개화기 사과 '후지'와 '홍로'의 품종간 저온 피해율, 호르몬과 유리당 분석 (Analysis of Freezing Injury Rate, Hormone and Soluble Sugars between 'Fuji' and 'Hongro' Apple Trees in Flowering Period)

  • 최근 지구온난화에 의한 이상기상으로 겨울철 한파와 봄철 저온에 의한 농작물 피해가 심각하게 발생하고 있다. 특히 과수의 개화기 저온피해는 꽃눈의 생육단계에 따라 차이가 있으며 발육이 진전될수록 내한성이 약해져 개화 직전부터 낙화 후 1주까지 한계온도가 다르게 발생한다. 따라서 개화기가 빠른 사과 '홍로'가 '후지'보다 피해가 심각한 것이 일반적이나 2020년 4월 저온피해는 개화기가 늦은 '후지'의 피해가 심하게 발생하여그 원인을 분석하고자 본 연구를 수행하였다. 충북 보은군 사과나무 '후지'와 '홍로'를 동시에 재배하는 2농가를 대상으로 품종 간 피해율 실태조사를 실시하였다. 또한 정확한 품종 간 비교 분석을 위하여 생육단계가 동일한 시료를 선택하여 인위적으로 저온처리(-2.0℃, -4.0℃)를 하여 피해 정도를 조사하고, 원인 분석을 위해 조직 내 유리당과 호르몬 함량을 분석하였다. 실태조사 결과 2농가 모두 '후지'가 '홍로'보다 피해율이 높았으며, 특히 B농가(저지대, 평지) '후지'의 경우 피해율이 60.5%로 가장 높았다. 또한 동일한 생육단계의 시료를 사용한 인위적 저온 처리 시험결과에서도 '후지'와 '홍로' 품종 간 피해율에 유의한 차이가 있었다. 유리당 함량은 저온 피해율이 낮았던 '홍로'가'후지'보다 높았으며, 호르몬 분석 결과 정상 꽃눈보다 손상된 조직에서 ABA, IAA와 SA 함량이 높게 나타났다. 따라서 본 연구 결과 조직내 유리당 함량이 높으면 저온 피해율이 낮은 것을 확인할 수 있었으며, 특히 저온 피해율은 sorbitol 함량과 부의 상관관계를 이루고 있다.

응급실을 통하여 입원한 체간부 관통상 환자에 관한 임상적인 고찰 (Clinical Patterns of Penetrating Torso Injury at Emergency Department)

  • Background: This study was undertaken to evaluate the clinical pattern of the patients with penetrating torso injury. We retrospectively analyzed the clinical symptoms, mechanism of injury, injury type including injured organ, and ultimate outcome of treatment. Our purpose of the study was to establish guideline of management in penetrating torso injury. Methods: This study consists of an analysis of a consecutive series of 94 patients with penetrating injury of trunk treated at one general hospital during 7year period (from January 1995 to April 2003) who was admitted through in our emergency department. All data were collected from the medical records and entered in a database for analysis on the following: age, sex, mechanism of injury, vital sign at admission, clinical outcome including hospital mortality, length of hospital stay, length of intensive care unit stay, requirement of crystalloid fluid and blood product. Results: Among 94 patients, there were 68 men and 26 women, with ages ranging from 19 to 82 years (average 38.2 years). The most frequent mechanism of injury was violence by others including rob (n=54, 57.4%) followed by suicidal attempt (n=24, 25.5%) and accidental injury (n=16, 17.0%). No injury was inflicted from gun. In 37 patients, systolic blood pressure at admission was under 90mmHg. The time interval from injury to admission, and from admission to operation was 57.8minutes and 4hour 12minutes each. Laparotomy was required in 70 patients, thoracotomy in 5 patients, and 3 patients required thoracotomy and laparotomy. Among 94 patients, an average of 1.7 organs were injured. The small bowel and colon were the organs most commonly wounded followed by liver, mesentery, pleura. Of the 94 patients, 6 died for an overall mortality rate of 6.4%, and two of them were not related with hemorrhage. The average length of hospital stay was 18.1 days, and 40 patients required ICU care. Conclusion: Of the 94 patients who were admitted from penetrating torso injury, no patient was injured from firearm. Overall mortality rate was 6.4%. In our hospital, firearm injury was relative rare.

