신성 고혈압 백서의 Renin Secretion 조절의 특성 (Characteristics of Control Mechanism of Renin-Angiotensin System in Two Kidney One Clip Goldblatt Hypertension)

  • It has long been suggested that the change of renin-angiotensin system is responsible for the increased arterial blood pressure in the experimental hypertension. But the exact nature of the cause and maintenance of early and late Phase of renal hypertension is still controversial. Increased renin-angiotensin system has been suggested. To clarify the altered renin-angiotensin system in the early phase of two kidney one clip Goldblatt hypertension(2K1C GH), experiments were carried out in the rats of 3,7, and 14 days of 2K1C GH rats, sham-operated, and control rats. Responses of the plasma renin activity to the intravenous infusion of L-isoproterenol were dose-dependent. Responses of the plasma renin activity to the intravenous L-isoproterenol in 2K1C GH rats were not different from sham-operated control rats. Hypotensive responses of the 2K1C GH rats were not different from sham-operated rats. Suppression by intravenous infusion of angiotensin II of plasma renin activity showed a dose-dependent manner. Suppression by angiotensin ll of plasma renin activity was attenuated or abolished in the early phase of 2K1C GH rats. Intravenous infusion of arginine vasopressin(AVP) showed a dose-dependent suppression of plasma renin activity, Attenuated responses by AVP of plasma renin activity were noticed in the early phase of 2K1C GH rats. These results suggest that the altered renin-angiotensin system in the early phase of the two kidney one clip Goldblatt hypertension may be caused by failure of the short loop negative feedback control mechanism.

Continuous-infusion Ifosfamide and Doxorubicin Combination as Second-Line Chemotherapy for Recurrent or Refractory Osteosarcoma Patients in China: a Retrospective Study

  • Objective: The aim of this retrospective study was to evaluate the feasibility and efficacy of response to continuous-infusion ifosfamide and doxorubicin combination as second-line chemotherapy for patients with recurrent or refractory osteosarcoma. Materials and Methods: Eighteen recurrent or refractory osteosarcoma patients who were treated with continuous-infusion ifosfamide and doxorubicin combination between May 1999 and April 2011 were included in the analysis. Ifosfamide at $12g/m^2$ was administered by intravenous continuous infusion over 3 days, and doxorubicin $60mg/m^2$ was administered as an intravenous bolus injection on day 1. The combination therapy was repeated every 3 weeks. Treatment was continued until evidence of disease progression or unacceptable toxicity. Results: The patients (ages 7-53 years) received a total of 42 cycles of chemotherapy (median: 2 courses; range: 2-5 courses). The overall response rate was 0% and the disease control rate was 22.3%, with four patients having stable disease. The median time to progression and overall survival time were 2 months (range: 2-5 months) and 9 months (range: 3-29 months), respectively. Major severe toxicities were leucopenia 7 (38.9%), nausea and vomiting 3 (16.7%) and alopecia 9 (50%). There were no treatment-related deaths. Conclusions: In our experience, continuous-infusion ifosfamide and doxorubicin combination therapy at this dosage and schedule was found to be well tolerated and moderate effective, which could be considered as salvage therapy for patients with recurrent or refractory osteosarcoma. Further assessment is necessary to confirm the safety and efficacy of this treatment.

Ketorolac의 지속적 정주가 Morphine정맥 PCA에 미치는 효과 (The Effect of Continuous Infusion of Ketorolac on Morphine IV PCA)

