The Relationship Between Levels of Serum Testosterone and Prostate-specific Antigen in Healthy Men: An Integrative Review (성인 남성의 혈청 테스토스테론 농도와 전립선 특이항원의 관계에 대한 통합적 고찰)

  • Purpose: The purpose of this study was to review primary research exploring the correlations between the levels of serum testosterone and the prostate-specific antigen (PSA) in healthy men without prostate diseases. Methods: An integrative review was conducted using the Whittemore & Knafle (2005) framework. The keywords, 'testosterone & prostate-specific antigen', 'testosterone & PSA' and 'healthy men' were used to search peer-reviewed publications in six databases. Among 1,959 searched articles, eleven articles were selected after excluding articles that do not meet inclusion criteria. Literature quality was moderate (Level 3). Results: As a result of this study, it was confirmed through the nine articles that healthy adult men showed no significant correlation between the serum testosterone and the PSA. Conversely, two articles presented that the serum PSA correlate positively with the testosterone. In particular, it is inferred that the effect of the serum testosterone and the PSA secreted into a 24-hour circadian rhythm with different amplitudes and slopes would have had great influence. However, it does not consider the factors affecting the testosterone and the PSA, such as race, liver disease, and BMI, so there is insufficient empirical data to clearly explain the relationship between the testosterone and the PSA. Conclusion: The correlation between the serum testosterone and the PSA in healthy adult men is insignificant in relation to the circadian rhythm of the testosterone and the PSA secretion. It is that a large-scale study including various influencing factors using new biochemical indicators such as pro PSA be conducted in the future.

An Integrative Literature Review on Sexual Abuse Prevention Education Programs for Elementary School Students in South Korea (한국의 초등학생을 대상으로 한 성폭력 예방 교육 프로그램에 관한 통합적 고찰)

  • Purpose: The purpose of this study was to review sexual abuse prevention education program for Korean elementary school students. Methods: Whittemore and Knafl's integrative review methods were used and Gough's weight of evidence was employed as a quality appraisal tool. Articles published in Korean or English were identified through electronic search engines and scholarly web sites using three keywords: "elementary school student", "sexual abuse", and "prevention education". Peer-reviewed articles published between 2000 and 2018 were included in this review. Results: Twelve articles met the inclusion criteria and were appraised as being high-quality. Among the 12 selected studies, seven were descriptive, while five were intervention studies. Sexual abuse prevention education programs were effective in improving perceptions, knowledge, attitudes, and preventive behaviors among elementary school students. However, deficiencies were found in the variety of educational methods, utilization of experts, and standardization of the content of sexual abuse prevention education. Conclusion: We need to provide various educational methods that are appropriate for specific developmental stages, and the sexual abuse prevention content should draw upon the current sexual education program administered to this population. Furthermore, parents and trained teachers or school health teachers should be included to provide effective education programs for elementary students.

An Integrative Review on the Contents and Effectiveness of Depression and Anxiety Interventions applied to Unmarried Mothers Living in Residential Facilities (시설에 거주하는 미혼모에게 적용된 우울 및 불안 감소를 위한 중재의 통합적 문헌고찰)

  • Purpose: The purposes of this study was to review the literature on intervention for treating anxiety and depression among unmarried mothers living in facilities, and to understand core that could promote the development of more effective interventions. Methods: Key words in English and Korean were used to search through eight electronic databases-PubMed, Cochrane Library, EMBASE, CINAHL, RISS, DBpia, NDSL, and the National Assembly Library. Results: Ten studies were ultimately selected for the integrative review and were evaluated in terms of contextual and methodological quality. The studies consisted of seven quasi-experimental studies and three case report studies. The selected studies utilized music, art, forest therapy, dancing, education, and play programs to change mothers' perceptions, emotions, and behavior and to improve their relationships with their babies or others. Conclusion: It is important to consider mothers' self-awareness and emotional expression, and to improve their relationships with their babies or others as core elements when developing intervention programs for anxiety and/or depression among unmarried mothers living in residential facilities.

Communication Experiences and Needs among the Critically Ill with an Artificial Airway: An Integrative Review (인공기도를 가진 중환자실 환자의 의사소통 경험과 요구에 관한 통합적 문헌고찰)

  • Purpose : The aim of this integrative review was to describe communication experiences and needs among the critically ill with an artificial airway and evaluate relevant intervention studies. Methods : Using the method employed by Whittemore and Knafl (2005), we searched for papers on PubMed, CINAHL, and Web of Science published from January 1, 2015 to September 25, 2020. A total of 26 papers (11 descriptive studies, 15 experimental ones) were identified and evaluated. Results : Communication experiences while having an artificial airway, identified either by patients' recall or direct observation, were predominantly negative. However, positive experiences were reported in patients with improvements in physical conditions and functions. Patients' communication needs were diverse, encompassing physical, psycho-emotional, social, spiritual, and medical issues. Interventions tested included alternative and augmentative communication strategies, communication intervention packages, and voice restorative devices. Physical, psychosocial, and communication-related patient outcomes were evaluated using various methods including self-report surveys, interviews, and observations. Conclusions : The findings from this review represent the state of science regarding communication of the critically ill whose vocal abilities are inhibited by the presence of an artificial airway. Future studies with rigorous experimental designs and measures are warranted to better understand and support the complex needs of this highly vulnerable patient population.

