serum iron also relatively increased compare to strengthening energy group(補氣).

Green Synthesis to Develop Iron-Nano Formulations and Its Toxicity Assays

  • Objectives: In the past few years, herbal medicines have gained popularity over synthetic drugs because of their natural source and minimal side effects which has led to a tremendous growth of phytopharmaceuticals usage. With the development of nanotechnology, it provides alternative approaches to overcome several limitations using nano-formulations. In spite of considerable quantity of antianemic preparations with different iron forms available, currently additives are used and represented in modern pharmaceutical market. Iron deficiency anemia is a major global public health problem which particularly affects pregnant women, children and elderly persons. The situation is complicated because of disadvantages and drug side effects from existing antianemic medicines. There is a great demand for the development of new antianemic preparations. Green synthesis of iron oxide nanoparticles, possess high potential in this field. Methods: Our study focuses on developing green synthesis of iron oxide nanoparticles (IONPs) of 10-50 nm with spherical shape where different dosages were used -1 mg/kg, 10 mg/kg and 100 mg/kg for exposure in Wistar albino female rats for 28 days. The toxicity was assessed using various parameters such as measurements of the rat body and organ mass, hematology, biochemical evaluation and histopathological examinations. Results: No significant differences were observed in body and organ weights. Hematological indices also indicated no significant differences whereas biochemical factors showed increase in levels of direct bilirubin and globulin of medium as well as high dose and SGPT levels were increased only in high dose. The major organs (heart, kidney and liver) showed histopathological alterations in 10 and 100 mg/kg whereas brain showed only in 100 mg/kg. Conclusion: The toxicity of IONPs was found to be more significant when the concentration was increased; however, low doses can be used for further investigation as an antianemic preparation.

울산시 여고생의 철분영양상태와 빈혈에 관한 연구 (A Study of Iron Status and Anemia in Female High School Students in Ulsan)

  • This study was designed to assess the iron nutritional status and anemia of high school students. 383 female subjects in Ulsan Metropolitan city were evaluated using a questionnaire, and a measurement of hematological indices. The average height and weight of the respondents were 161.24$\pm$4.90 cm and 53.12$\pm$6.37kg, repectively. The average BMI(body mass index) was 20.43$\pm$2.26 which was in the normal ramge. The average hemoglobin(Hb) concentration of the subjects was 13.14$\pm$0.97g/dl, and the average hematocrit(Hct) level was 40.84$\pm$17.40%. Transferrin saturation{TS(%)} was 20.86$\pm$10.32%, and the ferritin by Hct(<36%), 27.2%by TS(<14%),26.6% by ferritin(<12 ng/ml). As for clinical symptoms, the greatest number of respondents reported that they experienced ‘decreased ability to concectrate’. Mean daily intakes of iron were 14.89$\pm$4.48 mg and heme iron intakes were 5.04$\pm$2.13 mg, which was 29.6% of total iron intake. The total iron binding capacity(TIBC) was negatively correlated with Hb concentration(r= -0.222, p<0.01). Serum ferritin was positively correlated with Hb concentration(r= - 0.323, p<0.05) and negatively correlated with TIBC(r= -0.367, p<0.01). TS(%) was positively correlated with Hb concentration(r= 0.402, p<0.01) and positively correlated with serum ferritin(r=0.413, p<0.01). As for the correlation between blood biochemistry and clinical symptoms related to anemia, the Hb concentration was negatively correlated with ‘shortening of breath when going upstairs(p>0.05)’ and ‘cold hands and feet’ significantly(r= -0.109, p<0.05). The level of Mean corpuscular volume(MCV) was negatively correlated with ‘feel dizzy when standing up’,‘tired out easily’, and ‘decrease ability to concentrate’ significantly(p<0.05). In particular, the level of Fe was negatively correlated with ‘shortening of breath when going upstairs’ and ‘feeling blue’ significantly(p<0.01). These results suggest That the prevalence of iron deficiency of female high school students is very high, therefore guidelines for diet supports and nutrition education to improve their iron status should be provided.

