Multi-vessel intractable coronary spasm development in a patient with aborted sudden cardiac death: a case study with intravascular ultrasound findings

  • Coronary spasm generally occurs in patients with minimal atherosclerotic plaque lesion, and it has a rather favorable prognosis. However, in some cases, coronary spasm may induce myocardial infarction and even sudden cardiac death (SCD). Here, we report a case in which multi-vessel intractable coronary vasospasm suddenly occurred in a diffuse atherosclerotic lesion after percutaneous coronary intervention (PCI) in a patient with aborted SCD. We identified the characteristics of the spasm portion in intravascular ultrasound (IVUS) images and conducted percutaneous cardiopulmonary bypass support-PCI with stenting as treatment. Intima and media thickening and a large attenuated plaque burden with rupture were identified in IVUS images at the obstructive spasm portion.

  • Intravascular papillary endothelial hyperplasia or Masson's hemangioma is a rare vascular tumor. The reactive proliferation of endothelial cells in this disease mimics other benign or malignant vascular proliferation such as angiosarcoma or Kaposi's sarcoma. It is important to make an accurate distinction to avoid confusion with these malignant tumors. This would facilitate a proper diagnosis, which is essential so that the patient is not subjected to unnecessarily aggressive or inappropriate treatment.

Effects of Guizibokryungwhan Herbal-Acupuncture on the Intravascular Coagulation Induced by Endotoxin in Rats (계지복령환약침(桂枝茯笭丸藥鍼)이 Endotoxin으로 유발(誘發)된 흰쥐의 혈전증(血栓症)에 미치는 영향(影響))

  • This study was designed to investigate the effects of Guizibokryungwhan Herbal-acupuncture on the intravascular coagulation. At first 36 mice were divided normal group(n=6) and experimental group(n=30). Normal group had no treatment and experimental group was induced intravascular coagulation by injected endotoxin 0.5mg/kg into the caudal vein. The experimental group was divided Sample-A(n=12), Sarnple- B(n=12) and control group(n=6). Control group was administrated normal saline. Sample-A was injected Guizibokryungwhan Herbal-acupuncture manufactured by extraction method and Sample-B was injected Guizibokryungwhan Herbal-acupuncture manufactured by water-alcohol method. Then Sample-A and Sample-B was injected high and low concentration. All treatments were done to Kwanwon acupuncture point once a day for 5 days s.c.. The results were following. 1. Compared with the control group, the platelet count was significantly increased in high-concentrated Sample-A and Sample-B group. 2. Compared with the control group, the concentration of fibrinogen was significantly increased in all experimental groups. 3. The prothrombin time was sjgnificantly shortened in high-concentrated Sample-A and Sample-B group as compared with the control group. 4. The activated partial thromboplastin time was significantly shortened in high-concentrated Sample-A group as compared with the control group. 5. The Guizibokryungwhan Herbal-acupuncture has a concentration-dependent effects on the intravascular coagulation.

Effect of He-Ne laser intravascular irradiation by live blood analysis (생혈구분석을 통한 He-Ne laser 정맥혈관내 조사의 효과)

  • Objectives : The purpose of this study was to investigate the effect of Intravascular Laser Irradiation of Blood(ILlB) by the live blood analysis. Methods : We had analysed the changing forms of the live blood samples with Ultra Darkfield Microscope before and after Intravascular Laser Irradiation of Blood. Results : 1. Somatid did not showed significant change. 2. In the rouleau of red blood cells was decreased significantly. 3. In the morphological change of red blood cells, Burr cell, Ovalocyte and Poikilocyte were decreased significantly, but Acanthocyte and Target cell were increased significantly. 4. In the abnormal matters in plasma, the Cholesterol cristal did not showed significant change, but the Aggregation of platelet, Lipids, Spicule, Leucocyte, Uric acid cristal did showed a little significant decrease. Conclusion : These findings suggest that live blood analysis is useful to judge the effect of treatment and diagnosis in oriental medicine, and with the effect of Intravascular Laser Irradiation of Blood, it had showed significant effect on rouleau of red blood cells, morphological change of red blood cells and abnormal matters in plasma.

  • This paper reports the development and performance evaluation of a backend system for real-time IVUS (Intravascular Ultrasound) imaging. The developed backend system was designed to minimize the amount of logic and memory usage by means of efficient LUTs (Look-up Tables), and it was implemented in a single FPGA (Field Programmable Gate Array) without using external memory. This makes it possible to implement the backend system that is less expensive, smaller, and lighter. The accuracy of the backend system implemented was evaluated by comparing the output of the FPGA with the result computed using a MATLAB program implemented in the same way as the VHDL (VHSIC Hardware Description Language) code. Based on the result of ex-vivo experiment using rabbit artery, the developed backend system was found to be suitable for real-time intravascular ultrasound imaging.

