Efficacy of sucrose application in minimizing pain perception related to dental injection in children aged 3 to 9 years: a randomized control trial

  • Background: Dental fear and anxiety are significant challenges in managing behavior in children. Oral administration of sucrose or sweet-tasting solutions has shown effectiveness in reducing procedural pain in infants and neonates. This study aimed to investigate whether pre-application of sucrose solution had an effect on minimizing pain perception during injection and to assess the potential impact of the child's age and sweet preference. Methods: A randomized control clinical trial was conducted on 60 children aged 3-9 years requiring buccal infiltration injections. Following parental consent, demographic data of the children were recorded. Sweet preferences was assessed using a modified forced-choice test. Children were equally and randomly allocated into study (sucrose) and control groups using a lottery method. Sucrose solution or distilled water, respectively, was applied to the lateral surface of the tongue for 2 min. Topical anesthetic was applied at the site of injection, followed by local anesthesia administration. The children rinsed their mouths thrice with water immediately after anesthetic injection. A video was recorded during injection which was then scored by three blinded examiners on the Sound Eye Motor (SEM) scale. The children also self-evaluated using Wong-Baker Faces Pain Rating Scale (WBFPS). Results: The mean SEM scores and WBFPS scores were analyzed using the Kruskall-Wallis test. The mean SEM score in the study group was 1.37 ± 0.61, compared to 3.17 ± 0.87 in the control group, showing a statistically significant difference (P < 0.001). Mean pain scores assessed by WBFPS in the study group were 0.60 ± 1.4, while in the control group, they were 6.27 ± 2.33, also showing a statistically significant difference (P < 0.001). Children with a sweet preference demonstrated a subjective reduction in pain perception. Conclusion: Application of sucrose before dental injections in children helps to minimize pain upon injection across all age groups.

Simultaneous Determiniation of Ar/$N_2$Ratios in Groundwater (지하수에 용해된 질소, 아르곤 가스의 동시측정)

  • Previously reported Ar/N$_2$ratios in groundwater have been measured by single ion monitoring (Barnes et al., 1975; Vogel et al., 1981; Mariotti et al., 1988). The detector geometry and flared flight tube in VG Optima isotopic ratio mass spectrometer appeared to be fortuitously aligned for the simultaneous measurement of Ar/N$_2$ratios. Method development included mechanical adjustments to optimize the mass spectrometer for Ar/N$_2$ratio measurements followed by development of a preparation system for the extraction of air-saturated water samples. Samples containing known Ar/N$_2$ratios were used to assess accuracy and precision, and to test the applicability of methods for measurements of aqueous Ar/N$_2$ratios. The results indicated that the prepared air-saturated water samples were almost identical to the predicted Ar/N$_2$ratios (p <0.001). Groundwater samples were collected from on-going research sites, Shelton and Grand Island, Nebraska. Samples from the Grand Island sludge injection site form a lower boundary for worldwide reported Ar/N$_2$ratios. These lower Ar/N$_2$ratios can be explained by the production of nitrogen gas from this site, where denitrification was reported previously.

Prediction of Optimal Gluteal Intramuscular Needle Length by Skinfold Thickness Measurements in Korean Adults (피부주름두께 측정을 통한 성인의 둔부 근육주사 바늘의 최적 길이 예측)

  • Purpose: This study was conducted to assess optimal needle length for gluteal intramuscular injections (IM) via simple skinfold thickness (SFT). Methods: For this study, 190 healthy adults were recruited and grouped into eight groups according to gender and body mass index (BMI) (kg/$m^2$). The Korean Society for the Study of Obesity criteria defines a BMI under 20 as underweight, 20.1-22.9 as normal, 23-24.9 as overweight and over 25 as obese. For each participant, the SFT of dorsoguteal (DG) and ventrogluteal (VG) sites were measured using a caliper. Subcutaneous tissue thickness was acquired through ultrasonic images. Results: For men in the overweight and obese groups at the DG site, for the obese group at the VG site, and for women in the normal weight, overweight and obese groups at both sites, the mean subcutaneous tissue thickness exceeded 1.84 cm, the minimal length for a 1 inch needle used for IM. At the DG site, optimal intramuscular needle length (OINL) was 1.4 times in women and 1.0 times in men compared to SFT. At the VG site, OINL was 1.3 times in women and 0.9 times in men compared to SFT. Conclusion: The results of this study suggest that SFT is a reliable index to determine optimal needle length with minimal effort prior to IM.