초등학교 아동의 사고예방행동과 사고발생의 관련성 연구 (A Study on the Relationship between Injury Preventive Behaviors and Accidents for Elementary School Children)

  • Nowadays according to the complexity of our daily life due to the development of industry and transportation various kinds of injuries happen. Since 1993 the death rate among 5-14 age children by injuries amount to about 57%. Furthermore, the injury of children can give a direct damage to their own family and it can also bring a serious loss to the country as well as the community where they live in accord with degree of an injury. From this reason, the importance of safety education has been increasing. In this study, it is aimed to clarify the effect of injury preventive behaviors on injury incidence of the 6th grade students in the elementary school. This study is accomplished by the survey sampled from 9 elementary schools in seoul, each of which belong to different district. The survey took for 15 days from the date of september 10th 1996 to september 25th 1996 and the results were as follows; 1. For demographic characteristic, sexual distribution was 48.7% of male students and 47.1% of female students. The required time from home to school was 10-19 minutes(51.8%), and the most way of their going to school is to walk(89.7%). Mothers who graduated from high school were 46.6% and 49.4% of fathers were above university graduated level. The most vocation of mothers was housewife as 56.7%. 2. The results of analysis on the injury rates, 75.8% of children experienced more than one injury and the most was play injury. With regards to sites where injuries happened, the room, the kitchen, and the living room in the house ranked first(38.3%) and then injuries at school such as on the playground, in the classroom, and on the stairs was 34.1%. 3. Considering the rate of injury incidence in accordance with demographic characteristics, male students experienced more injuries than female students(p〈0.05). For the way from home to school, children who went to school by bicycle experienced the most injuries and then on foot, by bus in order of which differences were statistically significant(p〈0.05). 4. From the comparison of parents' safety education practice and the injury incidence, the less often parents practice safety education, the more often children experience injuries(p〈0.05). 5. The results of analysis on the effect of injury preventive behaviors on accidents children who didn't act injury preventive behaviors experienced much more injuries than those who did injury preventive behaviors of which differences were statistically significant(p〈0.01). 6. From the results of analysis of factors effecting on the elementary school children's injuries, children without injury preventive behaviors had more effect on accidents than those who had injury preventive behaviors as odds ratio 2.06(p〈0.01), and the odds ratio of male students compared with female students was 1.47(p〈0.05).

수도권지역 일개 전문 응급의료센터로 내원한 외상환자의 연령군별 특성 (Epidemiologic Analysis of the Trauma Patients Visiting an Urban Specialized-emergency Medical Center)

  • Background: Although trauma has been the major cause of death in korea, there are few reports describing the epidemiologic characteristics of trauma victims according to age-group. Thus, this study aimed to describe the epidemiologic characteristics of trauma victims visiting an urban emergency medical center in Korea. Method: This study was conducted with all trauma victims who visited the emergency departments of Asan Medical Center from April 11, 2005 to May 10, 2005. After the subjects were divided into three groups such as child (${\leq}14$ years old), adult(15-64 years old) and elderly(${\geq}65$ years old), the prehospital courses, injury mechanisms, injury severities and clinical results were compared. Result: Among total 5,927 patients who visited the emergency medical center, 896(15.1%) patients were trauma victims. Although child under the age of 15 comprised 28.7% of the total trauma victims, less severe injuries(injury severity score < 9) accounted for 97.7% of the cases. The hospitalization rate for injury among child was 21.5%. however, although elderly aged 65 comprised only 5.9% of the total trauma victims, more severe injuries(injury severity score ${\geq}9$) accounted for 30.2% of the cases. The hospitalization rate for injury among elderly was 56.4%. Conclusion: Patterns of injury and clinical results by age-group were considerably different. less severe and non-hospitalized injuries were more common in child than other age-groups. while severe and hospitalized injuries were common in elderly.

