Diagnostic Value of Susceptibility-Weighted MRI in Differentiating Cerebellopontine Angle Schwannoma from Meningioma

  • Background: Differentiation of cerebellopontine angle (CPA) schwannoma from meningioma is often a difficult process to identify. Purpose: To identify imaging features for distinguishing CPA schwannoma from meningioma and to investigate the usefulness of susceptibility-weighted imaging (SWI) in differentiating them. Materials and Methods: Between March 2010 and January 2015, this study pathologically confirmed 11 meningiomas and 20 schwannomas involving CPA with preoperative SWI were retrospectively reviewed. Generally, the following MRI features were evaluated: 1) maximal diameter on axial image, 2) angle between tumor border and adjacent petrous bone, 3) presence of intratumoral dark signal intensity on SWI, 4) tumor consistency, 5) blood-fluid level, 6) involvement of internal auditory canal (IAC), 7) dural tail, and 8) involvement of adjacent intracranial space. On CT, 1) presence of dilatation of IAC, 2) intratumoral calcification, and 3) adjacent hyperostosis were evaluated. All features were compared using Chi-squared tests and Fisher's exact tests. The univariate and multivariate logistic regression analysis were performed to identify imaging features that differentiate both tumors. Results: The results noted that schwannomas more frequently demonstrated dark spots on SWI (P = 0.025), cystic consistency (P = 0.034), and globular angle (P = 0.008); schwannomas showed more dilatation of internal auditory meatus and lack of calcification (P = 0.008 and P = 0.02, respectively). However, it was shown that dural tail was more common in meningiomas (P < 0.007). In general, dark spots on SWI and dural tail remained significant in multivariate analysis (P = 0.037 and P = 0.012, respectively). In this case, the combination of two features showed a sensitivity and specificity of 80% and 100% respectively, with an area under the receiver operating characteristic curve of 0.9. Conclusion: In conclusion, dark spots on SWI were found to be helpful in differentiating CPA schwannoma from meningioma. It is noted that combining dural tail with dark spots on SWI yielded strong diagnostic value in differentiating both tumors.

뇌기저부 수막종의 임상분석 및 수술성적 (Clinical Analysis and Surgical Results of Skull Base Meningiomas)

  • 저자들은 10년동안 수술을 시행받았던 212례의 수막종 환자중 뇌기저부에 위치한 61례에 대한 임상 분석을 시행한 결과 다음과 같은 결과를 얻었다. 1) 본 연구기간중 뇌기저부 수막종 환자는 61례로 전체의 29%를 차지하였으며, 평균 추적기간은 약 52개월이었다. 2) 성비는 여성이 남성보다 약 2배 많았으며, 평균연령은 52세였다. 3) 위치별로는 후두개와가 가장 많았으며, 전체적으로는 접형골연부, 천막부 및 소뇌교각부가 대부분을 차지하였다. 4) 호발증상 및 징후로는 두통, 뇌신경마비 및 소뇌징후 순으로 나타났다. 5) 종양제거정도는 심슨등급 I, II로 전적출한 경우가 82%이었으며, 심슨등급 III로 아전적출한 경우가 18%이었다. 6) 병리조직결과는 양성이 85%로 대부분을 차지하였으며, 비정형성과 악성은 각각 10%, 5%를 차지하였다. 7) 술후 보조적 치료는 악성, 부분적출 및 재발한 경우에 사용하였다. 8) 술후 합병증으로는 뇌척수액누출, 뇌신경마비 및 간질 발작 순이었다. 9) 술후 사망한 경우는 수술후 사망한 1례와 종양 재발에 의한 사망 2례이었다. 10) 재발은 약 15%로 심슨등급 III와 악성인 경우에 높았으며, 재발 위치는 천막부, 접형골연 및 소뇌교각부 순이었다. 결론적으로 뇌기저부 수막종의 수술은 종양 주변부의 중요한 구조물이 위치함에 따라 낮은 사망률 및 합병증 발생률을 가지고 수술적 적출이 어렵지만, 술전 방사선학적 소견의 정확한 이해와 적절한 접근법의 선택, 뇌기저부 재건술이 술후 합병증의 감소 및 종양적출을 위해 필수라고 생각한다.

Cystic Hemangiopericytoma in the Third Ventricle

  • Primary intracranial hemangiopericytoma is rare and resemble meningioma on imaging study. It shows meningeal attachment, and is usually isointense with gray matter on T1-weighted MR image with heterogeneous enhancement and prominent vascular flow voids on T2-weighted image. Cystic type of hemangiopericytoma is very rare and only 3 cases have been reported in the literature which arised in the middle fossa, cerebellum, and occipital area. Ventricular hemangiopericytomas were reported in 9 cases, and all of them were solid type. Authors experienced a peculiar case of cystic hemangiopericytoma in the 3rd ventricle and report it with review of the literature.

