Objective: This study aimed to get information on the current status of therapies to date for non-alcoholic fatty liver disease (NAFLD), including non-alcoholic steatohepatitis (NASH). Methods: All randomized clinical controlled trial (RCT)-derived papers for NAFLD or NASH were reviewed via PubMed Database. Results: 39 RCTs met the review criteria, of which 15 and 24 papers were for NAFLD and NASH, respectively. 83% of the papers were released since 2006, and 30 studies were conducted for western medicines, antioxidants and lifestyle intervention whereas nine trials were done using herbal medicine or acupuncture which showed positive outcome. Conclusions: NAFLD and NASH are new epidemic disorders which can be a target of traditional Oriental medicine. This study will be helpful for the Oriental medicine-based strategies or therapeutic development for them.
Objective This study was performed to investigate whether Bee Venom Pharmacopuncture(BVP) could be a more effective modality than Warm Needling(WN) in relieving pain and symptoms of knee osteoarthritis(OA). Design Prospective, randomized and controlled clinical trial. Setting Single center trial in Korea Patients 49 volunteers with knee OA participated in the study. All the participants were screened through an inclusion and exclusion criteria. 33 participants were completed the clinical trial. Intervention The subjects were randomly assigned to one of two groups. One group received BVP(n=18), while the other group received WN(n=15). Sixteen sessions of BVP or WN were given at the pain region of the problematic knee for 8 weeks. Primary outcome measure is the Korean translation of Western Ontario and McMaster Universities Osteoarthritis Index scores(Korean WOMAC, KWOMAC). Secondary outcome measure is the physical health scores based on the 36-Item Short-Form Health Survey(SF-36) and Patient Global Assessment(PGA). KWOMAC and SF-36 were measured third (baseline, 4 and 8 weeks). PGA was measured twice(4 and 8 weeks). Results BVP group showed significant decrease compared to WN group in pain, function and total scores of KWOMAC according to the Mann-Whitney U-test. In the PGA, BVP group, compared to WN group, showed a significant increase. Conclusions BVP was more effective in relieving pain of knee OA than WN. These findings suggest that BVP is a promising alternative for treating knee OA.

Purpose: To evaluate the therapeutic effects of copper reduction on angiogenesis-related factors in patients with glioblastoma multiforme treated by gamma knife radiosurgery. Materials and Methods: In the present block randomized, placebo-controlled trial, fifty eligible patients with a diagnosis of glioblastoma multiforme who were candidates for gamma knife radiosurgery were randomly assigned into two groups to receive daily either 1gr penicillamine and a low copper diet or placebo for three months. The intervention started on the same day as gamma knife radiosurgery. Serum interleukin-6 (IL-6), tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$), vascular endothelial growth factor (VEGF) and copper levels were measured at baseline and after the intervention. The serum copper level was used as the final index of compliance with the diet. In order to control probable side effects of intervention, laboratory tests were conducted at the beginning, middle and end of the study. Results: The patients had a mean age and Karnofsky Performance Scale of 43.7 years and 75 respectively. Mean serum copper levels were significantly reduced in intervention group. Mean survival time was 18.5 months in intervention group vs. 14.9 in placebo group. VEGF and IL-6 levels in the intervention group were also significantly reduced compared to the placebo group and $TNF-{\alpha}$ increased less. Conclusions: It seems that reducing the level of copper in the diet and dosing with penicillamine leads to decline of angiogenesis-related factors such as VEGF, IL-6 and $TNF-{\alpha}$. Approaches targeting angiogenesis may improve survival and can be used as a future therapeutic strategy.
Objectives: This study was conducted to evaluate the impact of a School-based Depression Awareness Education Program on depression knowledge and help-seeking orientation for adolescent. Methods: The students of two high schools were allocated to the intervention (n=221) or control group (n=223) in this cluster-randomized trial. The school-based depression awareness education was a 3-hour program. The intervention group received pre, post and follow up tests while the control group received pre and follow up tests after two months. Depression knowledge, attitude to friends'depression, and help-seeking for professional assistance was evaluated at each test. Differences in the two month change between the intervention and control groups were analyzed by t-test or chi-square test. Results: There were positive changes in depression knowledge, response to friend's depression, and attitude to professional help-seeking in the post test compared to the pretest among intervention group. Improvement of depression knowledge was greater in the intervention group than in the control group (P=0.011), but there were no significant differences in response to friend's depression (P=0.268) or help-seeking orientation (P=0.263) after two months. Conclusions: School-based Depression Awareness Education resulted in significant positive changes in depression knowledge in high school students, but made no attitudes difference to professional help-seeking.
