Interferon-γ 투여가 쥐에서의 Bleomycin 유도 폐 섬유화에 미치는 영향 (The Effect of Interferon-γ on Bleomycin Induced Pulmonary Fibrosis in the Rat)

  • 연구배경 : 폐 섬유화증의 진행에는 세포외 간질의 대사에 관여하는 gelatinase가 중요한 역할을 하는 것으로 알려져 있으며, gelatinase의 작용은 사이토카인을 비롯한 여러 가지 요소에 의해 조절되는 것으로 알려져 있다. 지금까지 $interferon-{\gamma}$($IFN-{\gamma}$)는 폐섬유화를 억제하는 것으로 알려져 있지만 gelatinase의 작용에 $IFN-{\gamma}$가 미치는 영향은 잘 밝혀져 있지 않다. 이에 저자들은 bleomycin 유도 백서 폐 섬유화 모델에서 $IFN-{\gamma}$가 폐 섬유화에 미치는 영향과MMP-2, -9과 이의 길항제인 TIMP-1, -2 그리고 Th-2 사이토카인의 변화에 미치는 영향을 조사함으로써, $IFN-{\gamma}$가 폐 섬유화증에 미치는 영향과 기전을 연구하고자 하였다. 재료 및 방법 : Sprague-Dawley계의 수컷 흰쥐를 정상 대조군, bleomycin 군, bleomycin+$IFN-{\gamma}$ 군의 세 군으로 나누어, bleomycin 군 과 bleomycin+$IFN-{\gamma}$ 군은 bleomycin sulfate를 생리식염수에 섞어 기관 내로 주입하였고(6 U/kg), bleomycin+$IFN-{\gamma}$ 군은 $IFN-{\gamma}$를 14일간 근육주사 하였다($2{\times}105U/kg$). 실험동물은 3, 7, 14, 28일에 폐를 얻어 hematoxylineosin 염색과 Masson's trichrome 염색을 하여 폐의 염증 반응과 섬유화 정도를 관찰하였고, 폐 조직 내의 hydroxyproline 함량, MMP-2, -9, TIMP-1, -2에 대한 Western blot, zymography와 reverse zymography, IL-4와 IL-13에 대한 ELISA검사를 시행하였다. 결 과 : 1. 폐 염증은 bleomycin 투여 7일째에는 $IFN-{\gamma}$를 투여한 경우 더 심하게 일어났으나(bleomycin 군 : bleomycin+$IFN-{\gamma}$ 군=$2.08{\pm}0.15:2.74{\pm}0.29$, P<0.05), 폐 섬유화는 bleomycin 투여 후 28일째 $IFN-{\gamma}$ 투여에 의해 유의하게 감소되었다(bleomycin 군 : bleomycin+$IFN-{\gamma}$ 군=$3.94{\pm}0.43:2.46{\pm}0.13$, P<0.05). 2. 폐 내 hydroxyproline 함량은 bleomycin 투여 28일 후 $IFN-{\gamma}$ 투여에 의해 유의하게 감소되었다(bleomycin 군 : bleomycin+$IFN-{\gamma}$ 군=$294.04{\pm}31.73{\mu}g/g:194.92{\pm}15.51{\mu}g/g$, P<0.05). 3. Bleomycin 투여에 의해 MMP-2 단백질의 양이 증가되는 소견이 Western blot에서 관찰되었는데 MMP-2의 증가는 bleomycin 투여 14일에 최고에 이르렀으며, $IFN-{\gamma}$의 투여에 의해 bleomycin에 의한 MMP-2의 증가는 억제되었으나 대조군 수준으로 감소하지는 않았다. 4. Zymography 검사에서 bleomycin 투여 3일째에는 활성화된 형태의 MMP-2가 $IFN-{\gamma}$에 의해 유의하게 증가되었으나(bleomycin 군 : bleomycin+$IFN-{\gamma}$ 군=$209.63{\pm}7.60%:407.66{\pm}85.34%$, P<0.05), 투여 14일 후에서는 $IFN-{\gamma}$에 의해 활성화된 상태의 MMP-2가 유의하게 감소되었다(bleomycin 군 : bleomycin+$IFN-{\gamma}$ 군=$159.36{\pm}20.93%:97.23{\pm}12.50%$, P<0.05). 5. Bleomycin을 투여한 후 bleomycin 군과 bleomycin+$IFN-{\gamma}$ 군 모두에서 IL-4의 양이 감소되었으나, 두 군간 차이는 통계적으로 유의하지는 않았고, IL-13은 별 다른 차이가 없었다. 결 론 : Bleomycin에 의한 백서 폐 섬유화 모델에서 $IFN-{\gamma}$는 초기 폐 염증을 증가시키지만 후기 폐 섬유화는 억제시킨다는 것을 알 수 있었다. $IFN-{\gamma}$가 폐 섬유화를 억제하는 것은 MMP-2의 활성화를 억제하기 때문인 것으로 생각한다. 또한 $IFN-{\gamma}$에 의한 MMP-2의 기능 억제는 Th-2 사이토카인을 억제하여 이루어지는 것 같지는 않았다. 본 논문을 기초로 향후 $IFN-{\gamma}$가 MMP-2의 활성화를 조절하는 기전과 MMP-2의 활성화가 폐섬유화에 미치는 영향에 대한 연구가 필요할 것으로 생각한다.

