실활치 표백술 후 치관내 수복물의 미세누출에 관한 비교 (THE COMPARISON OF MICROLEAKAGE ON INTRACORONAL RESTORATION AFTER NON-VITAL BLEACHING)

  • The purpose of the present study were to evaluate microleakage of a fourth generation dentin-bonding agent following a walking bleaching treatment, to determine the effect of temporary postbleaching dressing with calcium hydroxide on microleakage and to investigate the effect of delayed intracoronal restoration on microleakage. The results of this study were as follows : 1. Bleached groups showed more microleakage than unbleached group 2. Immediately restored group following bleaching procedure showed the highest microleakage score. 3. One-week delayed restorations showed less microleakage but there were no statistically significant difference between group II and III. 4. Provisional dressing with calcium hydroxide had no influence on microleakage. It is necessary to know the time that has elapsed from the bleaching treatment to the restoration procedure to achieve optimal seal, as well as to reduce the risk of microleakage in adhesive restoration.

  • The purpose of the present study were to evaluate microleakage of a fourth generation dentin-bonding agent following a walking bleaching treatment, to determine the effect of temporary postbleaching dressing with calcium hydroxide on microleakage and to investigate the effect of delayed intracoronal restoration on micro leakage. Forty extracted non-carious incisors were selected and conventional root canal treatment was performed. The teeth were randomly divided into 4 groups of 10 specimens each.(omitted)

하악 제1대구치 협측 소와의 맹출 전 방사선 투과상과 수복의 연관성 (Relationship between Pre-Eruptive Buccal Pit Radiolucency and Restoration in Mandibular First Molar)

  • Pre-eruptive intracoronal resorption (PEIR)은 맹출 전 치아의 치관 내에서 나타나는 발달 상의 결함을 의미한다. 본 연구의 목적은 파노라마 방사선 사진의 미맹출 하악 제1대구치 협측 소와에서 PEIR로 의심되는 방사선 투과상과 맹출 중인 하악 제1대구치에서 협측소와의 수복 필요성과의 관계를 조사하는 것이다. 실험군은 맹출 중일 때 협측 소와에 한정된 수복이 필요한 병소가 관찰되었던 35개의 하악 제1대구치를 대상으로 하였고, 대조군은 맹출 후 건전했던 하악 제1대구치 64개를 포함하였다. 실험군은 대조군에 비하여 맹출 전 파노라마 방사선 사진에서 측정된 협측 소와의 크기가 통계적으로 유의하게 컸다. 실험군 내에서 성별, 나이, 치아형성단계, 수복 방법에 따른 협측 소와 크기의 유의한 차이는 없었다. Receiver operating characteristic 커브를 그렸을 때, 커브 아래 면적이 $0.813{\pm}0.047$으로 방사선 사진 상의 협측 소와 크기를 근거로 수복 필요성을 예측했을 때 중등도의 정확성을 보였다. 파노라마 방사선 사진에서 맹출 전 하악 제1대구치의 협측 소와 부위에서 방사선 투과상이 관찰되는 경우 임상적으로 PEIR을 의심하고 주기적인 관찰과 맹출 후 검사가 필요할 수 있다.

  • 맹출 전 치관 내 흡수(pre-eruptive intracoronal resorption; PEIR)는 미맹출치 치관의 법랑상아경계하방에 위치한 방사선 투과상의 흔치 않은 병소이다. 맹출 전 치아에서의 PEIR은, 흡수 진행속도에 따라 외과적 노출술 후 수복 혹은 지속적 관찰로 치료가능하다. 하지만 맹출 후에는 우식 병소 발달의 위험이 높아 임상가의 대처가 늦을 경우 근관치료 등을 포함하는 심화된 수복치료를 진행하여야 한다. 본 증례는 어린이에서 PEIR로 인해 감염된 미성숙 영구치의 임상적 특징 및 치료에 관하여 보고하고자 한다.

  • Invasive cervical resorption is a relatively uncommon form of external root resorption. It is characterized by invasion of cervical region of the root by fibrovascular tissue derived from the periodontal ligament. This case presents an invasive cervical resorption occurring in maxillary lateral incisor, following damage in cervical cementum from avulsion and intracoronal bleaching procedure. Flap reflection, debridement and restoration with glass ionomer cement were performed in an attempt to repair the defect. But after 2 mon, more resorption extended apically. Considering root stability and recurrence potential, we decided to extract the tooth. Invasive cervical resorption in advanced stages may present great challenges for clinicians. Therefore, prevention and early detection must be stressed when dealing with patients presenting history of potential predisposing factors.

