  • Interstital cystitis(IC) is a disease of the bladder that is characterized by inveterate irritation due to non-specific chronic inflammation. The symptoms of IC are chronic pain, frequency, urgency and sleep deprivation These symptom's are contained to the Lunbing(淋病) in oriental medicine. We observed 4 cases of patient with Interstitial cystitis, the results are as follows. Methods & Results : All patients were treated by the same method. Treatment was performed by means of Hapgokja(合谷刺, The needle arrived to the bladder wall) with electroacupuncture(electric stimulation was 5mA mixed pulse, low frequency 2Hz and high frequency 30Hz, intensity was adjusted to inside the scope where the patients perseveres.) The electrodes were placed CV2 to CV3, both K11, K12. Conclusions : 1. As the result, uses the Hapgokja(合谷刺), with the electroacupuncture, symptoms are remarkably improved and got the satisfactory effect in the treatment. 2. In Hunner's ulcer type, the first treatment was not satisfactory, and second time, after 15 times treatment, there were maintain urgency, 8 times nocturia, 70 percents bladder pain. 3. 3 patients of nonulcer type were treated for 44.8 times(average) and improved started after 5 times treatment. After 2 month's treatment, there were no frequency, no urgency, and remain 1 or 2 nocturia, 30 percents bladder pain.

  • Purpose: The purpose of this study is to identify the real condition of urinary incontinence(UI), interstitial cystitis(IC), atrophic viginitis(AV) for elderly women and analyze the cognition of traditional korean medicine(TKM) for them. Methods: We utilized questionnaire from May to June, 2010. Questionnaires were taken from 125 women using senior welfare center and nursing home, aged over 65 up to 92. The data were analyzed by $X^2$-test using SPSS/PC ver 18.0 program. Results: The prevalences of UI, IC and AV symptoms were 50.4%, 40.8%, 56%, respectively. The average I-QoL score for UI was $82.62{\pm}21.16$, and the average ICSI score for IC was $8.16{\pm}2.50$. After adjustment for each of the variables considered in this study, alcohol was associated with UI and age, BMI(body mass index) were associated with IC. Most of respondents have no experience(94.4%) or don't know (79.2%) about TKM for UI, IC and AV. 44 women(37.3%) indicated that they weren't willing to use TKM for UI, IC and AV. Reasons for not taking TKM were because of 'no knowledge of TKM(34.1%)' and 'more accustomed to western treatment(34.1%)'. Conclusion: Although the prevalence of UI, IC, AV for elderly women was high, the actual percentage of treatment for these diseases was low, furthermore, patients were not aware of TKM and had very few experiences of TKM for these diseases. The development and increased promotion about TKM program for elderly women's urogenital diseases is needed.

  • Purpose: The O'Leary Symptom Index and Problem Index(ICSI/ICPI) have been used widespread in clinical studies and research. This research was aimed to determine the reliability and validity of the ICSI/ICPI-K that was translated into Korean. Methods: The research design was a methodological study to test the measurements. Subjects were 301 elderly women in Seoul, Chungnam and Gyungbuk. They were recruited through a formal announcement and they responded after informed consent. Data was analyzed with SPSS 15.0. The internal consistency was assessed by Cronbach's alpha coefficient and the criterion validity was assessed by Pearson correlation with the Geriatric Depression Scale(GDS) and inter-subscales. Results: The elderly women were on average 74.3 years old. As to the reliability of the scale, ICSI/ICPI-K had a Cronbach alpha of .832. In addition, the results showed that ICSI/ICPI-K was valid through the Pearson correlation. That is, the ICSI-K and ICPI-K had a significant relationship with GDS(r=.295; r=.374, respectively) and strong correlations of subscale to subscale(ICSI-K & ICPI-K: r=.676). The most common symptom of the lower urinary tract by the ICSI/ICPI-K was nocturia 249(83.3%) followed by frequency 187(62.8%), urgency 148(49.8%). and pelvic pain 71(23.9%) in the elderly women. Conclusion: We can conclude that the ICSI/ICPI-K is reliable and valid in Korean elderly women. It needs to be evaluated in the clinical area.

