상악 전치부 임플란트지지 수복금관의 주변 연조직 제원 변화에 관한 누년적 연구 (Alterations of the soft tissue dimensions around implant-supported singletooth replacements in the maxillary anterior region - A retrospective longitudinal study -)

  • This study was performed to investigate the soft tissue changes around single implant-supported crowns during followup periods. Twenty patients(31 implants) whose single missing tooth in the maxillary anterior region had been replaced with an single implant-supported crown were recruited for the study. Crown length, soft tissue level and papilla height at the single implant-supported crowns were measured at follow-up examination and calculated from the slides taken at time of crown placement. as well Papilla index was scored from the slides taken at the time of crown placement and follow-up examination. A very little amount of recession occurred and the soft tissue level moved more apically and the papilla height increased significantly (p<0.01). Especially, both mesial and distal papilla index at single implant-supported crowns increased significantly during follow-up periods (p<0.001). When the two slides taken at the time of crown placement and follow-up were compared simultaneously, except one site, papillae size increased at all sites. From the results of the study, the interdental papillae at the single implant-supported crowns seemed to regenerate significantly and their crown margins were stable during follow-up periods. Hence it is indicated that various surgical interventions at on early stage to enhance soft tissue esthetics arourd single implants may be unnecessary.

An analysis on the factors responsible for relative position of interproximal papilla in healthy subjects

  • Purpose: This study examined the factors that can be associated with the appearance of the interproximal papilla. Methods: One hundred and forty-seven healthy interproximal papillae between the maxillary central incisors were examined. For each subject, a digital photograph and periapical radiograph of the interdental embrasure were taken using a 1-mm grid metal piece. The following parameters were recorded: the amount of recession of the interproximal papilla, contact point-bone crest distance, contact point-cemento-enamel junction (CEJ) distance, CEJ-bone crest distance, inter-radicular distance, tooth shape, embrasure space size, interproximal contact area, gingival biotype, papilla height, and papilla tip form. Results: The amount of recession of the interproximal papilla was associated with the following: 1) increase in contact point-bone crest, contact point-CEJ, and CEJ-bone crest distance; 2) increase in the inter-radicular distance; 3) triangular tooth shape; 4) decrease in the interproximal contact area length; 5) increase in the embrasure space size; and 6) flat papilla tip form. On the other hand, the amount of gingival recession was not associated with the gingival biotype or papilla height. In the triangular tooth shape, the contact point-bone crest distance and inter-radicular distance were longer, the interproximal contact area length was shorter, and the embrasure space size was larger. The papilla tip form became flatter with increasing inter-radicular distance and CEJ-bone crest distance. Conclusions: The relative position of the interproximal papilla in healthy subjects was associated with the multiple factors and each factor was related to the others. A triangular tooth shape carries a higher risk of recession of the interproximal papilla because the proximal contact point is positioned more incisally and the bone crest is positioned more apically. This results in an increase in recession of the interproximal papilla and flat papilla tip form.

정중구개부의 미니 임플랜트 식립과 관련된 절치관의 해부학적 연구 (Anatomic study of the incisive canal in relation to midpalatal placement of mini-implant)

  • 정중구개봉합부에 교정용 미니 임플랜트를 식립하고자 할 때 절치관의 해부학적 위치를 파악하여 이를 피하여 식립하여야 한다. 본 연구에서는 절치관의 해부학적 위치를 절치유두나 치관의 교두첨 등의 임상적 지표를 이용하여 예측할 수 있는 해부학적 자료를 마련하고자 하였다. 성인 남녀 25명의 진단 모형과 전산화 단층촬영(CT) 자료를 통해 절치관의 해부학적 형태, 절치유두와 절치관의 전후방적인 위치의 관련성 및 치관 교두첨의 전후방 위치에 따른 절치관 후방 정중구개봉합부의 골두께를 계측하였다. 계측 결과 중절치 사이 설측 치간유두에서 절치공 후연까지 구개면상의 거리는 치간유두-절치유두 후연 길이의 약 1.7배에 해당하였으며 절치유두 후연에서 절치공 후연까지의 구개면상의 거리는 평균 6.15 ${\pm}$ 1.75 mm였다. 절치공 후연의 전후방적 위치는 교두첨을 기준으로 평가 시 제1소구치의 설측교두보다 약간 전방에 해당하였으며, 이 부위 직후방에서는 골표면에 수직으로 식립하는 것보다 교합평면에 수직으로 식립할 때 골두께가 더 두꺼웠다. 또한 절치관의 폭경이 구개에서 4.03 ${\pm}$ 0.64 mm인 것을 고려할 때 절치관의 측방에 식립코자 하는 경우 정중에서 3 mm 이상 측방에 식립하는 것이 바람직하다. 교정용 미니 임플랜트를 정중구개부에 식립할 때 본 연구 결과를 절치공의 위치를 짐작하는 데 활용할 수 있을 것이다.