The Effect of Hongdeung-tang on Endoxotin-induced Disseminated Intravascular Coagulation (홍등탕(紅藤湯)이 내독소로 유발된 파종성혈관내응고증에 미치는 영향)

  • Objective : This experimental study was carried out to investigate anti-coagulation effects and mechanism of Hongdeung-tang on the dissentimenated intravascular coagulation Methods : Each 10 rats were classified as normal gorup, control group and treated group. The control group was given a 0.5mg/ml endotoxin intra-vein injection and was occurred Disseminated Intravascular Coagulation(DIC). The treated group were orally administrated 1ml Hongdeung-tang concentration once a day for 6 days and then it was treated the same as the control group. We took the blood from heart and were investigated number of Platelet and white blood cell(WBC), fibrinogen concentration, prothrombin time(PT), activated partial thromboplastin time(APTT), interleukin-6(IL-6), Tumor Necrosis $Factor-{\alpha}(TNF-{\alpha})$, Prostaglandin $E2(PGE_2)$ from them. Results : There were a statistically significant increase of platelet and fibrinogen concentration in the treated group compared to the treated group. The PT was a significantly reduced in treated group compared to the control group. There was no significant change in numbers of WBC and APTT between control and treated group. On the contrary, the number of PT, IL-6, $TNF-{\alpha}$, $PGE_2$ were significantly decreased in treated group compared to the control group. Conclusion : DIC which was occurred by endotoxin intravenous injection seems to related to the increase of IL-6, $TNF-{\alpha}$, $PGE_2$ and Hongdeung-tang has an effect that supress those factors. we can assume Hongdeung-tang has an anti-coagulation effect by those supressing effect.

Effect of Gwaluzisiltang and Gwaluzisiltanggami on the Intravascular coagulation induced by Endotoxin in Rats (과루지실탕(瓜蔞枳實湯) 및 그 가미방(加味方)이 ENDOTOXIN으로 유발(誘發)된 혈전(血栓)에 미치는 영향(影響))

  • This study was performed to investigate the effects of Gwaluzisiltang and Gwaluzisiltanggami on the intravascular coagulation. The experimental group divided two groups : one group was the intravascular coagulation induced by endotoxin in rats, another group was the formation of paw edema by contusion in rats, and then these rats were treated with liquid extract of Gwaluzisiltang(Sample I) and Gwaluzisiltanggami(Sample II), which were administered orally. Then the numbers of platelets, concentration of fibrinogen, Prothrombin time and FDP(fibrin-fibrinogen degradation Products)were measured. The results were as follows : 1. The effects of the Intravascular coagulation 1) Platelet was increased significantly in the sample I compared with control group. 2) Fibrinogen was increased significantly in the sample I compared with control group. 3) Prothrombin time was shortened significantly in the sample I and more shortened in the sample II compared with control group. 4) FDP was decreased significantly in the sample I and more decreased in the samplem II compared with control group. 2. The effect of the formation of paw edema by contusion in rats. 1) The rate of paw edem was decreased significantly after five hours in the sample I 2) Platelet was increased significantly in the sample I compared with control group. 3) Fibrinogen was decreased in the sample I and sample II compared with control group, but it is not significant 4) Prothrombin time was shortened significantly in the sample II compared with control group. According to the above results, it is considered that the Gwaluzisiltang and Gwaluzisiltanggami seem to be applicable disease related to thrombosis, because they obtained significant effects on the experimental method which are based on the oriental medical theory-the principle of phlegm and blood stasis have the same source and disease (痰瘀同源, 痰瘀同病).

  • Intravascular papillary endothelial hyperplasia (IPEH, Masson's hemangioma) is a non neoplastic reactive endothelial proliferation most commonly located in the skin or subcutaneous tissues although it has been reported in multiple locations throughout the body. This lesion may arise from malformed or normal vessels primarily, and may develop with hemangioma, pyogenic granuloma, or lymphangioma. This lesion, though benign, is clinically important since it may present as a mass and be confused histologically with angiosarcoma. The authors report a 27 years old patient with a mass in his forearm which results in intravascular papillary endothelial hyperplasia.

Development of a High-Speed Endoscopic OCT System and Its Application to Three-Dimensional Intravascular Imaging in Vivo (고속 내시경적 OFDI 시스템 개발과 이를 이용한 3차원 생체 혈관 내부 이미징)

  • Intravascular optical coherence tomography (OCT) enables imaging of the three-dimensional (3D) microstructure of a blood vessel wall. While 3D vascular visualization provides detailed information of the vessel wall and intraluminal structures, a longitudinal imaging pitch that is several times bigger than the imaging resolution of the system has limited true high-resolution 3D imaging. In this paper we demonstrate high-speed intravascular OCT in vivo, acquiring images at a rate of 350 frames per second. A 47-mm-long rabbit aorta was imaged in 3.7 seconds, after a short flush with contrast agent. The longitudinal imaging pitch was 34 micrometers, comparable to the transverse imaging resolution of the system. Three-dimensional volume rendering showed greatly enhanced visualization of tissue microstructure and stent struts, relative to what is provided by conventional intravascular imaging speeds.