  • Background: Ketorolac($Tarasyn^{(R)}$) is a non-steroidal anti-inflammatory drug(NSAID) which has shown to be an effective postoperative analgesic available parenterally, and when combined with morphine can reduce its requirement. The analgesic efficacy and adverse effects of continuous infusion of ketorolac added to morphine IV PCA was evaluated in 60 women after abdominal hysterectomy. Methods: Patients were assigned to receive either morphine intravenous(IV) bolus followed by morphine IV patient controlled analgesia(PCA), or ketorolac 30mg IV and continuous IV infusion at 4.0mg/hr in combination with the above regimen. The authors evaluated PCA morphine used, pain assessment(verbal pain intensity score and visual analogue scale) and side effects at 2, 4, 6 and 24hrs during pain control. Results: Continuous infusion of ketorolac decreased the PCA morphine usage significantly(30.4 ---> 19.6 mg : p=0.007) at 24hrs postoperatively. Significant differences were seen favoring ketorolac infusion in pain intensity and visual analogue scale both at rest and during movement. There were no differences in incidences of deep sedation, nausea & vomiting. But the ketorolac group they complained of dizziness more than morphine only group. Little pruritus was recorded in either groups. Conclusions: The authors conclude continuous IV infusion of ketorolac in conjunction with morphine PCA provide effective analgesia after low abdominal surgery.

근거기반 중심정맥 주입요법 간호실무지침 개정 (Updates of Evidence-Based Nursing Practice Guidelines for Central Venous Infusion Therapy)

  • Purpose: This study was conducted to update nursing practice guidelines for intravenous infusion published in 2017. Methods: The guideline update process was carried out using 22 steps developed by NICE and SIGN. It was agreed to update domains related to central venous infusion therapy. Contents related to peripheral infusion would be updated later. Results: Updated guidelines for central venous infusion therapy consisted of 6 domains and 195 recommendations. The number of recommendations by domain was 11 for general instruction, 14 for central vascular access devices (CVAD) and add-on devices, 13 for nursing management before insertion of CVAD, 30 for management during insertion of CVAD, 51 for management after insertion of CVAD, and 76 for complications. A grade was 29 (14.9%), B grade was 87 (44.6%), and C grade was 79 (40.5%) in the strength of recommendations. A total of 37 (19.0%) recommendations were newly developed and 23 (12.3%) previous recommendations have been modified. The newly developed recommendations were mainly related to the infection control methods. Conclusion: The updated guideline is focused on safe maintenance of central venous infusion therapy. Through this guideline, it is hoped to minimize the occurrence of complications and improve the standardization and efficiency of nursing practice.

Effects of VFAs and Glucose Infusions on Ruminating Behavior of the Fasted-Goats

  • To test their roles in rumination behavior, metabolites (acetic acid, propionic acid, butyric acid and glucose) were intravenously infused into the jugular vein of goats during fasting. The heads of four female goats tested were locked in a stanchion of cages in an experimental room. Ruminating behaviors with regard to number of ruminations, ruminating time, number of remastications and remasticating time were significantly decreased by acetic acid infusion (p < 0.05), and tended to be depressed more on values in butyric acid infusion or glucose infusion than those in pre-infusions, but propionic acid infusion decreased a little. Those data suggest that rumination receptors sensitive to VFAs and glucose are more likely to be situated in the area, where they would respond too blood levels.

아동간호사의 말초정맥관 관련 교육 및 간호수행에 영향을 미치는 융합적 요인 (Convergence factors affecting on education and nursing performance related to peripheral intravenous catheterization among pediatric nurses)

  • 본 연구는 아동간호사의 말초정맥관 관련 간호교육프로그램의 기초자료를 얻기 위해 125명의 아동간호사를 대상으로 시행된 서술적 조사연구로, 최희강 외(2016)의 아동의 말초정맥주사관리 프로토콜과 간호교육평가원의 핵심 기본간호술 중 정맥주사프로토콜의 일부를 사용하여 수집된 자료를 SPSS 21.0으로 분석하였다. 아동간호사의 말초정맥관 관련 교육수행이 가장 낮았고, 부작용관리수행이 가장 높았다. 일반적특성에 따른 간호수행 하위영역에 부분적으로 유의한 차이가 있었다. 교육과 간호수행 간 정적 상관관계가 있었고(r=.486, p<.001), 간호수행에 영향을 미치는 요인은 교육(${\beta}=.472$), 근무부서(${\beta}=.216$) 및 결혼상태(${\beta}=.169$)이었다(adj $R^2=0.314$, p<.001). 본 연구결과는 대상자 수를 확대하여 반복 연구 후 아동간호사의 말초정맥관 관리 교육프로그램의 기초자료로 활용될 수 있다.

