LNG선용 버터플라이밸브의 유동특성에 관한 연구 (제1부 : CFD해석과 실험결과의 비교) (Flow Characteristics of Cryogenic Butterfly Valve for LNG Carrier (Part 1 : CFD Analysis and its Comparison with Experimentation))

  • Butterfly valves are widely used as control valves for industrial process. For the definition of optimum configuration of the valve, wide range of related studies has been actively conducted in the case of working fluids of water or air under the normal temperature. Recently, internal flow and performance characteristics of cryogenic butterfly valve for LNG carrier take a growing interest in the field of research and development. Therefore, present study is aimed to investigate the internal flow and performance characteristics of the cryogenic butterfly valve because the study result for the valve can be hardly found at present. Part 1 of this paper describes the study result of a butterfly valve model under the condition of the normal temperature. Succeeding Part 2 of this paper will describe the internal flow characteristics of a cryogenic butterfly valve for LNG carrier. The results of Part 1 show that pressure loss coefficients and flow rate coefficients obtained by the present experiment and CFD analysis agree well each other. Moreover, internal flow visualization for the valve by CFD analysis and PIV measurement have revealed complicated flow patterns of the internal flow field in detail.

  • To improve the efficiency and lifetime of filters, we analyze the element distribution at the entrance and exit by using the flow inside a filter, confirming that the internal structure affects the filter efficiency. The flow in the pipe is predicted through computer simulations, and the filtration efficiency of each element is compared through experiments. The efficiency and lifetime of the filter are indirectly improved through the element distribution at the filter outlet according to the internal structure. Because pressure loss from the structure inevitably occurs, the efficiency and lifetime of filters against pressure loss must be considered.

A Case Study on the Use of Megestrol Acetate and Korean Medical Treatment for the Loss of Appetite and Weight Loss in Patients with Amyotrophic Lateral Sclerosis(ALS)

  • Objectives : Weight loss and loss of appetite in amyotrophic lateral sclerosis patients are common symptoms, these are associated with survival as well as nutritional condition. The changes in weight and appetite were reported after treatment using Megestrol acetate and Korean medical treatments. Methods : Amyotrophic lateral sclerosis patient with weight loss and loss of appetite were treated by administering megestrol acetate a known anti-malignant tumor agent and Korean medical treatments such as acupuncture, pharmacopuncture and herbal medicine. The changes in weight and appetite were checked using body mass index(BMI) and simplified nutritional appetite questionnaire(SNAQ). Results : There was a more than 17 kg increase in weight. BMI increased from 15.94 $kg/m^2$ to 21.97 $kg/m^2$ and SNAQ score increased from 7 to 16. Owing to several side effects, Megestrol acetate was stopped after which only Korean medical treatments were provided. After which the lasting effects in BMI and appetite could be seen. Conclusion : There are few studies on weight loss and loss of appetite in ALS patients. Using only megestrol acetate can cause several considerable side effects. Which respect to this, the findings in this study could open up new clinical guideline possibilities.

표준담배추출물과 Lipopolysaccharide로 유발한 만성폐쇄성폐질환 동물모델에서 쌍화탕의 폐손상 및 근감소 억제 효과 (Inhibitory Effects of Ssanghwa-tang on Lung Injury and Muscle Loss in a Cigarette Smoke Extract and Lipopolysaccharide-induced Chronic Obstructive Pulmonary Disease Mouse Model)

  • Objectives: This study evaluated the effects of Ssanghwa-tang (SHT) on lung injury and muscle loss in a COPD mouse model. Methods: C57BL/6 mice were challenged with cigarette smoke extract and lipopolysaccharide, and then treated with two concentrations of SHT (250 and 500 mg/kg). After sacrifice, the bronchoalveolar lavage fluid (BALF) or lung tissue was analyzed by cytospin, ELISA, real-time PCR, flow cytometry analysis, and H&E and Masson's trichrome staining. The grip strength of COPD mice was measured using a grip strength meter. The running time of COPD mice was measured by a treadmill test. Muscle tissue of the quadriceps was stained with H&E and Masson's trichrome staining. Results: SHT significantly inhibited the increase in neutrophil numbers in BALF and significantly decreased immune cell activity in BALF and lung tissue. It also significantly inhibited the increase in TNF-α, IL-17, and MIP2 in BALF. Real-time PCR analysis revealed that the mRNA expression of TNF-α, IL-17, MIP2, and TRPV1 in lung tissue showed a significant decrease compared with the control group. Lung tissue damage was significantly reduced in the histological analysis. The grip strength and running time of the COPD mice showed a significant decrease compared with the control group. In histological staining, SHT was found to reduce the damage to muscle tissue. Conclusions: This study indicates that SHT can be used as a therapeutic agent for COPD patients by inhibiting lung injury and muscle loss.

보중익기탕 투여 후 화학항암제로 인한 전신쇠약감 및 탈모호전 증례보고 (A Case Report of Chemotherapy-induced Alopecia Treated with Bojungikki-tang)

  • This case study describes the effect of Bojungikki-tang on chemotherapy-induced alopecia. Alopecia is a well-documented cause of distress to patients undergoing cancer treatment, but no approved pharmacological treatment exists for chemotherapy-induced hair loss. A 70-year-old female diagnosed with a cholangiocarcinoma and liver metastasis received chemotherapy, including gemcitabine and cisplatin, every three weeks. As a result of the continuous chemotherapy, she suffered various toxicity-related side effects, including bone marrow suppression, general weakness, nausea, peripheral numbness, and hair loss. Bojungikki-tang was initially administered to improve the patient's general weakness and fatigue. After three months of treatment, the patient's hair loss and general condition improved, and the color of the new hair was dark, despite the chemotherapy. The treatment did not improve other symptoms, such as bone marrow suppression and peripheral numbness. This case suggests that Bojungikki-tang could have a beneficial effect on chemotherapy-induced alopecia.