실험적 폐색전증에서 조직형플라스미노겐활성체의 투여방법이 혈액응고기전에 미치는 영향 (Effects of the Dosing Regimen of Tissue-type Plasminogen Activator on Blood Coagulation System in Experimental Pulmonary Embolism)

  • 연구배경 : 조직형플라스미노겐활성체는 생리적인 플라스미노겐활성체이므로 혈전의 섬유소에 결합된 플라스미노겐만을 활성화시키는 특이성이 있다고 알려져 있지만, 폐색전증에서 치료목적으로 대량투여하는 경우에도 섬유소에 대한 특이성이 유지되어 전신적인 출혈경향을 유발하지 않는가에 대해서는 논란이 있다. 또한 동량의 약제를 투여하더라도 조직형플라스미노겐활성체의 투여시간에 따라 혈전의 용해효과가 달라진다고 보고되었지만, 이러한 경우에 혈액의 응고기전이 어떠한 영향을 받는지는 명확하지 않아 이를 규명해보고자 본 연구를 시행하였다. 방법 : 실험견에 자가혈병으로 대량의 폐색전증을 유발시킨후 대조군은 특이치료없이, $t-PA_{15}$ 군은 15분동안, 그리고 $t-PA_{180}$군은 3시간에 걸쳐, 조직형플라스미노겐활성체를 체중(kg)당 1mg씩 정맥투여하였다. 그리고 실험과정중 시간에 따라, 실험동물의 혈색소치, 적혈구용적, 백혈구수, 혈소판수, 프로트롬빈시간, 활성부분트롬보플라스틴시간, 섬유소원농도, 트롬빈시간, 플라스미노겐 및 ${\alpha}_2$-항플라스민의 농도, 제 V 및 VIII응고인자의 농도, 그리고 섬유소원분해산물의 농도를 측정하였다. 결과: 1) 일반혈액검사소견근 세 군간에 유의한 차이가 없었다. 그러나 실험과정중 혈색소치와 적혈구용적 및 혈소판수는 감소하고, 백혈구수는 계속 증가하는 경향을 보였다. 2) 프로트롬빈시간, 활성부분트롬보플라스틴시간 및 트롬빈시간은 $t-PA_{15}$군이 치료후 15분~60분 사이에, $t-PA_{180}$군은 치료후 30분~180분 사이에 연장되었다가 정상으로 회복되었다. 3) 섬유소원농도 ${\alpha}_2$-항플라스민의 농도, 제 V 및 VIII응고인자의 농도는 $t-PA_{15}$군 및 $t-PA_{180}$ 군에서 모두 감소하였으나 $t-PA_{15}$군은 $t-PA_{180}$군에 비해 조기에 회복되는 양상을 보였고 플라스미노겐의 농도는 급격히 증가하였다가 회복되는 양상을 보였다. 4) 섬유소원분해산물의 농도는 폐색전후 증가하였고, $t-PA_{15}$군 및 $t-PA_{180}$ 군에서는 조직형플라스미노겐활성체 투여후 더욱 증가하였는데 $t-PA_{180}$군에서 더 현저하였다. 결론 : 폐색전증에서 치료목적으로 대량의 조직형플라스미노겐활성체를 투여하는 경우, 약제의 투여시간이 180분일때보다는 투여시간이 15분일때 출혈이 발생할 가능성이 적다. 따라서 동량의 조직형플라스미노겐활성체를 투여하더라도 약제의 투여시간을 달리함으로써 출혈의 부작용을 줄일 수 있을 것이다.

