A Case of Membranous Glomerulonephritis Associated with Small Cell Lung Cancer Being Complete Remmission with Chemotherapy (항암치료 후 완전관해된 소세포암과 동반된 막성사구체신염 1예)

  • Paraneoplastic nephrotic syndrome can be diagnosed from its clinical and immunological features. The development of several types of glomerular injury in patients with cancer have been recognized, and are considered as paraneoplastic syndrome. Most prominent are the occurrence of membranous glomerulonephritis in patients with carcinomas. We report a case of a 60-year-old-man with small cell lung cancer presenting as nephrotic syndrome. A renal biopsy revealed membranous glomerulonephritis. Six lots of chemotherapy were administered, which led to a complete tumor response with total resolution of the nephrotic syndrome following treatment.

Incidence of Severe Crown Gall Disease on Tetraploid Cultivars of Grape in Korea

  • The main grape producing regions in Korea were surveyed for the occurrence and relative incidence of crown gall disease on grapevine. The results of the survey showed that the disease tended to affect tetraploid cultivars, which produce a large and sweet fruit but are very weak to cold weather. Incidence of crown gall disease was high on the tetraploid cultivars, Kyoho (Gerbong), Daebong, and Black Olympia while it was low on the diploid cultivars, Cambell Early and Sheridan. The disease incidence was very high on Anseong, Cheonan, and Chincheon, where the major growing areas of tetraploid cultivars and grapevines were burried in the winter to protect from freeze injury, whereas it was low in Yungdong, Kimcheon, and Nonsan. Crown gall disease did not increased with grapevine age on both Cambell Early and Kyoho. It remained low in Cambell Early, but high for all ages on Kyoho, Galls were found on the crown region and mid part of the trunk, but more galls were on small branches and canes on Kyoho grapevines. More than one third of ZKyoho grapevines inspected had galls on multiple locations on grapevines. On Kyoho, 56.3% of the galls covered more than 50% of the crown gall disease affects severely on the cultivar.

Analysis of the Characteristic of Railroad(level-crossing) Accident Frequency (철도 건널목 사고의 발생빈도 특성분석 연구)

  • Railroad traffic accident consists of train accident, level-crossing accident, traffic death and injury accident caused by train or vehicle, and it is showing a continuous downward trend over a long period of time. As a result of the frequency comparison of train accidents and level-crossing accidents using the railway accident statistics data of Railway Industry Information Center, the share of train accident is over 90% in the 1990s and 80% in the 2000s more than the one of level-crossing accidents. In this study, we investigated time series characteristic and short-term prediction of railroad crossing, as well as seasonal characteristic. The analysis data has been accumulated over the past 20 years by using the frequency data of level-crossing accident, and was used as a frequency data per month and year. As a result of the analysis, the frequency of accident has the characteristics of the seasonal occurrence, and it doesn't show the significant decreasing trend in a short-term.

A Retrospective Review of Iatrogenic Skin and Soft Tissue Injuries

  • Background Even though the quality of medical and surgical care has improved remarkably over time, iatrogenic injuries that require surgical treatment including injuries caused by cast and elastic bandage pressure, extravasation, and dopamine-induced ischemia still frequently occur. The goal of this study was to estimate the incidence and analyze the distribution of iatrogenic injuries referred to our department. Methods A retrospective clinical review was performed from April 2006 to November 2010. In total, 196 patients (116 females and 80 males) were referred to the plastic surgery department for the treatment of iatrogenic injuries. We analyzed the types and anatomic locations of iatrogenic complications, along with therapeutic results. Results An extravasation injury (65 cases, 37.4%) was the most common iatrogenic complication in our study sample, followed by splint-induced skin ulceration, dopamine-induced necrosis, prefabricated pneumatic walking brace-related wounds and elastic bandage-induced wounds. Among these, prefabricated pneumatic walking brace-related complication incidence increased the most during the 5-year study period. Conclusions The awareness of the very common iatrogenic complications and its causes may allow physicians to reduce their occurrence and allow for earlier detection and referral to a plastic surgeon. We believe this is the first study to analyze iatrogenic complications referred to a plastic surgery department in a hospital unit.

COMPLICATIONS ASSOCIATED WITH OPEN REDUCTION OF MANDIBULAR FRACTURES (관혈적 정복술로 치료한 하악골 골절에서의 합병증)

  • Purpose: The aim of this study was to evaluate the complications of open reduction and internal fixation of mandibular fracture with miniplates. Patients and Methods: A total of 134 patients who presented with 196 fractures were analyzed retrospectively. Complications were evaluated for factors such as age, the site of fracture, the severity of fracture, delayed operation, preoperative wound contamination, the site and disposition of teeth in the fracture line and midfacial fractures. Statistical analysis was used to compare complications to risk factors. Results: Of the 134 patients, 20 patients had some form of postoperative complications and complication rate was 14.9%. These complications included infection, plate fracture, malocclusion, wound dehiscence, nerve injury and nonunion. There was a significant correlation between complication rate and the severity of fracture, preoperative wound contamination and the site of the fracture and disposition of teeth(p<0.05). Conclusion: The occurrence of postoperative complications in the treatment of mandibular fractures was related to the severity of fracture, preoperative wound contamination and the site of the fracture and disposition of teeth.

