급성호흡부전 호전 후 기관지폐포세척에서 지방과립구가 확인된 지방색전증후군 1예 (A Case of Fat Embolism Syndrome of Fat Globules Found in BAL Fluid after Recovery from Acute Respiratory Failure)

  • 지방색전증후군은 외상 등의 원인으로 발생한 지방색전에 의해 유발되는 전신 질환으로 급성호흡부전 및 의식변화를 초래할 수 있다. 저자들은 평소 질병이 없던 22세 남자로 교통사고에 의한 대퇴골 간부 및 다발성 골절을 입고 개인병원에서 외부고정장치 수술 받은 후 다음 날부터 호흡곤란과 의식저하 보여 본원에 전원된 환자에서, 증상 발현 당시는 급성호흡부전증을 동반한 지방색전증후군으로 추정 진단하고, 증상 호전 후 시행한 기관지 폐포세척액에서 대식세포내 지방과립구를 확인하여 보고한다. 급성호흡부전증의 급성기에 기관지내시경을 시행하는 것은 호흡부전의 원인 감별에 도움이 되지만 때로 환자에게 위험을 초래할 수도 있으므로, 임상적으로 지방색전증후군이 추정되는 환자에서 회복기에 기관지폐포세척검사를 시행하는 것은 확진에 도움이 될 수 있으리라 추정된다.

단순 골낭종에서 발생한 병적 골절 (Pathologic Fracture of Unicameral Bone Cyst)

  • 목적: 단순 골낭종의 병적 골절 환자를 대상으로 단순 골낭종의 관해에 미치는 예후 인자에 대하여 알아보고자 하였다. 대상 및 방법: 2001년부터 2010년까지 본원에 내원한 단순 골낭종을 동반한 병적 골절 환자 13명을 대상으로 하였으며, 평균 추시 기간은 26개월(3-90개월)이었다. 성별은 남자 11명, 여자 2명이었으며, 평균 나이는 10.2세(6-16세)였다. 발생 부위는 근위 상완골 9명, 상완골 간부 2명, 근위 대퇴골 1명, 근위 경골 1명이었다. 단순 골낭종의 병적 골절에 대하여 보존적 치료로 단순 석고 고정 후 골절 유합을 시도하였으며, 1명은 일차적으로 골이식 및 관헐적 정복술과 내고정술을 시행하였다. 5명의 환자에서 치료 경과 중 스테로이드 주사를 시행하였으며, 2명에서 골이식을 시행하였다. 병적 골절 치료 중의 단순 골낭종의 변화 및 연령, 낭종의 크기, 성장판 침범 정도에 따른 단순 골낭종의 예후 인자에 대해 분석하였다. 결과: 병적 골절 후 전례에서 평균 8.2개월에 골유합을 얻었으며, 13명의 환자 중 4명(31%)에서 골낭종의 완전 관해를 보였다. 발병 나이에 따른 단순 골낭종 관해의 통계적 차이는 없었지만(p=0.42), 낭종의 크기가 클수록, 성장판에 근접한 경우 관해를 얻기가 어려웠다(p=0.05, p=0.03). 결론: 단순 골낭종의 병적 골절에서 관해가 이루어질 수 있으나, 낭종의 크기가 크거나 성장판에 근접한 경우에는 좀 더 적극적인 치료가 필요할 것으로 보인다.

요수근 관절의 골절-탈구에서 단요월상인대의 손상 형태에 따른 치료 결과의 비교 (Comparison of the Outcomes according to the Injury Type of the Short Radiolunate Ligament in Fracture-Dislocation of the Radiocarpal Joint)