  • Interstitial cystitis/bladder pain syndrome (IC/BPS) is a debilitating chronic disorder characterized by suprapubic pain and urinary symptoms such as urgency, nocturia, and frequency. The prevalence of IC/BPS is increasing as diagnostic criteria become more comprehensive. Conventional pharmacotherapy against IC/BPS has shown suboptimal effects, and consequently, patients with end-stage IC/BPS are subjected to surgery. The novel treatment strategies should have two main functions, anti-inflammatory action and the regeneration of glycosaminoglycan and urothelium layers. Stem cell therapy has been shown to have dual functions. Mesenchymal stem cells (MSCs) are a promising therapeutic option for IC/BPS, but they come with several shortcomings, such as immune activation and tumorigenicity. MSC-derived extracellular vesicles (MSC-EVs) hold numerous therapeutic cargos and are thus a viable cell-free therapeutic option. In this review, we provide a brief overview of IC/BPS pathophysiology and limitations of the MSC-based therapies. Then we provide a detailed explanation and discussion of therapeutic applications of EVs in IC/BPS as well as the possible mechanisms. We believe our review will give an insight into the strengths and drawbacks of EV-mediated IC/BPS therapy and will provide a basis for further development.

  • Dimethyl sulfoxide (DMSO) is widely used in chemistry and biology as a solvent and as a cryoprotectant. It is also used as a pharmaceutical agent for the treatment of interstitial cystitis and rheumatoid arthritis. Previous reports described DMSO as being reduced by methionine-S-sulfoxide reductase (MsrA). However, little is known about the DMSO reduction capability of methionine-R-sulfoxide reductase (MsrB) or its effect on the catalysis of methionine sulfoxide reduction. We show that mammalian MsrB2 and MsrB3 were unable to reduce DMSO. This compound inhibited MsrB2 activity but did not inhibit MsrB3 activity. We further determined that DMSO functions as an inhibitor of MsrA and MsrB2 in the reduction of methionine sulfoxides via different inhibition mechanisms. DMSO competitively inhibited MsrA activity but acted as a non-competitive inhibitor of MsrB2 activity. Our study also demonstrated that DMSO inhibits in vivo methionine sulfoxide reduction in yeast and mammalian cells.

  • Hypospadias is a rare congenital malformation of the urethra reported in cattle. The urethral lumen of male indigenous Korean calf is open along the ventral aspect of the penis in the perineal region. Renal abscess and renal stone formation causing urinary tact infection has not been reported in hypospadia calves. The objective of this study was investigation for renal abscess and renal stone formation through autopsy. Histopathological examination and laboratory tests were performed. At autopsy, the pustules were formed on the right renal cortex, and the renal medulla abscess were formed on right and left part of the renal pelvis. Histopathological finding, this case was diagnosed as severe acute suppurative and necrotizing pyelonephritis, and severe chronic interstitial nephritis with fibrosis and moderate multifocal acute cystitis with edema. Milky exudate of the kidney has been identified as Actinomyces meyeri using the VITEK-2 system for identification of bacteria, and the stone has been identified as carbonate apatite using FT-IR system for quantification analysis. This case report describe the hypospadias complicated with urinary tract infection due to carbonate apatite stones and Actinomyces meyeri.

