Purpose: Difficult organs or locations or inadequate tube intubations can cause complications. There are some cases in which the tube location changes or the tube is removed due to processing inside the organ while installing the stylet or rapid stylet removal. Thus, this study aimed to evaluate and develop an integrated intubation tube with stylet (IITS) for easier intubation of organs in emergency cases and reduce complications caused by the stylet. Methods: This study used a "Laerdal Airway Management Trainer". For stylet intubation, procedure No. 14 of the national practical test protocol was followed, but the removal step was omitted. In this study, each emergency case was intubated with an IITS, in which the stylet was not inserted or removed separately even though it has the function of an organ intubation stylet. Results: The existing classic ET intubation method had a success rate of 100% and had an average intubation time of 21.75 seconds, The developed IITS method was also successful in all cases and had an average intubation time of 15.78 seconds. Conclusion: Application of an IITS is expected to reduce intubation time and decrease inappropriate depth and intubation failure due to stylet removal, therefore improving the efficiency of airway maintenance.

U-health Service는 환자와 의료진과의 언제 어디에서나 의료서비스를 제공하는 것을 지칭하며 정보통신 기술과 보건의료를 융합한 서비스로 정의하고 있다. 그러나 환자의 의료정보, 개인정보 유출과 같은 사례가 발생하고 있고, 또한 네트워크를 통하여 데이터 송수신을 계승하므로써 기존 유 무선기반의 보안위협사항을 계승하는 취약점이 있다. 그러므로 본 논문에서는 U-Health Service에서 발생하는 취약점을 보완하고자 통합 인증스키마를 설계하여 안정한 개인정보에 대한 관리 시스템을 제안하였다. 제안프로토콜에서는 ID-Based Encryption을 활용하여 사용자 등록, 의료기관과 사용자간의 인증, 데이터 통신 암 복호화, 사용자 정보 폐기에 대한 프로토콜을 설계하였으며, 이에 따른 기존시스템 및 PKI Based 기반 통신과정과 보안성과 안전성에 관하여 분석하였다.

As medical service industry is transformed into the market centered on consumers, medical service quality patients recognize is emphasized as the powerful means to ensure competitive advantage among hospitals in the fierce medical markets. Many researches have been done on the definition of medical service quality, developing a scale to measure it, patient satisfaction, hospital repurchase and oral transmission intention, but integrated studies have not been done sufficiently on the patient' cognitive emotional aspects. For these research purposes, based on service relation, service quality, physical surroundings, human services, corporate reputation, service value, emotional response, customer satisfaction and repurchase intention, this paper suggests a theoretical modeling composed of hypotheses on the relations of each theoretical variable. In addition, the moderating effect of service relationships is investigated based on the structural equation model.

최근 병원은 ICT기술을 접목한 고품질의 의료서비스를 제공하는 허브병원으로서의 기능을 수행하기 위해 병원의 정보화를 통하여 직무능력을 향상하고 경영자의 의사결정지원시스템을 통하여 고도화된 통합의료정보시스템을 구축하고 있다. 본 연구에서는 환자정보와 진료정보의 신속한 조회로 진료편의성을 극대화 할 수 있도록 통합의료정보시스템에 OCS(Order Communication System)시스템과 HIS(Hospital Information System)시스템을 연동하여 최적화된 맞춤형 모바일 헬스케어 진료환경을 설계하여 환자의 치료 및 진료를 신속하고 정확하게 수행할 수 있는 체계적인 의료시스템을 연구하였다.

We build up distributed database of medical picture and design and realize H/W & S/W of special image workstation. We build up high speed image transmission system for distributed database and retrieval of various medical pictures in w ard through image transmission system and realize integrated image diagnosis. This system improves medical service by speedy diagnosis and enables more precise diagnosis by integrated image diagnosis through distributed database. In economical view this system curtails huge cost of film processing and transmission, which make medical expense cheaf, because it does not use film. We built up PACS in pediatric hospital of Seoul National University Hopital and tested the system with various medical pictures and showed that speedy integrated image diagnosis is possible.

The purpose of the study is to establish priorities in integrated medicine policy. Delphi method was used in conducting a survey on integrated medicine specialists regarding the policies of integrated medicine and actions. For specialists in integrated medicine, total 17 persons selected included scholars who performed minimum one study on integrated medicine, medical staff who performing integrated medical service at institutions that implemented integrated medical care or those who were involved in creating or implementing policies on integrated medicine. The first Delphi-method survey conducted from Apr. 14 through 18, while the second survey was carried out from Apr. 21 through 25. To prioritize the policies based on the results of the mini Delphi-method survey, Analytic Hierarchy Process(AHP) was performed for 15 persons answered to mini Delphi-method from May 2 through 16, 2014. From the Delphi-method survey, four policies and 16 actions were obtained. AHP showed the first priority placed on 'Building the Infrastructure of Industrialization in the Integrative medicine' of the 4 policies and 'Developing new diagnostic and treatment instruments' was given the highest priority of the 16 actions. For implementation of integrated medicine policy, the 4 policies and the 16 actions found in this research require government support and should be urgently implemented according to its priority.

