Experience of spinal stenosis, herniation of intervertebral disk patients with low back pain under non-surgical treatment (비수술적 요법을 받는 척추관협착증, 추간판탈출증 환자의 요통 경험)

  • The purpose of the study was to explore the process of experience of spinal stenosis, herniation of intervertebral disk patients with low back pain under non-surgical treatment. 10 participants attended in-depth individual interview. Data analyzed using the Grounded theory methodology of Corbin and Strauss(2015). A core category emerged as 'Compromise with my body in the swamp of pain'. Central phenomenon was 'Constrained life by severe pain'. The process of the low back pain experience included three phases: 'cognition', 'coping', and 'adaptation'. This finding will be helpful for understanding the low back pain experience and be as fundamental data as for developing and applying nursing intervention program according to the low back pain experience process.

Treatment of Intervertebral Disc Disease in Dogs: 56 Cases (1999~2001) (개에서 추간판 질환의 치료 56례(1999-2001))

  • Fifty six dogs diagnosed with intervertebral disk disease between January 1999 and August 2001 were reviewed to characterize signalments and prognosis. Seventy-five percent was chondrodystrophoid breeds (42 dogs). Mean age was 5.1$\pm$3.0 year-old and fifty-four percent had acute onset. Interspaces between eleventh thoracic vertebrae and fourth lumbar vertebrae were affected most commonly. Success rate of conservative treatment to dogs with paresis was 83% and that of operation to dogs with paralysis was 67%. Six dogs were recurred and 50% of them had recovered again. There was significant correlation between success rate of treatment and the degree of clinical signs but there was no significant correlation between success rate of treatment and rate of onset, status of clinical signs, duration of clinical signs.

  • Objective: This study examined a case in which insomnia and pain due to herniated intervertebral disks were improved by treatment with an oriental herbal and the administration of Uhwangchungsim-won. Case Summary: Three patients with insomnia diagnosed with herniated intervertebral disks were treated with a series of Korean medical therapies, including the herbal medication Uhwangchungsim-won, acupuncture, herbal acupuncture, and physical therapy. We used the Numerical Rating Scale (NRS) to measure subjective sleep states and the European Quality of Life Five Dimensions (EQ-5d) Scale to evaluate the therapeutic effect. Following treatment, subjective sleep states and the overall scores for pain had improved. Conclusion: The results indicate that Korean medical therapies with Uhwangchungsim-won have beneficial effects on insomnia for patients with herniated intervertebral disks.

  • Since the first report of an operation for prolapsed intervertebral disk by Mixter and Barr[1934], many thousands of operations have been successfully performed without incident. Linton and White in 1945 reported the vascular complication, but perforation of large vessels is rare complication of operation for prolapsed disk. A medical student, aged 22 years, was performed to a disk operation [L4-5, Rt. on May 1977.] From postoperative 10th day, palpitation, generalized edema and substernal pain were noted, and 2 months later, wide pulse pressure [70-80 mmHg], continuous bruit and thrill on the Rt. low abdomen were followed. Aortography revealed arteriovenous fistula between just proximal to abdominal aortic bifurcation and inferior vena cava. So, fistulectomy [Resection of proximal 2 cm of C.I.A., Rt, including fistula opening and end to end anastomosis] was performed on July, 77. During follow up study, remained fistula between Rt. internal lilac artery and lilac vein was found 2 months later. Re-operation [Double ligation of the Rt. internal lilac artery] was don on January 1978. Postop. results were excellent, except impossible to ejaculation.

  • A psoas compartment block has been used to provide anesthesia for orthopedic surgical procedures and analgesia for post-operative pain. Currently, this block is advocated for relieving pain in the lower extremity and pelvic area resulting from various origins. We report a case of a 69-year-old male patient who had gait abnormality with posterior pelvic and hip pain, which were both aggravated by hip extension. From the magnetic resonance image, the patient was found to have a laterally herniated intervertebral disc at the L2/3 level, which compressed the right psoas muscle. This was thought to be the origin of the pain, so a psoas compartment block was performed using 0.25% chirocaine with triamcinolone 5mg, and the pain in both the pelvis and hip were relieved.

