Objectives: Oral and maxillofacial surgeons must gain mastery of various approaches to the midface due to the increasing incidence, complexity, and severity of presenting midfacial fractures. Unlike in the case of other body parts, the need to preserve facial aesthetics makes it more difficult for the surgeon to select an approach for managing the facial injuries. The midfacial degloving (MFD) approach is a combination of intraoral and intranasal incisions made to access the midface without any external incision. The aim of the present study was to evaluate the efficacy of MFD in maxillofacial surgery and to assess its advantages and complications. Materials and Methods: The MFD approach was used in five cases, with three cases treated with open reduction and internal fixation and two cases operated on for posttraumatic deformity. Nasal dorsum augmentation was completed in three cases and nasal osteotomy was performed in one case. The bicoronal flap technique was combined with MFD for frontal bone augmentation in one case. The intraoperative time required for flap completion and the ease of performing the planned procedures were noted. Postoperative evaluation was done for reduction, aesthetics, function, and complications. Results: Access was excellent for performing all planned procedures. Average time spent for flap elevation and exposure of the midface was 63 minutes. Complications like postoperative swelling, infraorbital nerve paresthesia, and intranasal crusting were all transient. No long-term complications like stenosis of the nose, sneer deformity, or weakness of the facial muscles were noticed. Additionally, no complications were noted when MFD was combined with bicoronal flap. Conclusion: Though the MFD approach is technically demanding and takes more time than other facial approaches, it should be learned and applied by maxillofacial surgeons in selective cases, as it provides complete exposure of the midface without facial scarring.

As the number of single-person households increases, the consumption of bottled water is increasing. In addition, as the public's interest in radioactivity increases, interest in the field of living radioactivity is also increasing. Since drinking water is an essential element in our daily life, it must be safe from radioactivity. In this study, gamma radiation of drinking spring water was measured and internal exposure dose evaluation was performed to determine its harmfulness. K-40 and uranium-based radioactivity analysis was performed through a high-purity germanium detector, and as a result, drinking water was detected somewhat higher than that of mixing water. Since there is no regulation on the natural radioactivity concentration in Korea, it was compared with the U.S. Environmental Protection Agency Drinking Water Regulations and World Health Organization standard. As a result, there were some items that exceeded standards. Internal exposure was evaluated according to the effective dose formula of ICRP 119. As the result was derived that a maximum of 1.17 mSv per year could be received. This result means that the dose limit for the general public may be exceeded, and it was judged that it is necessary to set an appropriate standard value and present a recommendation value through continuous monitoring in the future.
Background: Food consumption is one of the most important routes for radionuclide intake for the public; therefore, there is the need to have a comprehensive understanding of the amount of radioactivity in food products. Consumption of radionuclide-contaminated food could increase potential health risks associated with exposure to radiation such as cancers. The present study aims to determine radioactivity levels in some food products (milk, rice, sugar, and wheat flour) consumed in Mali and to evaluate the radiological effect on the public health from these radionuclides. Materials and Methods: The health impact due to ingestion of radionuclides from these foods was evaluated by the determination of activity concentration of radionuclides 238U, 232Th, 40K, and 137Cs using gamma spectrometry system with high-purity germanium detector and radiological hazards index in 16 samples collected in some markets, mall, and shops of Bamako-Mali. Results and Discussion: The average activity concentrations were 9.8±0.6 Bq/kg for 238U, 8.7±0.5 Bq/kg for 232Th, 162.9±7.9 Bq/kg for 40K, and 0.0035±0.0005 Bq/kg for 137Cs. The mean values of radiological hazard parameters such as annual committed effective dose, internal hazard index, and risk assessment from this work were within the dose criteria limits given by international organizations (International Commission on Radiological Protection and United Nations Scientific Committee on the Effects of Atomic Radiation) and national standards. Conclusion: The results show low public exposure to radioactivity and associated radiological impact on public health. Nevertheless, this study stipulates vital data for future research and regulatory authorities in Mali.