Utility of sodium tetradecyl sulfate sclerotherapy from benign oral vascular lesion

  • Background: Hemangioma and vascular malformation are benign vascular lesions that often occur in cephalic and cervical region. Currently, surgical resection, laser therapy, angiographic embolization, use of steroids, and sclerotherapy are used as treatments. Case presentation: This study reports three cases of benign vascular lesions that are remarkably treated by sodium tetradecyl sulfate (STS) injection, of which occurred in oral cavity and around the mouth. Three percent of STS was diluted with 0.9 % of normal saline, and it was injected to the lesion site at least once. The result of treatment was evaluated based on clinical findings. Conclusion: Surgical treatment of hemangioma and vascular malformation occurred in oral cavity is not normally used because of esthetic issues and potential hemorrhage. On the other hand, sclerotherapy using STS is an effective therapy compare to surgical treatment. Despite the number of STS injection was different for each patient, all three patients had reached satisfactory level through the treatment with gradual diminution of lesions.

  • By understanding the construction process of sea dikes and the current state of the thickness and speed of fluid in the bottom layer protection work of final closure gaps, a construction method applicable for the blocking of bottom layer work will be selected. The three construction methods selected will be tested in site through various methods, and the reinforcement of bottom layer protection and impervious effect will be verified. The verification results are as follows: 1) The overall riprap layer were 0.5~1.0m thicker than planned so that the grouting depth and grout input amount increased 2) The applied construction methods permeability of riprap layers were improved from $\alpha{\times}10^{-2}cm/s$ before the construction to $\alpha{\times}10^{-4}cm/s$ after construction. 3) The results of core extraction in order to grossly verify the hardening time and durability allowed the identification of grout injection effect. The amount of filling of the injection was difficult to judge because the slime in many areas made the reading of borehole photography difficult.

Performance Analysis of an Inert Gas Generator for Fire Extinguishing

  • Present study deals with performance analysis of an inert gas generator (IGG) which is to be used as an effective mean to suppress the fire. The IGG uses a turbo jet cycle gas turbine engine to generate inert gas for fire extinguishing. It is generally known that a lesser degree of oxygen content in the product of combustion will increase the effectiveness of fire suppressing. An inert gas generator system with water injection will bring advantages of suffocating and cooling effects which are considered as vital factors for fire extinguishing. As the inert gas is injected to the burning site, it lowers the oxygen content of the air surrounding the flame as well as reduces the temperature around the fire as the vapour in the inert gas evaporates during the time of spreading. Some important aspects of influencing parameters, such as, air excess coefficient. $\alpha$, compressor pressure ratio, $ pi_c$, air temperature before combustion chamber, $T_2$, gas temperature after combustion chamber, $T_3$, mass flow rate of water injection, $M_w$, etc., on the performance of IGG system are investigated. Calculations of total amount of water needed to reduce the turbine exit temperature to pre-set nozzle exit temperature employing a heat exchanger were made to compare the economics of the system. A heat exchanger with two step cooling by water and steam is considered to be better than water cooling only. Computer programs were developed to perform the cycle analysis of the IGG system and heat exchanger considered in the present study.

  • U.J.S.(Ultra Jetting System) is a new ground improvement method registered as a Utility Model No.0205798, which has fundamentally improved the existing jetting method of J.S.P.(Jumbo Special Pattern System). In this study, the uniaxial compressive strengths of improved soil-grout structures by U.J.S. and J.S.P. which have been conducted on the construction site are compared. Also, the differences between the U.J.S. and J.S.P. are analyzed by considering the role of the auger bit, the injection distance measured from the axis of boring tubes, and angle of injection measured from the horizontal. The specimens of soil-grout structures are taken from the improved soils by using the U.J.S. and J.S.P. The uniaxial tests for the samples are conducted after the curing period of 28 days. The uniaxial compressive strengths and the coefficients of elasticity of surface and distance from the axis of boring. This study shows that the mean strength of the improved structure by J.S.P. is 1.9 times greater than by J.S.P.

