A Case of Korean Medicine Treatment for a Patient with Acute Bacterial Prostatitis Who Did Not Improve with Antibiotics (항생제 치료 후 호전되지 않은 급성 세균성 전립선염 환자의 한의학적 치험 1례)

  • Objectives: The purpose of this study was to report the clinical effects of pharmacopuncture, acupuncture, and ohrimsan on prostatitis that was not improved by antibiotics. Methods: The patient underwent So-Yeom pharmacopuncture injection, acupuncture, and ohrimsan. A numerical rating scale (NRS) was used to obtain a sense of residual urine and the National Institute of Health Chronic Prostatitis Symptom Index (NIH-CPSI) was used to evaluate the treatment effects. Results: NRS for the sense of residual urine decreased from 70 before treatment to 0 after treatment. The NIH-CPSI was 19 on the first day of admission and decreased to 8 on the day of discharge. No side effects were observed after treatment. Conclusions: The traditional Korean medicine of pharmacopuncture, acupuncture, and ohrimsan is effective for treating prostatitis that is not improved by antibiotics.

  • This study was performed to investigate the cause, symptom, treatment of carpal tunnel syndrome through Western medicine and Dong-Eui-Bo-Kham(東醫寶鑑). Results & conclusions 1. Carpal tunnel syndrome is a common peripheral nerve entrapment syndrome that is characterixed by pain, numbness, sensory disturbance along the dsitribution of the meridian nerve in hand 2. Treatment of carpal tunnel syndrome have included wrist immobilization, anti-inflammatory drug, local injection of steroid, nerve block and surgical decompression. 3. Carpal tunnel syndrome seems to be similar with numbness(痺證). The causes were usually pathogenic Wind, Cold, Dampness. 4.Acupuncture, herbal medicine, herbal acupuncture were used for treatment of carpal tunnel syndrome. We considered that more study to find various and effective methods oriental medicine for carpal tunnel syndrome should be made.

The Experimental Study on the Suppression Effect of Asthma and Immune Response Improvement of Platycodi Radix Herbal-acupuncture (길경약침(桔梗藥鍼)의 천식억제(喘息抑制) 및 면역조절효과(免疫調節效果)에 대(對)한 실험적(實驗的) 연구(硏究))

  • Objectives : The aim of this study was to investigate the effect of Asthma-depression and Immunoregulation with PR-HAS(Herbal-acupuncture with Platycodi Radix infusion solution) injection at Joksamni(St36) on ovalbumin-induced asthma in mice. Methods : C57BL/6 mice were sensitized and challenged with OVA(ovalbumin) for 12 weeks(once a week). The experimental group(OVA-PR-HA) wase treated with concentrations(1%) of PR-HAS at Joksamni(St36) for the later 8 weeks(3times/week). The second experimental group(OVA-Needle prick) was treated with Needle-Prick at Joksamni(St36) for the later 8 weeks(3times/week). Results : 1. The weight and total cells in the mice lung treated with PR-HA decreased significantly compared with that of control group. 2. Total leukocytes and eosinophils in BALF of the mice group treated with PR-HA decreased remarkably compared with those of control group. 3. The sticking of collagen on histological analysis of lung sections, the mice group treated with PR-HA decreased significantly compared with those of control group. 4. The concentrations of IL-4, IL-5, IgE in BALF, and IL-4, IL-5, IL-13 in serum of the mice group treated with PR-HA decreased significantly compared with those of control group. 5. The number of $Gr-1^+/CD11b^+\;and\;CD11b^+$ cells in the lungs of the mice group treated with PR-HA decreased significantly compared with that of control group. 6. The numbers of $CCR3^+\;cells,\;CD4^+\;cells\;and\;CD8^+\;cells$ in the lungs, and $CD3e^+/CD69^+$ in the lungs of the mice group treated with PR-HA decreased significantly compared with those of control group. 7. The mRNA expression of ${\beta}-actin,\;TNF-{\alpha}$, IL-4, IL-5,IL-13 in the mice group treated with PR-HA with RT-PCR decreased significantly compared with those of control group. Conclusion : The concentrations of IL-4, IL-5, IgE in BALF, and IL-4, IL-5, IL-13 in serum of the mice group treated with PR-HA decreased significantly compared with those of control group. The number of $Gr-1^+/CD11b^+\;and\;CD11b^+$ cells in the lungs of the mice group treated with PR-HA decreased significantly compared with that of control group. Effects of High Frequency Electroacupuncture on c-fos Expression in the PAG and Hippocampus of Adjuvant Induced Rat Arthritis Pain Model (고빈도 전침차푹이 류마토이드 관절염 통증 모델의 뇌 c-fos 발현에 미치는 영향)