왕느릅나무 껍질 열수 추출물의 마우스에서의 in vivo 면역조절 효과 (Immune-modulation Effect of Ulmus macrocarpa Hance Water Extract on Balb/c Mice)

  • 약용식물은 염증, 감염증, 암과 같은 질병에 광범위하게 사용되고 있으며, 전통적으로 오래 동안 사용되어 왔다. 왕느릅나무(Ulmus macrocarpa Hance)는 느릅나무의 한 종으로서 중국, 일본을 비롯해 한국 전역에 분포하고 있으며, 줄기와 뿌리의 껍질을 유백피라 하여 한의학에서 피부염, 유방염, 부종 등에 사용하여 왔다. 이 연구의 목적은 유백피의 열수 추출물(Ulmus cortex water extract, UCWE)이 면역조절 기능을 가지고 있는지를 조사하는 것이다. UCWE 식이농도 30 mg/kg, 100 mg/kg, 300 mg/kg의 3개 군으로 나누어 14일 동안 식이한 후 면역반응을 측정하였다. IL-2, IL-12, IFN-${\gamma}$ 의 혈중 농도가 UCWE를 식이한 군에서 유의적으로 증가하였으며, lymphokine activated killer cells (LAK)을 이용한 세포매개 세포독성 시험에서도 정상군에 비해 UCWE를 식이한 군에서 유의적으로 증가하였다. 그러나, 간, 신장, 비장, 흉선의 무게변화는 UCWE를 식이한 군과 정상군과의 차이가 없었다. 이는 300 mg/kg 고농도의 2주간 식이가 장기에 영향을 거의 미치지 않는다는 것을 암시한다고 생각된다. 따라서, UCWE는 내부 장기에는 영향을 주지 않으며 면역기능을 향상시키는 효과를 가진다고 여겨진다.

식육단백질과 친수성 콜로이드의 상호결합 특성을 이용한 저지방 육제품 제조기술 개발 - II 모델연구결과를 이용한 저지방 소시지의 개발 (Development of Low-fat Meat Processing Technology using Interaction between Meat Proteins and Hydrocolloids-II Development of Low-fat Sausages Using the Results of Model Study)

