An Effects of Radiation Dose Assessment for Radiation Workers and the Member of Public from Main Radionuclides at Nuclear Power Plants (원전에서 발생하는 주요 방사성핵종들이 방사선작업종사자와 원전 주변주민의 피폭방사선량 평가에 미치는 영향)

  • In a primary system at nuclear power plants (NPPs), various radionuclides including fission products and corrosion products are generated due to the complex water conditions. Particularly, 3H,14C,58Co,60Co,137Cs,and131I3H,14C,58Co,60Co,137Cs,and131I are important radionuclides in respect of dose assessment for radiation workers and management of radioactive effluents. In this paper, the dominant contributors of radiation exposure for radiation workers and the member of public adjacent to NPPs were reviewed and the process of dose assessment attributable to those contributors were introduced. Furthermore, the analysis for some examples of radiation exposure to radiation workers and the public during the NPP operation was carried out. This analysis included the notable precedents of internal radiation exposure and contamination of demineralized water occurred in Korean NPPs. Particularly, the potential issue about the dose assessment of tritium and carbon-14 was also reviewed in this paper.

  • Background: The International Commission on Radiological Protection (ICRP) recommendations and the Federal Guidance Report (FGR) published by the U.S. Environmental Protection Agency (EPA) have been widely applied worldwide in the fields of radiation protection and dose assessment. The dose conversion coefficients of the ICRP and FGR are widely used for assessing exposure doses. However, before the coefficients are used, the user must thoroughly understand the derivation process of the coefficients to ensure that they are used appropriately in the evaluation. Materials and Methods: The ICRP provides recommendations to regulatory and advisory agencies, mainly in the form of guidance on the fundamental principles on which appropriate radiological protection can be based. The FGR provides federal and state agencies with technical information to assist their implementation of radiation protection programs for the U.S. population. The system of radiation dose assessment and dose conversion coefficients in the ICRP and FGR is reviewed in this study. Results and Discussion: A thorough understanding of their background is essential for the proper use of dose conversion coefficients. The FGR dose assessment system was strongly influenced by the ICRP and the U.S. National Council on Radiation Protection and Measurements (NCRP), and is hence consistent with those recommendations. Moreover, the ICRP and FGR both used the scientific data reported by Biological Effects of Ionizing Radiation (BEIR) and United Nations Scientific Committee on the Effects of Atomic Radiation (UNSCEAR) as their primary source of information. The difference between the ICRP and FGR lies in the fact that the ICRP utilized information regarding a population of diverse races, whereas the FGR utilized data on the American population, as its goal was to provide guidelines for radiological protection in the US. Conclusion: The contents of this study are expected to be utilized as basic research material in the areas of radiation protection and dose assessment.

Factors Associated with Beliefs among Adults in Korea about the Health Effects of Thirdhand Smoke on Children (한국 성인의 3차 흡연으로 인한 아이의 건강 유해성 인식과 관련 요인)

  • Objectives: Thirdhand smoke (THS) refers to residual tobacco smoke pollutants that remain after cigarette smoking. The purpose of this study was to determine predictors of the belief among adults in Korea that THS is harmful to children. Methods: A population-based sample of 3,000 adults (1919 years) among a web-based panel in Korea was obtained in November 2016. The respondents filled out questionnaires, including THS beliefs, smoking status, presence of voluntary smoke-free rules at home, and support for the implementation of smoke-free regulations at home. THS beliefs were assessed with a statement that staying in a room today where people smoked yesterday can harm the health of children. Results: Overall, 92.2 % of adults believed that THS exposure could harm the health of children. Respondents who were nonsmokers, believed that SHS exposure were associated with pneumonia of children, lived in home with complete smoke-free rule at home, and supported the implementation of smoke-free regulation at home were more likely to believe that THS could harm for health of children. Conclusions: The majority of Korean adults believed that THS exposure could harm the health of children. Predictors identified in the study can be useful to address the risk of THS beliefs and promote implementation of tobacco control policies.

A Study for the Safety on the Flame Exposure of the Propane Cylinder (소형 프로판 용기의 화염 노출에 대한 안전성 연구)

  • To evaluate the safety of propane cylinder, the flame test was performed by the flame exposure scenario of propane cylinder. The cylinder which was exposed in a flame was rapidly occurred to increase the internal pressure by liquid expansion, if so it cause of physical explosion. Therefore, the cylinder which was applied with thermal pressure relief device sholud be not bursted and the propane should be discharged to outside safely. The flame average temperature that was around of cylinder is 600C600C, and then it increased 700C700C by discharged propane. The result of flame test, the cylinder was deformed, but it was not bursted. The regulations of flame exposure test for propane cylinder were not restricted, this paper can be applied to restrict the flame test if the cylinder is possible to expose the flame. Also, the results is expected as reference for estimation of the pressure cylinder performance.

