종격동 결핵성 농양: 2례 보고 (Mediastinal Tuberculous Abscess - Report of two cases -)

  • Tuberculosis is a chronic disease caused by Mycobacterium tuberculosis, which usually affects the lung but may cause lesions in any organ or tissue of the human body. Mediastinal lymph node involvement is common feature of intrathoracic tuberculosis in children. Sometimes the lymph node may be enlarged and it causes compressive symptoms. Recently we experienced two cases of tuberculous abscess at middle mediastinum. The abscess seemed to be originated from the mediastinal lymphadenitis, and caused the symptoms. Operation was performed by median sternotomy and by posterolateral thoracotomy incision respectively for the purpose of relieving symptoms and diagnosing the mediastinal mass. The symptoms were relieved completely and postoperative course was uneventful.

  • A 22 year old soldier was discovered on routine study for discharge to have a mass in the posterior mediastinum He was admitted due to high fever and improved by antibiotic treatment for 2 weeks. Chest PA, apicogram, myelogram and CT scan demonstrated enlargement of the neural foramen at the T1 level with erosion of the posterior aspect of the vertebral body and the pedicle contiguous with the intrathoracic mass, A myelogram showed a large extradural defect at the T1 level on the left. There were no clinical signs of cord compression. A standard posterolateral thoracotomy incision was made with extension to high thoracic vertebra. The 3rd rib was resected and the angles of the posterior portion of 1st and 2nd ribs were cut and rib heads were removed. Extrapleural neurilemmoma 6x6 cm was resected intrathoracically. And after removal of the pedicle and the lamina, intraspinal extradural mass 3 X 2 cm was resected carefully with trivial tearing of the dura which was sealed with gel-foam and pleura. There was ma postoperative neurological complication.

Supradiaphragmatic Heterotopic Liver Presenting as a Pleural Mass: A Case Report

  • Abnormally located liver tissue has been described in the vicinity of the liver proper, near anatomical structures such as the gallbladder, the umbilical fossa, the adrenal gland, the pancreas, and the spleen. Supradiaphragmatic ectopic liver is a rare finding, but has been reported to have been found in the intrathoracic cavity and in the pericardium. In the majority of supradiaphragmatic ectopic liver cases, there was an accompanying transdiaphragmatic pedicle of the main liver body into the abdominal cavity. In a minority of supradiaphramatic ectopic liver cases, the liver was completely separated from the abdominal cavity without a connection between the thorax and the abdomen, with accompanying diaphragmatic anomalies. We describe one case of intrathoracic ectopic liver in a patient with a previous history of lower chest wall trauma, and a brief review of the English-language medical literature on this topic.

비디오 흉부수술의 평가 (The Evaluation of Video-Assisted Thoracic Surgery)

  • Over the past few years, video-assisted thoracic surgery [VATS] has been used increasingly for intrathoracic pathologic problems as a less invasive operative techniques. Today it is viewed as a sparing and safe alternative to thoracotomy for a wide spectrum of indications. Using video-assisted operative thoracoscopy, we performed consecutive 150 operations on 148 patients during the initial 2 years of our experience from July 1992 with the following indications: pneumothorax [n=53], hyperhidrosis [n=29], mediastinal mass [n=23], pleural disease [n=13], diffuse parenchymal or interstitial lung disease [n=12], benign pulmonary nodule [n=7], metastatic lung mass [n=3], primary lung cancer [n=3], bronchiectasis [n=2], malignant pericardial effusion [n=2], endobronchial tuberculosis [n=1], esophageal achalasia [n=1], and pulmonary parenchymal foreign body [n=1]. There were no death, and overall complicaton rate was 24.0%[n=36]. The most prevalent complication was persistent air leakage [longer than 5 days] in 14 cases [9.3%]. Persistent pleural effusion [longer than 5 days] occurred in 6 cases [4.0%]. Six patients were converted to an open thoracotomy because of inability to control the operative bleeding [n=3], failed adhesiolysis in bronchiectasis [n=2], and radical excision of an lung cancer [n=1]. Pneumothorax recurred in 3 cases[2.0%]. Other complications were Horner`s syndrome, diaphragm tears, temporary phrenic nerve palsy, hoarseness, subsegmental atelectasis, transient respiratory difficulty, and esophageal mucosal tear. The advantages of this minimally traumatizing operative technique lie in improved visualization, decreased pain, shortened hospital stay, and less postoperative morbidity. The indications of VATS has been extended increasingly to intrathoracic pathologies, but its role in the managements of primary lung cancer and esophageal disease remains to be defined.

Surgical Removal of Intrathoracic Lipoma from Herniated Omentum through the Diaphragmatic Defect in an Abyssinian Cat

  • A nine-year-old neutered male cat was presented with chronic cough and vomiting. Thoracic radiography showed regions of fatty opacity in the right caudoventral region. On positive contrast celiography, contrast agent did not move into thoracic cavity. Computed tomography revealed 7-mm diameter of defect at the right diaphragmatic crus and a 2-mm diameter defect at the left ventral diaphragmatic crus. Through the right diaphragmatic defect omental herniation was confirmed by the presence of contrast enhanced omental vessel running across the diaphragm. On exploratory thoracotomy, the omentum protruded into the thorax through the right diaphragmatic defect, and it contained a yellowish lipomatous mass. The protruded omentum containing a mass in the thorax was removed, and the right diaphragmatic defect was closed. Histopathologic examination revealed that the protruded omentum showed normal omental structure and the adipose mass showed lipoma surrounded by fibrous tissue. In conclusion, a thorough examination is necessary to confirm the origin of the mass located near the diaphragm.