  • 목적: 요수근 관절 탈구는 고에너지 손상에 의해 발생하며 요골 경상돌기 골절 및 단요월상인대 손상을 동반한다. 단요월상인대는 요골 부착부에서 파열되기도 하지만 요골 월상골와의 전방연에서 견열 골편을 동반하여 발생하기도 한다. 이 연구의 목적은 요골 경상돌기 골절이 동반된 요수근 관절 탈구에서 단요월상인대의 손상 형태 및 치료 방법에 따라 결과의 차이가 있는지 알아보고자 한다. 대상 및 방법: 요수근 관절 탈구로 수술을 시행한 18명을 대상으로 하였다. 요수근 관절 탈구는 Dumontier 등의 방법을 이용하여 Group 1 (순순한 탈구 또는 요골의 작은 견열 골절)과 Group 2 (주상골와의 1/3 이상을 침범한 요골 경상돌기 골절)로 분류하였다. Group 2는 단요월상인대의 부착부가 파열되거나 작은 견열 골절을 동반한 경우를 2A, 비교적 큰 견열 골절을 동반하여 내고정이 가능한 경우를 2B로 분류하였다. 전자는 단요월상인대의 직접 봉합으로, 후자는 작은 나사를 이용한 견열 골편의 내고정으로 치료하였다. 최종 추시에서 통증, 관절 운동 범위와 파악력, 영상 검사를 확인하였다. 치료 결과는 patient-rated wrist evaluation(PRWE), modified Mayo wrist score (MMWS)를 이용하여 평가하였다. 결과: 모든 증례는 Group 2 (2A 6명, 2B 12명)로 분류되었다. 굴신 운동 범위는 건측의 79%, 파악력은 72.9%를 보였다. Group 2A가 2B보다 신전/굴곡/회전에서 더 큰 운동 범위를 보였다. 요사위/척사위/회외전과 통증, 파악력 회복은 차이는 없었다. PRWE, MMWS에서는 두 군의 차이가 없었다. 합병증으로 외상성 관절염 7예, 관절 불안정 5예가 있었다. 결론: 단요월상인대가 손상될 때 요골 월상골와의 전방연에서 발생한 견열 골절은 치료 결과에 영향이 없었다. 그러나 견열 골편이 전위되거나 회전되어 탈구의 정복을 방해하기 때문에 주의해야 하며, 골편에 부착하는 단요월상인대의 기능을 복원하기 위해 해부학적 정복과 견고한 내고정이 필요하다.

관절면을 침범한 경골 외측 고평부 골절에 대한 관절경을 이용한 정복 및 내고정술 (Arthroscopically-Assisted Reduction and Internal Fixation of Intra-Articular Fractures of the Lateral Tibial Plateau)

  • 목적: 경골 외측 고평부 골절에서 추가적인 피질골 창을 내지 않고 관절경을 이용한 정복술 및 골이식술 없이 내고정한 후 이에 따른 결과를 평가해 보았다. 대상 및 방법: 2009년 3월부터 2017년 3월까지 3차원 컴퓨터 단층촬영(computed tomography, CT)에서 관절면의 함몰 및 전위가 5 mm 이상이면서 Schatzker 분류상 II형에 해당하는 경골 외측 고평부 골절에 대해 관절경을 이용하여 수술적 치료를 하고 최소 18개월 이상 추시가 가능한 27예를 대상으로 하였다. 경골 피질골에 추가적인 창을 내지 않고 전외측 연골판하방 삽입구(inframeniscal portal)를 통해 골막거상기(freer elevator)를 이용하여 감입되어 있는 골편을 정복하여 관절면의 정복을 얻은 후 골이식 없이 5.0 mm 혹은 6.5 mm 유관나사를 이용하여 내고정술을 시행하였다. 결과 판정은 임상적 결과와 방사선적 결과로 나누어 Rasmussen의 방법을 이용하여 분석하였고 유관나사 제거술을 받은 13예의 환자에 한해서 관절경을 이용하여 2차 추시를 시행하였다. 결과: 최종 추시에서 모든 예에서 골유합을 얻었고, 평균 골유합 기간은 8.7주(8-12주)였다. Rasmussen 기준에 따라 방사선적 결과는 25예(92.6%)에서 우수 또는 양호의 만족스러운 결과를 얻었고 임상적 결과는 24예(88.9%)에서 우수 또는 양호의 결과를 얻었다. 2차 추시를 시행한 환자 13예에서 섬유연골로 잘 치유됨을 확인하였고 술 후 외래 추시 중 CT 촬영한 8예에서 골결손 부위 골이식 없이 유합된 소견을 확인하였다. 결론: 경골 외측 고평부 골절에서 추가적인 피질골 창을 내지 않고 관절경을 이용한 정복 및 골이식 없이 시행한 내고정술은 관절면을 정확히 정복할 수 있는 유용한 방법이며 임상적으로 양호한 결과를 얻을 수 있는 방법으로 생각된다.

