Complications of the retromandibular transparotid approach for low condylar neck and subcondylar fractures: a retrospective study

  • Objectives: The goal of this study was to evaluate the rates of complications, morbidity, and safety with the transparotid approach. Materials and Methods: A retrospective study was conducted and consisted of 53 surgically treated patients in the past five years for low condylar neck and subcondylar fractures. Only patients with malocclusion and who underwent open reduction with internal fixation with the retromandibular transparotid approach were included. The examined parameters were postoperative suboptimal occlusion, deflection, saliva fistula, and facial nerve weakness. Results: Fifty-three patients had an open reduction with internal fixation on 55 sides (41 males, 77.4%; mean age, 42 years [range, 18-72 years]). Four patients (7.5%) experienced transient facial nerve weakness of the marginal mandibular branch, but none was permanent. Four patients had a salivary fistula, and 5 patients showed postoperative malocclusion, where one needed repeat surgery after one year. One patient showed long-term deflection. No other complications were observed. Conclusion: The retromandibular transparotid approach is a safe procedure for open reduction and internal fixation of low condylar neck and subcondylar fractures, and it has minimal complications.

  • The purpose of this research is to develop the management strategy for a take-out specialty sore on the basis of SWOT analysis which requires analysis of internal and external environmental informations. The survey was performed in order to gather the internal and external customer informations for a take-out specialty store. The questionnaires developed were distributed to customers (N=188) and employees (N=23) at 5 take-out specialty store branches from April 25, 2002 to April 29, 2002. The statistical data analysis was completed by SPSS WIN 10.0 for descriptive analysis, factor nalaysis, and Pearson's correlation. IPA(Importance-Performance Analysis) was applied in order to identify the critical management issues which would be explained with the organization's weakness or strength. SWOT analysis was performed through identifying the organizational strength and weakness as internal environment factors and external environmental opportunity and threat as uncontrollable external factors.

Partial spontaneous remission of small cell lung carcinoma with neurologic symptom (신경계 증상을 동반한 부분적으로 자연완화된 소세포폐암)

  • Small cell lung carcinoma (SCLC) is a cancer that shows aggressive behavior, early spread to distant sites, and frequent association with distinct paraneoplastic syndromes. Spontaneous remission of cancer, particularly of SCLC, is a rare biological event. Cases involving spontaneous regression of SCLC were reported, and were associated with paraneoplastic syndromes of the nervous system. This article reports on a 78-year-old man with SCLC in remission, with neurological symptoms. The patient visited the hospital because of generalized weakness, and imaging studies revealed a mass in the lower lobe of the left lung, pathological evaluation showed SCLC. The patient refused oncologic treatment and was treated only with conservative care. In follow-up study the diameter of the mass had decreased from initial 32 mm, 9 months after admission to 20 mm, 17 months after admission to 13 mm. The patient kept complaining of generalized weakness, dizziness, and paresthesia of limbs. We assumed that, in this case, the spontaneous remission of lung cancer was related to the immunologic response directed against the tumor, which is believed to be an important factor in the pathogenesis of paraneoplastic neurologic syndromes.

  • Recently, usual minor stress is getting more important in somatization disorder. This study is a clinical report of a patient with somatization disorder induced by stress treated with typical orintal medical therapy reinforcing the weakness of heart and gall blaader(Herb-med, acupunture, etc.) in combination with EFT. STAI, BDI were compared between before and after treatment. The results show the typical onntal medical therapy reinforcing the weakness of heart and gall blaader in combination with EFT is efficient in the treatment of Somatization disorder.

Characteristics of Atopic Dermatitis Patients Based on Personal Health Records (PHR) according to Age and Sasang Constitution (연령과 사상체질에 따른 개인건강기록(PHR)에 기반한 아토피피부염 환자의 특성)

  • Objectives The purpose of this study is to identify the characteristics of atopic dermatitis patients based on the personal health records (PHR) according to age and Sasang Constitution, and to find basic data for daily life management using the personal health record. Methods We analyzed the characteristics of atopic dermatitis patients based on PHR (symptoms, emotional state, SCORAD Index, five viscera weakness questionnaire, etc.), and Sasang Constitution collected for 6 weeks in 34 patients (or guardians). Results Each item of the SCORAD Index score in children and adolescents showed that children and adolescents have significantly higher edema than adults. However, repeated measurement of the SCORAD Index showed a significant positive improvement over time regardless of age and constitution. In terms of the symptoms of atopic dermatitis, there was a significant correlation between age and swelling, quality of life and effusion, pruritus (itch) and sleep disturbance in relation to the general characteristics of the subject. In the five visceral weakness scores, adults had significantly higher spleen weakness, liver weakness, kidney weakness scores than in children and adolescents. All results showed a high tendency to have a high lung weakness score, which was more pronounced in children and adolescents. Conclusion This study is meaningful to confirm the correlation between SCORAD Index items related to the symptoms of atopic dermatitis, and to identify trends for specific items by ages and Sasang Constitution.

