The clinical study on 1 case of Patient with Tremor by Bok-Jin (복진을 통한 시호가용골모려탕 투여로 호전된 진전 환자 1예)

  • Background : Tremor is a rhythmic, involuntary muscular contraction characterized by oscillations (to-and-fro movements) of a part of the body. Tremor is the common symptom clinically. Objectives : The purpose of this study is to show a case of patient with tremor improved by medication of Bok-jin-centered. Patients & Methods : The patient was 60-year-old woman who complained Cephalic and cheilo tremor. She was treated by oriental medicine with acupuncture, herb medication, physical therapy through the abdominal diagnosis, Bok-Jin. Results : The Cephalic and cheilo tremor was reduced after oriental medical therapy. Conclusion : After the treatment, tremor and other symptoms were improved. This result suggested that Bok-jin is the significant diagnosis and medication of Bok-jin-centered is recommanded.

Hemifacial Spasm Treated with Acupuncture and Electro-acupuncture in 2 Patients: Case Report (반축얼굴경련증 환자 2례에 대한 침 및 침전기 자극술 치험례)

  • Hemifacial spasm is a facial nerve disorder characterized by episodic involuntary facial muscle contraction. In this study, the effect of Acupuncture and Electro- acupuncture therapy for two patient with hemifascial spasm were evaluated. Two patients with hemifacial spasm were treated with Acupuncture and Electro-acupuncture and evaluated with Scott's scale and Treatment satisfaction grade. In Case 1, the grade of spasm intensity classified by Scott's description improved from 3 to 1. In Case 2, the grade improved from 4 to 2. Patients were satisfied with the treatment in both cases. Acupuncture and Electro-acupuncture seem to be effective in the treatment of hemifacial spasm.

  • In most persons with disabilities had poor oral hygiene because of less attention and ability to perform a dental care. So the increased prevalence and severity of dental disease were common oral state. Although most persons with disabilities need a adequate dental treatment, it is often very difficult to treat because of noncooperativity and involuntary muscle movements. Dental treatments under sedation and general anesthesia were make to provide a high-quality dental service because of decreased anxiety and fear associated dental treatment in persons with disabilities. The dental professionals must be able to select and apply the proper sedation methods in agreement with the characteristics of the disabilities, general conditions, sedation experience and capacity of dentist, type and time of dental treatment, equipments of dental clinic, consent of patient's protector. The proshodontic treatment procedures, such as abutment preparation, dental impression taking process, try-in process of prosthesis and adjustment of occlusion, are difficult even for patients without disabilities. Those procedures are more difficult to patients with disability because it's too hard to control breathing and muscle. In this report, we performed prosthetic dentistry procedures to three patients with disabilities under dental sedation and general anesthesia.

  • Spasmodic dysphonia is a focal laryngeal dystonia that results in involuntary spasms during speech. The etiology of spasmodic dysphonia is not yet defined, but it is presumed to be a neurological abnormality of central nervous system motor function. The treatment of choice for spasmodic dysphonia is botulinum toxin injection directly at the laryngeal muscles. However botulinum toxin injection requires repeated procedures. Many different kinds of surgical treatments have been introduced but the recurrence rate is still high. So we performed myomectomy with LASER and neurectomy with specially designed electrical surgical knife which can cut recurrent laryngeal nerve branch selectively with its noble curved section. We report a case of a 43-year-old male patient with spasmodic dysphonia treated by thyroarytenoid myoneurectomy.

  • Tremor is a rhythmic and involuntary muscular contraction characterized by oscillations of body parts. Tremor often accompanies neurological disorders associated with aging. Although the disorder is not life-threatening, it can be responsible for functional disability and interpersonal relationship problems. We have recently examined a number of tremor-patients. one of them was improved significantly through the herbal medicine (Yeokgansan-gamibang) and acupuncture, so we report it for the better treatment.

  • Having gone through socioeconomic changes from an agricultural to a post-industrial society, mental health system accordingly has been changed. In Korea where the socioeconomic change has been so rapid, the mental health system and law have been behind the socioeconomic system. Post-industrial society needs more humanized and advanced mental health system, however, Korean mental health law reflects ideology of Korean society in industrial age. This paper attempts to assess the significance Korea's Mental Health Law against the backdrop of socioeconomic changes. A substantial part of the report is devoted to identifying discrepancies between Korea's Mental Health Law and the perspectives of post-industrial ideology and areas for improvement. Improvement in mental health law should take place in line with the changes occurring in socioeconomic environments, the social concept of family, and the public awareness of human rights. Korea's mental health law should be changed in a way to improve hospitalization procedure for people with mental illness, promote the opening of mental health facilities and improve the quality of the lives of the mentally ill. Further changes are deemed necessary in the public and media view of mental illness. Also, the national budget will have to be increased with a view to raise the social rights of those with mental illness to receive quality rehabilitation services.

