A군 연구균 감염 후 급성 사구체 신염과 동반된 침습성 수막구균감염증 1례 (Case of Combined Acute Poststreptococcal Glomerulonephritis and Acute Invasive Meningococcal Disease)

  • 본 저자들은 A군 연쇄구균 감염 후 급성 사구체신염과 동반된 수막구균감염증(수막구균 뇌수막염, 수막구균혈증)에 걸린 7세 남아를 경험하였는데 A군 연쇄구균 감염 후 급성 사구체 신염으로 인해서 C3가 감소하여 수막구균감염증에 감염이 되었는지, 역으로 가능성은 떨어지지만 수막구균감염증의 결과로 C3가 감소하였고 사구체신염이 생긴 것인지 전후 관계가 확실하지 않은 증례를 문헌고찰과 함께 보고하는 바이다.

Functional Genomics of Vibrio vulnificus: from Survival to Toxigenesis

  • Understanding the molecular pathogenesis of the multifaceted host-pathogen interaction is critical in the development of improved treatment and prevention, as well as elucidating how certain bacteria can circumvent host defenses, multiply in the host, and cause such extensive damage. Disease caused by infection with V. vulnificus is remarkable for the invasive nature of the infection, ensuing severe tissue damage, and rapidly fulminating course. The characterization of somatic as well as secreted products of V. vulnificus has yielded a large list of putative virulence attributes, whose known functions are easily imagined to explain the pathology of disease. These putative virulence factors include a carbohydrate capsule, lipopolysaccharide, a cytolysin/hemolysin, elastolytic metalloprotease, iron sequestering systems, lipase, and pili. However, only few among the putative virulence factors has been confirmed to be essential for virulence by the use of molecular Koch's postulates. This presentation describes molecular biological characterization of the virulence factors contributing to survival as well as to toxigenesis of V. vulnificus.

설사 환자에서의 원인균 분리동정에 관한 연구 (Study on the Identification of Enteropathogenic Bacteria from Diarrheal Patients)

  • The specimens were collected from 89 diarrheal patients who had visited Pusan National University Hospital from June to September 1985. They were cultured and tested for the bacteriological identification of causative agents. In this study we identified 5 strains of Salmonella species, 5 strains of Shigella species, 2 strains of Y. enterocolitica, and 17 strains of enteric pathogenic E. coli. Enteric pathogenic E. coli were classified into enterotoxigenic E. coli, enteropathogenic E. coli, and enteroinvasive E. coli by serological type. We tried to isolate V. cholerae and V. parahaemolyticus too but we cannot find them out.

A tdcA Mutation Reduces the Invasive Ability of Salmonella enterica Serovar Typhimurium

  • We previously observed that the transcription of some flagellar genes decreased in Salmonella Typhimurium tdcA mutant, which is a gene encoding the transcriptional activator of the tdc operon. Since flagella-mediated bacterial motility accelerates the invasion of Salmonella, we have examined the effect of tdcA mutation on the invasive ability as well as the flagellar biosynthesis in S. Typhimurium. A tdcA mutation caused defects in motility and formation of flagellin protein, FliC in S. Typhimurium. Invasion assays in the presence of a centrifugal force confirmed that the defect of flagellum synthesis decreases the ability of Salmonella to invade into cultured epithelial cells. In addition, we also found that the expression of Salmonella pathogenicity island 1 (SPI1) genes required for Salmonella invasion was down-regulated in the tdcA mutant because of the decreased expression of fliZ, a positive regulator of SPI1 transcriptional activator, hilA. Finally, the virulence of a S. Typhimurium tdcA mutant was attenuated compared to a wild type when administered orally. This study implies the role of tdcA in the invasion process of S. Typhimurium.

Non-Invasive Colorimetric Magneto Loop-Mediated Isothermal Amplification (CM-LAMP) Method for Helicobacter pylori Detection

  • More than half the world's population is thought to be infected with Helicobacter pylori. Although the majority of infected people are asymptomatic, H. pylori infection may cause gastric ulcers and deadly gastric cancer. Owing to the difficulty and invasiveness of current routine culture and diagnostic methods, a highly sensitive and specific noninvasive assay for H. pylori is of interest. This study highlighted the design and performance of a colorimetric magneto loop-mediated isothermal amplification (CM-LAMP) assay to detect H. pylori in spiked saliva samples. LF primers were coated on magnetic nanoparticles by carbodiimide-induced immobilization and functionally used for solid-phase amplification. During the LAMP reaction at 66℃, biotin-tagged FIPs were incorporated into LAMP amplicons. The colorimetric signal developed after the addition of NeutrAvidin horseradish peroxidase conjugate (NA-HRP) and ABTS. None of the tested microorganisms, including closely related bacteria, was shown positive by the CM-LAMP assay except H. pylori isolates. This novel platform was highly specific and 100-fold more sensitive (40 CFU/ml or 0.2 CFU per reaction) than the PCR and conventional LAMP assays for the detection of H. pylori in spiked saliva. Our results demonstrated the feasibility of using this noninvasive molecular diagnostic test to detect H. pylori in saliva samples.