Physical Therapy Approach and Management for Lymphedema : Expert Opinion (림프부종의 물리치료적 접근과 관리 : 전문가 견해)

  • Background : Lymphedema is a progressive disorder characterized by the impairment of lymph flow from tissues to the blood circulation system. This occurs as a result of damage to the lymphatic system. Complex decongestive therapy (CDT) is a multimodal, conservative therapeutic approach that is used for the management of lymphedema. CDT consists of a combination of compression therapy, manual lymphatic drainage, exercise, and skin care. Purpose : This study aimed to provide a review of available physical therapy interventions as well as general care guidelines for patients with lymphedema. Methods : The recommendations and guidelines for physical therapy management, medical management, and general information were reviewed from the following sources: 1) The American Physical Therapy Association, 2) The Norton School of Lymphatic Therapy, and 3) The International Society of Lymphology. This review contains general information, including the medical management and the importance of physical therapy in lymphedema. Physical therapy management should be based on an assessment of the patients' presenting impairments, including based on inclusion or exclusion of physical therapy interventions. This review also outlines a step-by-step approach that starts with disease diagnosis and progression all the way through to rehabilitation as an outpatient. Conclusion : Depending on the patients' journey to recovery and the requirement for rehabilitation, physical therapy interventions should focus on the patients' needs including pain, appearance, physical function and general rehabilitation. We hope that this review will provide information on evidence-based physical therapy and general care to patients with lymphedema.

Association of Hemodialysis and Sarcopenia : A systematic review (혈액투석과 근감소증의 연관성에 관한 문헌고찰)

  • Purpose : This study aimed to investigate the relationship between the characteristics of hemodialysis patients and the occurrence of sarcopenia through a comprehensive literature review. Methods : A systematic literature search was conducted to identify eligible studies in the Cochrane library, PubMed and Embase. In this review, we included all papers published since the initiative's inception and summarized results as of december 2022. Studies that investigated association between sarcopenia diagnosis and hemodialysis patients (aged≥18 years) were included. Ultimately, 16 studies met our selection criteria. The risk of bias was assessed using the Newcastle-Ottawa scale. Results : Fourteen of the sixteen studies (88 %) reported that significant association between sarcopenia diagnosis and hemodialysis patients. However, two studies reported no association between sarcopenia diagnosis and hemodialysis patients. As a factor statistically related to sarcopenia in hemodialysis patients, Mortality (6 studies, 38 %), age (5 studies, 31 %), body composition (4 studies, 25 %), physical activity (2 studies, 13 %), diabetes (2 studies, 13 %), cardiovascular abnormalities (1 studies, 6 %), nutritional status (3 studies, 19 %), and gender (3 studies, 19 %). Conclusion : Our findings highlight the necessity of developing a physical therapy program that accurately reflects the health status of hemodialysis patients. To further investigate the association between the diagnosis of sarcopenia and hemodialysis patients, it is recommended to conduct large-scale longitudinal studies using standardized diagnostic criteria and evaluation methods, as well as analyze potential risk factors. Consequently, this study emphasizes the importance and potential of developing physical therapy programs that effectively address the health consequences associated with hemodialysis. The significance of this research lies in its ability to provide valuable insights and lay the foundation for future studies focused on developing preventive and therapeutic interventions targeting muscle wasting syndrome resulting from hemodialysis.

Neurofeedback Training for Upper Extremity Functional Recovery in Stroke Patients : A systematic review (뇌졸중 환자의 팔 기능 회복을 위한 뉴로피드백 훈련 : 체계적 고찰)

  • Purpose : To identify trends in studies on the intersection of neurofeedback (NF) training and rehabilitation interventions for patients with stroke, as conducted over the past decade. Methods : This review included studies published from 2012 to 2022. A comprehensive database search was conducted using keywords such as "stroke", "electroencephalogram (EEG)", "brain wave", "nerve feedback", and "upper extremity function". Results : A total of 703 studies were initially retrieved. Of these, literature predating 2012, duplicate literature, non-experimental studies, and studies that did not target patients with stroke were excluded. After this screening, we retrieved the full texts of 15 articles and re-checked whether each study met the inclusion criteria of this study. The advisory members who participated in this study consisted of people with doctoral degrees and more than 5 years of clinical experience related to the rehabilitation of patients with stroke. Disagreements were resolved through discussions. Ultimately, 8 papers met the inclusion criteria and were included in the final analysis. Despite differences in the type and duration of NF training, the combination of NF training and rehabilitation intervention was found to be effective in promoting the functional recovery of the upper extremities, eliciting positive EEG changes, and inducing neurological changes in the brain. Conclusion : A meta-analysis involving a wider search range is needed in future studies. In addition, efforts are required to generalize the clinical application of these interventions. This can be achieved by supplementing the research methodologies through extensive review studies that encompass a diverse array of study designs.