The Kori-Unit 1 nuclear power plant, which is scheduled to be decommissioned after permanent shutdown, is expected to generate large amounts of various types of radioactive waste during the decommissioning process. Among these, nuclear reactors and internal structures have high levels of radioactivity and the dismantled structure must have the proper size and weight on the primary side. During decommissioning, it is important to prepare an appropriate and efficient disposal method through analysis of the disposal status and the legal restrictions on wastes generated from the reactors and internal structures. Nuclear reactors and internal structures generate radioactive wastes of various levels, such as medium, very low, and clearance. A radiation evaluation indicates that wastes in the clearance level are generated in the reactor head and upper head insulation. In this study, a clearance waste safety evaluation was conducted using the RESRAD-RECYCLE code, which is a safety evaluation code, based on the activation evaluation results for the clearance level wastes. The clearance scenario for the target radioactive waste was selected and the maximum individual and collective exposure doses at the time of clearance were calculated to determine whether the clearance criteria limit prescribed by the Nuclear Safety Act was satisfied. The evaluation results indicated that the doses were significantly low, and the clearance criteria were satisfied. Based on the safety assessment results, an appropriate metal recycle and disposal method were suggested for clearance, which are the subject of the deregulation of internal structures of nuclear power plant.
Objectives : This study investigated the biochemical mechanisms of anti-proliferative and pro-apoptotic effects of the water extract of Dan-Seon-Tang (DST) in human leukemia U937 cells. Methods : U937 cells were exposed to DST and growth inhibition was measured by MTT assay. Results : Exposure of U937 cells to DST resulted in the growth inhibition in a concentration-dependent manner. This inhibitory effect was associated with morphological changes and apoptotic cell death such as formation of apoptotic bodies, increased populations of apoptotic-sub G1 phase and induction of DNA fragmentation. The induction of apoptotic cell death in U937 cells by DST was associated with up-regulation of death receptor 4 (DR4) and down-regulation of Bid, surviving and cellular inhibition of apoptosis protein-2 (cIAP-2) expression. DST treatment also induced the proteolytic activation of caspase-3, caspase-8 and caspase-9, and a concomitant degradation of caspase-3 substrate proteins such as poly (ADP-ribose) polymerase (PARP), phospholipase (PLC)-γ1, β-catenin and DNA fragmentation factor 45/inhibotor of caspase activated DNAse (DFF45/ICAD). Furthermore, apoptotic cell death by DST was significantly inhibited by caspase-3 specific inhibitor z-DEVD-fmk, demonstrating the important role of caspase-3. Conclusions : These findings suggest that herb prescription DST may be a potential chemotherapeutic agent for the control of human leukemia U937 cells; further study is needed to identify the active compounds.
Background and Objective : Allergy is defined as an altered reactivity to an antigen, which can result in pathologic reactions upon subsequent exposure to that particular antigen. This study was to evaluate the effect of oriental materia medica on IL-5 secretion in the mouse spleen cell. Methods : We used the splenocytes of mouse 8 weeks after its birth, and then cultivated those into the 2 experimental groups and a control group for 48 hours. The culture media of the experimental groups were made of 1μg/ml, 10μg/ml oriental materia medica, representative. And the culture media of control group was given no oriental materia medica. Then, we assayed the quantity of cytokine-expression by the sandwich ELISA. The quantities of cytokine-expression of the experimental groups were compared with that of the control group, which was standardized. These methods were used for all of the oriental materia medica treated. Results : Some oriental materia medica inhibit the secretion of IL-5 in both 1μg/ml and 10μg/ml culture media. These were Acori Rhizoma, Luffae Fasciculus Vascularis, Amomi Rotundi Fructus, Schisandrae Fructus, Biotae Semen, Clematis armandii, Dioscoreae Sativa Rhizoma, Coicis Semen, Sophorae Flos, Oroxyli Semen, Aurantii Semen, Pini Nodi Lignum. Conclusion : This study indicates that some oriental materia medica inhibit the secretion of IL-5 and are beneficial for allergic disease.