DEVELOPMENT OF ROLLOVER CRITERIA BASED ON SIMPLE PHYSICAL MODEL OF ROLLOVER EVENT

  • The high potential for injury involved in rollover accidents warrants the development of a system to protect passengers against such events. To effectively implement such a protection system, it is first necessary to determine rollover criteria (i.e., real time states which indicate the occurrence of rollover events). In this paper, several rollover criteria have been developed based on simplified physical models. Such accidents are first classified into two types, untripped and tripped, according to the main cause that initiates the rollover. Characteristics of these rollover situations are identified by applying appropriate principles of dynamics to corresponding simplified physical models. Two main rollover criteria, Rotational Kinetic Energy (RKE) and Initial Kinetic Energy (IKE), are then introduced based on these characteristics. ADAMS/View simulations have been performed to verify the feasibility of the introduced rollover criteria. ADAMS/Car simulations have also been conducted to get more realistic rollover data with a complete vehicle model. Results of these experiments reveal that our established criteria prove useful for predicting whether actual rollover occurs or not.

Delayed contralateral traumatic carotid cavernous fistula after craniomaxillofacial fractures

  • A carotid-cavernous sinus fistula is a rare condition in which an abnormal communication exists between the internal or external carotid artery and the cavernous sinus. It typically occurs within a few weeks after craniomaxillofacial trauma. In most cases, the carotid-cavernous sinus fistula occurs on the same side as the craniomaxillofacial fracture. We report a case of delayed carotid-cavernous sinus fistula that developed symptoms 7 months after the craniomaxillofacial fracture. The fistula developed on the side opposite to that of the craniomaxillofacial fracture. Based on our experience with this case, we recommend a long follow-up period of 7-8 months after the occurrence of a craniomaxillofacial fracture. We also recommend that the follow-up should include consideration of the side contralateral to the injury.

Factors Influencing on Pressure Ulcer Incidence among Older Patients with Hip Fracture in a Hospital (고관절 골절로 입원한 노인 환자의 욕창 발생 위험요인)

  • Purpose: This study aimed to identify the incidence and risks for pressure ulcer among older patients with hip fracture. Methods: The subject were 215 older patients suffering from hip fracture who were admitted for surgical operation from January 1, 2012 to April 30, 2016 in a university-affiliated hospital. The incidence of pressure ulcer was collected retrospectively through medical record review and the risk factors were analyzed using Cox's proportional hazard model. Results: Out of the total, 32 patients (14.9%) developed pressure ulcer with the average occurrence period being 4.72 (${\pm}3.81$) days. Stage II pressure ulcer was the most common at 72.0%. Risk factors included ambulation status before injury (p= .039), spinal anesthesia (p= .029), and stay at intensive care unit after operation (p= .009). Conclusion: Despite pressure ulcer prevention efforts, the incidence remained relatively high. Considering the identified risk factors, more efforts is needed for early detection and prevention of pressure ulcers in such patients.

NRF2 activation by 2-methoxycinnamaldehyde attenuates inflammatory responses in macrophages via enhancing autophagy flux

  • A well-controlled inflammatory response is crucial for the recovery from injury and maintenance of tissue homeostasis. The anti-inflammatory response of 2-methoxycinnamaldehyde (2-MCA), a natural compound derived from cinnamon, has been studied; however, the underlying mechanism on macrophage has not been fully elucidated. In this study, LPS-stimulated production of TNF-α and NO was reduced by 2-MCA in macrophages. 2-MCA significantly activated the NRF2 pathway, and expression levels of autophagy-associated proteins in macrophages, including LC3 and P62, were enhanced via NRF2 activation regardless of LPS treatment, suggesting the occurrence of 2-MCA-mediated autophagy. Moreover, evaluation of autophagy flux using luciferase-conjugated LC3 revealed that incremental LC3 and P62 levels are coupled to enhanced autophagy flux. Finally, reduced expression levels of TNF-α and NOS2 by 2-MCA were reversed by autophagy inhibitors, such as bafilomycin A1 and NH4Cl, in LPS-stimulated macrophages. In conclusion, 2-MCA enhances autophagy flux in macrophages via NRF2 activation and consequently reduces LPS-induced inflammation.

  • Acupuncture, which is a procedure of inserting needles in a specific portion of the body to control pain, is conducted widely. The procedure is considered relatively safe because of its minimal side effects and low occurrence (1 in 200,000). On the other hand, its side effects include neurological damage, infection, hematoma, emphysema, etc. The authors report a case of cervical neurological damage that occurred after an acupuncture procedure.