  • 본 연구는 유화형 소시지와 유사한 물성을 갖는 저지방 소시지를 제조하기 위하여 실시하였다. 이전의 모델연구결과를 통해 얻은 최적조건을 저지방 육제품 제조에 적용하였으며 제조한 저지방 소시지의 pH는 6.29~6.34, 수분 74~76%, 지방<3% 및 단백질 15~18%인 반면, 유화형 소시지는 pH 6.51, 수분 56%, 지방 26.9% 및 단백질은 13.2%이었다. 친수성 콜로이드를 단독으로 첨가한 저지방 처리구는 무침가구와 비교할 때 가열수율, 보수력 및 색도에 영 향을 미치지 않았으나, CN을 첨가한 처리구의 보수력은 유화형 대조구와 KF 및 LBG를 각각 단독으로 첨가한 처리구에 비하여 오히려 낮았다. 물성특성에서는 저지 방 대조구와 CN을 첨가한 저지방 처리구가 유화형대조구보다 탄력성 및 응집성이 높았고, KF와 LBG를 각각 첨가한 처리구는 부서짐성과 경도 및 탄력성이 낮았다. KF, CN 및 LBG의 2종 내지 3종을 복합으로 첨가하여 저지방 소시지를 제조하였을 때 유화형 대조구에 비하여는 보수력은 떨어졌으나, KF와 LBG(KLL) 및 KF와 CN과 LBG(KCL)를 복합으로 첨가한 처리구는 단독 첨가구에 비하여 보수력이 증진되었다. 물성적인 특성에 있어서는KF와 LBG를 혼합하여 첨가한 처리구를 제외하고 다른 혼합 첨가구는 유화형 대조구와 비교적 유사한 물성적인 성상을 가졌으며 그 중 KF와 CN의 복합처리구가 유화형 대조구와 가장 유사한 물성을 가졌다. 이상의 결과에서 볼 때 기존의 유화형 대조구와 유사한 물성을 갖는 저지방 소시지를 개발하기 위하여 친수성 콜로이드의 단일첨가보다는 보수력이 좋은 KF 혹은 LBG등과 경도를 높여 주는 CN과의 복합지방대체제의 첨가가 더욱 효율적임을 알 수 있었다.

소규모 지역간 결핵관리사업 성과에 대한 시계열분석 - 전국 234개 시.군.구 보건소의 환자등록자료를 중심으로 - (Time Series Analysis on Outcomes of Tuberculosis Control and Prevention Program between Small Areas in Korea - with Patient Registry Data of 234 City.County.District Public Health Centers -)

  • 연구배경 : 이 연구는 우리나라 각 기초자치단체에서 발행하는 "통계연보" 등을 활용하여 결핵관리사업의 성과를 결핵예방사업, 환자발견 및 지속관리사업, 그리고 결핵 치료사업 측면에서 소규모 지역간 사계열분석 모형에 의해 연도별 인구천명당 결핵등록률 및 결핵 치료순응도 등의 성과지표로 산출하여 비교분석하고 이에 영향을 미치는 요인을 규명하고자 하였다. 방법 : 전국 234개 시 군 구 기초자치단체별로 발간된 "통계연보"나 광역자치단체별 "통계연보"를 원주시청 행정자료실, 강원도청 행정자료실, 통계청 민원실 등을 방문, 1980년도부터 2000년도까지 순차적으로 열람하여 결핵관리사업등 관련 정보를 지난 1년여동안 추적 조사하였다. 결핵관리사업의 성과지표인 연도별 결핵유병률 등을 산정, 이를 종속변수로 하여 다중 희귀모형을 구축하여 Micro TSP와 SAS 패키지를 이용하여 확률론적안 시계열분석을 하였다. 결과 : 1998년까지의 지역별 현황을 비교하면, 인구지표 중 인구이동률을 제외한 나머지 변수들의 경우 모두 구지역, 시지역, 군지역의 순으로 유의하게 감소하였다. 사회 경제학적 지표로는 인구천명당 자동차 등록대수, 지방세, 담배소비세 모두 시 군 구지역간에 유의한 차이를 보였다. 또한 보건의료지표는 시 군 구지역간 인구천명당 의사수 및 병상수 모두 통계적으로 유의하게 지역 규모의 크기에 따라 감소하였다. 지역별 관련 결핵지표의 시계열 분포의 변화를 비교하면, BCG접종률의 경우 1980년부터 1984년까지 약간의 증가 추세를 보이다가 그 이후 1993년도까지는 일정한 평형 수준 (plateau state) 을 유지한 이후 다시 감소추세를 보였다. 1985년 이전에는 시지역이, 1985년 이후에는 군지역의 접종률이 타지역에 비해 유의하게 높았다. 폐결핵양성자수 시점유병률, 폐결핵 음성자수 시점유병률 및 결핵사망률의 분포를 보면, 세가지 결핵지표 모두 1981년 이후 지속적으로 감소하는 영향을 보여주되, 군지역이 가장 높았고 시간이 경과할수록 시 군 구지역간 차이도 점차 줄어들었다. 이는 지역 특성상 민간의료기관의 분포가 적은 군 지역에서의 결핵등록 및 관리사업이 상대적으로 시 구(광역도시)지역보다 보건소 중심으로 용이하게 운영되어지고 있음을 시사한다. 결핵 치료순응도의 경우 기간유병률이나 사망률과는 반대로 1982년 이후 점차 증가하는 추세였다. 결론 : 이 연구결과는 현재 보건소 중심으로 관리되어 매년 집계, 보고, 발간되는 시 군 구 기초자치단체별 "통계연보"의 결핵보건사업 결과정보에 대한 이용의 실증연구에 해당된다. 따라서 지난 제7차까지 시행해 오던 전국결핵실태조사 대신에 향후 시행될 결핵정보 감시체계 운영등의 기초자료로서 가능성을 제공해주고 있다. 결국 이를 바탕으로 결핵관리사업과 관련 지역 특성에 맞는 지역보건의료계획 수립의 기초자료 및 정부의 국가결핵보건사업 기획의 근거를 제공함으로써 결핵관리 대책 수립시 우선순위 결정과 예산배분, 기대효과 산정에 필수적인 정보로 활용되기를 기대한다.

