국내 모성적응증진 프로그램의 효과에 대한 체계적 문헌고찰 (Effect of Intervention Programs for Improving Maternal Adaptation in Korea: Systematic Review)

  • Purpose: The purpose of this study was to determine the current state of nursing intervention for maternal adaptation and its' effectiveness in Korea by utilizing a systematic review. Methods: The PICO(Population-Intervention-Compar ator-Outcome) strategy was established, and 1,720 pieces of literature published during the last ten-year period from four electronic databases were reviewed. Eighteen references that met inclusion and exclusion criteria were finally selected for systematic review. The quality of references using critical appraisal checklist for experimental studies were evaluated, and then systematic review and meta-analysis were conducted. Results: All 18 references were quasi-experimental research design. Most interventions were provided at the hospital and postpartum care center. Maternal adaptation interventions appeared to be of many types, and particularly maternal role education programs were the most common. Confidence in maternal role was used as the most common variable for the maternal adaptati on. Various interventions for helping maternal adaptation in the postnatal period improved maternal confidence, moth er-infant attachment, maternal satisfaction and mother-infant interaction effectively. Conclusion: A diversity of nursin g interventions in postnatal period improved various aspects of maternal adaptation. Randomized controlled trials and longitudinal studies are needed in order to verify the effect of interventions for maternal adaptation more clearly.

  • Acupuncture treatment and control group interventions in parallel-group randomised trials of acupuncture are not always precisely reported. In an attempt to improve standards, an international group of experienced acupuncturists and researchers devised a set of recommendations, designating them STRICTA : STandards for Reporting Interventions in Controlled Trials of Acupuncture. In a further consensus-building round, the editors of several journals helped redraft the recommendations. These follow the CONSORT format, acting as an extension of the CONSORT guidelines for the specific requirements of acupuncture studies. Participating journals are publishing the STRICTA recommendations and requesting prospective authors to adhere to them when preparing reports for publication. Other journals are invited to adopt these recommendations. The intended outcome is that interventions in controlled trials of acupuncture will be more adequately reported, thereby facilitating an improvement in critical appraisal, analysis and replication of trials.

지역사회 거주 심장질환 환자들을 위한 모바일 건강중재: 통합적 문헌고찰 (Mobile Health Interventions for Community-Dwelling Patients with Heart Diseases: An Integrative Review)

  • Purpose : The purpose of this study was to review mobile health (mHealth) interventions based on studies from online databases for community-dwelling patients with heart diseases. Methods : Six databases (RISS, KISS, PubMed, CINAHL, EMBASE, and SCOPUS) were searched to select studies conducted from January 1 to September 30, 2010. After this, quality appraisals were carried out using the Scottish Intercollegiate Guidelines Network checklist and a total of 11 studies were selected. Results : The selected 11 studies included 7 randomized controlled studies, 1 quasi-experimental study and 3 pilot studies. The main components of mHealth interventions included symptom monitoring at home, provisions for individualized messages for health management using text messaging, telephone or smart phone applications, and running websites for symptom monitoring or health education. Intervention periods varied from 6 weeks to 12 months. The findings of the studies suggested that the mHealth interventions were effective in improving self-management of heart diseases, quality of life, and decreasing symptoms. Conclusions : The results of the review suggested that mHealth interventions had positive effects on community-dwelling patients with heart diseases. More mHealth intervention studies need to be conducted in Korea to aid community-dwelling patients with heart diseases.

  • Purpose: The purpose of this study was to identify trends in health-related interventions on children attending Community Child Care (CCC) centers, which are part of a health policy to provide after-school care for vulnerable children in South Korea. Methods: From 2007, 109 papers were analyzed using the scoping study method. Results: The number of studies increased steadily between 2007 and 2016. Most studies were based on the social sciences, and the participants were mainly elementary school students. Psychological and social interventions were the most common types of interventions with socio-psychological indicators as measurements. In addition, only a few studies had a clearly defined conceptual framework. The majority of studies did not explicitly indicate that they followed ethical considerations. Conclusion: It is necessary to develop health-related interventions for children attending CCC centers using diverse subjects, types, and evaluation methods, along with improvements in the quality of research methodology. Furthermore, it is essential to clearly articulate and implement ethical considerations in research targeting vulnerable children.

Systematic Review of Meaning-centered Interventions for Adolescents

  • Purpose: This study analyzes meaning-centered intervention studies conducted in Korea and abroad to compare and confirm their characteristics and results. Methods: A literature search using keywords in English and Korean was performed using nine electronic databases in December 2017. Search participants included adolescents, and interventions conducted in meaning-centered intervention studies were selected for evaluation. A Risk of Bias Assessment tool for non-randomized studies was used for quality assessment. Results: All studies were based on quasi-experimental designs. The semantic intervention included topics such as freedom, choice, responsibility, pain, death, finding value of life, purpose and meaning of life, and becoming a master of my life. All studies used logotherapy, and included meaningful interventions that influenced the meaning and purpose of the life of adolescents. The life satisfaction of adolescents changed significantly when there is a positive and open relationship with their parents. Therefore, an educational program for parents based on meaning therapy is desperately needed. Conclusion: Meaning-centered interventions were found to be effective interventions for exploring the meaning of life not only for adolescents with problems but also for healthy youth, and as such could be used as basic data for the development of an appropriate intervention for enhancing their life.