A Literature Review on Overseas Intervention Study for Feeding Problems in Children with Autism Spectrum Disorders (자폐 스펙트럼 장애 아동의 섭식 문제에 대한 중재의 국외 문헌 연구)

  • Purpose : The purpose of this study provided an overview of the general status and recent intervention approaches in overseas research related to feeding problems in children with autism spectrum disorder (ASD). This review aims to explore interventions for feeding problems in order to provide higher quality follow-up research directions and implications, particularly focusing on providing recommendations for future research in the context of domestic studies. Methods : Analyzing studies published in international journals from 2017 to 2023. This review involved six selected articles, through Embase, Pubmed, RISS, KISS database search engine. A literature analysis that includes inclusion and exclusion criteria, six selected articles were examined. The literature analysis categorized the general status of the research and intervention approaches and treatment components into intervention, treatment settings and therapists, and dependent variables, respectively. Results : Among feeding intervention approaches, parent education interventions based on behavioral therapy had the highest proportion, followed by multidisciplinary interventions. To maintain the effectiveness of interventions over the long term and to generalize them to the home environment, parent education that utilizes parents as mediators is considered a crucial factor. The most commonly observed effects as dependent variables were changes in the consumption of disliked foods, health foods and alterations in feeding behavior. Conclusion : This study introduces various intervention approaches for addressing feeding problems in children with autism spectrum disorder (ASD), focusing on the positive effects demonstrated by active intervention research in abroad. Furthermore, it underscores the need for additional research in Korea to validate the efficacy of these feeding intervention methods. Lastly, the study outlines future research directions aimed at developing feeding programs to support children with ASD and their families coping with feeding issues.

Current Status and Development Direction Through a Review of Yoga Therapy Literature (요가치료 문헌 고찰을 통해 본 현황과 발전 방향)

  • Background: Integrative medical research is needed to explore the development direction of new yoga therapy. Purposes: A systematic literature review is conducted to analyze the current status of clinical research on yoga therapy into scientific categories, and to explore the content and development direction of yoga therapy. Methods: Through electronic databases such as RISS, NDSL, DBpia, e-article, and KISS, 530 domestic academic papers were selected from 2010 to 2018 and the final 28 were extracted based on PRISMA guidelines. Results: As a result of the study, in terms of quality, it remained at 3b of the CEBM level of evidence, and in terms of quantity, the number of experimental groups in the entire clinical study was 288, so domestic clinical studies of yoga therapy are insufficient. It was found that 80% of yoga therapy was exercise therapy focusing on asana movements. This seems to be due to a lot of researchers in the field of physical education. Conclusions: These results indicate that understanding and practice of the Ashtanga-yoga's training system and Pancha-kosha theory from the perspective of integrative medicine are necessary. In other words, yoga therapy is required to develop into an integrated mind-body therapy program that integrates holistic healing yoga based on individual mental and physical constitution, meditation therapy based on Ayurveda, and exercise therapy.

Association between Periodontitis and Preeclampsia: a Systematic Review (임신기 치주질환과 자간전증 발생간의 연관성)

  • Purpose : Preeclampsia(PE) is a pregnancy-specific disease which is characterized by hypertension and proteinuria. This disease occurs in about 2-8 % of pregnancies in developing countries and remains among the major causes of maternal and neonatal mortality and morbidity throughout the world. Although the causes of PE are not fully understood, the infection has been considered as the main risk factor for this diseases. Periodontal disease may provide a chronic burden of endotoxin and inflammatory cytokines and the disease has been considered as risk factors of systemic illnesses including cardiovascular disease, atherosclerosis, and cerebrovascular ischemia. So, studies performed over the last 15 years have suggested that periodontal disease may be associated with adverse pregnancy outcomes such as PE. However, this association has not been found in all populations. The aim of this review article was to evaluate whether periodontal status and the presence of specific periodontal pathogens may influence the incidence of PE. Methods : Many research articles searched at the electronic databases(MEDLINE; 2000 to July 2017) including search term as periodontal disease and preeclampsia. Result : There were 10 case-control studies and 5 cohort studies meeting our inclusion criteria. The results showed that maternal periodontitis (adjusted odds ratio: 1.5 to 9.3) was associated with preeclampsia in 15 epidemiological studies. Conclusion : It is clear that maternal periodontitis is a risk factor associated with preeclampsia, emphasizing the importance of periodontal care in prenatal programs.