Purpose: This study was done to examine the effect of an integrated care service which included a combination of oriental and western care on health outcomes in elderly patients with degenerative arthritis. Methods: A prospective comparative design was used. Data were collected from May 1, 2008 to June 30, 2009 from 85 elderly patients with degenerative arthritis in the lower extremities who were followed in a hospital out-patient department for 8 weeks. The integrated care service group (n=36) received a combination of physical therapy, acupuncture, western medicines or herbal medicines, and the western care group (n=49) received physical therapy or western medicines. Functional independence, walking speed, rotation balance, pain intensity, service satisfaction and total medical costs for the two groups were compared at 8 weeks. Results: Functional independence (t=2.14, p=.036) and walking speed (t=2.51, p=.014) improved significantly in the integrated care group while pain intensity improved significantly in the western care group (t=3.35, p=.002). The integrated care group reported higher scores for service satisfaction (t=2.09, p=.041) and higher medical costs than the western care group (t=2.15, p=.035). Conclusion: The results suggest that integrated care services are effective modalities to improve mobility and quality of life for elders with degenerative arthritis.

최근의 정보통신기술(ICT)은 관련기술과의 융합으로 모든 시스템을 통합하는 형태로 발전하고 있다. 이런 시대의 흐름과 마찬가지로 의료산업은 근래에 많은 발전 및 응용에 관한 연구가 활발히 진행되고 있다. 의료정보시스템도 의료 IT의 정보시스템들이 통합되는 방향으로 급변하게 진화해가고 있으며, 앞으로도 그 가속도는 더 할 전망이다. 특히 유비쿼터스 환경에서의 의료정보산업은 최근에 큰 전환기를 맞고 있다. 특히 정부의 정보통신부 IT839 전략으로 핵심기술 및 신규 서비스로 다양한 응용서비스 시범사업과 관련 요소기술 개발이 활발히 진행되고 있다. 따라서 본 논문에서는 의료정보시스템의 최근 동향을 업계 및 시스템적으로 분석하고, 유비쿼터스 컴퓨팅 환경에서의 미래의 통합의료정보시스템의 응용에 대한 발전 방향을 제시하고자 한다. 그리고 국가 주도의 산업성장으로서 유비쿼터스 환경의 구축을 위한 병원 응용 시스템 구축을 제시하며, USN(Ubiquitous Sensor Network) 환경의 IT응용 서비스가 실용화하고 있는 실정에서 통합의료정보를 위한 환자 진료의 서비스 강화를 도모하도록 통합의료정보시스템을 제안 및 설계하였다. 그리고 병원의 실제 EMR, HIS, PACS의 호환을 위한 솔루션을 제시하였다.

Lower limb amputees are increasing due to various reasons. It is difficult for lower limb amputees to walk without an assistive device such as a prosthetic leg. In this paper, an electronically controlled knee-type prosthetic leg with a 4-bar linkage structure for lower limb amputees was developed. The knee-type prosthetic leg has a 4-bar linkage structure and assists walking by using an integrated drive module. The torque is 90 Nm, the rotation speed is up to 120 deg, and it weight 1.9 kg, so it is lighter than a commercial prosthetic leg, so it can be used for a long time because there is less fatigue when walking. An integrated control board was developed by applying various sensors and microprocessor. The motor drive and encoder are built into the integrated drive module. The integrated control board and integrated drive module communicate using CAN. When a lower limb amputee wears a knee-type prosthetic leg and walks, it shows a shape similar to the swing phase graph of a normal people, and it is possible to walk naturally while walking.

최근 전문의 응급의료센터 당직을 규정하는 것을 골자로 응급의료법이 개정되면서 응급의료센터 시스템에 많은 변화가 생길 것 이다. 응급환자가 응급의료센터를 찾을 경우 응급실 근무의사 ${\rightarrow}$ 해당과 인턴 ${\rightarrow}$ 1 2년차 전공의(레지던트) ${\rightarrow}$ 3 4년차 레지던트를 거쳐야 전문의 진료를 받을 수 있던 것을 응급실 근무의사 ${\rightarrow}$ 해당과 당직전문의 진료 또는 호출로 그 절차가 간소화됐다. 문제는 이런 시스템을 하려면 모든 진료과목에 대해 당직전문의를 배치해야 한다. 하지만 모든 병원이 모든 진료 과목에 대해 전문의를 배치하는 것은 어렵다. 본 논문에서는 모바일 기기를 이용하여 사용자 근처의 응급의료센터가 있는 병원과 해당 병원이 어떤 전문의가 있는지와 병원의 정보와 응급실의 사용현황을 개정되는 응급의료법을 위해 통합 응급실 관리 시스템을 설계한다.