The Effect of the Acupuncture Therapy in Combination with Soyeom Pharmacopuncture Therapy on the Improvement of the Symptoms of the Patients with Herniated Intervertebral Disk of L-spine in His Initial Stage of Hospitalization (소염약침 병행치료가 요추추간판탈출증 환자의 입원초기 증상 호전에 미치는 영향)

  • Objectives: Herniated Intervertebral Disk(HIVD) of L-spine is one of the most common diseases that cause back pain and radiating pain. The aim of this study is to find out if the acupuncture therapy combined with Soyeom Pharmacopuncture therapy can improve the symptoms of the patients suffering from the disease more effectively than the acupuncture only therapy. Methods: We separated 30 patients into two groups. Group A was treated with the acupuncture only therapy, and Group B was treated with Soyeom Pharmacopuncture therapy in addition to the acupuncture therapy. We have observed patients for 7 days. We checked the Visual Analog Scale(VAS) for 4 times and did Straight Leg Raising Test(SLRT) twice. Results: 1. For 7 days from the admission day, the VAS score has been decreased significantly in both Group A and Group B. 2. For 7 days from the admission day, the VAS score of Group B has been appeared signifi cantly lower than that of Group A, so that we can know that patients in Group B has more rapidly improved than patients in Group A 3. From the admission day to the 3rd hospital day, the improvement index of VAS scroe of Group B has appeared significantly higher than that of Group A, but after 5th hospital day, there was no significance between the improvement index of VAS score of Group A and that of Group B. 4. For 7 days from the admission day, the SLRT(degree) has been improved significantly in both Group A and B, but there was no significance between the SLRT(degree) of Group A and that of Group B. Conclusion: We can use Soyeom Pharmacopuncture therapy with acupuncture therapy to make the patients suffering from back pain and radiating pain improve more rapidly.

Comparison Study on CNR and SNR of Thoracic Spine Lateral Radiography (흉추 측면검사 영상의 CNR과 SNR 측정의 비교 연구)

  • This study was proven for the T-spine breathing technique in lateral projection, using computer radiography (CR), charge coupled device (CCD), indirect digital radiography (IDR) and direct digital radiography (DDR). All images were evaluated and compared with CNR and SNR measured with the mean pixels and the standard deviation as setting ROI of spinous process, pedicle, vertebral body, intervertebral foramen and intervertebral disk using Image J. In experiment results of 4 type detectors, T-spine breathing technique was indicated as excellent in ROI of spinous process, pedicle, vertebral body, intervertebral foramen and intervertebral disk. As T-spine breathing technique indicated excellent images compared to the existing T-spine lateral radiography, this method would be useful for elderly patients who have difficulty in deep exhalation. This study was indicated the application possibility of T-spine breathing technique by presenting contrast to noise ratio (CNR) and signal to noise ratio (SNR) with quantitative value in 4 type detectors.

Evaluation of quantitative on T-spine exhalation technique and T-spine breathing technique of natural breathing (T-spine exhalation technique과 자연스런 호흡 상태에서 촬영하는 T-spine breathing technique의 정량적 평가)

  • Measurements of CNR(Contrast to Noise Ratio) and SNR(Signal to Noise Ratio) of T-spine breathing technique (TBT) using spontaneous breathing and T-spine exhalation technique (TET) with full exhalation were carried out, and with which the more appropriate method was suggested. Both TBT and TET were examined in a sample of fifty-three patients who visit to our hospital for spinal disease from June 2012 to November 2012. All images were evaluated with CNR measured from the differences between the mean pixels and contrast density as setting ROI of spinous process, pedicle, vertebral body, intervertebral foramen, and intervertebral disk using Image J. SNR was measured with the mean pixels and the standard deviation as setting ROI of vertebral body using Image J. In CNR comparison and SNR comparison of TET and TBT, TBT was indicated as excellent in ROI of pedicle, vertebral body, intervertebral foramen and intervertebral disk, and statistical analysis were significant(p<.01). As TBT indicated excellent images compared to the existing T-spine lateral radiography, T-spine lateral radiography would be reestablished and significant as applying to various medical institutions.
