Purpose: The current study aims to identify the effects of proprioceptive neuromuscular facilitation (PNF) on pain and grip strength in patients with cervical herniated intervertebral disc (HIVD) and provide effective interventions for such patients. Methods: Included in this study were 20 subjects (10 men, 10 women) in their 30s-50s who had been diagnosed with cervical HIVD. Intervention methods included PNF neck patterns, dynamic reversal of antagonists, and a combination of isotonics. The visual analogue scale (VAS) was used to measure pain and dynamometers were used to measure grip strength. Results: After four weeks of muscle strengthening exercises, neck pain was significantly reduced, and hand grip strength significantly increased. Conclusion: PNF can be utilized as an effective intervention to reduce the pain of patients with cervical HIVD and to increase grip strength.

Objective: Spinal instrumentation without fusion often fails due to biological failure of intervertebral joints (spontaneous fusion, degeneration, etc). The purpose of this study is to investigate the influence of fixation rigidity on viability of intervertebral joints. Methods: Twenty pigs in growing period were subjected to posterior segmental fixation. Twelve were fixed with a rigid fixation system(RF) while eight were fixed with a flexible unconstrained implant(FF). At the time of the surgery, a scoliosis was created to monitor fixation adequacy. The pigs were subjected to periodic radiological examinations and 12pigs (six in RF, six in FF) were euthanized at 12-18months postoperatively for analysis. Results: The initial scoliotic curve was reduced from $31{\pm}5^{\circ}$ to $27{\pm}8^{\circ}$ in RF group (p=0.37) and from $19{\pm}4^{\circ}$ to $17{\pm}5^{\circ}$ in FF group (p=0.21). Although severe disc degeneration and spontaneous fusion of facet joints were observed in RF group, disc heights of FF group were well maintained without major signs of degeneration. Conclusion: The viability of the intervertebral joints depends on motion spinal fixation. Systems allowing intervertebral micromotion may preserve the viability of intervertebral discs and the facet joint articular cartilages while maintaining a reasonably stable fixation.
Methodology for finite element analysis of vertebral column and pedicle screw system, which circumvents the tremendous difficulties in geometric, material, and structural modeling, is proposed. The simplification is focused on the modeling of the cancellous bone in vertebral body the intervertebral disc. and the instrumented internal fixation devices. Each proposed modeling technique is justified to result in reasonable accuracy. These methods are believed to be suitable for the development of pedicle screw systems, not only because modeling itself is much simpler. but also because reliable empirical data for disc stiffness may be incorporated with little additional effort, and presumably frequent design change may be easily reflected on the analysis.
Four patients underwent lumbar surgery. In all four patients, the dura was minimally torn during the operation. However, none exhibited signs of postoperative cerebrospinal fluid leakage. In each case, a few days after the operation, the patient suddenly experienced severe recurring pain in the leg. Repeat magnetic resonance imaging showed transdural nerve rootlets entrapped in the intervertebral disc space. On exploration, ventral dural tears and transdural nerve rootlet entrapment were confirmed. Midline durotomy, herniated rootlet repositioning, and ventral dural tear repair were performed, and patients' symptoms improved after rootlet repositioning. Even with minimal dural tearing, nerve rootlets may become entrapped, resulting in severe recurring symptoms. Therefore, the dural tear must be identified and repaired during the first operation.

The object of this study is to report a clinical effect regarding the conservative treatment of the patients who were diagnosed as the herniated intervertebral lumbar disc(HIVD). The patients were treated by the oriental medical conservative treatment including acupunture, Bee venom therapy and Chu-na therapy. We estimated by visual analogue scale and pain rating score subjectively. And we examined the patients with SLR test and valsalva test. Oriental medical conservative treatment improved HIVD subjuctively and objectively.

Results & Conclusions : In this study, the first MRI examination of HIVD of L-spine patients was performed at the first visit and re-examination of MRI was done after the treatment. We assessed clinical symptoms by using Numerical rating scales(NRS). In each case, the size of the disc herniation was considerably reduced in MRI image. NRS was also reduced significantly.

Gellan gum as a natural polysaccharide has good heat resistance, acid resistance and enzymes resistance. However, one of the drawbacks of gellan gum might be the lower mechanical strength. In this work, gellan gum scaffolds were mixed with poly(lactic-co-glycolic acid) (PLGA) microsphere in order to improve mechanical properties. The gellan gum scaffolds with various contents of PLGA microsphere were prepared for the regeneration of disc tissues. To evaluate the mechanical strength of hybrid structure of gellan gum and PLGA microsphere, compression strength of the fabricated scaffolds was measured. MTT analysis, SEM observation, histological evaluation and RT-PCR were performed to confirm the effect on the cell growth and extracellular matrix secretion. As a result, it showed the best cell proliferation and extracellular matrix secretion in gellan gum sponge containing 50% PLGA microspheres. In conclusion, this study confirmed that the hybrid structure of gellan gum and PLGA microspheres was found suitable in regeneration of the intervertebral disc.

Recently intradiscal electrothermal therapy is introduced, which is a new and minimally invasive technique fer the treatment of discogenic low back pain. This procedure involves the percutaneous threading of a flexible catheter into the disc under fluoroscopic guidance. The catheter, composed of thermal resistive coil, heats the posterior annulus of the disc, causing contraction of collagen fibers and destruction of afferent nociceptors. This study tries to investigate the effects of the important factors of this procedure such as heat source temperature and heat applying time on the temperature distribution within the intervertebral disc. This study utilized both computer simulation and the experiment for the verification of finite element analysis. FE analysis was carried out with ANSYS v7.0 (ANSYS Inc, USA) using 10,980 number of brick element and 12,551 number of node. The functional spinal units of 5 month old swine were used for the experiment and the temperature was monitored using 10 channel temperature measurement device MV200. Through this study, it was able to analyze the temperature range of inner intervertebral disc by two mechanisms which are known to alleviate pain clinically. The results showed that when the heat source temperature was kept up 80 degree for 1,020 seconds, the temperature of inner annulus reached at 45 degree up to the distance of 15.6mm from heat source, which explains coagulation of inner annulus by heat. When the same heat source was used, the temperature of inner nucleus reached at 60 degree up to the distance of 9mm from heat source, which explains contraction of inner nucleus by heat.

A 57-year-old female diagnosed with L5-S1 lumbar intervertebral disc herniation, suffering from severe pain despite taking tapentadol received combined Korean medicine treatment, including acupotomy, acupuncture, pharmacopuncture, and herbal therapies for 53 days. To assess pain, Numeric Rating Scale (NRS) and lumbar range of motion (ROM) were checked daily from the day of admission. Moreover, the Oswestry Disability Index (ODI) and European Quality of Life-5 Dimensions (EQ-5D) were used to evaluate function and quality of life. After combined Korean medicine treatment, reabsorptioin of intervertebral disc was confirmed by radiological examination; pain reduced from NRS 5~7 to NRS 1~2; lumbar ROM in extention increased from 20° to 30°; and function and quality of life improved. The results suggest the possibility that a combined Korean medical treatment, including acupotomy, can be used as an alternative to opioids for pain management of lumbar vertebral disc herniation.
