A Literatural study on the hemorrhoids and hemorrhoids complicated by anal fistula (痔瘡과 痔瘻에 對한 文獻的 考察)

  • A Literatural study on the etiological factors, classification, prescription of hemorrhoids and hemorrhoids complicated by anal fistula following results were obtained. 1. The cause of hemorrhoids are long time sit, long time gate, overfatigue, overeating, imbalance of stool( constipation or diarrhea), pregnant fertility(overfatigue after childbirth, insufficiency of middle warmer energy), uncontrol sexual excess, pathgenic factors of wetness, heat, wind, dry, genetic cause, excess of anxiety, pile up of heat poison, weakness of entrails and viscera. The cause of hemorrhoid complicated by anal fistula are attack of external wind, heatness, dry, fire, wetness(pathgenic factors), inapporiate treatment and chronic disease, greasy diet, excess of anxiety, constipation, uncontrol sexual excess, obstacle of circulation of vital energy and blood on anal site. 2. Classification of hemorrhoids are female hemorrhoids, male hemorrhoids, pulse hemorrhoids, intestines hemorrhoids, vital energy hemorrhoids, wine hemorrhoids, blood hemonhoids, flowing hemorrhoids. Classification with other method are external hemorrhoids, internal hemorrhoids, mixed hemorrhoids, excrescence hemorrhoids, nipple homorrhoids. External hemorrhoids is classified of varicosis of hemorrhoidal vein, connective tissue form, thrombus form. Classification of hemorrhoid complicated by anal fistula are simple lower hemorrhoid, lower mixed hemorrhoid, deep hemorrhoid, outer of one hole hemorrhoid, a horseshoe hemorrhoids. Once more classificated of four are space of sphincter muscle form, penetration sphincter muscle form, upper of sphincter muscle form, outer of sphincter muscle form. 3. Therapy method of hermorrhoid and hemorrhoid complicated by anal fistula are internal method, fumigation method method, ointment, method of close with medicine, necrotizing method, hot medicated compress( gxternal method), injection, insertion, bind, (operation) and acupuncture therapy (the others method) 4. Herb medicine for many used of internal method are Scutellaria baikalensis George(黃芩), Coptis japonia Makino(黃連), Rehmania giutinosa Liboschitz ex Fischer & Meyer(生地黃), Poncirus trifoliata Refinesque(枳殼), Sanguisorba officinalis Linne(地楡), Sophora japonica L.(槐花), Cnidium officinale Makino (川芎), Astragalus membranaceus Bunge(황기), Angelica gigas Nakai (當歸). 5. Herb medicine for many used of fumigation are Schlechtendalia Chinesis J. Bell (五倍子), Artemisia Vulgaris L. var indica Maxim(艾葉), Poncirus trifoliata Refinesque (枳殼), Nepeta japonica Maximowicy(荊芥), And herb medicine for many used of ointment are Calomelas(輕粉), Alum(白礬), Boswellia carterii Birdwood(乳香), Os Draconis Fossilia Ossis Mastodi(龍骨).

Change in Risk of Dropout Due to Bleeding during Bloodletting-Cupping Therapy (습식 부항 시술시 사혈량에 따른 부항 탈락 위험도 탐색)