  • The trend of x-ray image sensor has been evolved from an amorphous silicon sensor to a crystal silicon sensor. A crystal silicon X-ray sensor, meaning a X-ray CIS (CMOS image sensor), is consisted of three transistors (Trs), i.e., a Reset Transistor, a Source Follower and a Select Transistor, and a photodiode. They are highly sensitive to radiation exposure. As the frequency of exposure to radiation increases, the quality of the imaging device dramatically decreases. The most well known effects of a X-ray CIS due to the radiation damage are increments in the reset voltage and dark currents. In this study, a pixel array of a X-ray CIS was made of 20×20pixels20×20pixels and this pixel array was exposed to a high radiation dose. The radiation source was Co-60 and the total radiation dose was increased from 1 to 9 kGy with a step of 1 kGy. We irradiated the small pixel array to get the increments data of the reset voltage and the dark currents. Also, we simulated the radiation effects of the pixel by MCNP (Monte Carlo N-Particle) simulation. From the comparison of actual data and simulation data, the most affected location could be determined and the cause of the increments of the reset voltage and dark current could be found.

  • Hereditary hemorrhagic telangiectasia(Osler-Rendu-Weber Syndrome) is characterized by telangiectasia of the skin and mucous membranes and intermittent bleeding from vascular abnormalities. About 20% of patients with this syndrome have pulmonary arteriovenous fistulas. Pulmonary arteriovenous fistula is uncommon malformation which has an abnormal connection between the pulmonary capillary bed, in which venous blood in the pulmonary artery is shunted through the fistula into the pulmonary vein without exposure to alveolar oxygen and result in unoxygenated, desaturated systemic arterial blood, polycythemia, cyanosis and clubbing. Death often results from cerebral abscess and rupture of the malformation with massive hemorrhage. Therapeutic intervention is recommended for all symptomatic patients because of the risk of those serious complications. Treatment options include surgery and transcatheter obliteration with steel coils or detachable balloons. Therapeutic embolization has the advantages that multiple bilateral pulmonary arteriovenous fistulas can be occluded and also that the procedure can be repeated if necessary. Recently we experienced a case of the multiple bilateral pulmonary arteriovenous fistulas associated with telangiectatic change of hepatic artery and multiple angiodysplasia on the gastric mucosa in 41 years old female patient who had mild dyspnea of exertion(NYHA class II). clubbing finger, severe iron deficiency anemia. She was treated with embolization technique using steel coils and iron replacement. After the therapeutic embolization. significant improvement of dyspnea of exertion with disappearance of multiple pulmonary nodule on follow-up simple chest x-ray was noted. During the subsequent six months follow-up period, she had the improvement of symptoms and iron deficiency anemia.

Radial Probe Endobronchial Ultrasound Using Guide Sheath-Guided Transbronchial Lung Biopsy in Peripheral Pulmonary Lesions without Fluoroscopy

  • Background: Radial probe endobronchial ultrasound-guided transbronchial lung biopsy (RP-EBUS-TBLB) has improved the diagnostic yield of bronchoscopic biopsy of peripheral pulmonary lesions (PPLs). The diagnostic yield and complications of RP-EBUS-TBLB for PPLs vary depending on the technique, such as using a guide sheath (GS) or fluoroscopy. In this study, we investigated the utility of RP-EBUS-TBLB using a GS without fluoroscopy for diagnosing PPLs. Methods: We retrospectively reviewed data from 607 patients who underwent RP-EBUS of PPLs from January 2019 to July 2020. TBLB was performed using RP-EBUS with a GS without fluoroscopy. The diagnostic yield and complications were assessed. Multivariable logistic regression analyses were used to identify factors affecting the diagnostic yields. Results: The overall diagnostic accuracy was 76.1% (462/607). In multivariable analyses, the size of the lesion (≥20 mm; odds ratio [OR], 2.06; 95% confidence interval [CI], 1.27-3.33; p=0.003), positive bronchus sign in chest computed tomography (OR, 2.30; 95% CI, 1.40-3.78; p=0.001), a solid lesion (OR, 2.40; 95% CI, 1.31-4.41; p=0.005), and an EBUS image with the probe within the lesion (OR, 6.98; 95% CI, 4.38-11.12; p<0.001) were associated with diagnostic success. Pneumothorax occurred in 2.0% (12/607) of cases and chest tube insertion was required in 0.5% (3/607) of patients. Conclusion: RP-EBUS-TBLB using a GS without fluoroscopy is a highly accurate diagnostic method in diagnosing PPLs that does not involve radiation exposure and has acceptable complication rates.

