Effectiveness of Korean Medicine Treatment on Lumbar Intervertebral Disc Herniation Patients' Fear-Avoidance Belief (요추 추간판 탈출증 환자에 적용된 한의학적 치료가 공포-회피 반응에 미치는 효과)

  • Objectives This study is planned to classify effectiveness of Korean medicine treatment on lumbar intervertebral Disc herniation patients' fear-avoidance belief. Methods We measured 84 patients with lumbar intervertebral disc herniation in Jaseng Korean Medicine Hospital. We used numeric rating scale, oswestry disability index, fear-avoidance belief questionaire for evaluating effectiveness of oriental medicine treatment on lumbar intervertebral disc herniation. And We used Magentic Resonance Imaging(MRI) for classifying the patient who has lumbar intervertebral disc or not. Results 1. There was statistical effectiveness of korean medicine treatment on lumbar intervertebral disc herniation patients' fear-avoidance belief (p<0.01). ${\Delta}NRS$ and ${\Delta}ODI$ (r=0.587) was Most statistically correlative. ${\Delta}ODI$ and ${\Delta}FABQ$ had a little of statistically correlation (r=0.313). ${\Delta}NRS$ and ${\Delta}FABQ$ has no statistically correlation (r=0.145). Conclusions This study suggests that screening for the korean medicine treatment reduces fears and negative belief of patients with the lumbar disc herniation and prevents chronic physical symptoms that come from them so that it can be a great help to controlling patients' pain clinically.

Magnetic Resonance Imaging for Each Type of Herniated Lumbar Intervertebral Disc (요추 추간판 탈출증의 유형별 자기공명영상 소견)

  • The classification of herniated intervertebral lumbar disc type is clinically important, as treatment method may be slightly different according to the type of herniated intervertebral disc. When 450 patients who suffered from herniated intervertebral lumbar disc were tested with Magnetic Resonance Imaging (MRI) to find out the type of herniated disc, the following findings were given : 1. The age of the patients investigated ranged from 15 to 83, and their mean age was 40. 2. The male patients were twice as many as the female patients, since the ratio of males to females was 300 : 150. 3. 118 patients suffered from a single herniated disc, and 332 patients were attacked with multi-herniated disc. 4. The types of single herniated disc were a protrusion for 50 patients (40%) and an extrusion for 40 patients (34%). The part of being herniated was an intervertebral disc between $L_4-L_5$ for 95 patients(80%) and the same disc between $L_5-S_1$ for 22 patients(19%). 5. The types of multi : -herniated disc were an protrusion for 67 patients(20%) and an extrusion for 70 patients(21%). Besides, 90 patients (28%) were the case that protrusion and extrusion coexisted simultaneously. The parts of being herniated were $L_3-L_4,\;L_4-L_5$ and $L_5-S_1$ for 53 patients(16%), $L_3-L_4$ and $L_4-L_5$ for 57 patients(17%), and $L_4-L_5$ and $L_5-S_1$ for 139 patients(42%).

A Study for Clinical and Radiological Correlation between Lumbar Lordotic Angle, Lumbar Intervertebral Disc Angle and Lumbar Spinal Stenosis (요추 전만 각도 및 요추 추간판 각도와 요추 척추관 협착증의 상관관계에 관한 연구)

  • Objectives The purpose of this study is to find out clinical and radiological correlation between lumbar lordotic angle, lumbar intervertebral disc angle and lumbar spinal stenosis. Methods Total 250 patients' who had visited Bu-Chun Jaseng Hospital of Korean Medicine lumbar lordotic angle, intervertebral disc angle of L4/5 and dural sac dimension of L4/5 were measured by X-ray and MRI films. We analysed correlation between lumbar lordotic angle, intervertebral disc angle of L4/5 and lumbar spinal stenosis in terms of clinical and radiological aspect. Results 1. The mean intervertebral disc angle of L4/5 were $10.72{\pm}3.98^{\circ}$, the mean lumbar lordotic angle were $41.97{\pm}11.73^{\circ}$ and the mean dural sac dimension of L4/5 were $133.18{\pm}45.46mm^2$. 2. This study shows that dural sac dimension of L4/5 was inversely reated to intervertebral disc angle of L4/5 by statistically (p<0.05). 3. There was visible difference regarding intervertebral disc angle of L4/5 between patients who had been diagnosed with lumbar spinal stenosis by clinically and patients who had not been diagnosed with lumbar spinal stenosis by clinically; The former's angle was relatively higher than the latter's (p<0.05). Conclusions There was a statistical significance between intervertebral disc angle of L4/5 and lumbar spinal stenosis in single-segment.

