Comparison of the fit accuracy of zirconia-based prostheses generated by two CAD/CAM systems

  • PURPOSE. The purposes of this study are to evaluate the internal and marginal adaptation of two widely used CAD/CAM systems and to study the effect of porcelain press veneering process on the prosthesis adaptation. MATERIALS AND METHODS. Molar of a lower jaw typodont resin model was prepared by adjusting a 1.0 mm circumferential chamfer, an occlusal reduction of 2.0 mm, and a $5^{\circ}$ convergence angle and was duplicated as an abrasion-resistant master die. The monolithic crowns and copings were fabricated with two different CAD/CAM system-Ceramil and Zirkonzahn systems. Two kinds of non-destructive analysis methods are used in this study. First, weight technique was used to determine the overall fitting accuracy. And, to evaluate internal and marginal fit of specific part, replica technique procedures were performed. RESULTS. The silicone weight for the cement space of monolithic crowns and copings manufactured with Ceramil system was significantly higher than that from Zirkonzahn system. This gap might cause the differences in the silicone weight because the prostheses were manufactured according to the recommendation of each system. Marginal discrepancies of copings made with Ceramil system were between 106 and $117{\mu}m$ and those from Zirkonzahn system were between 111 and $115{\mu}m$. Marginal discrepancies of copings made with Ceramil system were between 101 and $131{\mu}m$ and those from Zirkonzahn system were between 116 and $131{\mu}m$. CONCLUSION. Marginal discrepancy was relatively lower in Ceramil system and internal gap was smaller in Zirkonzahn system. There were significant differences in the internal gap of monolithic crown and coping among the 2 CAD/CAM systems. Marginal discrepancy produced from the 2 CAD/CAM systems were within a reported clinically acceptable range of marginal discrepancy.

  • Purpose: This study was to compare adaptations of lithium disilicate CAD/CAM crowns fabricated with different scanning systems. Methods: This study selected the mandibular right first molar as an abutment for experiments and produced 10 working models. Lithium disilicate crowns appropriate for each abutment were produced by using a CEREC$^{(R)}$ CAD/CAM system. The independent t-test was then used to compare and analyze the data obtained from the two groups(${\alpha}$=0.05). Results: Mean(SD) adaptation were $86.93(22.82){\mu}m$ for the InS group, $88.42(26.77){\mu}m$ for the ExS group. They were no statistically significant differences between groups for adaptation(p>0.05). Conclusion: Within the limitations of this study, the results suggest that the accuracy of all investigated optical scanner were satisfactory for clinical use. Further assessment and improvement of the lithium disilicate ceramic for the fabrication of FPDs is evidently still required.

CAD/CAM 밀링 시스템을 활용한 단일 치관과 3본 교의치의 3D 적합도 평가 (Three-dimensional evaluation of the internal adaptation of single and three-unit fixed dental restoration by CAD/CAM milling system)

  • Purpose: The purpose of this paper was to evaluate the occurrence of errors regarding adaptation by conducting a three-dimensional assessment comparing the bridge type dental restoration after the cutting process, which has multiple abutments, with a single type dental restoration. Methods: By using ten identical files obtained by scanning the master model, thirty designs were created consisting of ten maxillary right first premolars and ten maxillary right first molars with single crown abutments, along with ten bridge designs with the identical abutment. A 5-axis milling machine was used to produce the design file. The produced denture prostheses were scanned using a silicone replica for a STL file. An evaluation was conducted using 3D analysis software on the master model and each of the thirty data files. Results: The RMS value of the pre-molar (14) was $38.4{\pm}4{\mu}m$ for single and $54.7{\pm}6{\mu}m$ for bridge abutment; therefore, a statistically significant difference was observed for single and bridge designs although both shared the same abutment form (P<.05). Also, the RMS value of the molar (16) was $47.6{\pm}2{\mu}m$ and $56.6{\pm}5{\mu}m$ for the single and bridge designs, respectively, thereby presenting a statistically significant difference (P<.05). Conclusion: As a result, dental prosthesis fabricated using the single method presented better internal adaptation outcomes.

