Fiberoptic bronchoscope and C-MAC video laryngoscope assisted nasal-oral tube exchange: two case reports

  • In cases of multiple facial trauma and other specific cases, the anesthesiologist may be asked to convert an oral endotracheal tube to a nasal endotracheal tube or vice versa. Conventionally, the patient is simply extubated and the endotracheal tube is re-inserted along either the oral or nasal route. However, the task of airway management can become difficult due to surgical trauma or worsening of the airway condition. Fiberoptic bronchoscopy was considered a novel method of airway conversion but this method is not useful when there are secretions and bleeding in the airway, or if the anesthesiologist is inexperienced in using this device. We report a successful airway conversion under the aid of both, a fiberoptic bronchoscope and a C-MAC video laryngoscope.

Effects of Oral Care with 0.1% Chlorhexidine and Tantum Solution on Oral Status and Incidence of Oral Pathogens among Intubated ICU Patients: A Pilot Study (0.1% 클로르헥시딘과 탄툼액을 이용한 구강간호가 기관내관을 삽입한 중환자실 환자의 구강상태 및 구강내 세균 발생에 미치는 효과)

  • Purpose: The purpose of this pilot study was to investigate the effect of oral care using 0.1% chlorhexidine and tantum solution on oral status and incidence of oral pathogens among intubated ICU patients. Methods: The study subjects were 26 intubated patients of MICU in a university hospital. Oral care was provided with 0.1% chlorhexidine solution to the experimental group (n=12) and with tantum solution to the control group (n=14), 3 times a day for 6 days. After the intervention, the oral status of each subject was checked using structured method, and oral swab culture was done to count colonized bacterial pathogens. Results: The mean oral status score of experimental group was significantly higher than that of control group (U=42.50, p=.031). The mean incidence of oral pathogens in experimental group was smaller than that of control group, but the difference was not statistically significant. Conclusion: The oral care with 0.1% chlorhexidine solution is more effective than with tantum solution for improving oral status of intubated ICU patients. However, additional studies with larger sample size will be needed to figure out it's effects on the incidence of oral pathogens.

Oral Hygiene Care of Endotracheal Intubated Patients in Korean Intensive Care Units : A Scoping Review (기관내관을 적용중인 국내 중환자실 성인 입원환자의 구강간호에 대한 주제범위 문헌고찰 )

  • Purpose : This scoping review was designed to identify gaps in knowledge and guide future directions for clinical nursing practices and research on oral hygiene care for endotracheally intubated patients in Korean intensive care units. Methods : We conducted a scoping review using the methodological framework proposed by Arksey and O'Malley. PubMed, CINAHL, RISS, Science On, and DBpia databases were searched. Two reviewers independently selected the studies and extracted data. A total of 445 studies were identified, of which 17 were included in the final analysis. Results : No research has been conducted from 2021 to the present. A total of 12 instruments were utilized in 13 studies to assess the effectiveness of oral hygiene care. All studies investigating the effectiveness of oral hygiene care have consistently documented that chlorhexidine has the most significant effect. The frequency and time of oral hygiene care were each examined in one study. Conclusion : A research study is necessary to develop a measurement tool for assessing the effectiveness of oral hygiene care suitable for endotracheal intubated patients in intensive care units. Interventional studies should be conducted to determine the effects of chlorhexidine and the appropriate frequency and time of oral hygiene care. Thereafter, revision of domestic evidence-based clinical practice guidelines by integrating these results will be necessary.

Preventive Effect of Garlic Administration on Respiratory Toxicity Induced through Intratracheal Instillation of Fine Dust (PM10) in Rats (실험동물 랫드를 이용한 미세먼지 기도노출에 따른 호흡기계 독성에 대한 마늘의 예방효과 탐색)

  • Objectives: Exposure to fine dust (PM10) could contribute to the occurrence of cardiovascular disease or respiratory abnormalities. Since garlic is known to possess an anti-oxidative stress effect, the present study was performed to evaluate the effect of garlic intake on fine dust-mediated pulmonary toxicity. Methods: Rats were intratracheally instilled with fine dust at 15 mg/kg body weight (BW)/day for five days following five-day intragastric intubation of garlic at 0.7 or 1.4 g/kgBW/day, or 13.1 mg/kgBW/day S-allyl-cysteine (SAC) as a reference component in garlic. Blood and bronchoalveolar lavage fluid (BALF) were collected. Results: Deposit of fine dust was visually and histopathologically observed in the lungs. Body weight gain during the instillation period was significantly lowered in all the groups instilled with fine dust. Neutrophil numbers in blood were significantly elevated in the fine dust alone group, but this alteration was diminished in the groups administered with garlic. Levels of serum glutathione were lower in the rats instilled with fine dust alone, and this decrease in the glutathione level seems dose-dependently compensated among the groups administered with garlic. Similar findings were observed in the BALF with statistical significance. Typical pulmonary histopathological observation related with inflammation was demonstrated in the lungs of the rats exposed to fine dust alone, whereas such histopathologic findings were not improved in the groups administered with garlic. Conclusion: The present study suggests that garlic intake could alleviate fine dust-mediated pulmonary or systemic toxicities. Further investigation is necessary to delineate the mechanism of garlic-mediated effects on pulmonary function.

Physiologic Changes During Bronchoscopy in Mechanically Ventilated Patients (기계환기중인 환자에서 기관지내시경 검사에 따른 생리적 변화)

  • Background : Bronchoscopy in patients on mechanical ventilation is being performed much more frequently. However, there is little data on the changes in physiologic parameters and no established mechanical ventilation protocol during bronchoscopy. A decreasing or the removal of positive end-expiratory pressure (PEEP) during bronchoscopy may precipitate severe hypoxemia and/or derecruitment. Methods : Our standardized mechanical ventilation protocol, without changing the PEEP level, was used during bronchoscopy. The physiological parameters were measured during the bronchoscopic procedure. Results : During bronchoscopy, respiratory acidosis, elevation of peak pressure, elevation of heart rate and auto-PEEP were developed, but were reversible changes. Procedure-related gross barotraumas or other severe complications did not developed. Conclusion : No serious complications developed during bronchoscopy under our standardized mechanical ventilation protocol when the PEEP level remained unchanged. The procedure time should be kept to a minimum to decrease the exposure time to undesirable physiological changes.