  • Objectives : This study was to investigate the effect of high frequency electroacupuncture at $ST_{36}$ acupuncture point on the Complete Freund's Adjuvant (CFA) induced rat arthritis pain model. Methods : Arthritis was induced by intradermal injection of CFA into base of tail. Experimental groups were divided into 4 groups; Normal, Control and Jok-Samri ($ST_{36}$) and Non-Acupuncture point (NA). Normal group, non-arthritic group, was injected with normal saline,and the others groups were injected CFA. $ST_{36}$ group was treated by 120 Hz electroacupuncture at $ST_{36}$ acupuncture point, and NA group was treated by 120 Hz electroacupuncture at non-acupuncture point. Each groups were evaluated by the change of c-fos positive neurons in periaqueductal gray (PAG) and hippocampus by using an image analyzer and a microscope. Results: - In the PAG region, the number of fos-positive cells in the $ST_{36}$ group ($42.37{\pm}5.08$) were significantly (p<0.05) decreased compared with the control group ($64.56{\pm}6.35$). - In the PAG region, the number of fos-positive cells in the NA group were meaninglessly decreased compared with the control group - In the Cornu Ammonis(CA)l region of hippocampus, the number of fos-positive cells in the $ST_{36}$ group ($7.00{\pm}1.08$) and NA group ($5.56{\pm}2.01$) were significantly (p<0.05) decreased compared with the control group ($13.81{\pm}1.24$). - In the dentate gyrus region of hippocampus, the number of fos- positive cells in the $ST_{36}$ group ($10.75{\pm}0.98$) and NA group ($6.56{\pm}0.78$) were significantly (p$26.45{\pm}1.82$). Conclusions : It is expected that high frequency electroacupuncture can be used a treatment of arthritic pain.

Single Intramuscular-dose Toxicity of Anti-inflammatory Pharmacopuncture in Rats

  • Objectives: This study was performed to analyze the toxicity of the test substance, anti-inflammatory pharmacopuncture (AIP), when used as a single intramuscular-dose in 6-week-old, male and female Sprague-Dawley rats and to find the lethal dose. Methods: The experiment was conducted at Biotoxtech according to Good Laboratory Practices. Twenty (20) female and 20 male Spague-Dawley rats were divided into 4 groups of five 5 female and 5 male animals per group. The rats in the three experimental groups received single intramuscular injections with 0.1-$m{\ell}$, 0.5-$m{\ell}$ and 1.0-$m{\ell}$/animal doses of AIP, Groups 2, 3, and 4, respectively, and the control group, Group 1, received a single intramuscular injection with a 1.0-$m{\ell}$ dose of normal saline. Clinical signs were observed and body weight measurements were carried out for 14 days following the injections. At the end of the observation period, hematology, clinical chemistry, histopathological tests and necropsy were performed on the injected parts. Results: No deaths occurred in any of the groups. Also, histopathological tests showed that AIP had no effect on the injected parts in terms of clinical signs, body weight, hematology, clinical chemistry, and necropsy. Conclusions: As a result of single intramuscular-dose tests of the test substance AIP in 4 groups of rats, the lethal dose for both males and females exceeded $1.0m{\ell}$/animal. Therefore, AIP is a relatively safe pharmacopuncture that can be used for treatment, but further studies should be performed.