터보펌프 인듀서의 내부 유동 해석 (An Interal Flow Analysis of Turbo Pump Inducer)

  • The internal flow in the rocket pump inducer of LE-7 engine for H-II rocket was predicted at design and off-design flow rates using CFD code, CFX- Tascflow. In this numerical study, the performance curve of inducer coressponding to flow rates variation and the internal flow in the front of blade leading edge show good agreement between the calculations and the measurements. Backflow is appeared at suction side of leadinge edge tip, and this region is extended to upstream as flowrate decrease. Because of backflow, pressure loss coressponding to meridinal coordinate occupy 50% from inlet domain to leading edge. By this phenomena, pressure loss in front of blade leading edge take a great effect to inducer performance.

횡류수차 노즐형상이 성능과 내부유동에 미치는 영향 (Effect of Nozzle Shape on the Performance and Internal Flow of a Cross-Flow Hydro Turbine)

  • The purpose of this study is to examine the effect of nozzle shape on the performance and internal flow of a cross-flow hydro turbine. CFD analysis for three kinds of nozzle shape is conducted to simulate the effect of nozzle shape. The results reveal that relatively narrow nozzle width is effective to increase the turbine efficiency and output power. Almost output power is achieved at Stage 1. Therefore, optimum design of the nozzle shape is necessary to improve the turbine performance. Recirculation flow in the runner passage decreases the turbine efficiency and output power because the flow make hydraulic loss and collision loss in the region. Air should be put into the runner passage and the recirculating flow should be suppressed by the air layer in the runner.

Gastric Ulceration and Bleeding with Hemodynamic Instability Caused by an Intragastric Balloon for Weight Loss

  • Obesity in the United States is a medical crisis with many people attempting to lose weight with caloric restriction. Some patients choose minimally invasive weight loss solutions, such as intragastric balloon systems. These balloon systems were approved by the Federal Drug Administration (FDA) in 2015-2016 and have been considered safe, with minimal side effects. We report a patient with a two-day history of melena, abdominal pain, hypotension, and syncope which developed five months after placement of an intragastric balloon. Esophagogastroduodenoscopy with balloon removal revealed a small 8-mm gastric ulcer in the incisura. This gastric ulcer probably developed secondary to mechanical compression of the stomach mucosa by the gastric balloon which contained 900 mL of saline. The FDA is now investigating five deaths since 2016 associated with these second-generation balloons. Clinicians should be aware of these complications when evaluating patients with gastrointestinal complications, such as bleeding.

혈구삼화탕 투약 후 복부팽만이 호전된 기능성 소화불량 환자 증례보고 (A Case Report of Improvement in Abdominal Distension in a Patient with Functional Dyspepsia Using Hyeolgusamhwa-tang)

  • Objective: The aim of this case report was to describe the effect of Hyeolgusamhwa-tang (絜矩三和湯 ) on abdominal distension caused by Qi (氣脹) in a patient with functional dyspepsia. Methods: A female Korean patient was treated with Hyeolgusamhwa-tang and acupuncture for 14 days. We observed objective symptoms, such as bloating, nausea, belching, loss of appetite, and heartburn, and we also measured her abdominal circumference. Results: After treatment, her abdominal circumference was decreased from 87 cm to 83.5 cm, and her food intake was improved from the baseline. The other objective symptoms, like general weakness, loss of appetite, abdominal discomfort, and heartburn, were improved or disappeared without any adverse events reported. However, we could not find any significant differences for some symptoms, such as nausea. Conclusions: This case suggests that Hyeolgusamhwa-tang and acupuncture therapy could be effective in reducing functional distension in patients diagnosed with distension caused by Qi (氣脹).

Treatment Outcomes of Patients with Multidrug-Resistant Tuberculosis: Comparison of Pre- and Post-Public-Private Mix Periods

  • Background: This study compared the treatment outcomes of patients with multidrug-resistant tuberculosis (MDR-TB) before and after the implementation of public-private mix (PPM). Factors affecting treatment success were also investigated. Methods: Data from culture-confirmed pulmonary MDR-TB patients who commenced MDR-TB treatment at Pusan National University Hospital between January 2003 and December 2017 were retrospectively reviewed. Patients were divided into two groups in terms of PPM status: pre-PPM period, patients who commenced MDR-TB treatment between 2003 and 2010; and post-PPM period, patients treated between 2011 and 2017. Results: A total of 176 patients were included (64 and 112 in the pre- and post-PPM periods, respectively). 36.9% of the patients were resistant to a fluoroquinolone or a second-line injectable drug, or both. The overall treatment success rate was 72.7%. The success rate of post-PPM patients was higher than that of pre-PPM patients (79.5% vs. 60.9%, p=0.008). Also, loss to follow-up was lower in the post-PPM period (5.4% vs. 15.6%, p=0.023). In multivariate regression analysis, age ≥65 years, body mass index ≤18.5 kg/m2, previous TB treatment, bilateral lung involvement, and extensively drug-resistant (XDR)- or pre-XDR-TB were associated with poorer treatment outcomes. However, the use of bedaquiline or delamanid for ≥1 month increased the treatment success. Conclusion: The treatment success rate in MDR-TB patients was higher in the post-PPM period than in the pre-PPM period, particularly because of the low rate of loss to follow-up. To ensure comprehensive patient-centered PPM in South Korea, investment and other support must be adequate.