Antiproliferative Activity of Piceamycin by Regulating Alpha-Actinin-4 in Gemcitabine-Resistant Pancreatic Cancer Cells

  • Although gemcitabine-based regimens are widely used as an effective treatment for pancreatic cancer, acquired resistance to gemcitabine has become an increasingly common problem. Therefore, a novel therapeutic strategy to treat gemcitabine-resistant pancreatic cancer is urgently required. Piceamycin has been reported to exhibit antiproliferative activity against various cancer cells; however, its underlying molecular mechanism for anticancer activity in pancreatic cancer cells remains unexplored. Therefore, the present study evaluated the antiproliferation activity of piceamycin in a gemcitabine-resistant pancreatic cancer cell line and patient-derived pancreatic cancer organoids. Piceamycin effectively inhibited the proliferation and suppressed the expression of alpha-actinin-4, a gene that plays a pivotal role in tumorigenesis and metastasis of various cancers, in gemcitabine-resistant cells. Long-term exposure to piceamycin induced cell cycle arrest at the G0/G1 phase and caused apoptosis. Piceamycin also inhibited the invasion and migration of gemcitabine-resistant cells by modulating focal adhesion and epithelial-mesenchymal transition biomarkers. Moreover, the combination of piceamycin and gemcitabine exhibited a synergistic antiproliferative activity in gemcitabine-resistant cells. Piceamycin also effectively inhibited patient-derived pancreatic cancer organoid growth and induced apoptosis in the organoids. Taken together, these findings demonstrate that piceamycin may be an effective agent for overcoming gemcitabine resistance in pancreatic cancer.

Development of an On Site Diagnostic Tool to Detect Neuropsychiatric Impairment due to Chronic Organic Solvent Exposure (만성유기용제 폭로에 의한 정신신경학적 이상소견의 현장진단방법 개발에 관한 연구)

  • A study has been conducted on developing questionnaires to serve as on site diagnostic tools for the early detection of neuropsychiatric impairment among workers chronically exposed to low-level organic solvents. Two drafts of tentative questionnaires have been developed as follows; several question items were selected from questionnaires which were administered to workers exposed to organic solvents in previous studios and were grouped into each symptom category based on the presence of its association using Guttman scaling method, then these selected items were reviewed by neuropsychiatry specialists. The final draft of the questionnaire (total symptom score=36) was developed by selecting 33 question items which had more than a 0.88 Guttman coefficient of reproducibility in each symptom category from a pilot study in which these tentative questionnaires were administered to workers manufacturing soles. Three plants using organic solvents and one plant never using organic solvents as a control group were selected to test the reliability and validity of the developed questionnaires. The major organic solvent in the workplace environment detected by a personal air sampler and GC/MSD nab toluene. The concentration of toluene in air from the department using organic solvent was statistically different from that of the department never using organic solvent. The concentration of toluene from almost all of the workplace did not exceed the allowable level. There was no statistically significant difference between the concentration of urinary hippuric acid from the workers of the department using organic solvent and that of the department never using it. Total symptom score of the plant never using organic solvents was 9.8 and those of the three plants using organic solvents were 15.6, 14.7, and 13,7 respectively. In order to evaluate the validity of the questionnaires, the workers from two different departments of the plant in which usage of organic solvents are totally dofferemt were compared. The total symptom score was 17.8 for workers of the department using organic solvent and 13.5 for the department never using organic solvent and scores of each symptom group between exposure and non-exposure group also showed statistically significant difference. The finding that total symptom score of the exposure group increased as the duration of employment increased indirectly suggests the usefulness of the developed questionnaire to assess the health effects of chronic exposure to organic solvents. The correlation coefficient, which was calculated to evaluate the test-retest reliability, was 0.581 (p=0.001). The coefficient of Crohnbach which reflects the internal consistency of the questionnaire was 0.91. In conclusion, the reliabilty of the questionnaire was well maintained over the time lapse between the two administrations of the questionnaire and despite the seasonal difference.

  • A 6-month-old orange winged Amazon parrot (Amazona amazonica) was presented for evaluation of weight loss, anorexia, and abdominal distension for two months. Clinical and laboratory examinations revealed evidence of hepatic failure, including transudative ascites, increased serum serum aspartate aminotransferase and bile acids, and decreased albumin-to-globulin ratio. Hepatic fibrosis with bile duct hyperplasia and chronic hepatitis was diagnosed by liver biopsy. The cause of this condition remained unclear, but exposure to hepatotoxins was suggested.