흉곽내 갑상선종 1예 보고 (Intrathoracic Goiter)

  • Substernal goiter may be defined as any thyroid enlargement that has 50 to 100 % of its mass inferior to the thoracic inlet. Ectopic substernal goiters are rare, and most substernal gaiters arise from cervical thyroid gland. Fifteen to fifty percent of these patients are asymptomatic. Symptoms, when present, are usually the result of tracheal or esophageal compression. Symptoms are often positional. Standard chest roentgenograms are often diagnostic, but computed tomographic or radioactive iodine scans may be helpful. In symptomatic patients or those in whom explorations are undertaken for diagnostic purpose or exclude carcinoma, surgical removal is indicated. Although cervical thyroids with substernal extension may be safely and successfully removed through a cervical incision, primary substernal goiters by definition derive their blood supply from within the thorax and are better approached by splitting the sternum or through a posterolateral thoracotomy. Recently we experienced a child fist sized secondary posterior mediastinal goiter in 55-year old female. The mass was completely removed through right posterolateral thoracotomy without any complications. The postoperative courses were uneventful.

원발성 종격동 지방육종 -1예 보고 - (Primary Mediastinal Liposarcoma - 1 Case Report -)

  • A case of liposarcoma was reported in 52 year-old female. She had the operation history due to mediastinal lipoma at other Hospital before 26 months ago. Chest X-ray revealed a huge soft tissue mass- density at the entire right lung field, and left middle and lower lung field at admission. At the lateral film, the mass was located in the anterior and middle mediastinum. Transsternal bilateral thoracotomy was performed, followed by extirpation of liposarcoma, wedge resection of superior vena cava, angioplasty of superior vena cava, and then partial pericardiectomy. The post-operative treatment was 5500 rad irradiation. Post-operative course was uneventful, that was noticed by OPD follow-up for 10 months. Primary liposarcoma of the mediastinum is very rare tumor. This tumor grows to an enormous size, and symptoms are referable to compression of the contiguous intrathoracic structures. The treatment of choice is surgery in all cases. Such an approach serves to establish a tissue diagnosis, to relieve the patients* symptoms, and may results in a cure sometimes. Radiotherapy or/and chemotherapy seems to be ineffective, but should be further studied.

  • 기관내 지방종은 문헌상 매우 희귀한 종양으로 알려져 있다 본 환자는37세의 건강한 남자로 간질성와 호흡곤란의 증상으로 지난 6개월간 기관지천식의 진단하에 치료받아 오던중,본원에서 흉부전산화 단층촬영과 기관지내시경검사상 흉부기관의 내강에 위치한 구형의 종양이 기관내강의 대부분을 막고 있음을 알고 기관지내시경검사하에 용종제거술루우프를 이용하여 전기소작과 더불어 종양을 완전히 제거하였다 환자는 수술후 3일 째 퇴원하였으며,수술후 1, 6개월에 시행한 기관지내시경상 재발없이 양호한 상태를 유지하고 있다.

누두흉을 동반한 선천성 기관지낭종1 치험례 (A Case of Funnel Chest Associated with Congenital Bronchogenic Cyst)

  • A three year and seven month old girl with moderate depression deformity of the sternum associated with a huge well defined homogenous hazy mass density of the upper half of the right hemithorax on plain chest x-ray had developed, exertionaI dyspnea (Figs1, 2 and 3). Correction of the funnel chest was carried out with modified Ravitch procedure and resection of the intrathoracic cystic mass was performed through an anterolateral thoracotomy incision in one stage operation satisfactorily (Figs. 7 and 8). On exploration, the mass, $15{\times}12{\times}10$cm in size, was connected to the bronchus at 1cm a bove the carina by a stalk (Fig. 4). The outer surface showed abundant vasculature. The specimen was filled with mucoid material; the inner surface was much trabeculated. glistening and smooth (Fig. 5 and 6). yficroscopically, the cyst was lined with simple or pseudostratified ciliated columnar epithelium. The cystic wall was composed of loose fibrous connective tissue, muscle layers, cartilages with some lymphocytic infiltration (Fig. 9). Isolated cases of funnel chest deformity and congenital bronchogenic cystic disease are not uncommon; however, the assocbtion of the two conditions is yery rare. Therefore. report and review of the literature was done.

Combined Intrathoracic and Intraperitoneal Splenosis after Splenic Injury: Case Report and Review of the Literature

  • Splenosis is defined as an autotransplantation of the splenic tissue after splenic rupture or splenectomy, and occurs most frequently in the peritoneal cavity. Splenosis is usually asymptomatic and is found incidentally. We report a case of combined intrathoracic and intraperitoneal splenosis in a 54-year-old male who worked as a miner for 10 years in his twenties, and was a current smoker. He was referred to our hospital for further evaluation of an incidental left diaphragmatic mass. Positron emission tomography-computed tomography and bronchoscopy were performed to evaluate the possibility of malignancy. There was no evidence of malignancy, but the spleen was not visualized. Reviewing his medical history revealed previous splenectomy, following a dynamite explosion injury. Therefore, splenosis was suspected and technetium-99m-labeled heat-damaged red blood cell scan confirmed the diagnosis. Radionuclide imaging is a useful diagnostic tool for splenosis, which could avoid unnecessary invasive procedures.