Background: The purpose of this study was to provide the basis for the treatment intervention by identifying the treatment effect when rehabilitation intervention is applied to patients with dysphagia due to stroke and by comparing the results of the treatment mediation according to the differences of the treatment methods and frequency. Design: Randomized Controlled Trial. Methods: 30 people diagnosed with dysphagia due to stroke were divided in accordance with the differences in treatment mediation techniques and treatment frequency- traditional swallowing rehabilitation coupled with neuromuscular electrical stimulation group and only neuromuscular electrical stimulation group/ 5 times per week group and 2 times per week group, and ten weeks of treatment intervention was performed. Paired t test was employed to show the efficacy of treatment intervention, Independent sample t test was used to compare the results according to difference and number of treatment intervention techniques. Results: There was a significant positive effect of treatment on traditional swallowing rehabilitation coupled with neuromuscular electrical stimulation group, only neuromuscular electrical stimulation group, 5 times per week group and 2 times per week group (p<0.05). There was no statistically significant difference in treatment effect between traditional swallowing rehabilitation coupled with neuromuscular electrical stimulation group and only neuromuscular electrical stimulation group (p<0.05). There was no statistically significant difference in treatment effect between 5 times per week group and 2 times per week group (p<0.05). Conclusion: There was no significant difference according to the technique or number of treatments of swallowing rehabilitation treatment interventions, but it was confirmed that rehabilitation intervention for dysphagia showed positive treatment effect.
Purpose: This study aimed to develop an integrated health promotion program for cancer survivors residing in the community based on the shared care model, and evaluate its effectiveness. Methods: A quasi-experimental trial was conducted. The participants consisted of 35 cancer survivors with completed intensive cancer therapy at the cancer hospital. The intervention group (n=20) and the control group (n=15) were recruited from among a district home cancer patient registrations. The intervention group participated in an integrated health promotion program based on the MAPP (Mobilizing for Action through Planning and Partnership) development process. The program consisted of physical, psycho-social and body image units. The participants were assessed before the program, and immediately after the program. Data were collected between July 1 and September 2, 2018 using FACT-G quality of life (QOL), distress thermometer (DT), and resilience. The data were analyzed by performing a χ2 test, Fisher's exact test, Mann-Whitney test, and ranked ANCOVA using SPSS. Results: The intervention group reported a higher QOL overall and significantly higher social/family well-being than the control group. Distress was significantly lower in intervention group than in the control group. Resilience had no significant difference between the two groups. Conclusion: These findings indicate that the integrated health promotion program base on the shared care model and MAPP development process could be effective intervention for improving social/family well-being and the QOL, and reducing distress of cancer survivors at home. Community health center nurses need to provide intervention to support self-care competency for cancer survivors' comprehensive care with physical, psycho-social, and body image to help them adjust their life to a moderate risk group in the community.
PURPOSE: This study examined the effects of a myofascial release technique with preservation physical therapy on low back pain, range of motion, and physical function of patients in acute traffic accidents METHODS: Twelve patients with traffic accidents were divided randomly into two groups of six patients each who met the selection criteria. The training was conducted for 45 minutes each time, for two weeks and four times per week. The experimental group received four myofascial release techniques and with preservation physical therapy. The control group received only preservation physical therapy. RESULTS: The VAS test result was statistically significant after the intervention in both groups (p < .05). After the intervention, there was no significant difference between the experimental group and control group. The Schober test result was statistically significant after the intervention in both the experimental and control groups (p < .05), and there was a significant difference between the experimental group and control group after the intervention (p < .05). The KODI assessment result was significant after the intervention in both the experimental and control groups (p < .05), and there was a significant difference between the experimental group and control group after the intervention (p < .05). CONCLUSION: The myofascial release technique with preservation physical therapy had a positive effect on low back pain, range of motion, and body function in acute traffic accident patients.
