Radiographic Evaluation of Femoral Tunnel Placement During ACL Reconstruction (전방 십자 인대 재건술시 대퇴 터널의 위치에 대한 방사선학적 평가)

  • The isometric position for the graft is important in the anterior cruciate ligament reconstruction surgery. It is well known that the femoral position is more critical than tibial side. But, there is few deciding method of proper graft position after the anterior cruciate ligament reconstruction surgery. So we planned to evaluate the ideal femoral isometric position with 6 adult cadavaric femurs and exact radiographs. After the insertion of femoral interference screw on ideal isometric position, we obtained roentgenograms of true lateral view and 10, 20, 30, 45 degree internal and external rotation views. Then we measured the shortest distance from the posterior cortical margin of lateral femoral condyle to posterior border of interference screw on the radiographs. We also measured true distance between posterior cortical margin of lateral femoral condyle to the posterior margin of femoral tunnel after cutting of distal femur. Based on this study, we could not determine the permissible rotation degree of radiographs. But we concluded that if the distance between posterior cortical margin of lateral femoral condyle and posterior border of interference screw ranges 4.5-6.5mm on the lateral view, the femoral position is considered as a relatively ideal isometric good position.

Short-Term Results of Subpectoral Tenodesis of the Proximal Biceps Tendon Using by Interference Screw (간섭나사를 이용한 흉근하 상완 이두근 건 고정술의 단기 추시 결과)

  • Purpose: Our purpose was to retrospectively analyze clinical results of subpectoral tenodesis of the proximal biceps tendon using an interference screw. Materials and Methods: We reviewed 23 cases of patients receiving tenodesis of the proximal biceps tendon between January 2008 and January 2009 for whom we had follow-up data for at least 1 year. Twenty-three cases were operated on using subpectoral tenodesis; 16 of these cases had a rotator cuff tear. The results were judged using a visual analog scale (VAS), ASES, tenderness on the biceps groove, fixation failure and the degree of deformity (BAD). Results: VAS and ASES scores were significantly improved in all patients by the time of the final observation. There were no significant complications or fixation failures. The patients without a tear of the rotator cuff had a better result than patients with a tear of the rotator cuff, but the difference between the two groups was not significant (p>0.05). Conclusion: In patients with pathology of the long head of the biceps brachii, benefits of subpectoral interference screw tenodesis include pain relief, maintenance of functional biceps, muscle strength, and cosmesis. Subpectoral biceps tenodesis using interference screw fixation appears to be a promising, reproducible, reliable technique for addressing anterior shoulder pain related to pathology of the long head of the biceps brachii.

Tenodesis of Long Head of the Biceps Brachii Tendon with Bioabsorbable Interference Screw (체내 흡수성 간섭 나사를 이용한 상완 이두건 장두건 고정술의 임상적 결과)

  • Purpose: This study reports the clinical results of the tenodesis of long head of the Biceps brachii tendon with bioabsorbable interference screw by minimal open procedure. Materials and Methods: Ten cases of 10 patients (7 male, 3 female) were included in this study. The average age was 45.8 years old and the average period from the symptom onset to operation was 13.7 months. Average preoperative ASES score was 38.5. The causes of injury was; sports activities in 4 patients, unknown in 4 patients, industrial accident in 1 patient and traffic accident in 1 patient. The average follow up period was 12.1 months. Tenodesis with bioabsorbable interference screw by minimal open precedure was performed in all cases. Results: The ASES score improved to 87.5 at last follow up period and 6 cases had full range of motion of the shoulder. 4 cases had mild limited range of motion of the shoulder without any problem in normal daily activity. Conclusion: It was assumed that tenodesis of long head of the biceps brachii tendon with bioabsorbable interference screw by minimal open precedure was one of the good methods with good clinical results.

Clinical results of arthroscopic tenodesis of the proximal biceps tendon using by interference screw (관절경적 상완 이두근 건고정술의 임상적결과)

  • Purpose: To evaluate follow-up after the clinical results of arthroscopic assisted tenodesis of the proximal biceps tendon using an interference screw. Materials and Methods: We reviewed 30 cases of patients receiving tenodesis of the proximal biceps tendon between January, 2008 and January, 2010 who treated with arthroscope. The results were judged by VAS, ASES, tenderness in the biceps groove, fixation failure and the degree of deformity. Results: VAS, ASES scores increased by a statistically significant during the final observation in all patients than before surgery (p<0.05). For those with rotator cuff tear had low ASES results (p<0.05), respectively. But, tenderness in the biceps groove was 17% (5 cases), pepeye deformity and incresead BAD (biceps apex distance) were 10% (3 cases). Conclusion: Arthroscopic tenodesis of the proximal biceps tendon using an interference screw to patients with proximal lesion in their biceps tendon, can achieve good results, but it is possible to groove tenderness and popeye deformity.