12주 동안의 저열량식사와 행동수정요법이 비만여성의 체격지수와 생화학적 영양상태에 미치는 영향 (Effect of 12-week Low Calorie Diet and Behavior Modification on the Anthropomeric Indices and Biochemical Nutritional Status of Obese Woman)

  • This study was conducted to investigate the effect of a 3 week low calorie diet (LCD) and a 9 week of behavior modification (BM) program on the weight loss, mineral and vitamin status in 22 obese women. The subject were healthy, obese (PIBW> $120\%$) women aged 20 - 50 Yr and not taking any medications known to influence body composition, mineral or vitamin metabolism During the LCD program, subjects were provided commercial liquid formulas with 125 kcal per pack and were instructed to have a formula for replacement of one meal and at least one regular meal per day within the range of daily 800 - 1200 kcal intake. During the BM program the subjects weekly attended the group nutrition counseling session to encourage themselves to modify their eating behavior and spontaneously restrict their energy intakes. The BM program focused on stimulus control, control of portion sizes and modification of binge eating and other adverse habits. The initial mean energy intake of subjects was 2016.9 $\pm$ 129.8 kcal ($100.8\%$ of RDA) and dropped to 1276.5 $\pm$ 435.7 kcal at the end of a 3 week of LCD program and elevated to 1762 $\pm$ 329.3 kcal at the end of a 9 week of BM program. Carbohydrate, protein and fat intakes were significantly decreased at the end of the LCD but carbohydrate was the only macro nutrient that showed significant decrease (p < 0.05) at the end of the BM program compared to baseline. Calcium and iron intakes decreased significantly (p < 0.01, respectively) with no significant changes in other micronutrients at the end of the LCD. The mean weight of the subjects decreased from 73.8 $\pm$ 8.0 kg to 69.2 $\pm$ 7.7 kg with LCD and ended up with 67.7 $\pm$ 7.1 kg after 9 weeks of BM. The 3 weeks of LCD reduced most of the anthropometric indices such as BMI, PIBW, fat weight, wast-to-hip ratio and subscapular and suprailiac skinfold thickness. The 9 weeks of behavior modification showed slight change or maintenance of each anthropometric measurements. Weight loss and decreased WHR with the diet program induced significantly decreased systolic blood pressure. SGOT, SGPT and serum insulin levels with improved serum lipid profiles. Biochemical parameters related to iron status such as hemoglobin, hematocrit were significantly decreased (p < 0.01) at the end of the LCD. But their mean values were within normal range. The mean serum 25 (OH) vitamin $D_3$ level significantly increased after whole diet program. Serum folate level significantly decreased after 12 weeks of diet program. In conclusion 3 weeks of LCD brought 4.6 kg reduction in body weight without risk of iron, zinc or vitamin D deficiency and 9 weeks of the BM was effective to maintain nutritional status with slightly more weight reduction (1.5 kg). However calcium intake and serum folate should be monitored during the LCD and BM because of increased risk of deficiencies.

만성(慢性) 간질환(肝疾患)의 빈혈상(貧血像) (Anemia of Chronic Liver Diseases)

  • The pathogenetic mechanisms of anemia in patients with chronic liver disease were observed. Seventeen patients with moderate to advanced hepatic diseass were studied by various methods. Only patients without previous blood loss were included: 14 had cirrhosis, 2 had active chronic hepatitis, and one had inferior vena cava obstruction with associated liver cirrhosis. The followings were the results: 1. The anemia based on red blood cell count, Hb., and Ht. was found in 76.5-78.6% of the patients. 2. Red cell indices indicated that normo-macrocytic and normochromic anemia was present is the majority of the patients. 3. No evidence of megaloblastic anemia was found on the basis of the morphological examinations. 4. Serum iron, TIBC, % saturation and iron content in the bone marrow indicated that iron deficiency anemia was present in about half of the patients. 5. In the view of the erythrocyte dynamics, primary increase in the red cell destruction was ascribed to the cause of the anemia. 6. Decrease in the red cell survival time was not correlated with MCV, % saturation and S.L. ratio. Also, hemoglobin level was not correlated with MCV, % saturation and $T_{50}Cr$. Therefore, multiple causes may be involved in the pathogenesis of the anemia. 7. Anemia as determined by the red cell volume was found in only 60% of the patients. It may be possible that hemodilutional anemia is present.