Ovate Pontic을 이용한 상악 전치부 심미수복 (The Use of Ovate Pontics in the Maxillary Anterior Esthetic Zone: A Clinical Report)

  • 1933년 Dewey와 Zugsmith가 처음 ovate pontic을 소개한 이후 1990년대 말부터 임상적으로 널리 사용되고 있다. Ovate pontic은 심미적이고 위생적인 자연치의 emergence profile을 더 정교하게 재현할 수 있는 방법으로 알려져 있다. 특히, 상악 전치부가 상실된 경우 발치 시기부터 ovate pontic의 형태를 가진 레진 임시수복물을 제작해 줌으로서 치간유두를 보존시켜 치간공극의 발생을 최소화하여 심미적이고 발음의 이상이 없는 최종보철물을 제작할 수 있다. 본 증례에서는 치아상실부위에서 치간유두의 상실을 최소화하고 자연치와 유사한 emergence profile을 지닌 보철물을 제작하기위해 ovate pontic을 이용하여 처치한 치험예를 문헌고찰과 함께 보고하고자 한다.

A novel amnion-chorion allograft membrane combined with a coronally advanced flap: a minimally invasive surgical therapy to regenerate interdental papillary soft tissue recession - a six-month postoperative image analysis-based clinical trial

  • Objectives: Loss of the interdental papilla is multi-factorial and creates a multitude of problems. Autogenous connective tissue/biomaterial-based regeneration has been attempted for decades to reconstitute the black space created due to the loss of papilla. The aim of this present study was to regenerate papillary recession defects using an amnion-chorion membrane (ACM) allograft and to evaluate the clinical outcome up to six months postoperatively. Materials and Methods: Twenty patients with 25 Nordland and Tarnow's Class I/II interdental papillary recession defects were treated with ACM and coronal advancement of the gingivo-papillary unit via a semilunar incision on the labial aspect followed by a sulcular incision in the area of interest. A photographic image analysis was carried out using the GNU Image Manipulation software program from the baseline to three and six months postoperatively. The black triangle height (BTH) and the black triangle width (BTW) were calculated using the pixel size and were then converted into millimeters. The mean and standard deviation values were determined at baseline and then again at three and six months postoperatively. The probability values (P<0.05 and P≤0.01) were considered statistically significant and highly significant, respectively. An analysis of variance and post hoc Bonferroni test were carried out to compare the mean values. Results: Our evaluation of the BTH and BTW showed a statistically and highly significant difference from the baseline until both three and six months postoperatively (P=0.01). A post hoc Bonferroni test disclosed a statistically significant variance from the baseline until three and six months postoperatively (P<0.05) and a non-significant difference from three to six months after the procedure (P≥0.05). Conclusion: An ACM allograft in conjunction with a coronally advanced flap could be a suitable minimally invasive alternative for papillary regeneration.

상악 전치부 단일 임플란트 지지 수복금관의 주변 연조직반응의 단면적 연구 (Soft tissue reactions around implant-supported single-tooth replacements in the maxillary anterior region)