Evaluating Paraspinal Back Muscles Using Computed Tomography (CT) and Magnetic Resonance Imaging (MRI): Reliability Analysis and Correlation with Intervertebral Disc Pathology

  • Purpose: To investigate the reliability of CT and MRI for quantitative and qualitative analyses of lumbar paraspinal muscle fatty infiltration (PSFI) and correlation of PSFI with intervertebral disc pathology. Materials and Methods: Lumbar spine CT and MRI of 36 subjects were reviewed retrospectively. Two observers independently outlined lumbar paraspinal muscles at each mid-intervertebral disc level. Paraspinal muscles on CT and MRI were graded according to the Goutallier grading system (GGS). The area, mean value, and standard deviation (SD) of the Hounsfield unit (HU) were obtained. Intervertebral discs were assessed on axial image of T2WI at each level. Correlations between qualitative and quantitative data and intervertebral disc pathology, age, and sex were evaluated. Results: Inter- and intra-observer agreements for results of GGS on MRI were substantial (κ = 0.79) and moderate (κ = 0.59), respectively. Inter- and intra-observer agreements for results of GGS on CT were almost perfect (κ = 0.88) and substantial (κ = 0.66), respectively. Quantitative measurements of HU showed almost perfect inter- and intra-observer reliabilities (κ = 0.82 and κ = 0.99, respectively). There were statistically significant correlations between intervertebral disc pathology and PSFI at L1-2, L2-3, and L4-5 levels on MRI and at L1-2 and L3-4 levels on CT. Age showed significant correlation with results of GGS at all levels on CT and MRI. Conclusion: This study showed that GGS results and HU measurements could be useful for evaluating PSFI because they showed correlations with intervertebral disc pathology results at certain levels.

The Change of Lumbar Intervertebral Disc Size on Aging and Low Back Pain (노화와 요통에 따른 요추부 추간판 크기 변화)

  • The purpose of this study is to determine whether change of the lumbar intervertebral disc diameters and size is a risk factor for development of lumbar disc lesion and this changes are associated with aging. The L4-5 and L5-S1 intervertebral disc diameters and size were measured in 50 normal subjects and 50 low back pain patients in the age range $20{\sim}60$ by MRI. The data were analysed with independent t-test to differentiation between normal subjects and low back patients. We also analysed with one-way ANOVA, Pearson correlation and simple linear regression to differentiate between different age ranges. This study has shown that older subjects of both normal and low back pain patients have significantly larger transverse diameters and sizes of the L4-5 and L5-S1 intervertebral disc, compared with those of younger subjects. In addition, low back pain patients's disc sizes were larger than normal subjects but there was statistically no significant difference. The measurement of the intervertebral disc size can be considered as a clinically useful method for diagnosis of the low back pain and aging of the intervertebral disc. However, further study should be conducted to establish normal value of intervertebral disc size on large number of subjects.

Mini-partial lateral corpectomy and hemilaminectomy for the treatment of heavily protruded thoracolumbar intervertebral disc in small dogs

  • Five paraplegic dogs were diagnosed with thoracolumbar intervertebral disc disease with more than 50% compression of spinal cord. Because the lesions were determined to be disc extrusion on magnetic resonance imaging, a hemilaminectomy was initially performed, however, protruded discs were confirmed during surgery. To remove the protruded disc, modified partial lateral corpectomy (mini-PLC) was additionally performed. All dogs recovered to full ambulation within a median of 44 days without temporary deterioration or vertebral instability. Mini-PLC as described here enables successful removal of the protruded disc, while preserving vertebral stability in dogs for whom the use of hemilaminectomy is inevitable.