Ganciclovir resistance of human cytomegalovirus is associated with mutations in the viral UL97 gene and poses severe problems for immunocompromised patients. In this study, PCR-based restriction fragment length polymorphism and sequencing analyses detected the UL97 D605E mutation in all five clinical isolates from patients with ganciclovir-resistant human cytomegalovirus infection during prolonged ganciclovir therapy, whereas the M460V mutation was only present in 1 of 5 isolates. On the other hand, the detection rates of the D605E mutation in the stored available DNA samples from the donor and allogeneic stem cell transplantation recipients were 66.7% and 93.7%, respectively, suggesting that the presence of D605E mutation was not associated with the ganciclovir exposure. Although the D605E mutation may not be related to ganciclovir resistance, we suggest that this mutation could be an important molecular marker of human cytomegalovirus evolution in East Asian countries. Moreover, the restriction fragment length polymorphism method using the restriction enzyme HaeIII, which is generally used to detect the UL97 A591V mutation, could also detect the D605E mutation and may therefore be a useful tool for future research on the investigation of UL97 gene mutations.

Insulin autoimmune syndrome (IAS) is characterized by spontaneous hypoglycemia, extremely high serum insulin levels, and high titers of autoantibodies against endogenous insulin, in the absence of exogenous insulin injection. IAS often occurs following exposure to sulfhydryl-containing drugs, including alpha-lipoic acid (ALA). A 30-year-old woman without diabetes visited our outpatient clinic with recurrent hypoglycemia. She had been taken ALA for weight reduction since 3 weeks ago. Further hypoglycemia work up revealed very high insulin levels, C-Peptide levels and positive insulin antibodies. And conventional imaging examinations were negative for insulinoma or other pancreatic tumors. Finally, the diagnosis of Insulin autoimmune syndrome (IAS) was made. Following the cessation of ALA, hypoglycemia improved, with no medication, and the patient experienced no further hypoglycemic attacks over the next month. The use of ALA as a nutritional supplement is increasing. We report a case of IAS associated with ALA in a non-diabetic patient.
Background: Some studies have indicated an inverse relationship between cancer risk and sunlight exposure. Others have reported that the prognosis of some cancers such as prostate, colon, ovarian and non melanoma skin cancer, were affected by the season in which the cancer was diagnosed. In our study, we evaluated whether season is prognostic in Turkish patients with breast cancer. Materials and Methods: A total of 517 patients from Kayseri Training and Research Hospital were analysed retrospectively. Patients were divided into 4 groups according to season of cancer diagnosis: winter, spring, summer and autumn. The prognostic factors for disease free survival and overall survival were investigated. Results: No significant differences were found among groups regarding prognostic factors overall. Only estrogen receptor status and lymphovascular invasion were independent prognostic factors (p=0.001 and p=0.001 respectively). We found significantly differences for mean disease free survival among groups (p=0.019). Winter group had better mean DFS while summer group had worse DFS. Mean overall survival was similar in the four groups (p=0.637). Conclusions: The season is not an independent predictive factor. However, due to interaction with other factors, we think that the season of cancer diagnosis is important for cancer prognosis.
Background/Aims Potassium-competitive acid blockers are expected to be the next generation of drugs for the treatment of diseases caused by gastric acid. In 2015, vonoprazan fumarate, a novel potassium-competitive acid blocker, was approved by the Japanese health insurance system. Since its approval, patients refractory to vonoprazan can be encountered in clinical settings. We designed this study to clarify the pathophysiology of gastroesophageal reflux disease refractory to vonoprazan. Methods In this retrospective study, we involved patients who had refractory symptoms after administration of standard-dose proton pump inhibitors or vonoprazan and underwent diagnostic testing with esophageal high-resolution manometry and 24-hour multichannel intraluminal impedance and pH monitoring while using proton pump inhibitors or vonoprazan. Patients were diagnosed based on the Rome IV criteria for functional gastrointestinal disorders and diagnostic test results. Results Twenty-seven patients were analyzed during this study. Gastric pH ≥4 was sustained for a longer period of time, and the esophageal acid exposure time and number of acid reflux events were shorter in the vonoprazan group than in the proton pump inhibitor group. The percentage of patients diagnosed with acidic gastroesophageal reflux disease in the vonoprazan group was lower than that in the proton pump inhibitor group. Conclusions Intra-gastric pH and acid reflux were strongly suppressed by 20-mg vonoprazan. When patients with gastroesophageal reflux disease present symptoms after administration of 20-mg vonoprazan, the possibility of pathophysiologies other than acid reflux should be considered.