  • A 49-year-old female with a history of several neurosurgical and otolaryngologic procedures for occipital meningioma and cerebrospinal fluid leaks was diagnosed with pneumocephalus after a one hour flight on a domestic jet airliner. Despite multiple operations, the air appeared to enter the cranium through a weak portion of the skull base due to the low atmospheric pressure in the cabin. The intracranial air was absorbed with conservative management. The patient was recommended not to fly before a definite diagnostic work up and a sealing procedure for the cerebrospinal fluid leak site had been performed. Recent advances in aviation technology have enabled many people to travel by air, including individuals with medical conditions. Low cabin pressure is not dangerous to healthy individuals; however, practicing consultant neurosurgeons should understand the cabin environment and prepare high risk patients for safe air travel.

  • Rosai-Dorfman Disease[RDD] is an idiopathic histiocytic proliferation affecting lymph nodes. Although extranodal involvement has been reported in the skin, orbit, upper respiratory tract, of testes, the isolated intracranical involvement without associated lymphadenopathy is extremely rare. We report our experience with 1 case of an isolated intracranial RDD without associated lymphadenopathy and any other organ involvement. A 61-year-old male presented with an isolated well-circumscribed brain mass in the posterior fossa, preoperatively thought to be a meningioma. But histology and Immunohistochemistry confirmed that the lesion was RDD.

Postoperative Brain Swelling after Resection of Olfactory Groove Meningiomas

  • Objective : Postoperative brain swelling after resection of olfactory groove meningiomas by bifrontal interhemispheric transbasal approach is a knotty subject. Pathogenesis and predictive factors were investigated to prevent the problem. Methods : Eighteen patients of olfactory groove meningiomas who had undergone surgery were enrolled and retrospectively analyzed using their clinical and radiological data. Bifrontal inter hemispheric transbasal approach was used in all patients. Magnetic resonance imaging and transfemoral cerebral angiography were available for investigation in 18 and 14 patients respectively. Postoperative clinical course, tumor volume, peritumoral edema, tumor supplying vessels, and venous drainage patterns were carefully investigated in relation to postoperative brain swelling. Results : Seven patients [39%] developed clinically overt brain swelling after surgery. Among them, 4 patients had to undergo decompression surgery. In three patients, attempted bone flap removal was done by way of prevention of increased intracranial pressure resulted from intractable brain swelling and two of them eventually developed brain swelling which could be recovered without sequellae. Abnormal frontal base venous channel observed in preoperative angiography was significant predictive factor for postoperative brain swelling [p=0.031]. However, tumor volume, peritumoral edema, and existence of pial tumor supplying vessels from anterior cerebral arteries were failed to show statistical significances. Conclusion : To prevent postoperative brain swelling in olfactory groove meningioma surgery, unilateral approach to preserve frontal base venous channels or temporal bone flap removal is recommended when it is indicated.

Expression of the CXCL12/SDF-1 Chemokine Receptor CXCR7 in Human Brain Tumours

  • Purpose: Receptor 7 (CXCR7) has recently been characterized as a novel receptor for CXCL12/SDF-1 (stromal cell derived factor-1). Given the demonstrated importance of CXCL12/SDF-1 in angiogenesis and tumour metastasis, we hypothesized that CXCR7 may also play a role in tumour pathogenesis. Located in the limited space of the intracranial cavity, any brain tumours can be inherently serious and life-threatening. However, the expression of CXCR7 in pituitary adenoma, neurilemmoma or hemangioblastoma remains to be elucidated. Therefore, we aimed to determine the potential contribution of CXCR7 in the development of brain tumours. Methods: In this study we examined and quantified the mRNA expression of CXCR7 in four different human brain tumours - 27 patients with neurilemmoma (8 patients), pituitary adenoma (7 patients), hemangioblastoma (6 patients), or meningioma (6 patients) undergoing surgical resection in the West China Hospital of Sichuan University. There were 15 females and 12 males aged from 28 to 70 years old. Total RNA was isolated and mRNA was measured by quantitative real-time RT-PCR. One-way analysis of variance (ANOVA) was performed using SPSS 11.0 statistical software to compare the mRNA levels of CXCR7 among four groups. Results: We found that CXCR7 mRNA was detected in all tumour samples. Quantitative results showed that the levels of CXCR7 mRNA in brain tissues from patients with neurilemmoma or meningioma were significantly higher than those with pituitary adenoma or hemangioblastoma. Conclusions: The results suggest that the CXCR7 may play a role in progression, metastasis and angiogenesis of brain tumours.