구내 접근법에 의한 하악골 골절 치료에 대한 임상적 고찰 (A Clinical Review on the Transoral Approach to the Fractures of The Mandible)

  • The is a retrospective study on the transoral approach to open reduction of the Mandibular fractures. Our study was based on a series of 64 patients with mandibular fractures among 99 patients of facial bone fractures who had been treated by transoral approach with or without extraoral approach at Department of Oral and Maxillofacial Surgery, Yonsei Medical Center, Yonsei University from January 1981 to October 1988. We studied favorite sites of open reduction, fixation methods, results and prognosis related to transoral approaches of Mandibular fractures, and which compared with extraoral approaches. The results obtained are as follows : 1. The transoral open reduction was used more frequently in Mandibular fractures(64.6%) than Midfacial bone fractures(35.4%). Among 64 patients of mandibular fractures, 47 patients(73.4%) were treated only by transoral approach and others(26.6%) were treated by both trans- and extra-oral approach. Among 92 sites of mandibular fractures, 75 sites(81.5%) were treated by transoral approach and 17(18.5%) were treated by extraoral approach. 2. The most favorite site for transoral approach compared with extraoral approach was Symphysis(100%), and Angle(62.5%) was next in order of frequency on Mandibular fractures. 3. Direct Interosseous Wiring(DIW) was most commonly used for fixation(64.6%) and Miniplate osteosynthesis was used next in 28.1%. 4. Simple(39.1%) and Compound(52.2%) fractures were frequently indicated for transoral approach, however comminuted fractures were rarely indicated. 5. The direction of fracture lines on Angle of the mandible did not influence to determine whether transoral approach should be selected or not. However this area seemed to be more difficult to reduce exactly by transoral procedure than other areas because simultaneous superior and inferior fixation was applied predominantly on this area. 6. The success rate of reduction and fixation analyzed from us was more excellent in Direct Interosseous Wiring(29/53=54.7%) than in Rigid Internal Fixation(9/29=31.0%). But it might be depended upon various factors as like as sugeon's skill. 7. The postoperative complication due to transoral open reduction of mandible was not high(12.0%) and this rate was similar with other published reports.

거골 경부 골절 이후 발생한 부분적인 골괴사의 3차원 부피 분석 (Three-Dimensional Volume Analysis of Partial Avascular Necrosis after Talar Neck Fracture)

  • Purpose: The purpose of this study is to define the geographic patterns of partial avascular necrosis (AVN) of the talar body and to determine whether there were any predictors of both the location and occurrence of partial AVN. Materials and Methods: Nineteen patients with fracture of the talar neck treated by open reduction and internal fixation and followed up for more than 1 year were analyzed. The radiographs were examined 6 to 8 weeks after the operation for Hawkins sign and if it was not observed, magnetic resonance scans were performed. The three-dimensional analysis was performed using Mimics 17.0 (Materialise). The incidence of collapse and time to operative intervention was recorded. Results: Partial AVN of the talar body was observed in six out of 19 patients. The avascular segment of the talar body was located predominantly in the anterolateral portion. The average volume of the avascular segment was $289mm^3$, and it occupied 1% of total volume of the talus, and 10% of the talar dome. Collapse occurred in one patient in the area of the avascular process. There were no observable trends with regard to Hawkins classification, incidence of collapse, or time to operative intervention to the location of the avascular segment. Conclusion: Partial AVN can occur after fracture of the talar neck. The predominant location of the avascular segment was the anterolateral portion of the talar body. This information may be helpful to understanding the process of avascular necrosis of the talar body.

전위된 쇄골간부 골절에서 보존적 치료의 결과 (Results of the Closed Treatment of Displaced Middle-Third Fractures of The Clavicle)

  • Fractures of the clavicles are extremely common, accounting for 5 to 10% of all injuries in the adult skeletal trauma. Despite this, nonunions and symptomatic malunions after clavicular fracture are uncommon. When these complications occur, however, they may be associated with 11 pattern of disability that includes not only pain but also impairment of upper extremity function. The purpose of this study was to clarify the outcome of clavicular fractures treated by conservatively, and emphasize treatment and factors that possibly predispose a clavicular fracture to poor function or nonunion. We evaluated 9] consecutive fractures of the middle-third of the clavicle in adults which had been treated conservatively. There were 59 males and 33 females ranging in age from 17 to 80 years (average 36.8 years). Nine of the 91 fractures (10%) had developed nonunion, and 4 fractures (4.4%) reported unsatisfactory results. No patients had significant impairment of range of motion or strength as a result of the injury. We found that initial shortening at the fracture of≥20mm(6 cases) had a highly significantly association with nonunion(P<0.0001). Final shortening of 20mm or more (4 cases) was associated with an unsatisfactory result, but not with nonunion(P<0.0001). Our results have led us to recommend the open reduction and internal fixation of severely displaced fractures of the middle third of the clavicle in adults patients.

