A Case of Pilomatricoma with Extensive Ossification Arising the Posterior Neck (후경부에 발생한 광범위 골화를 동반한 모기질종 1예)

  • The pilomatricoma is a benign skin neoplasm arising from hair follicle matrix cells, and it comprises approximately 0.12% of all benign skin tumors. It occurs most commonly in head and neck. It usually tends to occur in childhood or school-age. Mainly, it exists in single subcutaneous or intradermal nodule and grows very slowly. The diagnosis is confirmed by histopathologic examination. Histopathologically, it is characterized by basaloid cells, enucleated shadow cells and calcifications in the shadow cell. However, it is rare to present extensive ossification in pilomatricoma. The treatment of choice is wide local excision. A 66-year-old man came to our clinic and he complained of two masses on right parotid tail and posterior neck. We performed excisional biopsy on posterior neck and partial parotidectomy. The pathology of former was pilomatricoma with extensive ossification. We report a rare unique case of pilomatricoma with brief review of literature.

Immunopreventive Effects against Murine H22 Hepatocellular Carcinoma in vivo by a DNA Vaccine Targeting a Gastrin-Releasing Peptide

  • There is a continuing need for innovative alternative therapies for liver cancer. DNA vaccines for hormone/growth factor immune deprivation represent a feasible and attractive approach for cancer treatment. We reported a preventive effect of a DNA vaccine based on six copies of the B cell epitope GRP18-27 with optimized adjuvants against H22 hepatocarcinoma. Vaccination with pCR3.1-VS-HSP65-TP-GRP6-M2 (vaccine) elicited much higher level of anti-GRP antibodies and proved efficacious in preventing growth of transplanted hepatocarcinoma cells. The tumor size and weight were significantly lower (p<0.05) in the vaccine subgroup than in the control pCR3.1-VS-TP-HSP65-TP-GRP6, pCR3.1-VS-TP-HSP65-TP-M2 or saline subgroups. In addition, significant reduction of tumor-induced angiogenesis associated with intradermal tumors of H22 cells was observed. These potent effects may open ways towards the development of new immunotherapeutic approaches in the treatment of liver cancer.

  • In vitro cell culture system has been proposed as a promising alternative model to in vivo skin irritation test. These studies were performed to screen the cytotoxicity effects of surfactants using normal human skin fibroblasts. Cell membrane integrity assessed by the leakage of lactate dehydrogenase (LDH) and mitochondrial integrity by MTT [3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyl tetrazolium bromides reduction test were affected in a dose dependent manner. The irritation potential of surfactants to human skin patch test, and the changes of capillary permeability by rabbit intradermal safety test were assessed as in vivo methods. Our results suggest that LDH leakage assay and MTT reduction test using cultured human fibroblasts could be predictive for the irritancy of various surfactants in human, and LDH assay is superior correlated with in vivo test (r=0.886) to MTT test with in vivotest (r=0.757).

  • The visceral dissemination of Borrelia burgdorferi in New Zealand White rabbits was evaluated following intradermal inoculation of $1{\times}10^8$ spirochetes. We inoculated Borrelia burgdorferi B31, B garinii KW1 and B afzehlii S13, respectively, and monitored the dissemination in the experimentally infected rabbits for 28 days. In the B burgdorferi B31-challenged group, the spirochetes were completely cleared in rabbits at day 1 and visceral dissemination was not demonstrated. However, B garinii KW1 and B afzelii S13 were found to successfully disseminate in visceral organs of rabbits during the experiment period of 28 days. And experimentally infection-derived immunological responses in rabbits were identified with enzyme-linked immunosorbent assay and immunoblot analysis. Based on these results, the differences in the virulence of Lyme borrelial strains were proved in rabbit model.

A Survey of paratuberculosis by immunological methods in dairy and Korean native cattle (면역학적인 방법에 의한 한우와 유우의 요네병 발생조사)

  • A immunological survey of paratuberculosis in dairy and Korean native cattles was conducted by enzyme linked immunosorbent assay(ELISA), complement fixation test(CFT), agar gel immunodiffusion test (AGID) and intradermal skin test(ID). Over all prevalence of pararuberculosis in cattles was 6.7%(109/1633) by ID, 7.5(205/2719) by AGID, 9.3% (245/2641) by CFT and 13.4%(363/2719) by ELISA. Prevalence in dairy cattle was higher than that of Korean native cattle. Of 70 ELISA-positive cattle, 23(28.6%) and 48(68.6%) cattles were classified as positive in the AGID and positive or suspect in CFT, respectively. Of 92 ELISA-suspect cattle, 32(34.9%) and 48(52.2%) cattles were classified as AGID-positive and CFT-positive or suspect, respectively. It was concluded that paratuberculosis is widespread in cattle of Korea.