두부외상후 심리사회적 예후 (Psychosocial Outcome after Head Injury)

  • Objective : This study was designed to evaluate the relationship between the initial neurosurgical or psychosocial factors and the psychosocial outcome. Patients and Methods : We analyzed 123 head-injured patients who were referred to the department of psychiatry for the evaluation of psychosocial function. We analyzed initial neurosurgical variables such as Glasgow Coma scale(GCS) score, skull fracture, CT finding, and psychosocial outcomes with regards to psychosis, personality change, depression, anxiety and IQ on Intelligence Scale. Results : Patients with mild head injury(GCS score 13-15, N=94, 76.4%) had better recovery rate on Glasgow Outcome Scale(GOS), less personality change than those with moderate or severe head injury. However, depression, anxiety and intelligence were not significantly different between two groups. The skull fracture(N=37, 30.1%) did not influence on the psychosocial outcome with reference to personality change, depression, anxiety and intelligence. The patients with abnormal CT findings(N=64, 52%) had lower recovery rate on GOS, more frequent tendency in psychosis, personality change and severe depression, less frequent in anxiety and mild depression, than patients with normal CT finding. However, levels of intelligence were not different between two groups. The patients with industrial accidents(IA) had lower educational level, milder head injury, more delay for the psychiatric evaluation (longer treatment period) than those with motor vehicular accidents(MVA). The psychosocial outcome with reference to personality change, depression, anxiety, intelligence were not different between two groups. Conclusion : These findings indicate that the more severe initial trauma, the poorer psychosocial outcome. However, it was frequently observed that patients with mild head injury suffered from mild anxiety and depression. Therefore mild head injury appeared to be more complicated by psychosocial stressors. The patients with IA, despite the fact that initial head injury was mild, required longer treatment period than MVA.

우리나라 자전거 손상 환자의 역학적 특징: 전국 55개 병원 응급실 환자를 대상으로 (The Epidemiology of Bicycle Injury in Korea: Patients Who Visited 55 Emergency Rooms)

  • Purpose: Bicycle riding is a healthy and cheap form of transportation and a popular form of recreation. This study aims to evaluate the epidemiology and the factors affecting the severity of bicycle-related injuries and to find effective methods for preventing injuries. Methods: This is a retrospective study of the patients'characteristics, the injury severity, the injury location, the effect of safety equipment on the bicycle injury based on information collected through the NEDIS (National Emergency Department Information System) from 1,284,429 patients who visited 55 emergency rooms nationwide. Results: During the research period throughout 55 emergency rooms 5,671 patients were seen because of bicycle injuries. The male-to-female ratio was 3.1:1, the median age 28 years old, and 42.6% of the patients were younger than 20 years old. Injury sites were mostly in the extremities(46.7%), the head(32.4%), and the face(14%), and 70.3% of severely injured patients had accompanying head trauma. Males aged 65 and older was possibly associated with severe head trauma. Of the patients who provided helmet information 4.8%(71patients) used helmets, and 95.2%(1392 patients) did not. In this group of patients providing helmet information, none of those wearing helmets died, but 0.4% of those not wearing helmets died. Conclusion: In this study, most bicycle injuries occurred in young adult men. From now, there seems to be a need for more effort on publicity activities on bicycle injuries and on the education of children and teenagers, who show a high incidence rate, and of senior citizens (over 65 years old) who show a high severity rate, about using a safety helmet to reduce the severity of injury.

저소득층 가정 부모의 아동안전사고 예방행위 실천에 영향을 미치는 요인 (Factors Affecting Parental Practices of In-home Injury Prevention for Young Children in Low-Income Families)

