H-DISS: Development of Integrated Management System for Healthcare Documents using Web Services and HL7 (H-DISS: 웹서비스와 HL7을 이용한 헬스케어 문서 통합 관리 시스템 개발)

  • Recently, as the technology has been blended with IT, BT or NT and technology in each field has been developed progressively with the people's needs for a healthy and abundance life. In the medical service field as well, related technologies such as medical equipment, medical information system, or medical business application have come to be diversified and advanced. However, most of these medical technologies are being developed independently without an interaction with other medical institutions or related companies, which causes many problems in the integration or interchangeability of them. This paper is intended to solve the problems generated during a direct information sharing between the functional interfaces of the present healthcare information sharing system. This paper develops a repository between the interfaces, which can save all the healthcare information from the institutions, and convert and publish it in various forms of document. In other words, recently HL7 protocol has been used for sharing the healthcare information in order to save the information in XML format, and develops the H-DISS(Healthcare-Documents Integrated Sharing System), a repository to manage and publish the saved document.

  • Due to their high degree of freedom to transfer and acquire light, fiber optics can be used in the presence of strong magnetic fields. Hence, optical sensing and imaging based on fiber optics can be integrated with magnetic resonance imaging (MRI) diagnostic systems to acquire valuable information on biological tissues and organs based on a magnetic field. In this article, we explored the combination of MRI and optical sensing/imaging techniques by classifying them into the following topics: 1) functional near-infrared spectroscopy with functional MRI for brain studies and brain disease diagnoses, 2) integration of fiber-optic molecular imaging and optogenetic stimulation with MRI, and 3) optical therapeutic applications with an MRI guidance system. Through these investigations, we believe that a combination of MRI and optical sensing/imaging techniques can be employed as both research methods for multidisciplinary studies and clinical diagnostic/therapeutic devices.

  • Recent reseach in the field of medical information systems has paid much attention to an ontology based medical terminology system to support clinical study and effective information search. This study aims to conduct research for further application or construction of ontology systems in Korea. This research reviews the theory of concept modeling and ontology. and analyses 4 cases of conceptual modeling of medical tenrminologies by ontology, The findings ot this study display these specific characteristics in medscal ontologies: (UHe standardization of terrrinology on MeSH. (2) The conceptualization of tenninology on UMLS. (l) ane. (2) are showed as untormal ontologies. (3) The theory of ontology integration in ON9, (4) The reference model of medical knowledge with formalization in GALEN. (3) and (4) are showed as formal ontologies. The application and construction of ontology should be differentiated according to the level of the proposed system, and then this analysis will provide useful information for the researcher and developer of the system.

치과용 DICOM encoder와 viewer의 특성과 개발

  • Information Technology has extended its scope to the medical field as well as dental field. Like medical field, network ststem for dental field requires acquisition, storage, and display of images. However, unlike the medical field, the system to integrate several information including medical images has not been developed according to industrial standard for management of digital image for medical use, so called DICOM conformance. which makes the digital environment in dental field more and more difficult and expensive for this standardization and comfortable communication in LAN and WAN. To solve this problem, the DICOM encoder and server has to be developed because the DICOM file can be easily retrieved with patient's information from the DICOM server in the system as DICOM file has the standard specification to integrate the patient's information. The information including image and other discrete data can be easily integrated in DICOM file and can be used without any difficulty for precise diagnosis and for contribution to the decision making for each treatment protocol. Therefore, the system composed of DICOM encoder and server in dental practive for DICOM file must be developed with prudent consideration of the several strategic factors: I) Enhanced diagnostic capability through the integrated information of image and clinical data. ii) Clinician-friendly interface to simulate the systemic treatment procedure in clinical practice iii) Implementation of multidisciplinary treatment protocol The development of DICOM encoder and server based on these strategic considerations will provide paperless and filmless hospital environments by the seamless integration and management of patient's history, several clinical data and clinical images through image processing for quantitative analysis. The system also allows clinicians to provide more predictable dental care for the patients.

Implement of Integration Compression Environment Using Medical Images

  • Large medical images in PACS are compressed for saving storage space and improving network speed. The integrated compression environment was designed and developed for uniting of various compression methods. Various compression algorithm-RLE compression, lossless JEPG, JPEG, was built into it, complying with DICOM. A image compression using DWT was also implemented in it. And a unified algorithm of lossless compression and lossy compression was designed to improve images quality and to make compression ratios high. And integrated compression environment was operating together with a database program for efficient and user-friendly management.