Significance of Feeding Induced Hypovolemia in Feed Intake Control of Goats Fed on Alfalfa Hay

  • The objective of this study was to examine whether feeding induced hypovolemia (decrease in plasma volume) acts on the regulation of feed intake in goats fed on dry forage. In order to prevent feeding induced hypovolemia, a 2 h intravenous infusion (16-18 ml/min) of isotonic mannitol solution was begun 1 h prior to feeding and continued until 1 h after the start of the 2 h feeding period. The intravenous infusion of isotonic mannitol solution (MI) decreased plasma osmolality by 1.0%, plasma total protein concentration by 4.2% and hematocrit by 5.9%, respectively. In comparison with no infusion (NI), MI significantly decreased thirst level by approximately 13%. At the completion of the 2 h feeding period, cumulative feed intake had been increased by 43% by MI. In conclusion, feeding induced hypovolemia in goats fed on dry forage increased thirst level more than the increase in plasma osmolality did. The results demonstrate that feeding induced hypovolemia is one of the factors controlling feed intake in goats fed on dry forage.

Effects of Xylazine or Acepromazine in dogs under constant rate infusion with alfaxalone

  • The anesthetic depth and cardiovascular effect of alfaxalone constant rate infusion in dogs premedicated with xylazine or acepromazine were evaluated. Ten dogs were randomly allocated into 2 groups. In group AA, dogs were premedicated with 0.02 mg/kg of intravenous acepromazine at 15 min before induction. In group XA 1.1 mg/kg of intravenous xylazine was premedicated at 5 min before induction. The anesthesia was maintained with 6 mg/kg/hr of alfaxalone after induction with 2 mg/kg alfaxalone in both groups. In both of groups, the qualities of induction were satisfactory without any adverse event, but adequate analgesia could not be provided, according to the withdrawal test. $PaO_2$ and $SaO_2$ implied a slight hypoxemia state in XA group, while those values of group AA were not significantly changed. The acepromazine and alfaxalone combination induce mild tachycardia. The bispectral index score were significantly decreased in group XA, compared with that in group AA. The premedication of xylazine before alfaxalone constant rate infusion in this study could provide adequate analgesia during 30 min, while the premedication with acepromazine could not.

  • Anxiety and fear is two main factors that keep patients from going to dental clinic. Especially, patients may feel implants operations are more traumatic. Intravenous conscious sedation for dental treatment can make patient comfortable and relaxable. Midazolam is more popular for sedation for dental treatment, but target-controlled infusion (TCI) of propofol and remifentanil is gaining wide popularity. A 54-year-old female patient who had severe dental phobia was referred to our dental hospital. She had past history of 2 times of hyperventilation and syncope during dental treatment. The patient showed a lot of dental anxiety and fear to dental treatments and stress reduction protocol was needed. We administered intravenous conscious sedation using target controlled infusion system with remifentanil and propofol. During sedation, we monitored the status of consciousness with bispectral index and vital signs. Dental treatment could be finished successfully without any problems.

Effects of Isopropyl Alcohol Infusions on the Ruminating Behavior of Goats

  • Metabolites, such as isopropyl alcohol (IPA) produced by rumen fermentation, were intravenously infused into a jugular vein of goats during feeding to explore the mechanism and roles of IPA in ruminating behavior (number of boli and ruminating time). Three female goats were confined in metabolism cages with a stanchion, The ruminating behavior measured by the number of ruminations, ruminating time, number of remastications, and remasticating time decreased (p<0,05) with intravenous IPA infusion. The IPA concentrations and VFA concentrations increased in the blood circulation. Our data suggest that sensitive receptors of rumination to IPA are more likely to be in an area such as the brain stem where they can respond to blood metabolite levels.