Long-Term Tropical Residency Diminishes Central Sudomotor Sensitivities in Male Subjects

  • Tropical natives (TROP) are capable of tolerating tropical heat because of their long-term adaptation to tropical environments. When exposed to heat stress, these natives tend to respond with lower sweat output, which is generally thought to be the result of heat acclimatization. The main objective of this study was to clarify central mechanisms inherent to suppressed thermal sweating in tropical natives (Malaysians) by comparing their sweating responses to those of temperate native (TEMP) (Koreans). This experiment was conducted in a thermoneutral climatic chamber ($24{\pm}0.5^{\circ}C,\;40{\pm}3%$ relative humidity). Heat loads were applied to each subject by the immersion of their lower legs in a hot water bath ($43^{\circ}C$ for 30 min). Sweat onset-time and sweat volume were compared between TROP and TEMP. The sweat onset-times on four selected points on the body ranged from 10.25 to 13.47 min in TEMP subjects, and from 16.24 to 17.83 min in TROP subjects (p<0.001). The local sweat volumes at the same sites ranged from 4.30 to $9.74 mg/cm^2$ in TEMP subjects, and from between 1.80 to $4.40mg/cm^2$ in TROP subjects (p<0.001). These results demonstrated a significant difference between TROP and TEMP subjects with regard to the manner in which they regulate their body temperatures when exposed to heat loads, and verified that long-term thermal adaptation blunts sweating sensitivities.

Validation of the Disaster Adaptation and Resilience Scale for Vulnerable Communities in Vietnam's Coastal Regions

  • Objectives: This study validated the Vietnamese version of the Disaster Adaptation and Resilience Scale (DARS) for use in vulnerable communities in Vietnam. Methods: This was a cross-sectional study involving 595 adults from 2 identified communities. The original DARS assessment tool was translated, and the validity and reliability of the Vietnamese version of DARS (V-DARS) were assessed. The internal consistency of the overall scale and its subscales was evaluated using Cronbach's alpha and McDonald's omega reliability coefficients. Confirmatory factor analysis (CFA) was employed to evaluate its construct validity, building upon the factor structure identified in exploratory factor analysis (EFA). Construct validity was assessed based on convergent and discriminant validity. Results: Following the established criteria for EFA, 8 items were removed, resulting in a refined V-DARS structure comprising 35 items distributed across 5 distinct factors. Both alpha and omega reliability coefficients indicated strong internal consistency for the overall scale (α=0.963, ω=0.963) and for each of the 5 sub-scales (all>0.80). The CFA model also retained the 5-factor structure with 35 items. The model fit indices showed acceptable values (RMSEA: 0.072; CFI: 0.912; TLI: 0.904; chi-square test: <0.01). Additionally, the convergent and discriminant validity of the V-DARS were deemed appropriate and satisfactory for explaining the measurement structure. Conclusions: Our findings suggest that the V-DARS is a valid and reliable scale for use within vulnerable communities in Vietnam to assess adaptive responses to natural disasters. It may also be considered for use in other populations.

자궁경부암 여성의 삶의 적응에 관한 근거이론적 접근 (A Grounded-theory Approach to the Process of Life Adaptation in Women with Cervical Cancer)

  • Purpose: The purpose of this study was to explore and describe the process of life adaptation in women with cervical cancer. Method: A grounded theory method with guided data collection and analysis was used. Fifteen women with cervical cancer who had some kind of treatment at the hospital were asked open-ended and descriptive questions with a guideline. All interviews were tape-recorded and transcribed verbatim. Result: The core category that emerged from the comparative analysis was "overcoming cancer" named as a process of life adaptation in the participants. The process of overcoming cancer evolved three stages - "admitting reality", "attempting health-care" and "continuing health-care". Depending on the paradigm model, the central phenomena of the experiences was "powerlessness". The internal factors motivating women to overcome cancer were "denial of cancer" and "desire for life". Strategies of overcoming cancer included "living as a cancer patient," "selective food eating," "steady exercising," "getting rid of stress," and "preparing for death". The intervening conditions, "supportive system," "forms of life," and "burden on family," influenced overcoming cancer. Conclusion: The results of this study could help clinical nurses to understand life adaptation in cervical cancer patients and establish efficient coping strategies in dealing with the problems they face.

디지털 구강스캐너로 모형 없이 제작한 전부지르코니아 수복물의 변연 및 내면 적합도 평가 (Evaluation of marginal and internal gap under model-free monolithic zirconia restoration fabricated by digital intraoral scanner)