  • Objectives : To investigate minimum pressure by verifying changes in pressure due to bleeding amount during bloodletting-cupping therapy. Methods : (1) We compared adhesion performance of four different cupping cups of same size: two disposable cupping cups(A, B) and two reusable cupping cups(A, B) each were vacuumed three times and kept in place for 10 minutes. (2) We vacuumed two different sized disposable cupping cups(A), size.1(InnerDiameter 48.8 mm) and size.3(InnerDiameter 39.1 mm), twice each(-200 mmHg) on silicon plate. We injected water and air at regular intervals in cupping cups by using a syringe, and then measured change of pressure in cupping cups and pressure at the time of dropout. Results : (1) Pressure reduction was $4.75{\pm}2.78%$ on average in the order of 'Disposable[A]>reusable[B]>Disposable[B]>reusable[A]', so that pressure retention performance of disposable cups can't be regarded as inferior to that of reusable cups. (2) Pressure of disposable cupping B(size.1) decreased by an average of -40.08 mmHg per 5 ml of water. At -24.8 mmHg, when 22 ml of water has been injected, cup has come off. Pressure of disposable cupping B(size. 3) decreased by an average of -99.4 mmHg per 5 ml of water. At -48.6 mmHg, when 13 ml of water was injected, cupping came off. Conclusions : Considering reduction rate of pressure due to water injection, in case of bleeding more than 15 ml, size.3 cup always comes off, therefore it needs to be re-operated at least once. Meanwhile, size.1 cup does not always come off in the same condition, depending on the initial pressure and therefore, re-operation may be considered.

A Case Report of Oriental Medical Treatment for the Left Lower Limb Monoplegia after Herniated Intervertebral Disc Operation at L-spine (요추 추간판 탈출증 수술 후 발생한 좌하지 단마비의 한의학적인 치료 증례보고)

  • Objectives: This study was performed to report the case of oriental medical treatment for the left lower limb monoplegia after herniated intervertebral disc(HIVD) operation at lumbar(L)-spine. Methods: A 38-year-old man who underwent lumbar HIVD operation at a hospital admitted with motor weakness of left lower limb, a little decreased left leg radiation pain and low back pain. We treated him by acupuncture, herbal medicine, bee venom injection moxibustion, cupping treatment physical therapy and measured with visual analogue scale(VAS), Oswestry disability index(ODI), Roland-Morris disability questionnaire(RMDO) and manual muscle test(MMT) from 8th February to 4th May 2011. Results: After treatment most symptoms decreased, VAS score changed from 10 to 3, ODI changed 71% to 37% RMDQ changed 13 to 8 and MMT changed from 3 to 5. Conclusions: Our study suggested that oriental medical treatments are significantly applicable to the monoplegia and pain after lumbar HIVD operation. And further studies ire required to identify underlying mechanism of the treatment.

A Clinical Case of Oriental Medical Treatment for the Paraplegia after Lumbar Epidural Nerve Block (요추 경막외 차단술 후 발생한 하지마비의 한의학적인 치료 효과 - 증례 보고 -)

  • Objectives : This study was performed to report the effect of oriental medical treatment in the paraplegia and pain after epidural nerve block. Methods : A 39-year-old woman who underwent epidural nerve block at a local clinic was admitted with motor weakness of lower limbs, severe lower radiating pain and decreased sensation when voiding and defecating. We treated her by acupuncture, a herbal medicine, a bee venom injection, moxibustion and cupping treatment and physical theraphy from 11th July 2008 to 14th October 2008. Results : After treatment, most symptoms decreased, VAS(Visual Analog Scale) score changed from 9 to 2. and examinated muscle power changed from 2-3 to 4-5. Conclusions : Our study suggested that oriental medical treatments are significantly effective in the paralplegia and pain after epidural nerve block. And further studies will be aid to identify underlying mechanism of treatment.

Case Report on Complete Response and 5 year Survival of Non-Small Cell Lung Cancer IIIB Patient Treated with Integrative Medicine (한양방통합치료로 완치되어 5년 생존에 도달한 3기B 비소세포성폐암 증례 보고)

  • Objectives: This is a five-year survival case report on non-small cell lung cancer (NSCLC) treated with western medicine and traditional Korean medicine. 44-year old man diagnosed NSCLC IIIB in 2015 went through two months' concurrent chemoradiotherapy (CCRT) first, however the cancer did not response. After CCRT he began to take intensive integrative therapy including acupuncture, abdominal moxibustion, wild ginseng pharmacopuncture, herbal medicinal product of ginseng, hyperthermia, Thymosin α1, and mega vitamin C injection for 4 years and 9 months. After three months of intensive treatment, the state of cancer changed first to partial response and then complete response. He finally succeeded in five-year survival without any recurrence or complication in 2020, We suggest that integrative approach including traditional Korean medicine can be a meaningful treatment option for nonoperable NSCLC. Further studies with more cases should be performed to establish proper treatment protocol of integrative medicine for lung cancer.