사례기반 추론기법과 인공신경망을 이용한 서비스 수요예측 프레임워크 (A Hybrid Forecasting Framework based on Case-based Reasoning and Artificial Neural Network)

  • 제조업에 있어서 판매 후 서비스 건수와 내용 등은 향후 서비스 제공을 위한 자원배분의 효율성 증진과 서비스 품질 향상을 위해서도 매우 중요한 정보이다. 따라서 기업들은 향후 발생하는 판매 후 서비스에 대해 정확히 예측하고 그에 따라 적절히 대처하는 능력을 확보할 필요성이 제조업을 중심으로 증가하고 있다. 그러나 실제로 이들 기업들이 활용하고 있는 서비스 수요예측 방법들은 전통적인 통계적인 예측기법이거나, 시뮬레이션을 기반한 기법들이다. 예를 들면, 전통적인 통계적인 예측기법으로는 회귀분석(regression analysis)의 경우, 다양한 제품모델에 대한 판매 후 서비스 발생 패턴이 선형적인 관계가 매우 적음에도 불구하고 선형으로 가정하여 추정한다는 점과 적정한 회귀식을 가정하여야 되며, 이러한 가정이 실제 경영환경에서는 매우 어렵다는 점 등이 기존의 예측기법들의 한계점으로 지적되고 있다. 본 연구에서는 디지털 TV 모델을 생산 판매 하는 A사의 사례연구를 통하여 최근 인공지능연구에서 각광을 받고 있는 사례기반추론(case-based reasoning; CBR) 기법을 활용한 서비스 수요예측 프레임워크를 제안하고자 한다. 또한, 사례기반추론에서 핵심적인 역할 중 하나인 유사 사례추출 방법에 있어서 가장 일반적인 nearest-neighbor 방법 이외의 유사 사례추출 방법을 제안하고자 한다. 특히, 본 연구에서 제안하는 유사 사례추출 방법은 인공신경망(artificial neural network)을 활용한 자기조직화지도(Self-Organizing Maps : SOM) 군집화 기법을 활용한 유사 사례추출 방식으로 이를 활용한 서비스 수요예측 프레임워크에 구현하고, 실제 기업의 판매 후 서비스 데이터를 활용하여 본 연구에서 제안하는 서비스 수요 예측 프레임워크의 유효성을 실증적으로 검증하고자 한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. 레닌 억제제가 실험적 사구체 콩팥염에 미치는 효과 - 예비 보고 (Effect of renin inhibition on an experimental glomerulonephritis - a preliminary report)