Acupuncture in Patients with a Vertebral Compression Fracture: A Protocol for a Randomized, Controlled, Pilot Clinical Trial

  • Objectives: A vertebral compression fracture (VCF) is characterized by back pain and fracture of a vertebral body on spinal radiography. VCFs of the thoraco lumbar spine are common in the elderly. In general, appropriate analgesics should be prescribed to reduce pain and, thus, promote early mobilization. The ideal treatment approach for VCFs has not been determined. In Korea, acupuncture and herbal medication have been used to treat VCFs for many years. There is empirical evidence that acupuncture might benefit patients with a VCF. However, no randomized, controlled, clinical trials evaluating the efficacy and the safety of acupuncture for treating a VCF have been published. Therefore, we designed a randomized, controlled, pilot, clinical trial to obtain information for the design of a further full scale trial. Methods: A five week protocol for a randomized, controlled, pilot, clinical trial is presented. Fourteen patients will be recruited and randomly allocated to two groups: a control group receiving interlaminar epidural steroid injections once a week for three weeks, and an experimental group receiving interlaminar epidural steroid injections plus acupuncture treatment (three acupuncture sessions per week for three weeks, nine sessions in total). The primary outcomes will be the pain intensity (visual analogue scale and PainVision$^{TM}$ system). The secondary outcome measurements will be the answers on the short form McGill pain questionnaire and the oswestry disability index. Assessments will be made at baseline and at one, three, and five weeks. The last assessment (week five) will take place two weeks after treatment cessation. This study will provide both an indication of feasibility and a clinical foundation for a future large scale trial. The outcomes will provide additional resources for incorporating acupuncture into existing treatments, such as nonsteroidal anti-inflammatory medications, narcotics and vertebral augmentation. This article describes the protocol.

Intravenous Single Dose Toxicity of Sweet Bee Venom in Sprague-Dawley Rats

  • Objectives: Anaphylactic shock can be fatal to people who become hypersensitive when bee venom pharmacopuncture (BVP) is used. Thus, sweet bee venom (SBV) was developed to reduce these allergic responses. SBV is almost pure melittin, and SBV has been reported to have fewer allergic responses than BVP. BVP has been administered only into acupoints or intramuscularly, but we thought that intravenous injection might be possible if SBV were shown to be a safe medium. The aim of this study is to evaluate the intravenous injection toxicity of SBV through a single-dose test in Sprague-Dawley (SD) rats. Methods: Male and female 6-week-old SD rats were injected intravenously with SBV (high dosage: 1.0 mL/animal; medium dosage: 0.5 mL/animal; low dosage: 0.1 mL/animal). Normal saline was injected into the control group in a similar method. We conducted clinical observations, body weight measurements, and hematology, biochemistry, and histological observations. Results: No death was observed in any of the experimental groups. Hyperemia was observed in the high and the medium dosage groups on the injection day, but from next day, no general symptoms were observed in any of the experimental groups. No significant changes due to intravenous SBV injection were observed in the weights, in the hematology, biochemistry, and histological observations, and in the local tolerance tests. Conclusion: The results of this study confirm that the lethal dose of SBV is over 1.0 mL/animal in SD rats and that the intravenous injection of SBV is safe in SD rats.

Research Trends on the Acupotomy Treatment for Knee Osteoarthritis : A Systematic Review (무릎 골관절염의 도침 치료에 대한 연구 동향 : 체계적 문헌 고찰)

  • This study aimed to review clinical studies about Acupotomy used for Knee Osteoarthritis. We searched the following 12 online databases (KISS, NDSL, RISS, OASIS, Earticle, Kmbase, MEDLINE/Pubmed, Cochrane library, Ebscohost, Ovid, CNKI, Wanfang), to find randomized controlled trials that used Acupotomy for knee osteoarthritis. The methodological quality of randomized controlled trials were assessed by using the Cochrane risk of bias tool and meta-analyses were performed. 16 randomized controlled trials were included. Total number of patients was 1169. The average duration of treatment was 3.14 weeks and most of the patients were treated once a week. The major treatment sites were ligaments, muscles, and tender nodules and the most used evaluation tool was the efficiency. We selected 4 studies and meta-analyzed them. All of the studies performed Acupotomy+sodium hyaluronate injection as a treatment group, and sodium hyaluronate injection as a control group. Meta-analysis showed positive results for Acupotomy+sodium hyaluronate injection in terms of efficiency rate compared to sodium hyaluronate injection. Also Meta-analysis showed positive results for Acupotomy+sodium hyaluronate injection in terms of WOMAC (pain, stiffness, function) compared to sodium hyaluronate injection. In this study, we reviewed studies about Acupotomy used for knee osteoarthritis. The studies showed that Acupotomy can significantly effective on knee osteoarthritis. But according to Cochrane Risk of Bias (RoB) evaluation method, most of the study's risk of bias was unclear. Threrefore, more high-quality studies will be needed.