Scolopendra Pharmacopuncture Combined with Electroacupuncture for the Treatment of Ganglion Cysts: A Retrospective Study

  • Objectives: Ganglion cysts require a sustainable treatment that suppresses their frequent recurrence. This study aimed to explore the clinical effects of Scolopendra pharmacopuncture (SP) and electroacupuncture on ganglion cysts. Methods: We retrospectively reviewed the patient records and follow-up reports for 20 patients with wrist ganglion cysts who received SP and electroacupuncture from April 2016 to March 2017. The cyst diameter, recurrence, visual analog scale (VAS) scores for pain, the Korean version of the disabilities of arm, shoulder, and hand (K-DASH) score, and the Korean version of the patient-rated wrist evaluation (K-PRWE) score before and after treatment were noted. Results: After treatment, the cyst diameter decreased significantly from 13.61 ± 6.41 mm to 5.15 ± 6.18 mm (p < 0.001), and VAS score for pain decreased from 1.31 ± 1.77 to 0.41 ± 0.33 (p = 0.021). Further, the K-DASH score decreased significantly from 8.97 ± 12.66 to 2.21 ± 7.39 (p = 0.016), and score for the function subscale of K-PRWE decreased from 11.37 ± 4.48 to 9.1 ± 3.67 (p = 0.046). No recurrences were reported from the followed-up patients. Any complication related to SP or electroacupuncture was not observed, except mild rash, itching, and swelling at the injection site in four patients. Conclusion: Combination of SP and electroacupuncture may be effective in treating ganglion cysts; further prospective studies with large population are needed to clarify the effect of SP and electroacupuncture.

Anti-inflammatory Effects of Ojeok-san in LPS-induced Inflammatory Rat Model (LPS유도 염증 동물모델에서 오적산의 항염증 효과)

  • Objectives: The aim of this study is to investigate the anti-inflammatory effects of Ojeok-san and compare the therapeutic effects according to its formation. Methods: We evaluated the anti-inflammatory effects of Ojeok-san using lipopolysaccharide (LPS) induced inflammatory animal model. Male SD rats were administered intra-orally with two different formulation types of Ojeok-san according to prescribed dosage. One hour later, to induce inflammatory responses, subsequent intra-peritoneal injection of LPS was conducted. After 5 hours later, serum TNF-α, IL-1β, IL-6 and PGE2 levels were measured by ELISA to assess the alteration of pro-inflammatory markers. Results: In our experiment, regardless of its formation, administration of Ojeok-san decreased TNF-𝛼, IL-1𝛽, IL-6 and PGE2 level in serum. Furthermore, LPS-induced toxicity of liver and kidney was not detected by Ojeok-san administration. Conclusions: The anti-inflammatory effect of Ojeok-san was shown in LPS-induced inflammatory model by decreasing pro-inflammatory markers, and there would be no significant difference in therapeutic effect between two formulation types of Ojeok-san.

Recent Domestic and International Trends on Non-Surgical Treatment of Lumbar Spinal Stenosis (요추 척추관 협착증의 비수술적 치료에 대한 최근 국내·외 연구 동향)