  • The aims of this study were firstly to investigate soft tissue reactions around single implant-supported crowns and secondly to compare soft tissue dimensions and conditions of the crowns in relation to interdental papillae, and lastly to investigate patients'esthetic satisfaction with their single implant-supported crowns according to the interdental papillae presence/absence. Twenty-nine patients (41 implants) whose single missing tooth in the maxillary anterior region had been replaced by single implant-supported crown participated for the study and various variables of soft tissue conditions, dimensions and crown dimensions were measured around the single implant-supported crowns at clinical examination and from study models and slides. The results showed that the soft tissue conditions around the single implantsupported crowns were similar to those around implants used for partially or totolly edentulous patients. Except for the high frequency of bleeding on probing, all other parameters revealed healthy conditions. The buccal sites of the crown had a shallow pocket comparing with other sites. At all sites of the crown, similar status of little inflammation was found. Mesial sites and central-incisor positioned implantsupported crowns had lower contact point position than distal sites and lateral-incisor positioned crowns, respectively. Mucositis index, probing depth and contact point position were significantly correlated with papillae index(p < 0.05). More inflammation and lower contact point position were found at the implant-supported crown with no interdental space than that with interdental space. Patients showed high esthetic satisfaction regardless of interdental space presence. The result indicated that, despite of their submucosal crown margins, single implantsupported crowns have soft tissue conditions as good as other implants used for the treatment of the different types of edentulism and a clinician can manipulate interdental papilla height by modifying crown shapes within the limits of not violating total esthetics.

한국인의 상악중절치 위치 및 길이에 관한 연구 (A Study on the Position and Length of the Maxillary Central Incisor in Koreans)

  • The arrangement of anterior artificial teeth is an important factor on the esthetics in the construction of complete dentures and prostheses. During construction of the complete denture the position of maxillary central incisor plays an important role in the arrangements of artificial teeth. The proper position of maxillary central incisor provides natural appearance of anterior teeth and the anterior reference point of occlusal plane. Many methods have been utilized to eatablish the position of the maxillary central incisor in complete denture prosthodontics. However, there has not been provided for the reference for Korean yet. This study was aimed to extablish a guide for the position of the maxillary central inciosr in Koreans. The horizontal and vertical distance between the maxillary central incisor and incisive papilla in Koreans were measured. 1. The mean value of horizontal distance from the maxillary central incisor to the incisive papilla had significant sexual difference, The average distance was 12.648mm in male and 11.385mm in female. 2. The horizontal distance of the incisive papilla had sexual difference, the average distance was 6.182mm in male and 5.622mm in female. 3. The vertical distance from the maxillary central incisor to the incisive papilla had not sexual difference, the average distance was 7.21mm. 4. The crown length of the maxillary central inciosrs from the distal interdental papilla had not sexual difference, the right central incisor was 6.40mm(mean) and the left central inciosr was 6.43mm(mean). 5. The clinical crown length of the maxillary central incisors had not sexual difference, the right central incisor 10.35mm(mean) and the left central incisor was 10.43mm(mean). 6. The shapes of the incisive papilla were pear(68.4%), oval(16%), triangular(6%), irregular(5%), rectangular(4%), and inverted pear(1%) shape.

Laser Doppler Flowmetry를 이용한 치은혈류량에 관한 연구 (A study of gingival blood flow using laser Doppler flowmetry)

  • The periodontal health has been evaluated clinically by various epidemiological indices, and in researches by measurement of gingival crevicular fluid. Laser Doppler flowmetry is a reliable and objective method that allows immediate measurement of erythrocyte flux in approximately one cubic mm of the capillary bed without disturbing the tissues. The purpose of the present study was to determine whether human gingival blood flow was different according to measuring area, measuring time, and sex or not. Forty volunteers with good general and periodontal health, aged early twenties and unmarried, were selected. Laser Doppler flowmetry($floLAB^{(R)}$, Moor Instruments Ltd., England) was applied to measure the gingival blood flow of marginal gingiva, interdental papilla, attached gingiva and alveolar mucosa. The blood flow of interdental papilla was measured at 9-10 AM, 1-2 PM, and 5-6 PM. The difference of blood flow according to measuring area and measuring time was statistically analyzed by one way AOVA and Dunkan test, and the difference of blood flow between men and women was statistically analyzed by t-test. (1) Mean blood flow was significantly higher in alveolar mucosa than in the gingiva(p<0.05), and there was no significant difference in blood flow between marginal gingiva and interdental papilla(p>0.1). (2) Mean blood flow was significantly higher at 5-6 PM than at 9-10 AM and 1-2 PM(p<0.05). But there was no significant difference in gingival blood flow between 9-10 AM and 1-2 PM(p>0.1). (3) There was no significant difference in gingival blood flow between men and women(p>0.1). The above results suggest that the measurment of gingival blood flow using laser Doppler flowmetry may be clinically applicable to early determination of gingival inflammation and evaluation of healing status, but further studies are necessary to standardize and simplify the measuring procedure.