Heart Failure Self-management Interventions: Evidence from Korean Patients

  • Purpose: The aims of this study were to (1) summarize the HF self-management interventions and (2) identify gaps and priorities for further investigation. Methods: A computer search of the literature over the past decade yielded 9 HF self-management studies. Six studies used non-experimental design; only 3 (33%) studies used quasi-experimental design which were included in this review. Results: The three pivotal HF self-management interventions studies were important because they highlight the positive effects of self-management compliance and other quality of life outcomes as well as discussing various issues. The informational booklet and telephone follow-up were the most common modalities in these interventions. The periodic telephone counseling offered in 2 studies effectively facilitated patient self-management compliance. There were some methodological flaws such as small sample sizes (range 8~21 in each group) and lack of experimental designs, long-term follow-up, and random group assignment. Additionally, the use of valid, reliable outcome measures is necessary to compare the effects of the interventions worldwide. Conclusion: More clinical evidence on HF self-management is needed using a larger sample size and the efficacy needs to be tested for various outcomes, including morbidity, mortality, and health care cost.

A Systematic Review: Effectiveness of Interventions to De-escalate Workplace Violence against Nurses in Healthcare Settings

  • Workplace violence (WPV) is an increasing cause of concern around the globe, and healthcare organizations are no exception. Nurses may be subject to all kinds of workplace violence due to their frontline position in healthcare settings. The purpose of this systematic review is to identify and consider different interventions that aim to decrease the magnitude/prevalence of workplace violence against nurses. The standard method by Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA, 2009) has been used to collect data and assess methodological quality. Altogether, twenty-six studies are included in the review. The intervention procedures they report on can be grouped into three categories: stand-alone trainings designed to educate nurses; more structured education programs, which are broader in scope and often include opportunities to practice skills learned during the program; multicomponent interventions, which often include organizational changes, such as the introduction of workplace violence reporting systems, in addition to workplace violence training for nurses. By comparing the findings, a clear picture emerges; while standalone training and structured education programs can have a positive impact, the impact is unfortunately limited. In order to effectively combat workplace violence against nurses, healthcare organizations must implement multicomponent interventions, ideally involving all stakeholders.

  • Purpose: This study examines the patient problems and nursing interventions during telephone consultations by nurses in an ophthalmology nursing unit. Methods: The data were collected during telephone consultations. A total of 13 nurses consulted 170 patients between September 29 and November 30, 2008 at a tertiary teaching hospital. Problems were raised by the patients and nursing interventions were provided to the patients by the nurses. The SPSS program was used to analyze the data. Results: There were 228 telephone inquiries by 170 patients. The problems were categorized into four groups based on a literature review and validated by 14 nurses: physical symptoms, medications, administrative matters, and follow-up care. Interventions provided by the nurses were categorized into six groups: instructing patients; providing information; providing background knowledge; assuring the patients; referring to the patients to other ancillary departments; and delivering doctor's orders. Conclusions: Nurse interventions for a specific patient problem varied depending on the nurse providing the consultation. The participating nurses expressed the need for a common protocol for telephone consultations. Thus, it would be beneficial to develop a guideline for telephone consultations to minimize practice variations among nurses.

자율신경균형 증진 간호중재를 위한 생행동적 이론적 기틀 구축: 세포노화 기전 기반으로 (A biobehavioral theoretical framework based on the mechanism of cellular aging for nursing interventions to promote autonomic balance)

  • Purpose: This study reviewed the pathophysiological mechanisms of cellular aging caused by psychological stress and aimed to establish a biobehavioral theoretical framework for nursing interventions to promote autonomic balance based on these mechanisms. Methods: A comprehensive literature review was conducted. Results: A review of the literature showed that the stress response increases the secretion of catecholamines and glucocorticoids, resulting in a greater allostatic load. This load induces inflammatory reactions and oxidative stress, shortening telomere length and damaging mitochondrial DNA, which can lead to cellular aging. Based on this mechanism, a biobehavioral theoretical framework for nursing interventions was established. This framework focuses on delaying or inhibiting the cellular aging process by acting on the stress response stage and improving autonomic balance. Conclusion: According to the proposed biobehavioral theoretical framework, stress-relieving nursing interventions may act on the mechanism of cellular aging caused by stress responses. We believe that this framework could expand our understanding of the biobehavioral aspects of stress and would facilitate efforts to use biomarkers to evaluate the effectiveness of stress-related nursing interventions at the cellular level.

  • The vast majority of the disaster prevention design research in Korea has focused on the macro-level interventions such as examining the existing systems and approaches, instituting revised policies, systematically establishing intervention programs, and evaluating the effectiveness of the interventions. However, little has focused on systematically challenging individuals' awareness using micro-level intervention. The present paper introduces approaches to micro-level intervention by infusing narrative-based simulation, and further suggests the implications on balancing the approaches of both micro-level and macro-level interventions.