  • Objectives The present study examined the recent domestic and international trends of non-surgical treatment of lumbar spinal stenosis (LSS). Methods The studies on non-surgical treatment of LSS were investigated via searching Korean web databases and PubMed. As a result, 39 studies were analyzed according to the authors, the types of study, the relationship to surgical treatment and the method of treatment. Results The first authors' occupations were identified as 17 doctors, 12 Korean medicine doctors, six physiotherapists, three chiropractic doctors and one doctor of science. The most common type of study in Korea was the case report and most of the studies were retrospective. Overseas, there were many prospective or experimental studies including randomized controlled trials (RCT). In Korea, non-surgical treatment was performed independently of surgical treatment in most cases. Among non-surgical treatments, the most researched treatment was traditional Korean medicine (TKM), followed by injection therapy, exercise therapy, and physical therapy. Conclusions Through this review, we can determine the effectiveness of TKM and its research direction. TKM should be studied experimentally including RCT and it should be conducted not only on acupuncture, but also on other treatment methods such as acupotomy, chuna, and herbal medicine. It is also necessary to conduct studies on TKM before and after surgery to compare the effects of surgery and TKM together, as well the effects on non-surgical treatments.

Anti-diabetic effect of medicinal plants used for lower wasting-thirst in streptozotocin-induced diabetic rats (Streptozotocin으로 유도된 당뇨 흰쥐에서 하소에 사용되는 한약의 항당뇨 효과 비교 연구)

  • Objectives : To prove the channel-tropism theory in herbology, we investigated the anti-diabetic effect of six herbal plants used for lower wasting-thirst in streptozotocin-induced diabetic rats. Methods : Diabetes was induced in male Sprague-Dawley rats by consecutive injection of streptozotocin (30 mg/kg i.p.) for 5 days. The rats were divided into normal control, diabetic control, and diabetic treatment with Lycii Radicis Cortex (LRC, 300 mg/kg); Corni Fructus (CF, 300 mg/kg); Bombyx Batryticatus (BB, 50 mg/kg); Lycii Fructus (LF, 300 mg/kg); Phellodendri Cortex (PC, 300 mg/kg); Epimedii Herba (EH, 300 mg/kg); and glibenclimide (10 mg/kg) as a reference drug. Herbal extracts or reference drug were administered orally for 28 days. The changes of body weight, food intake and water intake, and serological markers such as blood glucose, serum total cholesterol, triglyceride (TG), blood urea nitrogen (BUN), and creatinine (Cr) were measured. Results : The decrease of body weight and the increase of food and water intake in STZ-induced diabetic rats was improved by the administration of CF and LF. Also, the enhancement of blood glucose and serum total cholesterol, TG, BUN and Cr in STZ-induced diabetic rats was significantly inhibited by the administration of CF, BB, LF and glibenclimide. On the other hand, EH strongly inhibited the increase of BUN and Cr in the sera of STZ-induced diabetic rats. Conclusions : These results suggest that among six herbal medicines used lower emaciation of emaciation-thirst disease, CF, BB, LF and EH show a characteristics including the channel-tropism theory.

Proteomic Analysis for Neuroprotective Effect of Gastrodia elata Blume in the Substantia Nigra of Mice (천마의 흑질 내 도파민성 신경세포 보호 효과에 대한 단백체학적 분석)

  • Objectives : Parkinson's disease (PD) is a neurodegenerative disorder threatening the quality of life and highly occurred in over 65 years old. Gastrodia elata Blume (GEB), a traditional medicine used for the treatment of headache and convulsion, has been reported to have neuroprotective effect. This study was designed to investigate the neuroprotective effect of GEB and the proteomic changes in the substantia nigra (SN) of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP)-treated mice. Methods : Male eleven-week-old C57BL/6 mice were intraperitoneally injected with 30 mg/kg of MPTP at 24-h intervals for 5 days. Two hours after the daily MPTP injection, the mice were orally administered 800 mg/kg of GEB extract, which continued for 7 days beyond the MPTP injections, for a total of 12 consecutive days. Two hours after the final GEB administration, the brain samples were collected, and dopaminergic neuronal death and proteomic changes in the SN were evaluated. Results : GEB prevented the MPTP-induced dopaminergic neuronal death and regulated the expression of 11 proteins including thimet oligopeptidase, T-complex protein 1, glycine tRNA ligase, and pyruvate kinase isozymes M1. Conclusions : GEB prevents MPTP-induced dopaminergic neuronal death by regulating the proteins in the SN.