Halo Blight of Kudzu Vine Caused by Pseudomonas syringae pv. phaseolicola in Korea

  • Kudzu vine(Pueraria montana var. lobata) is an invasive climbing woody vine that envelops trees and shrubs, pressing physically and shutting out sunlight, which needs to be controlled. Kudzu vine pathogens were surveyed as a way to seek its biocontrol agents in 2002. Occurrence of a bacterial halo blight disease of kudzu vine was observed at several localities in Korea including Euiwang and Suwon in Gyeonggi Province, Daejon, and Gochang and Buan in Jeonbuk Province. Symptoms of brown to black spots with a surrounding yellowish halo appeared from June and lasted till the rainy season without much expansion, but accompanying often leaf blight and defoliation. Isolated bacteria were identified as Pseudomonas syringae pv. phaseolicola based on physiological and cultural characteristics, Biolog, fatty acid and 16S rDNA sequencing analyses. In artificial inoculation test, these bacteria produced the same halo spot symptoms on kudzu vine and bean plants. They also induced hypersensitive responses (HR) on tobacco, tomato, and chili pepper leaves. This is the first report of a bacterial disease of kudzu vine in Korea, and the bacterial pathogen can be used as a biocontrol agent against the pest plant.

근관감염균에 대한 polyphosphate의 항균효과 (ANTIBACTERIAL EFFECT OF POLYPHOSPHATE ON ENDODONTOPATHIC BACTERIA)

  • This study was performed to observe the antibacterial effect of polyphosphate (polyP) with various chain lengths (P3~P75) on virulent. invasive strains of P. gingivalis A7A1-28 and W50, and multidrug resistant E. faecalis ATCC29212. P. gingivalis strains were grown in brain-heart infusion broth (BHI) containing hemin and vitamin K with or without polyP. PolyP was added at the very beginning of the culture or during the exponential growth phase of the culture. Inhibition of the growth of P. gingivalis was determined by measuring the absorbancy at 540nm of the grown cells. Viable cell counts of the culture and release of intracellular nucleotide from P. gingivalis were measured. E. faecalis was grown in plain BHI with antibiotics alone or in combination with polyP(calgon: 0.1~1.0%) and the bacterial absorbancy was measured. The overall results suggest that polyP has a strong antibacterial effect on the growth of the virulent strains of P. gingivalis and the antibacterial activity of polyP seems largely bactericidal. accompanying bacteriolysis in which chelation phenomenon is not involved. Although polyP does not exert antibacterial activity against E. faecalis, it appears to increase antibacterial effect of erythromycin and tetracycline on the bacterium. Therefore, polyP alone or in combination with antibiotics may be developed as a candidate for the agent controlling oral infections including endodontic infection.

Empyema caused by Clostridium perfringens

  • Pleuropulmonary diseases caused by Clostridial species infections are rare, but have a mortality rate of up to 30%. Furthermore, older people are at greater risk of developing invasive clostridium infections, and the majority of reported cases of clostridium empyema have been attributed to iatrogenic trauma or aspiration. The authors report a case of spontaneous empyema caused by Clostridium perfringens. A 72-year-old woman was admitted to Kangwon National University Hospital for empyema. The patient had no history of trauma, a dental procedure, or aspiration, and was treated using empirical antibiotics and by drainage of pleural fluid. Bacteria species that cause empyema are usually not detected, but on the 4th day of admission, C. perfringens was isolated from the pleural space. The patient was continuously treated with antibiotics for C. perfringens and drainage, and was discharged 25 days after admission with almost a fully recovered status. Increased awareness of Clostrium species infection in the elderly is needed to ensure appropriate treatment.

Helicobacter pylori 감염진단에 있어 H. pylori Ag Stool 검사 (면역크로마토그라피법)의 임상적 유용성 (Clinical Usefulness of Helicobactor pylori Ag Stool Test (Immunochromatographic Assay) for Diagnosis of H. pylori Infection)

  • The aim of this study was to assess the Clinical Usefulness of Helicobacter pylori Stool Antigen (HpSA) immunochromatographic assay for the diagnosis of H. pylori infection. In this study, we had compared HpSA-immunochromatographic assay with CLO test and UBT test. From a total of 140 patients (M:F=88:52) with upper endoscopy, biopsy specimens were obtained for CLO test. Stool specimens was collected from all patients and tested using a HpSA-immunochromatic assay. H. pylori infection status was defined as infected if the results of both CLO test and UBT test were positive. CLO test and UBT test findings showed that 92 patients were H. pylori positive and 48 patients were H. pylori negative. According to this definition, the sensitivity, specificity, and positive or negative predictive value (PPV, NPV) of HpSA-immunochromatographic assay were 97.8%, 100%, 100%, and 96%, respectively. Cross reactivity test of HpSA-immunochromatographic assay were performed with 10 enteric bacteria strains in fecal habitat, and there were no false positive reaction. We evaluated the usefulness of HpSA assay for eradication therapy with 10 of 92 H. pylori positive patients, positive results of them at pre-eradication therapy were converted to negative at post-eradication. The HpSA-immunochromatographic assay is a highly sensitive and specific non-invasive diagnostic method for detection of H. pylori infection, a useful diagnostic method for H. pylori in post eradication stage.