Correlation Study Between Anemias with Higher MCV and Lower MCHC

  • The purpose of this study was to evaluate the significance of higher than normal mean corpuscular volume (MCV) and lower than normal mean corpuscular hemoglobin concentration (MCHC) in 20 patients. The hemoglobin (Hb), red blood cell indices (MCV, MCH, MCHC,), red cell distribution width (RDW), serum ferritin, serum iron (Fe) and total iron binding capacity (TIBC) were measured and the transferrin saturation were calculated. 18 (90.0%) cases were categorized as chronic diseases. Chronic renal failure, malignancy, and bleeding were all related to the cases. The mean values of hemoglobin was $9.5{\pm}2.14g/dL$; the MCV was $29.0{\pm}2.8fL$; the MCH was $30.9{\pm}1.0pg$; the MCHC was $31.2{\pm}0.5%$; the RDW was $17.0{\pm}3.6%$; serum Fe was $39{\pm}21{\mu}g/dL$; the TIBC was $219.7{\pm}108.8{\mu}g/dL$; transferrin saturation was $19.2{\pm}9.9%$ and ferritin was $445.5{\pm}499.6{\mu}g/L$ in the patients. The WHO criteria for hemoglobin of patients confirms anemia in 18 of the 20 (90.0%) cases. Anemia of chronic disease was shown in 11 (73.3%) cases; acute gastric ulcer with hemorrhage in 1 (6.7%) case; iron deficiency anemia in 1 (6.7%) case; 2 patients (13.3%) were of normal cases. There were changes in the baseline Hb level results of the 19 (95.0%) cases while no change was shown in 1 case. As a result, diseases associated with anemia and bleeding where the MCV is higher than the normal range and MCHC is lower than normal range are considered relevant findings.

중 고등학생 대상 영양교육 목표 설정을 위한 영양 관련 문제점 분석 (Setting Instructional Goals for Nutritional Education Program Through an Analysis of Problems Identified in Junior/senior High School Students)

  • For the purpose of developing 'Web-Based Nutritional Education Program', this study analyzed nutritional problems of junior/senior high school students and set appropriate instructional goals for nutritional education program. Survey questionnaires were distributed from October 21,2002 to October 26, 2002 to a total of 564 students at three junior high schools and five senior high schools located in Seoul. The total number of questionnaires collected and used in our study was 479, which is 84.9% of the total questionnaires distributed. According to the survey results, three student groups (male and female students at junior high schools and female students at senior high schools) had lower than an ideal body weight. In case of their Basal Metabolic Indices (BMI), the female students and junior high school students showed a tendency to be underweight, while the other three student groups were within the normal range. The result of the survey showed that all four groups of students showed a deficiency in nutrient intake of calories, calcium and thiamin when compared with Korean RDA. Particularly, in the case of the female students at senior high schools, the deficiency of iron intake was a problem. The survey about food attitudes and food habits of the students showed that the senior high school students had more problems when compared with their junior high school counterparts (p < .001). Therefore, the instructional goals of nutritional education program were set as follows: 1) To improve the level of understanding and the perception of junior and senior high school students about the importance of a dietary lifestyle. 2) To change food attitudes and nutritional knowledge related to individual dietary 1ifestyle, so as to maintain good health through the enhancement of the students' physical activities. We found out that it is critically important to factor in the existing problems of food attitudes and food habits, as well as nutrient intake among junior and senor high school students when setting objectives for nutritional education programs. 과체중 및 비만여성의 생식섭취가 체중감소 및 생화학적 영양상태에 미치는 영향 (The Effect of Uncooked Powdered Food on the weight toss and the Biochemical Nutritional Status in Overweight and Obese Women)