The Ability of Cervus Elaphus Sibiricus Herbal Acupuncture to Inhibit the Generation of Inflammatory Enzymes on Collagen-induced Arthritis Mice (녹용약침(鹿茸藥鍼)이 CIA 모델 생쥐의 염증인자 생성억제에 미치는 영향)

  • Backgrounds : Rheumatoid Arthritis(RA) is known as the chronic inflammatory diseasethat induces persistent inflammation in the joint cavity. The destruction of cartilage occurs as the result of bones destoyed by pannus, several influential cytokines induced by the synovial capsulitis, varieties of proteinases, $O_2$ radicals, and the secondary degenerative changes of articular cartilage. The type 2 collagen-induced arthritis model is used in recent experimental research on rheumatoid arthritis. Cervus elaphus sibiricus (Nockyong) has the effect of relieving pain by nourishing the muscles, joints, and bones. It is also known to be efficacious in promoting and enhancing the immune system. The objective of this study was to investigate the effect of Cervus elaphus sibiricus herbal acupuncture to inhibit the generation of proinflammatory enzyme on type 2 collagen-induced arthritis. I investigated the inhibition of mRNA transcription of MIF(macrophage migration inhibitory factor), $TNF-{\alpha}$(Tumor necrosis $factor-{\alpha}$) and MMP-9 (matrix metalloproteinase-9) of Cervus elaphus sibiricus herbal acupuncture using an in vitro test. Also investigated was the inhibition of differentiation of Th 1 cells and activation of cytokines(MIF, $TNF-{\alpha}$, IL-6, MMP-9), which are known to cause initial RA ,and are also related to the morphology of the synovial membranes of the joint capsule, by an in vivo test, using CIA(collagen induced arthritis) model mice. Materials & methods : The laboratory animals used in this experiment were 4 week-old DBA female mice, weighing approximately 20 grams, and adjusted to the laboratory environment. The experiment was divided into the normal group(NOR)-no treated group, control group(CON)-CIA induced group, and sample group(SAM)-Cervus elaphus sibiricus herbal acupuncture treated group. RA was induced in the mice via injection of $50{\mu}{\ell}$ C II mixed CFA. The Cervus elaphus sibiricus herbal acupuncture solution was applied on $GB_{35}$(陽陵泉) for 26 days from the 3rd day of RA inducement. The concentration of the solution was determined via a MTT assay. To research the effect on the expression of MIF, $TNF-{\alpha}$ and MMP-9 mRNA, RT-PCR was performed on synovial membrane cells from the knee joint of CIA mice. C II induced RA knee joint's histo-chemical synovial membrane was observed using a specimen model via the Hematoxilin and Eosin dying technique. Results : The expression of mRNA of RA-related cytokines such as MIF, $TNF-{\alpha}$, and MMP-9 dosedependently decreased in the cell from the synovial membranes of the joint, which is treated with Cervus elaphus sibiricus herbal acupuncture solution. In mice treated with Cervus elaphus sibiricusherbal acupuncture, the damage of synovial membranes of the joint was lessened, and differentiation of Th 1 cells was suppressed. The activation of RA-related cytokines such as MIF was suppressed, and the generation of $TNF-{\alpha}$ and MMP-9 showed a statistically significant decreas. Conclusions : It is speculated that Cervus elaphus sibiricus herbal acupuncture has the therapeutic effect of palliating the damage of the tissue impaired by RA by inhibition of the initial RA progression and by regulating excessive differentiation of Th 1 cell as it suppresses the generation of RA-related cytokines during the highest stage of RA by acting on pro-inflammatory enzymes.