  • Objectives: This study was to investigate the effect of high frequency electro-acupuncture (EA) at $ST_{36}$ acupoint on the Freund's Complete Adjuvant (FCA)-induced arthritis in rats. Methods : Arthritis was induced by intradermal injection of FCA into base of tail. Experimental groups were divided into 4 groups; Normal, Control, $ST_{36}$ and Non-Acupoint (NA). $ST_{36}$ group was treated by 120Hz EA at $ST_{36}$ acupoint. Body weight, paw edema volume and ankle joint thickness were measured after treatment. And we investigated the effects of 120Hz EA via WBC count, segmen neurophil, lymphocyte, PGE2 assay and NADPH-d histochemistry. Results and Conclusions : The mean of body weight and ankle joint thickness of $ST_{36}$ group was increased compared with control group. The mean of paw edema volume, WBC count, segment neutrophil and lymphocyte of were not decreased with control group.   • Loop-mediated isothermal amplification (LAMP) was developed to detect Mycobacterium tuberculosis complex (MTC) and non-tuberculous mycobacterium (NTM) genomic DNA in blood samples of Korea native cattle. A set of four primers, two outer and two inner, were designed from M. bovis and M. avium genomic DNA targeting the IS6110 and 16S rRNA gene, respectively. Based on 85 Intradermal Tuberculin Test (ITT) positive blood sample and using conventional PCR and LAMP, the agreement quotient (kappa), which measures agreement beyond chance were 0.93 (conventional PCR) and 0.97 (LAMP), respectively. The detection limit of the LAMP method was $2.0{\times}10^2$ copy/ml M. bovis and M. avium cells, compared to $2.0{\times}10^3$ copy/ml M. bovis and M. avium cells for conventional PCR. These results suggest that the LAMP is a powerful tool for rapid, sensitive, and practical detection of MTC and NTM in blood samples of Korea native cattle.

  • Anaphylactic reactions to anesthetic drugs could potentially produce life-threatening immune-mediated crisis. Most published reports are associated with neuromuscular blockers and anaphylactic reactions to inhalation anesthetics are rare. A 25-year-old male patient with no significant medical history and no previous abnormal drug reaction was scheduled for orthognathic surgery under general anesthesia. After uneventful anesthetic induction and nasotracheal intubation, generalized urticaria and erythema were detected during the maintenance period with $O_2-N_2O$-enflurane. No severe changes of vital signs and no ventilation problem were accompanied. The operation was cancelled and the cutaneous lesions were faded away during the recovery with 100% $O_2$. The skin-prick and intradermal tests showed that he was hypersensitive to all halogenated inhalation anesthetics including enflurane and not to intravenous anesthetics and neuromuscular blockers. The re-operation was safely carried out under intravenous anesthesia with propofol-fentanyl-vecuronium. We report this case of intraoperative anaphylactic reaction to enflurane with literature review.

  • Mahwang-Shin-Gung-San has been found inhibiting the mast cell-mediated anaphylactic reaction. This report describes an inhibitory effect of Mahwang-Shin-Gung-San (MSGS) on the immediate-type cutaneous allergic reactions. MSGS has concentration -dependently inhibited the ear swelling response induced by compound 48/80 in mouse by intradermal injection. The mast cells in mouse ear tissue undergone ear-swelling response by compound 48/80 were stained by alcian blue/nuclear fast red. MSGS significantly inhibited the compound 48/80-induced degranulation from mast cells in ear tissue. MSGS concentration-dependently inhibited the histamine release from the rat peritoneal mast cells (RPMC) by compound 48/80. We also studied the effect of MSGS on mast cell-dependent passive cutaneous anaphylaxis (PCA) activated by dinitrophenyl IgE antibody. MSGS showed potent inhibition of PCA by oral administration. These results indicate that MSGS inhibits immediate-type allergic reactions by inhibition of mast cell degranulation in vivo and in vitro.

An Easy Detection Method of Fasciola Eggs by Kim's Sedimental Tube (K-식(式) 침전관(沈澱管)에 의한 간질충란(肝蛭蟲卵) 간이검사법(簡易檢査法))

  • The studies carried out to investigate the detection method of Fasciola eggs from positive cattle feces in the intradermal reaction method with laborsaving composition of sieves and Kim's sedimental tube method. The results obtained are summarized as follows: 1. The detection method of Fasciola eggs was improved by 3 meshes (100mesh, 150mesh, 250mesh) for filteration of fecal fluid contained Fasciola eggs. 2. Third type of tube in the 3 kinds of sedimental tube had the highest recovers rate of Fasciola eggs. Thus, this type of tube was selected and called as Kim's sedimental tube. 3. A-II, A-III, B-III and C-III type by Kim's sedimental tube method had the recovery rates of eggs in the 1st drop as ranged 99 to 100% and, A-I, B-II, C-II as 71 to 91%. 4. These Type of tubes were useful to detect Fasciola eggs and to shorten the time of the diagnostic procedures. 5. The Kim's sedimental tube method showed high recovery rates and convenient procedures as compared with other detection methods reported. In addition, the EPG value can also be obtained by this method. Therefore, it is desirable that the Kim's sedimental tube method could be recommended to clinicians.