2 Clinical Reports of Schzophrenic Patients with TD (전광 환자의 지연성 운동장애 Tardive Dyskinesia 치험 2례)

  • Tardive Dyskinesia(TD) may be defined as a syndrome consisting of abnormal, stereotyped involuntary movements, which occurs relatively late in the course of drug treatment and in the etiology of which the drug treatment is a necessary factor like schizophrenia, The frequently involved parts of the body in the schizophrenic patients who have TD were tongue, upper extremity, lips and perioral area, jaws, lower extremity, muscle of facial expression, trunk respectively. The last few years have seen a number of well-conducted clinical, biochemical, neuropathological, and therapeutic studies in patients with TD, but there is no satisfactory treatment for the majority of patients with TD. I used herb medicines and some acupoints like GV20(百會), Extra-point(鎭靜), Extra-point(間谷), Liv20(行間), of the chennel of Gall bladder to treated 2 schizophrenic patients with TD from December 2003 to May 2004. The patients with TD were somewhat improved. Nonetheless for just 2 cases, I might thought that this acupuncture therapy could be helpful to treat schizophrenic patients with TD.However, further studies should be followed.

A Case Report of Intractable Hiccups after Laparoscopic Cholecystectomy (복강경 담낭절제술 후 발생한 난치성 딸꾹질 환자 증례)

  • Hiccups are defined as abrupt involuntary contractions of diaphragm and intercostal muscles with sudden closure of the glottis. Hiccups are one of the common transient symptoms but when hiccups last more than 48 hours, or have no response to treatment, they are defined as intractable hiccups. Intractable hiccups can induce multiple problems such as weight loss, dehydration and sleep disturbance. We report a 76-year-old male patient who suffered from intractable hiccups for 6 days after laparoscopic cholecystectomy. Hiccupping was so severe as to continue all day long and limit sleeping and eating. We succeeded in cure intractable hiccups with acupuncture and herbal formula; Gyulyeohwadam-tang.

  • Obturator nerve block has been commonly used for pain management to prevent involuntary reflex of the adductor thigh muscles. One of several options for this block is chemical neurolysis. Neurolysis is done with chemical agents. Chemical agents used in the neurolysis of the obturator nerve have been alcohol, phenol, and botulinum toxin. In the current case, a patient with spasticity of the adductor thigh muscle due to cervical cord injury had obturator nerve neurolysis done with botulinum toxin type B (BoNT-B). Most of the previous studies have used BoNT-A with only a few reports that have used BoNT-B. BoNT-B has several advantages and disadvantages over BoNT-A. Thus, we report herein a patient who successfully received obturator nerve neurolysis using BoNT-B to treat adductor thigh muscle spasm.

A Case of Combined Korean-Western Medicine Treatment on Chorea Hyperglycemia Basal Ganglia Syndrome Induced by Nonketotic Diabetes Mellitus (비케톤성 당뇨병으로 유발된 고혈당성 무도증의 한·양방 복합치료 증례보고)

  • Objectives: Chorea Hyperglycemia Basal Ganglia Syndrome(C-H-BG) is a syndrome with chorea-ballismus induced by hyperglycemia. This case study reports the clinical effect of Korean medicine treatment on C-H-BG. Methods: A 73-year-old male patient with left side chorea-ballismus visited Daejeon Korean Medicine Hospital. He had no diabetes mellitus(DM) history but the laboratory test result suggested nonketotic DM. We diagnosed him as C-H-BG and treated with herbal medicine, acupuncture and hyperglycemic medication. To evaluate chorea-ballismus, we checked Abnormal Involuntary Movement Scale(AIMS) score everyday. Results: After 14 days of treatment, the chorea-ballismus improved and AIMS score decreased from 38 to 4. Gait disturbance caused by the symptom disappeared. Conclusions: We suggest that Korean medicine treatment can improve the symptom of C-G-BG.