  • Background: Few studies have investigated unspecified or idiopathic pain associated with COIVD-19. This study aimed to provide the incidence rates of unspecified pain and idiopathic pain in patients with COVID-19 for 90 days after COVID-19 diagnosis. Methods: A propensity score matched cohort was used, including all patients with COVID-19 in South Korea, and analyzed their electronic medical records. The control group consisted of those who had not had tests for COVID-19 at all. Unspecified pain diagnoses consisted of diagnoses related to pain included in the ICD-10 Chapter XVIII. Idiopathic pain disorders included fibromyalgia, temporomandibular joint disorders, headaches, chronic prostatitis, complex regional pain syndrome, atypical facial pain, irritable bowel syndrome, and interstitial cystitis. Results: After matching, the number of participants in each group was 7,911. For most unspecified pain, the incidences were higher in the COVID-19 group (11.7%; 95% confidence interval [CI], 11.0-12.5) than in the control group (6.5%; 95% CI, 6.0-7.1). For idiopathic pain, only the headaches had a significantly higher incidence in the COVID-19 group (6.6%; 95% CI, 6.1-7.2) than in the control group (3.7%; 95% CI, 3.3-4.1). However, using a different control group that included only patients who visited a hospital at least once for any reasons, the incidences of most unspecified and idiopathic pain were higher in the control group than in the COVID-19 group. Conclusions: Patients with COVID-19 might be at a higher risk of experiencing unspecified pain in the acute phase or after recovery compared with individuals who had not had tests for COVID-19.

  • Background : Nonspecific interstitial pneumonia (NSIP) has been reported recently to have shown much better response to medical treatment and better prognosis compared with idiopathic UIP. However, clinical characteristics of idiopathic NSIP discriminating it from UIP have not been clearly defined. Method : Among 120 patients with biopsy-proven diffuse interstitial lung diseases admitted to the Samsung Medical Center between July 1996 and March 2000, 18 patients with idiopathic NSIP were included in this study. Retrospective chart review and radiographic analysis were performed. Results : 1) At diagnosis, 17 patients were female and the average age was $55.2{\pm}8.4$ years (44~73 years). The average duration from development of respiratory symptom to surgical lung biopsy was $9.9{\pm}17.1$ months. Increase in bronchoalveolar lavage fluid lymphocytes ($23.0{\pm}13.1%$) was noted. On HRCT, ground glass and irregular linear opacity were observed, but honeycombing was absent in all patients. 2) Corticosteroids were initially given to 13 patients, but the medication was stopped in 3 patients due to severe side effects. Further medical therapy was not possible in 1 patient who experienced streroid-induced psychosis. Herpes zoster (n=3), tuberculosis (n=1), avascular necrosis of the hip (n=1), cataract (n=2) and diabetes mellitus (n=1) developed during prolonged corticosteroid administration. Of the 7 patients receiving oral cyclophosphamide therapy, hemorrhagic cystitis hindered one patient from continuing with the medication. 3) After medical treatment, 14 of 17 patients improved, and 3 patients remained stable (mean follow-up ; $24.1{\pm}11.2$ months). FVC increased by $20.2{\pm}11.2%$ of predicted value and the extent of ground glass opacity on HRCT decreased significantly ($15.7{\pm}14.7%$). 4) Of the 14 patients who had stopped medication, 5 showed recurrence of NSIP and 2 became aggravated during steroid tapering. All patients with recurrence showed deterioration within one year after completion of initial treatment. Conclusion : Since idiopathic NSIP has unique clinical profiles and shows good prognosis, diagnosis different from UIP, and aggressive medical treatment are needed.

  • Purpose of the radio-therapy is maximize the radiation dose to the tumor while minimizing the dose to the critical organ. Carcinoma of the uterine cervix treatment are external irradiation or an interstitial brachtheraphy make use of isotope. Brachytherapy is a method of radiotherapy in advantage to achieve better local control with minimum radiation toxicity in comparison with external irradiation because radiation dose is distributed according to the inverse square low of gamma-ray emitted from the implanted sources. Authors make use of the patients data which 192Ir gives medical treatment intrcavity. Intracavitary radiation of the uterine cervix cancer, critical organ take $20\%$ below than exposure dose of A point in the ICRU report. None the less of the advice, Radiation proctitis and radiation cystitis are frequent and problematic early complications in patients treated with radiation for the uterine cervix cancer. In brachytherapy of uterine cervical cancer using a high dose rate remote afterloading system, it is of prime importance to deliver a accurate dose in each fractionated treatment by minimizing the difference between the pre-treatment planned and post-treatment calculated doses. Use of packing to reduce late complications intracavitary radiation of the uterine cervix cancer. Bladder and rectum changes exposure dose rate by radiotherphy make use of packing.