전위된 관절 내 종골 골절의 Ollier 접근법을 이용한 치료 (Treatment of Displaced Intra-articular Calcaneal Fracture using Ollier Approach)

  • Purpose: We evaluated the clinical results of the intraarticular calcaneal fractures treated using Ollier approach by inexperienced orthopaedic surgeon. Materials and Methods: Between August 2003 and May 2007, Of the total 46 cases, 12 cases (9 patients) of displaced intraarticular calcaneal fracture who underwent open reduction and internal fixation using Ollier approach were evaluated. The means of age was 50.5 years. According to the Sanders classification, there was no type I case and 8 cases of type II, 1 case of type III, and 3 cases of type IV. We evaluated the treatment result by assessing radiologic parameters ($B{\ddot{o}}hler$ angle, Gissane angle, and calcaneal height/width) and clinical outcomes (VAS and AOFAS score). Results: The means of follow-up period was 25.3 months. The means of B.hler angle was improved from $2.4^{\circ}$ to $26.1^{\circ}$. Radiologic and clinical union was achieved in all cases without additional procedures. Excellent result were noted in 2 cases, good in 5 cases, fair in 4 cases, and poor in 1 case. We experienced 2 cases of minor complications; 1 case of mild wound infection and 1 case of hypoesthesia on foot dorsum. Radiologic findings of subtalar arthritis were present in 2 cases. Conclusion: Ollier approach seems to be helpful to inexperienced orthopaedic surgeons for the treatment of intraarticular calcaneal fractures in that it enables them to achieve considerable clinical outcomes without serious complications.

스노보드 손상 환자에 있어서 손목 손상의 특성 (Characteristics of Wrist Injuries in Snowboarding)

  • Purpose: The purpose of this study was to analyze the characteristics and severity of wrist injuries in snowboarding. Methods: December 2005 to February 2008, Snowboarders who experienced wrist injures were included in this study. On the basis of the medical records and radiographic evaluation, the severity of distal radius fracture was classified according to the Arbeitsgemeinschaft fur Osteosynthesefragen/Association for the Study of Internal Fixation (AO/ASIF) classification. Results: Most of the injured snowboarders were a either of the beginner (35 cases, 46.1%) or the intermediate (27 cases, 35.5%) level. The most common cause of injury in snowboarding was a slip down (60 cases, 78.9%). Comminuted and articular fractures classified as AO types A3, B, and C, which required surgical reduction, made up 42.3% of the distal radial fractures in snowboarders. When we analyzed the differences in severity between the educated and the non-educated groups, an A2 type injury in the AO classification was the most common type of injury in the educated group (20 cases, 38.5%), it means less severe fractures ocurred in the educated group (p=0.045). The most frequent injury mechanism of fractures was slip down (48 cases, 63.2%), and a slip down backwards was the dominant type of slip down (36 cases, 75.0%) (p=0.031). Conclusion: Among the snowboarders in this study who suffered self-down injury to the wrist, more fractures were associated with a backwards slip down than with a forward slip down due to over extension. For educated snowboarders the severity of fracture was lower than it was for uneducated snowboarders.

Inferomedially impacted zygomatic fracture reduction by reverse vector using an intraoral approach with Kirschner wire

  • Background In inferomedially rotated zygomatic fractures sticking in the maxillary sinus, it is often difficult to achieve complete reduction only by conventional intraoral reduction. We present a new intraoral reduction technique using a Kirschner wire and its clinical outcome. Methods Among 39 inferomedially impacted zygomatic fractures incompletely reduced by a simple intraoral reduction trial with a bone elevator, a Kirschner wire (1.5 mm) was vertically inserted from the zygomatic body to the lateral orbital rim in 17 inferior-dominant rotation fractures and horizontally inserted to the zygomatic arch in nine medial-dominant and 13 bidirectional rotation fractures. A Kirschner wire was held with a wire holder and lifted in the superolateral or anterolateral direction for reduction. Following reduction of the zygomaticomaxillary fracture, internal fixation was performed. Results Fractures were completely reduced using only an intraoral approach with Kirschner wire reduction in 33 cases and through an additional lower lid or transconjunctival incision in six cases. There were no surgical complications except in one patient with undercorrection. Postoperative 6-month computed tomography scans showed complete bone union and excellent bone alignment. Four patients experienced difficulty with upper lip elevation; however, these problems spontaneously resolved after manual tissue lump massage and intralesional steroid (Triamcinolone) injection. Conclusions We completely reduced infraorbital rim fractures, zygomaticomaxillary buttresses, and zygomaticofrontal suture fractures in 84% of patients through an intraoral approach alone. Intraoral Kirschner wire reduction may be a useful option by which to obtain effective and powerful reduction motion of an inferomedially rotated zygomatic body.