The Effect of Acupotomy on Lumbar Herniated Intervertebral Disc: Report of a Case Series (요추 추간판 탈출증 환자에 대한 도침을 포함한 한방 복합치료 효과: 연속증례보고)

  • Objectives : This is a case series reporting of the improvement in patients with a lumbar herniated intervertebral disc by means of acupotomy combined with oriental medicine. Methods : We treated seven patients with a lumbar herniated intervertebral disc with acupotomy combined with oriental medical treatment. We checked the numeric rating scale(NRS), oswestry low back pain disability index(ODI) and range of motion(ROM). Results : NRS and ODI decreased, and ROM improved in all cases. Conclusions : This study shows acupotomy has a meaningful effect when applied on a lumbar herniated intervertebral disc.

  • Objectives : The purpose of this study was to report the clinical effects of needle-embedding acupuncture therapy combined with Korean medicine on a lumbar herniated intervertebral disc. Methods : We treated one patient with a lumbar herniated intervertebral disc with needle-embedding acupuncture therapy combined with Korean medical treatment. We checked the numeric rating scale(NRS), oswestry low back pain disability index(ODI) and straight leg raising test(SLRT). Results : In this case, the numeric rating scale decreased, while the straight leg raising test results improved. Conclusion : Needle-embedding acupuncture therapy could be effective for pain-reduction for a lumbar herniated intervertebral disc.

The Retrospective Comparative Study of Bee Venom Pharmacopuncture Therapy in Patients with Cervical Herniated Intervertebral Disc (경추 추간판 탈출증의 봉약침 병행 치료 효과에 대한 후향적 비교 연구)

  • Objective: This study was designed to evaluate the effect of Bee Venom Pharmacopuncture therapy in patients with Cervical Herniated Intervertebral Disc Methods: We investigated 30 cases of patients with Cervical Herniated Intervertebral Disc, who had been treated from October 2008 to December 2011. We divided patients into two groups: Group I was treated by Bee Venom Pharmacopuncture therapy and general acupuncture and Group II was treated by acupuncture therapy only. We measured the efficacy of treatments using the numerical rating scale(NRS). Results: 1. The treatment method for Group I was more effective than that of Group II in reducing the NRS on the tenth day after admission but there was no statistically significant difference between the results of two groups on the fifth day after admission and the tenth day after admission. Conclusion: The results of this study suggest that Bee Venom Pharmacopuncture therapy is effective in reducing pain for patients with Cervical Herniated Intervertebral Disc. Further clinical research is needed to verify these results and findings.

Clinical Study on 5th Lumbar - 1st Sacrum Facet Syndrome Patients' Radiological Finding (제5요추 - 제1천추간 후관절증후군 환자의 방사선학적 소견에 관한 임상적 연구)

  • Objectives : The 5th lumbar - 1st sacrum facet joint is unstable area from an anatomical viewpoint, so that it is clinically major causes of low back pain. The purpose of this study is to assess the difference of the Van Akkerveeken measurement and intervertebral disc angle, Ferguson angle betwoon the 5th lumbar - 1st sacrum facet syndrome patients and sample group patients.Methods : Van Akkerveeken measurement, intervertebral disc angle and Ferguson angle were measured in 30 patients who had 5th lumbar - 1st sacrum facet syndrome and 31 sample group patients.Results :1. Van Akkerveeken measurement of 5th lumbar - 1st sacrum facet syndrome patients is statistically larger that of than sample group patients.2. Intervertebral disc angle of the 5th lumbar - 1st sacrum facet syndrome patients is statistically larger that of than sample group patients.3. Ferguson angle is the 5th lumbar - 1st sacrum facet syndrome patients is statistically larger than that of sample group patients.Conclusions : These results suggest that Van Akkerveeken measurement and intervertebral disc angle is able to be used for diagnosis of facet syndrome.