Objective: This study attempted to find out if it changes the child's ability to perform daily life activities when visiting the familiar environment and daily living space of children with cerebral palsy and conducting a home activity support program for children and parents. Design: Randomized Controlled Trial Methods: Among 22 children aged 3 to 12 years old, they were assigned to the intervention group and control group. Of these, 12 boys and 10 girls participated in the study. Gross motor function measure and upper extremity function evaluation were used to measure the physical function of children with cerebral palsy, and self-care skills, mobility and social functions were evaluated in the pediatric evaluation of disability inventory. In addition, a parenting sense of competence was used to find out the efficacy of parents in raising children. After the pre-evaluation, basic rehabilitation treatment and intervention programs were applied to the intervention group, and only basic rehabilitation treatment was performed to the control group, and post-evaluation was performed 8 weeks later. Results: As a result of the study, among the items that measured the gross motor function, upper limb function, and daily life performance ability of the intervention group in the difference between the intervention group and the control group, statistically improved in personal processing and movement (p<0.05). In addition, the parenting sense of competence children in the intervention group was statistically significant (p<0.05). Conclusions: The home activity support program will help strengthen the ability of cerebral palsy children to perform daily life as a way to set mutually agreed goals with their families or children and achieve them in a familiar environment.
[Purpose] Excessive reactive oxygen species (ROS) induced by prolonged high-intensity exercise can cause structural and functional damage. Antioxidant polyphenol supplementation, which reduces ROS levels, may improve high-intensity exercise performance. We evaluated the effect of lychee fruit extract, which contains high levels of low-molecular-weight oligomerized polyphenols, on high-intensity exercise performance. [Methods] Ten male athletes were included in an open-label trial that consisted of control and intervention phases, with a 7-day washout period between phases. The participants were administered oligomerized lychee fruit extract for seven days, whereas no intervention was given in the control phase. High-intensity intermittent exercise and the Wingate test were performed. The power output, blood lactate levels, reactive oxygen metabolite levels, biological antioxidant potential, heart rate, and rate of perceived exertion were measured. [Results] The average power output was significantly higher in the intervention phase than in the control phase (P < 0.01), while the change in blood lactate levels was significantly lower in the intervention phase than in the control phase (P < 0.05). The average heart rate was significantly higher in the intervention phase than in the control phase (P < 0.05), without changing the rate of perceived exertion. Although there was no difference in reactive oxygen metabolite levels between the phase, the change in biological antioxidant potential was larger in the intervention phase than in the control phase (P = 0.06). The Wingate test showed no significant differences between the phase. [Conclusion] Short-term loading with oligomerized lychee fruit extract may increase performance during high-intensity intermittent exercise by improving metabolism.

Background: Breast cancer is the most common female malignancy in the world. Beta glucan can be a hematopoietic and an immune modulator agent in cancer patients. The aim of this trial was to determine the effect of beta glucan on white blood cell counts and serum levels of IL-4 and IL-12 in women with breast cancer undergoing chemotherapy. Materials and Methods: This randomized double-blind placebo-controlled clinical trial was conducted on 30 women with breast carcinoma aged 28-65 years. The eligible participants were randomly assigned to intervention (n=15) or placebo (n=15) groups using a block randomization procedure with matching based on age, course of chemotherapy and menopause status. Patients in the intervention group received two 10-mg capsules of soluble 1-3, 1-6, D-beta glucan daily and the control group receiving placebo during 21 days, the interval between two courses of chemotherapy. White blood cells, neuthrophil, lymphocyte and monocyte counts as well as serum levels of IL-4 and IL-12 were measured at baseline and at the end of the study as primary outcomes of the study. Results: In both groups white blood cell counts decreased after 21 days of the intervention, however in the beta glucan group, WBC was less decreased non significantly than the placebo group. At the end of the study, the change in the serum level of IL-4 in the beta glucan group in comparison with the placebo group was statistically significant (p=0.001). The serum level of IL-12 in the beta glucan group statistically increased (p=0.03) and comparison between two groups at the end of the study was significant after adjusting for baseline values and covariates (p=0.007). Conclusions: The findings suggest that beta glucan can be useful as a complementary or adjuvant therapy and immunomodulary agent in breast cancer patients in combination with cancer therapies, but further studies are needed for confirmation.