  • 목적: 본 연구의 목적은 실제 환자에서 디지털 구강스캐너로 모형 없이 제작한 보철물의 변연 및 내면 적합도를 평가하는 것이다. 대상 및 방법: 전향적 임상시험으로 시행한 본 예비 연구는 총 11개의 수복물을 대상으로 하였다. 구강스캐너(TRIOS, 3shape, Copenhagen, Denmark)로 디지털 구강인상을 채득한 후, 캐드 디자인 및 밀링 가공 과정을 통해 전부지르코니아 수복물을 제작하였다. 완성된 지르코니아관을 환자 구강 내에 시적하였고, 레플리카 술식으로 보철물-지대치 복제물을 얻었다. 이를 근원심, 협설 방향으로 잘라 변연오차, 변연간극과 축벽부, 선각부, 교합면부의 내면간극을 측정하였다. 통계처리는 Kruskal-Wallis 검정과 Mann-Whitney U 검정을 이용하여 통계적 유의성을 분석하였다(${\alpha}=.05$). 결과: 복제물을 통한 적합도 분석 결과, 근원심, 협설 절편 사이에는 통계적으로 유의한 차이가 없었다(P>.05). 변연간극에 불일치가 있었으며, 변연 오차가 변연간극 보다 컸다(P<.01). 결론: 본 연구의 한계 내에서, 구강스캐너로 모형 없이 제작한 전부지르코니아 수복물의 적합도는 임상적으로 허용할만한 결과를 보였다. 그러나 지르코니아관의 변연부위가 과풍융되는 경향이 있었으므로 주의 깊은 임상 적용 및 추적 연구가 요구된다.

Insect Adaptations to Changing Environments - Temperature and Humidity

  • The most important factors in environment that influence the physiology of insects are temperature and humidity. Insects display a remarkable range of adaptations to changing environments and maintain their internal temperature (thermoregulation) and water content within tolerable limits, despite wide fluctuations in their surroundings. Adaptation is a complex and dynamic state that widely differs in species. Surviving under changing environment in insects depends on dispersal, habitat selection, habitat modification, relationship with ice and water, resistance to cold, diapause and developmental rate, sensitivity to environmental signals and syntheses of variety of cryoprotectant molecules. The mulberry silkworm (Bombyx mori) is very delicate and sensitive to environmental fluctuations and unable to survive naturally because of their domestication since ancient times. Thus, the adaptability to environmental conditions in the silkworm is quite different from those of wild insects. Temperature, humidity, air circulation, gases and photoperiod etc. shows a significant interaction in their effect on the physiology of silkworm depending upon the combination of factors and developmental stage affecting growth, development, productivity and quality of silk. An attempt has been made in this article to briefly discuss adaptation in insects with special emphasis on the role of environmental factors and their fluctuations and its significance in the physiology of mulberry silkworm, B. mori.

Stress Adaptation of Escherichia coli as Monitored via Metabolites by Using Two-Dimensional NMR Spectroscopy

  • Escherichia coli responds to ever-changing external and internal stresses by rapidly adjusting its physiology for better survival. This adjustment occurs at all levels including metabolites as well as mRNAs and proteins. Although there has been many reports describing E. coli's adaptation to various stresses regarding transcriptomics or proteomics, only a few investigations have been reported regarding this adaptation viewed from metabolites' perspective. We applied four different types of stresses at four different doses as imposed by NaCl, sorbitol, ethanol, and pH to investigate the similarities or differences among the stresses, and which stress causes the largest perturbation of the metabolite composition. We profiled the metabolites under such external stresses by using two-dimensional NMR spectroscopy and identified 39 metabolites including amino acids, sugars, organic acids, and nucleic acids. According to our statistical analysis, the osmotic stress caused by sorbitol differentiated itself from others, while NaCl showed the largest dose dependent metabolic perturbations. We hope this work will form a foundation on which an approach to a successful protein production is systematically provided by a favorable metabolic environment by imposing proper external stresses.

한국판 사회적 외모불안 척도(Korean Version of the Social Appearance Anxiety Scale, K-SAAS) 타당화 (Validation of Korean Version of the Social Appearance Anxiety Scale)

  • Objective : To translate and adapt the Social Appearance Anxiety Scale into Korean and validate the Korean version of the social appearance anxiety scale, which measures the fear and anxiety about being negatively evaluated by others based on one's overall appearance, including body shape. Methods : For item translation and adaptation, six bilingual translators participated in the process of forward-adaptation and back-adaptation. Data were collected from undergraduate students. The sample size is 105 for Study 1 and 212 for Study 2. Classical item discrimination and difficulty analyses, exploratory factor analysis (EFA), confirmatory factor analysis (CFA), and reliability analysis were performed. Results : A unidimensional structure was found with a high internal consistency (Cronbach's α=0.95) and a high test-retest reliability (r=0.918). In addition, the concurrent validity was examined by correlations of the scale and several other scales measuring constructs related to social appearance anxiety. Conclusion : K-SAAS appears to be a reliable and valid scale for screening and assessing social appearance anxiety.
