노인의 수면생리와 노인에서 흔한 수면장애 (Sleep Physiology and Common Sleep Disorders in the Elderly)

  • Sleep changes substantially with age. There is a phase advance in the circadian sleep cycle and increased waking after sleep onset. The elderly people wake more frequently during the night and experience fragmented sleep and excessive daytime sleepiness. The prevalence of sleep disorders increases with age, and the composition of sleep disorders in the elderly differs from that in the young. The most frequently encountered sleep disorders are psychophysiologic insomnia, sleep disturbance due to dementia, sleeprelated respiratory disorder, restless legs syndrome and periodic limb movement disorder, and REM sleep behavior disorder. To treat the elderly sleep problem appropriately, it is important to know how sleep pattern changes as we age and to understand the cause of sleep-related symptoms. This article will review the sleep physiology and common sleep disorders in the elderly.

현침요법을 통한 울증 치료 2례 (The Case Study of a Patient with Ul-zeung who has treated by Hyun-chim therapy)

  • Hyun-chim Therapy was made for help the people who didn't learn breath training. The doctor acupuncture and treat 'Hyun-chi-sul' and it is a means by which patient can use 'universal Qi'. Ul-zeung comes from obstruction of qi by stress. The mind affect the body. The patient has depressed mood, irritable sign, chest discomfort, flank pain, angry stats and some strange feeling on the throat. In the case, two female patients, who complained of depression, insomnia, anxiety, palpitation, low energy, etc. We treated the patients with Hyun-chim Therapy. In result, the symptoms which they complained were improved.

디펜히드라민 음독 후 경기를 주소로 응급센터에 내원한 환자 1례 (A Case of Seizure in Diphenhydramine Overdose)

  • A 21-year-old woman ingested 1,250 mg of diphenhydramine in a single overdose. Diphenhydramine, a rare ingredient in over-the-counter medication, is used to treat insomnia in Korea. Toxicity is usually limited to anticholinergic symptoms. The standard approach to therapy for the treatment of diphenhydramine overdose is supportive care, including physostigmines and sodium bicarbonates. Here, we review the literature and for the first time report a case of acute diphenhydramine overdosage in Korea, complicated with seizures.

산후 우울증 환자 2례에 대한 임상보고 (2 Cases Report of Postpartum Depression Patient)

  • Purpose: The purpose of this study is to report the effect of oriental treatments to postpartum depression. Methods: We treated 41 years old woman and 38 years old woman who had the postpartum depression. In the beginning of treatment, they had anxiety, depression, insomnia, fatigue. They were treated by oriental treatments such as herb medicine, acupuncture, moxa therapy. And then We compared postpartum depression before treatment with one after treatment by Edinburgh postnatal depression scale(EPDS). Results: After the oriental treatments, the clinical symptoms were improved and EPDS was reduced (one reduces from 18 to 10, the other reduces from 17 to 8). Conclusion: Oriental treatments are effective on postpartum depression. After this, further approach and study on postpartum depression might be needed.

뇌경색 후 발생한 번조증에 대한 황련해독탕 약침의 병행 치험 1례 (A Case Report of the Agitation(Byun-jo) Caused by Cerebral Infarction Treated with Hwangryunhaedok-tang Herbal-Acupuncture Therapy)

  • Object : The purpose of this study is to evaluate the effect of Hwangryunhaedok-tang herbal-acupuncture therapy for Agitation(Byun-jo) caused by cerebral infarction. Methods : One patient was admitted who was suffering from insomnia, chest discomfort, anxiety, palpitation of Agitation(Byun-jo), after cerebral infarction. In the point of Differentiation of Syndromes(辨證), we diagnosed this patient as Excessive heat-fire syndrome of stroke(中風火熱證) and treated with Hwangryunhaedok-tang herbal-acupuncture therapy at CV17, BL15, GB20. Results : After Hwangryunhaedok-tang herbal-acupuncture therapy, symtoms of Agitation(Byun-jo) were improved remarkably, and the score of State-Trait Anxiety Inventory(STAI), Pittsburgh Sleep Quality Index(PSQI) were decreased. Conclusions : Hwangryunhaedok-tang herbal-acupuncture therapy has meaningful effect on the symptoms of Agitation(Byun-jo) caused by cerebral infarction and more researches should be followed.