Evaluation of P57, P53 and Ki67 Expression in Meningiomas

  • Objective : We conducted this study with the aim of predicting the biological behavior of meningiomas, and determining the benefits of associating histological subtype and grade with the expression of proliferative markers and tumor suppressor proteins. Methods : The study included 29 patients with primary intracranial and intraspinal meningioma diagnosed in the pathology laboratory of Konya City Hospital between January 2014 and December 2020. Clinicopathological characteristics of the patients including parameters such as age and gender were obtained from the hospital records. Histopathological findings were obtained by re-evaluating the preparations stained with Hematoxylin-Eosin, which were extracted from the archive, and by evaluating new sections obtained from paraffin blocks of patients stained with Ki67, p53, and p57 immunohistochemical stains. Results : A moderate correlation was found between tumor size and Ki67 proliferation index (PI) (p=0.003, r=0.530). There was no significant difference between grade I and grade II tumors in terms of p53 (p=0.184) and p57 (p=0.487) expressions. There were higher levels of Ki67 PI in grade II tumors. The histological subtypes of the tumor had no significant difference with Ki67 PI (p=0.018), p53 (p=0.662), and p57 (p=0.368) expressions. Conclusion : In order to obtain more definitive results, there is a need for studies, which are conducted with a greater number of patients and in multiple centers, and in which a long prospective follow-up is planned. The combination of histological, surgical, and imaging markers could make a more sensitive tool for predicting recurrence, and this could also be tested in future studies.

Surgical Experience of Infratentorial Meningiomas : Clinical Series at a Single Institution during the 20-Year Period

  • Objective : Based on surgical outcomes of patients with infratentorial meningiomas surgically treated at our institution, we analyzed the predictors for surgical resection, recurrence, complication, and survival. Methods : Of surgically treated 782 patients with intracranial meningioma, 158 (20.2%) consecutive cases of infratentorial location operated on between April 1993 and May 2013 at out institute were reviewed retrospectively. The patients had a median age of 57.1 years (range, 16-77 years), a female predominance of 79.7%, and a mean follow-up duration of 48.4 months (range, 0.8-242.2 months). Results : Gross total resection (Simpson's grade I & II) was achieved in 81.6% (129/158) of patients. Non-skull base location was an independent factor for complete resection. The recurrence rate was 13.3% (21/158) and the 5-, 10-, and 15-year recurrence rates were 8.2%, 12.0%, and 13.3%, respectively. Benign pathology, postoperative KPS over than 90, low peritumoral edema, and complete resection were significantly associated with longer recurrence-free survival rate. The 5-, 10-, and 15-year survival rates were 96.2%, 94.9%, and 94.9%, respectively. Benign pathology, postoperative KPS over than 90 and complete resection were significantly associated with a longer survival rate. The permanent complication rate was 13% (21/158). Skull base location and postoperative KPS less than 90 were independent factors for the occurrence of permanent complication. Conclusion : Our experience shows that infratentorial meningiomas represent a continuing challenge for contemporary neurosurgeons. Various factors are related with resection degree, complications, recurrence and survival.

뇌수막종에서 선형가속기를 이용한 방사선수술 효과 (Treatment of Intracranial Meningioma with Linac Based Radiosurgery)

  • 목적 : 뇌수막종 환자들을 대상으로 선형가속기를 이용한 방사선수술을 시행한 결과에 대하여 환자의 임상적 증상과 방사선학적 추적검사를 통해 그 효과를 알아보고자 하였다. 대상 및 방법 : 1988년 7월부터 1998년 8월까지 20명의 뇌수막종 환자들을 대상으로 6 MV 선형가속기를 이용하여 방사선수술을 시행하였다. 20명의 환자들 중 4명$(20\%)$은 남성, 16명$(80\%)$은 여성이었으며, 평균연령은 51세($22\~78$세)였다. 종양의 뇌내 위치는 측시상동 부위(parasagittal area)와 접형골 부위(sphenoid wing)가 8례$(40\%)$로 가장 많았다. 방사선 수술 전에 시행한 신경외과적 수술의 횟수를 보면 1회 시행한 경우가 11례, 2회가 2례, 3회 시행한 경우가 1례였으며, 수술을 시행하지 않고 방사선수술만을 시행한 경우는 6례였다. 종양의 평균 부피는 $5.72\;cm^3\;(0.78\~15.1\;cm^3)$였으며, 2차 시준기(secondary collimator)의 지름의 중간 값은 2 cm $(1\~3\;cm)$이었다. 종양변연부의 평균조사량은 19.6 Gy $(9\~30\;Gy)$였다. 추적관찰기간은 $2.5\~109$개월로 중앙값은 53개월이었다. 결과 : 방사선학적 검사와 신경학적 검사상 모두 반응률은 $95\%$였다. 방사선추적검사상 종양의 부피가 줄어든 경우는 5례$(25\%)$, 종양의 부피에 변화가 없는 경우는 14례$(70\%)$, 그리고 종양의 부피가 증가된 경우가 1례$(70\%)$였다. 종양의 부피가 증가된 1례를 제외한 19명의 환자중, 4명의 환자에서 추적 영상 검사상 중심부괴사의 소견이 보였다. 임상적 증상의 호전여부에 따른 신경학적인 검사상 증상의 호전이 있었던 경우는 9명$(45\%)$, 증상의 변화가 없었던 경우는 10명$(50\%)$였다. 증상이 악화되었던 1명$(5\%)$의 환자는 수술을 시행하여, 방사선 괴사의 소견을 보였다. 결론 : 뇌수막종 환자들에서 선형가속기를 이용한 방사선수술은 부작용 없이 시행할 수 있는 효과적인 치료법으로 생각된다.