  • Dermatomyositis is a rare and idiopathic inflammatory myopathy with a characteristic cutaneous manifestation. A 62-year-old female complained of polyarthralgia that lasted for many years. She was diagnosed with hypomyopathic dermatomyositis by the typical skin rash associated with dermatomyositis but without muscle involvement such as muscle weakness, elevated level of creatinine phosphokinase and aldolase. Her symptoms improved with treatment of hydroxychloroquine and prednisolone. We experienced a case of hypomyopathic dermatomyositis on 62-year-old female patient and report with review of literatures.

Case of Medullary Hemorrhage with Cavernous Angioma Treated with Oriental Medicine (해면혈관종과 병발한 연수출혈의 한방치료 1례)

  • Cavernous angiomas of the medulla oblongata are rare vascular malformations. We report a case of cavernous angioma with medullary hemorrhage showing symptoms of diplopia and weakness of right extremities. We treated a patient with herb medicine, acupuncture and moxibustion. This study suggests that oriental medicine therapy has a significant effect on the symptoms of medullary hemorrhage.

  • Exfoliative dermatitis is a name of a group of symptoms which is characterized by red rash, itching and scale in almost whole body. The patients are suffered from hypoalbuminemia, general weakness and fever sign in addition to characteristic skin lesions. So in the treatment of this disease, adequate symptomatic treatment is important with adequate topical skin management. We experienced a 67 year old Korean male patient diagnosed as exfoliative dermatitis based on clinical symptoms. We concluded that the symptoms of this patient are Sun-symptoms(癬症) and gave him Sunpunghwangi-san(宣風換肌散) and Chungdae-san(靑黛散). After taking herbal medicine, clinical improvement was noticed. Based on this experience, both internal and external use of oriental medicine have a good effect on exfoliative dermatitis.

  • Objective : The study was to investigate Diagnosis and Treatment Based on an Overall Analysis of Signs and Symtoms(證治) of a patient on stomach cancer stage IV by means of the clinical symptoms. Methods : The observation of the clinical progress was carried out by conducting Diagnosis and Treatment Based on an Overall Analysis of Signs and Symtoms(證治) with the patient diagnosed stomach cancer stage IV. Results : Treatments such as the invigoration of qi(補氣), the flow of qi(行氣), and the relieving pain(止痛) was given because the patient showed the qi deficiency of the spleen and stomach(脾胃氣虛). There were moderate effects for anorexia, indigestion, nausea, and general weakness, but there was not any clear effect for alleviation of abdominal pain except the first period. Specially, compared with two hospitalization treatments in 1998, the third hospitalization treatment did not show any apparent improvement. It was believed that this caused by the patients bodily weakness because of deterioration of anemia from bleeding in the progress of cancer. Conclusion : Diagnosis and treatment based on an overall analysis of signs and symtoms(證治) of a patient on stomach cancer stage IV had moderate effects on the improvement of the patients condition, but in this case we had difficulty in long-term observation because of short hospitalizations or insufficient examination by an oriental-western combined medicine group.

A Case Report of Non-cardiac Chest Pain in a Non-erosive Reflux Disease Patient Treated with Beewha-eum (비화음으로 호전된 비미란성 역류질환(Non-erosive Reflux Disease) 환자의 비심인성 흉통(Non-cardiac chest pain) 치험 1례)

  • Objective: The purpose of this study was to report the effectiveness of Korean medicine (Beewha-eum) on the treatment of non-cardiac chest pain (NCCP) in a patient with non-erosive reflux disease (NERD). Methods: The patient was diagnosed with a spleen-stomach weakness pattern identified by Korean medicine and was treated with herbal medicine (Beewha-eum). The severity of symptoms was assessed with a numerical rating scale (NRS) for chest pain, self-reported dyspepsia degree (%), Korean gastrointestinal symptom rating scale (KGSRS), gastrointestinal symptom score (GIS), and Functional Dyspepsia-Quality of Life(FD-QOL) score. Results: After Beewha-eum treatment, the severity of chest pain was decreased from NRS 8 to NRS 0 and the self-reported dyspepsia degree also decreased from 100% to 65%. The KGSRS score was decreased from 49 to 35, the GIS score was also decreased from 16 to 9, and the FD-QOL score was increased from 20 to 25. Conclusions: The study findings suggested that Korean medical treatment with Beewha-eum could be an effective option for treating NCCP in patients with NERD.