상악전치부에서 치아정출술을 이용한 치관연장의 증례보고 (CASE REPORT ON FORCED ERUPTION FOR CLINICAL CROWN LENGTHENING IN MAXILLARY ANTERIORS)

  • This case report presents two maxillary anterior cases for clinical crown lengthening by forced eruption. In the first case, clinical crown of maxillary right lateral incisor was almost lost by fracture. Forced eruption using intracoronal splint and elastic thread accomplished vertical root movement successfully. Then, post & core was inserted and final restoration was harmonious with adjacent teeth. In the second case, the crown portion of maxillary right central incisor was almost mutilated by secondary caries. Forced eruption using removable Hawley appliance and elastic accomplished vertical root movement successfully. Then, post & core was inserted and final restoration was placed. In conclusion, clinical crown lengthening by vertical root movement can be accomplished by a simple appliance without any sacrifice of periodontal support in selected patients. A clinical crown so created can be restored to adequate function and arch integrity without compromising adjacent teeth. Therefore, forced eruption is preferred in the anterior region of the dentition where esthetics is of major concern.

Effective application duration of sodium ascorbate antioxidant in reducing microleakage of bonded composite restoration in intracoronally-bleached teeth

  • Objectives: The aim of this study was to determine an appropriate application duration of sodium ascorbate (SA) antioxidant gel in reducing microleakage of bonded composite restoration in intracoronally-bleached teeth. Materials and Methods: Eighty endodontically-treated human incisors were randomly divided into eight groups: control, no bleaching; IB and DB, immediate and delayed bonding after bleaching, respectively; S10m, S60m, S24h, S3d and S7d, bleaching + SA gel for 10 min, 60 min, 24 hr, 3 day and 7 day, respectively. For bleaching, a mixture of 30% hydrogen peroxide and sodium perborate was applied for 7 day. All access cavities were restored using One-Step adhesive (Bisco Inc.) and then Aelite LS Packable composite (Bisco Inc.). The bonded specimens were subjected to 500 thermal cycles, immersed in 1% methylene blue for 8 hr, and longitudinally sectioned. Microleakage was assessed with a 0 - 4 scoring system and analyzed using nonparametric statistical methods (${\alpha}$ = 0.05). Results: Group IB showed a significantly higher microleakage than the control group (p = 0.006) and group DB a statistically similar score to the control group (p > 0.999). Although groups S10m, S60m, and S24h exhibited significantly higher scores than group DB (p < 0.05), the microleakage in groups S3d and S7d was statistically similar to that in group DB (p = 0.771, p > 0.999). Conclusions: Application of SA gel for 3 day after nonvital bleaching was effective in reducing microleakage of composite restoration in intracoronally-bleached teeth.

Fracture resistance of endodontically treated maxillary premolars restored by silorane-based composite with or without fiber or nano-ionomer

  • PURPOSE. This in vitro study investigated the fracture resistance of endodontically treated premolars restored using silorane-or methacrylate-based composite along with or without fiber or nano-ionomer base. MATERIALS AND METHODS. Ninety-six intact maxillary premolars were randomly divided into eight groups (n = 12). G1 (negative control) was the intact teeth. In Groups 2-8, root canal treatment with mesio-occlusodistal preparation was performed. G2 (positive control) was kept unrestored. The other groups were restored using composite resin as follows: G3, methacrylate-based composite (Z250); G4, methacrylate composite (Z250) with polyethylene fiber; G5 and G6, silorane-based composite (Filtek P90) without and with the fiber, respectively; G7 and G8, methacrylate-and silorane-based composite with nano-ionomer base, respectively. After aging period and thermocycling for 1000 cycles, fracture strength was tested and fracture patterns were inspected. The results were analyzed using ANOVA and Tukey HSD tests (${\alpha}$=0.05). RESULTS. Mean fracture resistance for the eight groups (in Newton) were G1: $1200{\pm}169^a$, G2: $360{\pm}93^b$, G3: $632{\pm}196^c$, G4: $692{\pm}195^c$, G5: $917{\pm}159^d$, G6: $1013{\pm}125^{ad}$, G7: $959{\pm}148^d$, G8: $947{\pm}105^d$ (different superscript letters revealed significant difference among groups). Most of the fractures in all the groups were restorable, except Group 3. CONCLUSION. Silorane-based composite revealed significantly higher strength of the restored premolars compared to that of methacrylate one. Fiber insertion demonstrated no additional effect on the strength of both composite restorations; however, it increased the prevalence of restorable fracture of methacrylate-based composite restored teeth. Using nano-ionomer base under methacrylate-based composite had a positive effect on fracture resistance and pattern. Only fiber-reinforced silorane composite restoration resulted in a strength similar to that of the intact teeth.
