통증환자에서 턱관절 경락음양 균형치료의 경추정렬 개선효과 (Temporomandibular joint yinyang balance treatment improves cervical spine alignment in pain patients, a medical imaging study)

  • Objectives : Temporomandibular joint (TMJ) balance is known to be intricately integrated with nervous system, cervical spine, and meridian system balance. This retrospective study with one-group pretest-posttest design reviewed cervical spine imaging data to provide evidence of spinal alignment improving effect of TMJ balance treatment. Methods : Cervical spine imaging data including computed tomography and simple x-ray of 25 cases with painful condition were reviewed to explore any change in cervical alignment on wearing the intraoral device for TMJ balance treatment of functional cerebrospinal therapy. Results : Cervical spine alignment significantly improved on wearing the intraoral device. Conclusions : TMJ balance treatment improves cervical spinal alignment, which may be a firm basis to proceed with further research of TMJ balance therapy as a way of balancing the whole-body meridian system.

치과 구내 X-선 촬영기의 사용연한에 따른 해상능에 관한 연구 (A STUDY OF THE RESOLUTION OF DENTAL INTRAORAL X-RAY MACHINES)

  • The purpose of this study was to assess the resolution and focal spot size of dental X-ray machines. Fifty dental X-ray machines were selected for measuring resolution and focal spot size. These machines were used in general dental clinics. The time of installation of the X-ray machine varies from 1 year to 10 years. The resolution of these machines was measured with the test pattern. The focal spot size of these machines was measured with the star test pattern. The following results were obtained: 1. The resolution of dental intraoral X-ray machines was not significantly changed in ten years. 2. The focal spot size of dental intraoral X-ray machines was not significantly increased in ten years. The statistical analysis between the mean focal spot size and nominal focal spot size was significant at the 0.05 level about the more than 3 years used machines.

  • Osteochondroma is one of the most common benign tumors that occur in the bone, but in the head and neck region, osteochondroma appears rarely. Malocclusion, temporomandiublar disorders and facial asymmetry can occur in most cases with osteochondroma of the mandibular condyle. Most surgeons prefer massive removal of osteochondroma and reconstruction of condyle, simultaneously, to prevent various complications, such as a lateral open bite on the contralateral side. We report a surgical treatment and reconstruction of osteochondroma on a mandibular condyle through intraoral approach.

  • 경추부 근긴장 이상증에 대해 턱관절 자세균형 개념에 따라 구강내 균형장치를 활용한 자세훈련법 등의 치료로 긍정적인 호전을 관찰했다. 증례 보고이기는 하지만 턱관절 자세균형을 활용한 치료법에서 긍정적인 치료효과가 이어지고 있어서 앞으로 더욱 체계적인 관찰과 연구가 필요할 것으로 사료된다.

  • A 22-year-old male patient had developed a submasseteric abscess secondary to a mandibular osteomyelitis at the age of 7 years old. The initial presentation at that time seems to be acute suppurative parotitis. The computed tomographic scans taken before surgery demonstrated diffuse deformity, sclerotic change and osteolytic lesion in the mandible. There was no marrow space on both sides of mandibular ramus and thin-walled cortical bone was seen. So, from the results of the computed tomography, the surgery was performed intraoral vertical ramus osteotomy (IVRO) instead of performing the more commonly used bilateral sagittal split ramus osteotomy (BSSRO). In this report, we present a case of surgical correction of mandibular prognathism with fibrous-osseous lesion of mandible with using IVRO.

  • Intraoral lipomas are a rare clinical entity, comprising only 0.1% to 5% of all benign tumors in the intraoral cavity. A 56-year-old woman suffering from diabetes presented with this relatively rare intraoral lipoma and was treated by surgical excision under general anesthesia. Because the mass was located adjacent to the mental foramen, a precise dissection was necessary to ensure minimal nerve damage. No abnormalities or recurrence was noted at 1-year follow-up and the patient did not complain of numbness. We studied the occurrence of oral lipoma in this diabetic patient and reviewed the relationship between oral lipoma and diabetes in the literature.

  • Cleft lip and palate(CLP) patients usually have midface deficiency and Class III malocclusion. Distraction osteogenesis (DO) has been used recently to correct the maxillary hypoplasia with stable and predictable result. Both external and internal devices that permit midface distraction are available, This case report describes intraoral DO for correction of the midface deficiency in a adult CLP patient with relapse following orthognathic surgery. The purpose of this report is to present advantages of the intraoral DO for the treatment of CLP, The relative and potential clinical indications, treatment planning, patient preparation, and possible vector control for DO are discussed.

Acoustic, Intraoral Air Pressure and EMG Studies of Vowel Devoicing in Korean

  • The devoicing vowel is a phonological process whose contrast in sonority is lost or reduces in a particular phonetic environment. Phonetically, the vocal fold vibration originates from the abduction/adduction of the glottis in relation to supraglottal articulatory movements. The purpose of this study is to investigate Korean vowel devoicing by means of experimental instruments. The interrelated laryngeal adjustments and aerodynamic effects for this voicing can clarify the redundant articulatory gestures relevant to the distinctive feature of sonority. Five test words were selected, being composed of the high vowel /i/, between the fricative and strong aspirated or lenis affricated consonants. The subjects uttered the test words successively at a normal or at a faster speed. The EMG, the sensing tube Gaeltec S7b and the High-Speech Analysis system and MSL II were used in these studies. Acoustically, three different types of speech waveforms and spectrograms were classified, based on the voicing variation. The intraoral air pressure curves showed differences, depending on the voicing variations. The activity patterns of the PCA and the CT for devoicing vowels appeared differently from those showing the partially devoicing vowels and the voicing vowels.

Determination and classification of intraoral phosphor storage plate artifacts and errors

  • Purpose: The aim of this study was to determine the reasons and solutions for intraoral phosphor storage plate (PSP) image artifacts and errors, and to develop an appropriate classification of the artifacts. Materials and Methods: This study involved the retrospective examination of 5,000 intraoral images that had been obtained using a phosphor plate system. Image artifacts were examined on the radiographs and classified according to possible causative factors. Results: Artifacts were observed in 1,822 of the 5,000 images. After examination of the images, the errors were divided into 6 groups based on their causes, as follows: images with operator errors, superposition of undesirable structures, ambient light errors, plate artifacts (physical deformations and contamination), scanner artifacts, and software artifacts. The groups were then re-examined and divided into 45 subheadings. Conclusion: Identification of image artifacts can help to improve the quality of the radiographic image and control the radiation dose. Knowledge of the basic physics and technology of PSP systems could aid to reduce the need for repeated radiography.

  • Optical-based imaging technology has high resolution and can assess images in real time. Numerous studies have been conducted for its application in the dental field. The current research introduces an oral camera that includes fluorescent imaging, a second study examining a 3D intraoral scanner applying a confocal method and a polarization structure that identifies the 3D image of a tooth, and finally, an optical coherence tomography technique. Using this technique, we introduce a new concept 3D oral scanner that simultaneously implements 3D structural imaging as well as images that diagnose the inside of teeth. With the development of light source technology and detector technology, various optical-based imaging technologies are expected to be applied in dentistry.