  • 목 적 : 급성 사구체 콩팥염 실험동물을 이용하여 레닌 억제제를 일정기간 투여한 후 정상군과의 비교를 통해 그 효과를 평가함으로써 임상연구 및 치료를 위해 유용한 지식을 제공하고자 이 연구를 시행하였다. 방 법 : 총 30마리의 재태연령 6주, 수컷 BALB/c 생쥐를 정상군(n=5), 정상치료군(n=5), 질병군(n=10), 질병치료군(n=10) 네 집단으로 분류하고 질병은 항사구체 기저막 항체를 주입하여 유발하였다. 질병유발과 동시에 레닌억제제를 osmotic mini pumps를 통해 피하로 투여하였고 질병유발 14일째 쥐를 도살하여 각 군별로 소변의 단백/크레아티닌 비와 각 개체의 사구체의 메산지움의 확장 정도를 점수화하여 비교하였다. 결 과 : 질병 유발 6일째 질병군에서 한 마리, 7일째 질병치료군중 한 마리가 사망하였다. 10일째 질병군 개체들에서 급성 병색이 나타나 각 군별로 질병 유발 10일째 소변의 단백/크레아티닌 비를 산출하였고 도살 후 떼어 낸 콩팥 조직에서 사구체의 메산지움의 확장 정도를 평가하였다. 소변의 단백/크레아티닌 비가 같은 군에 속한 다른 개체들과의 값보다 현저한 차이를 보인 질병군 한 개체를 제외한 후 측정한 결과 정상군은 $31.24{\pm}6.54mg/mg$, 정상치료군은 $23.38{\pm}13.60mg/mg$, 질병군은 $112.72{\pm}10.97mg/mg$, 질병치료군은 $114.07{\pm}32.30mg/mg$으로 질병군에서 의미 있는 소변 단백/크레아티닌 비의 상승을 확인할 수 있었고(P<0.05) 서로 다른 군들 사이에서는 차이를 나타내지 않았다. 메산지움의 확장 정도는 정상군은 $0.23{\pm}0.10$, 정상치료군은 $0.13{\pm}0.03$, 질병군은 $1.90{\pm}0.48$, 질병치료군은 $1.28{\pm}0.41$로 정상군과 질병군 사이에 유의한 차이가 있는 것으로 나타났고(P<0.05), 질병군과 질병치료군 사이에서도 P=0.008로 유의한 차이를 나타내었다. 결 론 : 급성 사구체 콩팥염 실험동물을 이용한 레닌 억제의 효과를 알아 본 이번 실험에서 소변의 단백/크레아티닌 비에서 차이는 뚜렷하지 않았지만 메산지움의 확장 정도를 평가한 병리학적 소견에 있어서는 의미 있는 차이를 확인 할 수 있었다.

유무선 통합 망에서 대량호 지능망 서비스의 성능 및 자원 최적화를 위한 동적 큐 관리자 (A Dynamic Queue Manager for Optimizing the Resource and Performance of Mass-call based IN Services in Joint Wired and Wireless Networks)

  • 본 논문은 유무선 통합망 환경하에서 대량호에 근거한 지능망 서비스의 호 완료율을 증가시키고 자원을 최적화하기 위한 정보 흐름 및 총괄서비스 로직을 설계한다. 이 로직을 구현하기 위하여 지능망 서비스제거에 호대기 서비스 특징을 적용하기 위한 동적 큐 관리자를 제시하는데, 이것을 유선 가입자뿐만 아니라 이동 단말을 소유한 무선 가입자에게도 적용하기위해 홈위치등록기와 서비스제어기간에 해당 서비스 가입자들의 이동성을 큐 관리자에게 통보하기 위한 서비스 등록 여부 프래그를 관리한다. 따라서, 이동 서비스 가입자가 로밍함에 따른 동적 서비스 그룹핑을 수행하고 M/M/c/K 큐잉 모델에 근거한 큐 길이를 동적으로 관리하여 자원을 최적화 할 수 있는 동적 큐 관리 메커니즘을 제안한다. 동적 큐 관리자에 의해 할당된 큐 길이를 결정하기 위해 서비스 증가율에 따른 가입자 단말 수와 실패율간의 관계 및 그에 따른 큐에서의 적정 대기 시간을 시뮬레이션 및 분석한다. 또한, 동적 큐 관리자를 구성하는 내부 서비스 로직과 SIB들간의 상호동작 및 자료 구조를 설계 및 구현한다.