  • This study was conducted to investigate the effect of an uncooked powdered food(UPF) on the weight loss and changes of the biochemical nutritional status for 20 overweight and 26 obese women in Iksan area. We just replaced common breakfast and dinner of the subjects with UPF. Their dietary intake status was evaluated by 24-hour recall method. Also anthropometric and biochemical measurements before and after the UPF program were estimated. The intake of energy, lipid and protein decreased and the quality of meals improved as people started to take UPF. Due to the energy loss by taking UPF, weight and body fat decreased significantly. The percentage of the body fat was high among the weight lost, from which we judged that the process of losing weight was successful. It is hard to predict whether weight loss will occur to people who are already in shape or not, but if overweight and obesity people regularly take UPF instead of other food products, we assume that UPF will help lessening the body fat. The most positive change among biochemical changes by taking UPF was the decrease of serum lipid contents. When overweight and obesity women replaced two out of three meals with UPF for 3 month period, no significant nutritional problems occurred. It seems that the 12weeks of UPF program used in this study was effective in improving anthropometric indices without producing the deficiency of iron or other susceptible nutrients.

성장통으로 발견된 하지 불안 증후군 1예 (A case of restless legs syndrome in a child presenting with growing pains)

  • 하지 불안증후군은 다리의 감각 둔화와 감각이상을 호소하며, 쉬고 있을 때 악화되고 움직이면 완화되는 특징을 보이는 증후군이다. 정확한 원인은 아직 알려진 바 없으며 아동기 시작되는 하지 불안증후군은 철분 결핍 및 가족력과 연관된 것으로 되어 있다. 또한 하지 불안증후군은 주의력 결핍 과잉행동 장애를 흔히 동반하며, 수면 다원 검사로 확진되는 주기성 사지 운동증과 같은 선상의 질병으로 여겨진다. 저자들은 하지 통증을 호소하는 5세 남아에서 어머니의 가족력을 발견하고 주의력 결핍 장애의 증상을 동반하며 수면 다원 검사로 진단된 경도의 주기성 사지 운동증으로 확진한 하지 불안 증후군을 진단하고 철분제 복용 후 증상 호전을 보인 사례를 경험하였기에 이를 보고하며, 성장통으로 내원하는 아이들에서 하지 불안 증후군을 감별할 것을 제안하는 바이다.

RDW를 이용한 빈혈의 재분류 (Modified Classification of Anemia by ROW)

  • 1986년 5월부터 1991년 6월까지 약 5년간 영남대학교 외과대학 부속병원에 내원하여 혈액학적 소견상 빈혈이 동반된 환자 210명과 정상인 200명 도합 410명을 대상으로 Coulter Counter S-plus II로 일반혈액검사를 실시하여 MCV와 RDW 및 여러 적혈구 지수를 얻어서 빈혈의 새로운 분류를 시도하였고 외국인에 비하여 빈혈의 병인 양상이 다른 한국인의 혈액질환의 감별진단에 도움이 되는 가를 알아 보고자 본 연구를 시행하였다. 거의 모든 경우에 MCV의 증감은 MCH와 MCHC의 증감을 동반하였으나 RDW와는 무관하였다. 따라서 저색소성빈혈 혹은 정색소성빈혈등의 용어보다는 이질성빈혈 혹은 동질성빈혈의 용어를 사용함이 빈혈의 형태학적 분류에 더욱 의미있는 것으로 사료되었다. 이질성소구성빈혈에는 철결핍성빈혈이 속하였고 이질성정구성빈혈에는 급성백혈병과 골수로성빈혈이 여기에 속하였으며 이질성대구성빈혈에는 거대적아구성빈혈과 용혈성빈혈이 속하였다. 동질성소구성빈혈에는 만성질환에 의한 빈혈이 여기에 속하였으며 동질성정구성빈혈에는 급성출혈, 만성백혈병 및 다발성 골수종등에 의한 빈혈이 여기에 속하고 동질성대구성빈혈에는 재생활량성빈혈이 여기에 속하였다. 진단적인 의의가 큰 혈색소병은 본 연구에는 관찰되지 않았지만 한국인에 가장 많은 빈혈의 원인인 철결핍성빈혈과 만성질환에 의한 빈혈의 감별진단에 RDW가 유용한 것으로 생각되었다.