  • Purpose: To estimate the initial and early phase fixation power of the human bone interference screw in reconstruction of the anterior cruciate ligament with bone-patellar tendon-bone allograft. Materials and Methods: The results of twenty eight knees of reconstruction with bone-patellar tendon-bone allograft were analysed in 6 weeks, 12 weeks, 6 months and one year following operation. Physical examination including Lachman test, flexion rotation drawer test and jerk test were performed. The KT-1000 measurement was performed at the same time. In Lachman test 0 $\~$2mm anterior displacement of the tibia was considered normal. The KT-1000 measurement of normal side was compared with operation side and the difference of the two was recorded. The MRI was checked at final follow-up. Results: All but one knee showed normal in physical examination. The failed case showed proximal migration of the graft due to insufficient number of interference screw fixation in widened tibial tunnel. Conclusions: The human cortical bone interference screw showed sufficient initial and early phase fixation power in reconstruction of the anterior cruciate ligament.

  • MWNTs have been widely investigated due to unique properties of such as good electrical conductivity and thermal stability in polymer composites industries. This paper established the procedure to fabricate PMMA/MWNT composites by a modular intermeshing co-rotating twin screw extruder with L/D ratio of 42. The electrical properties of PMMA/MWNT composites with different content of MWNT have been investigated. A sheet resistance percolation was observed at 4 wt% of MWNT for the melt processed composites. Sheet resistance of PMMA/MWNT composite film containing 4 wt% of MWNT was nearby $10^4{\Omega}/sq$ and this shows the possibility of potential application to EMI (Electronic Magnetic Interference) shielding materials. The characteristics of composites were analyzed by TGA, DSC, and SEM. In addition, MFI (Melt Flow Index) has been measured to analyze the rheological property.

  • Pretibial cyst formation is a rare complication after anterior cruciate ligament (ACL) reconstruction and there are only few cases reported. Pretibial cyst can be caused by variable reasons. Foreign body reaction due to breakdown of bio-absorbable screw for fixation, graft necrosis at tibial site, joint fluid leakage to tunnel and incomplete incorporation of graft to bony tunnel. The authors experienced one case of massive pretibial cyst after arthroscopic ACL reconstruction using bio-absorbable interference screw in 38-year-old male patient. Thus, authors report this rare case with literature view.

  • In this study, we designed tooth profile of the fly-cutter for antisymmetric rotor which is used in screw compressor. In order to verify this profile, we manufactured three different pairs(J46, N46, P46) of antisymmetric rotor using fly-cutter. We got the following conclusions from this study. (1) We obtained better contact condition using 3pairs of rotor which are manufactured by the fly-cutter. (2) We could prevent the cutter interference near bottom point of the robe of screw rotor.

  • In this study, screw machining system by use of a rotational tool such as an end-mill or a face cutter with Y axis off-set on a CNC turing center was developed for quick machining. In this system, It was possible to use different tools for different processes, and by off-setting the tool in Y direction by calculated amount it was possible to avoid tool interference problem which could occur within the central area at the end of a tool. In addition, machining a screw with a helix of up to 3 different leads combined and with tapered minor diameter was possible.

  • Recently, the interest on anatomical ACL reconstruction with double bundle technique is increased to reproduce the original load distribution, and kinematics of the knee. We developed an arthroscopic double bundle ACL reconstruction technique using autogenous quadriceps tendon with 2 splitted graft and patellar bone plug. The anteromedial bundle and posteolateral bundle of the ACL is replicated with each splitted graft of quadriceps tendon and fixed with biodegradable interference screw on the 2 femoral tunnels. The patellar bone plug of quadriceps tendon is fixed with biodegradable interference screw within the 1 tibial tunnel. We suggest that our technique using quadriceps tendon may be an alternative in arthroscopic double bundle ACL reconstruction.