  • As the paradigm shifts from treatment and provider-centered healthcare to prevention and consumer-centered healthcare, the integration of ICT convergence technology is calling for an era of digital healthcare industry revival in the Fourth Industrial Revolution. It is possible to provide individual customized medical services utilizing various medical data, and it is possible to provide various medical services that transcend time and space through integration with other industries. Such digital healthcare includes health, nutrition, exercise, and patient care, while the digital healthcare industry includes healthcare and IT related to medical devices, medical information systems, and healthcare platforms that can provide personal health and medical information. Due to the social demands of the aging and the increase of chronic diseases, digital healthcare is considered as an important policy in the fourth industrial revolution in Korea. In order for the digital healthcare industry to contribute to the prolongation of human life and the improvement of quality of life, it is urgent to develop related infrastructures, legal institutions, and prepare policies. In addition, it is important to activate convergent education to foster talents who will lead the digital healthcare industry. The purpose of this study was to examine the trends of the digital healthcare industry in the era of the fourth industrial revolution and the direction of government R & D policies, and to derive directions and suggestions for future development.

  • The use of a mobile agent in hospital environment offers an opportunity to deliver better services for patients and staffs. Furthermore, medical errors will be reduced because M-health system helps to verify the medical process. Optimized security protocols and mechanisms are employed for the high performance and security. Finally, a challenge in the near future will be converge the integration of Ubiquitous Sensor Network (USN) with security protocols for applying the hospital environment. We proposed secure authentication and protocol with Mobile Agent for ubiquitous sensor network under healthcare system surroundings.

Research on Community Knowledge Modeling of Readers Based on Interest Labels

  • Community portraits can deeply explore the characteristics of community structures and describe the personalized knowledge needs of community users, which is of great practical significance for improving community recommendation services, as well as the accuracy of resource push. The current community portraits generally have the problems of weak perception of interest characteristics and low degree of integration of topic information. To resolve this problem, the reader community portrait method based on the thematic and timeliness characteristics of interest labels (UIT) is proposed. First, community opinion leaders are identified based on multi-feature calculations, and then the topic features of their texts are identified based on the LDA topic model. On this basis, a semantic mapping including "reader community-opinion leader-text content" was established. Second, the readers' interest similarity of the labels was dynamically updated, and two kinds of tag parameters were integrated, namely, the intensity of interest labels and the stability of interest labels. Finally, the similarity distance between the opinion leader and the topic of interest was calculated to obtain the dynamic interest set of the opinion leaders. Experimental analysis was conducted on real data from the Douban reading community. The experimental results show that the UIT has the highest average F value (0.551) compared to the state-of-the-art approaches, which indicates that the UIT has better performance in the smooth time dimension.

A Design and Implementation of XML DTDs for Integrated Medical Information System (통합의료정보 시스템을 위한 XML DTD 설계 및 구현)

  • The advanced medical information systems usually consist of loosely-coupled interaction of independent systems, such as HIS/RIS and PACS. To support easier information exchange between these systems and between hospitals, and to support new types of medical service such as teleradiology, it becomes essential to integrate separated medical information and allow them to be exchanged and retrieved through internet. This thesis proposes an integrated medical information system using XML. We analyzed HL7 and DICOM standard formats, and designed an integrated XML DTD. We extracted information from HL7 messages and DICOM files and generated XML document instances and XSL stylesheets based on the proposed XML DTD. We implemented the web interface for the integrated medical information system, which supports data sharing, information exchange and retrieval between two different standard formats. The proposed XML-based integrated medical information system will contribute to solve the problems of current medical information systems, by enabling integration of separated medical informations and by allowing data exchange and sharing through internet. The proposed system with XML is more robust than web-based medical information systems developed by using HTML, because XML itself provides more flexibility and extensibility than HTML.

Effects of Air Pollution on Public and Private Health Expenditures in Iran: A Time Series Study (1972-2014)

  • Objectives: Environmental pollution is a negative consequence of the development process, and many countries are grappling with this phenomenon. As a developing country, Iran is not exempt from this rule, and Iran pays huge expenditures for the consequences of pollution. The aim of this study was to analyze the long- and short-run impact of air pollution, along with other health indicators, on private and public health expenditures. Methods: This study was an applied and developmental study. Autoregressive distributed lag estimating models were used for the period of 1972 to 2014. In order to determine the co-integration between health expenditures and the infant mortality rate, fertility rate, per capita income, and pollution, we used the Wald test in Microfit version 4.1. We then used Eviews version 8 to evaluate the stationarity of the variables and to estimate the long- and short-run relationships. Results: Long-run air pollution had a positive and significant effect on health expenditures, so that a 1.00% increase in the index of carbon dioxide led to an increase of 3.32% and 1.16% in public and private health expenditures, respectively. Air pollution also had a greater impact on health expenditures in the long term than in the short term. Conclusions: The findings of this study indicate that among the factors affecting health expenditures, environmental quality and contaminants played the most important role. Therefore, in order to reduce the financial burden of health expenditures in Iran, it is essential to reduce air pollution by enacting and implementing laws that protect the environment.