  • The irritable bowel syndrome(IBS) is the most common gastrointestinal disorder in clinical practice and is characterized by abdominal pain associated with a chronic disturbance of defecation. It is important to discriminate IBS from organic bowel disease. Based on clinical and epidemiologic studies, specific symptom criteria for IBS have been developed. The subject is a 29 year old man who has abdominal pain, chronic diarrhea, general weakness, dizziness, insomnia and has been diagnosed as irritable bowel syndrome. We classified this as Shaoyangin Mangyin(少陽人 亡陰證) and prescribed Hyeongbangjiwhangtang(荊防地黃湯) in accordance with the principle of pyoeumganggi(表陰降氣). In the result, we had the improvement of his symptoms.

  • Restless leg syndrome(RLS) is a sensorimotor disorder characterized by a distressing urge to move the legs and triggered by rest or inactivity. It is usually accompanied with insomnia. We experienced two cases of RLS among ischemic stroke patients. They complained uncomfortable sensation in their affected leg and urge to move the leg at night. We treated them with electro-acupuncture, moxibustion and herbal medicine until retiring. As a result, the symptoms including unpleasant sensation or pain, urge to move the leg and insomnia had withdrawn. These cases suggest the possibility that we can treat RLS with oriental medicine, especially in the case which is refractory to conventional medication or shows periodic leg movements during sleep.

한의학 고문헌 분석을 통한 무궁화의 약용부위와 기대효능 연구 (Study on the Medicinal Parts and Expected Efficacys of the Hibiscus syriacus by Literature Review on the Classics of Oriental Medicine)

  • Hibiscus syriacus is rarely used herb. For finding up new applications of this herb, the medicinal parts and expected efficacys that recorded in 24 classics of oriental medicine were inquired. As a result of the literature review, medicinal parts and expected efficacys of Hibiscus syriacus were as follows: The medicinal parts were mainly flower and bark. But the root, branch, leaf and fruit were mentioned too. Some unidentified medicinal parts seemed to be branch or bark. The mentioned symptoms that prescribed Hibiscus syriacus are intestinal 'wind', red-white dysentery, thirst with fever after dysentery, insomnia, 'wind' syndrom, leukorrhea, regurgitation, etc. And this herb was mentioned that applied externally for scabies, hemorrhoids, prolapse of the rectum, headache, extracted tooth, etc. Therefore, bleeding intestinal diseases, insomnia, vaginal or uterine hemorrhage(for internal use), scabies or mycosis, hemorrhoids. anal fistula, prolapse of the rectum, headache(for external use) were presented as expected efficacys of Hibiscus syriacus. In future, studies for verifying the efficacys are needed.

Acupuncture as a Complementary Treatment for Cancer Patients Receiving Chemotherapy

  • Background: Medical treatment for eliminating the side effects of cancer therapy may not always be efficacious. Acupuncture is one of the most widely accepted alternative and complementary therapies in use today. In this study, we investigated the efficacy of acupuncture in patients experiencing cancer treatment side effects, including nausea, vomiting, pain, poor sleep quality and anxiety. Materials and Methods: A total of 45 inpatients who underwent chemotherapy between February and April 2013 in the Oncology Department of Numune Hospital were included in our study. Acupuncture was administered to the patients one day prior to chemotherapy, on the day of chemotherapy and one day after chemotherapy. The patients were evaluated on nausea, vomiting, pain, sleep quality and anxiety before the chemotherapy and on the $4^{th}$ day of chemotherapy. Results: Of the 45 patients included in the study, 18 (40%) were female and 27 (60%) were male. A total of 25 (55.6%) had an elementary school education; 32 patients (71%) had stage 4 cancer and were treated with palliative chemotherapy (the patient characteristics are shown in Table 1). Statistically significant decreases (p<0.001) in pain, nausea, vomiting, insomnia and anxiety scores were observed after the acupuncture treatment compared to baseline. There were no differences in the age, gender, education level, stage or metastasis levels between the patient groups whose symptoms improved or were unchanged. Conclusions: Our study showed that acupuncture has positive effects in cancer treatment patients who experience nausea, vomiting, pain, poor sleep quality and anxiety as side effects of chemotherapy. Chemotherapy-related side effects in cancer patients could be decreased by the concurrent use of acupuncture.