  • Purpose: This study examined the characteristics of in-home injuries of children in low-income families and sought to identify the factors affecting parental in-home injury prevention practices. Methods: A cross-sectional descriptive survey design was applied, using questionnaires on in-home injury characteristics in children, parental in-home injury prevention practices, parental perceptions and knowledge on childhood injuries, and the Parental Stress Index. We queried 169 parents of children less than 5years of age who were enrolled in Nutrition Plus Projects at community health centers. Results: Overall, 92.7% of children had experienced in-home injuries, with sliding crashes and bumping injuries as the most frequent type of injury. The recovery rate with a scar after injury was 26.3%. Parental practices for in-home injury prevention were higher according parental age, educational status, and previous learning experiences regarding in-home safety and injury prevention. The two most significant factors affecting parental in-home injury prevention practices were age and parental perception of childhood injuries as being controllable and preventable. Conclusions: Considering the high risk of in-home childhood injuries in low-income families, safety education and the promotion of injury prevention practices for parents are recommended. The strategy to enhance the parental perception on preventing childhood injuries needs to be addressed.

Characteristics and Outcomes of Trauma Patients via Emergency Medical Services

  • Purpose: The aim of this study was to identify clinical outcome and characteristics of trauma patients via emergency medical services (EMS). Methods: Medical records of the trauma patients visiting the emergency department were retrospectively collected and analyzed from January 2015 to June 2016 in the single institution. Of 529 registered patients, 371 patients were transported by - were enrolled. The parameters including age, gender, injury mechanism, Glasgow coma scale on arrival, presence of shock (systemic blood pressure <90 mmHg) on arrival, time to arrival from accident to emergency room (ER), need for emergency procedures such as operation or angioembolization, need for intensive care unit (ICU) admission, injury severity score (ISS), the trauma and injury severity score, revised trauma score (RTS), length of stay, and mortality rate were collected. The SAS version 9.4 (SAS Institute, Cary, NC, USA) was used for the data analysis. Results: Arrival time from the field to the ER was significantly shorter in EMS group. However, overall outcomes including mortalities, length of stay in the ICU and hospital were same between both groups. Age, ISS, RTS, and injury mechanisms were significantly different in both groups. ISS, RTS, and age showed significant influence on mortality statistically (p<0.05). Conclusions: The time to arrival of EMS was fast but had no effect on length of hospital stay, mortality rate. Further research that incorporates pre-hospital factors influence clinical outcomes should be conducted to evaluate the effectiveness of such a system in trauma care of Korea.

실험교흔조직에서 생물학적 Amines정량에 의한 수상 후 경과시간 판정에 관한 연구 (A Study on the Time Course Changes of Biogenic Amines in Response to Bite Injury)

  • The present studies, the investigations were undertaken for an attemp to analyze time course change of biogemic amines in respons to bite injury in rats and guinea pigs, and obtained the following results that might be applicable to the forensic medicine and Forensic Odontology. The histamine concentrations measured at rat abdominal skins were decreased during the first 8 hours and recovered to control levels after 12 hours. However the serotonin concentrations were decreased during the first hours and increased during 4-8 hours, and then returned to control levels thereafter. In contrast with these results, both histamine and serotonin concentrations were tended to increase in guinea pig skins, but the concetration of histamine showed maximum rate of increase immediately after injury and those of sertonin showed maximum rate of increase at 30 minutes. The differencd in the response of histamine and serotonine after injury between rats and ginea pigs revealed that there is specials deffrence in the response of biogenic amines with regard to external stimuli. These results suggest that the determinition of biogenic amines in skin lesion of bite marks could be applied to the estimation of the elapsed time after antemortem injury.

Successful TAE after DCS for Active Arterial Bleeding from Blunt Hepatic Injury in a Child: A Case Report

  • Transcatheter arterial embolization (TAE) for blunt hepatic injury in children is not common and is especially rare after damage control surgery (DCS). We report a successful TAE after DCS on a child for massive bleeding from the left hepatic artery due to a motor vehicle accident. The car (a sport utility vehicle) ran over the chest and abdomen of a 4-year-old boy. On arrival, initial vital signs were as follows: blood pressure, 70/40 mmHg; heart rate, 149/min; temperature, $36.7^{\circ}C$; respiratory rate, 38/min. After resuscitation, computed tomography was done, and a suspicious contrast leakage from a branch of the left hepatic artery and a spleen injury (grade V) were found. TAE was performed successfully after DCS for a liver injury.