A Study for Reducing Pain from Injection of Lidocaine Hydrochloride (염산 Lidocaine 주입으로 초래되는 통증을 줄이는 방법에 대한 연구)

  • Local anesthetics produce pain during infiltration into skin. The relationship between local anesthetic-induced pain and pH of the local anesthetic solution has not been fully investigated. Commercial preparation of local anesthetics are prepared as acidic solutions of the salts to promote solubility and stability. And the acidity of local anesthetic solition may be related with the pain during infiltration of the solutione. So, we tried to neutralize the lidocaine hydrochloride solution which is one of the most frequently used local anesthetic agent. Sodium bicarbonate was used for neutralization. Sodium bicarbonate was mixed with lidocaine hydrochloride until the resulting pH of the solution become 7.4 which is identical to the acidity of body fluid. To identify the effect of neutralized lidocaine solution, we had a course of double blind test to 6 volunteers. Both forearm of each volunteer were injected with neutralized lidocaine and plain one, and the degree of pain was estimated by each volunteers. According to subjective description by the volinteers, everyone felt neutralized lidocaine injection site was less painful than plain lidocaine. We concluded that we could reduce pain from infiltration of lidocaine hydrochloride by neutralization of the anesthetic solution with sodium bicarbonate.

An Analysis of Pattern of Transforaminal Epidurography (경추간공 경막외 조영술의 양상에 관한 분석)

  • Background: A transforaminal epidural steroid injection has been used for the treatment of radicular arm or leg pain, which has the advantage of ventral epidural spreading compared to either an interlaminar or caudal approach. However, several factors are known to affect the epidural spread pattern of contrast dye; therefore, the injected medication can not be delivered to the target site. The objective of our study was to observe any differences in the contrast flow patterns according to several factors. Methods: A total of 34 epidurographies in 29 patients, who underwent fluoroscopically guided transforaminal epidural steroid injections, were evaluated. After confirming the location of the needle tip within the anterior superior aspect of the neural foramen in the lateral view, and at the 6 o'clock position to the pedicle in the anteroposterior view, 2 ml of contrast dye was injected. The contrast flow patterns of ventral, ventral and dorsal, and dorsal epidural filling were analyzed, according to age, gender, magnetic resonance imaging finding and history of previous back surgery. Results: Ventral contrast flow occurred in 30 out of the 34 epidurographies (88%). Both ventral and dorsal contrast flow patterns were observed 13 out of 16 (81%) patients in the older age group. Also, both ventral and dorsal contrast flow patterns were observed in 16 out of 18 (88%) patients with spinal stenosis. Conclusions: Transforaminal epidural steroid injections, performed uner fluoroscopy, provide excellent nerve root filling and ventral epidural spreading. Patients with spinal stenosis or an old age have both ventral and dorsal epidural spreading patterns.

Pharmacokinetics and Biodistribution in Mice of pCK-VEGF Expressing Human Vascular Endothelial Growth Factor (마우스에서 VEGF발현 Naked DNA 벡터인 pCK-VEGF의 약동력학 및 조직내 분포)

  • We recently developed a high efficiency expression vectors pCK, which drives a high level of gene expression in the skeletal muscles of mice. In this study, we investigated the pharmacokinetics and biodistribution of pCK-VEGF expressing human VEGF165 after intravenous or intramuscular administration. The quantity of pCK-VEGF in the tissues of mice was measured by the PCR method which has a detection limit of approximately 1 pg of the exogenously added plasmid. In the case of intravenous administration, the half life of the pCK-VEGF plasmid in the bloodstream was 1.68 min. After intra-muscular administration, the half life of pCK-VEGF plasmid in the bloodstream was 6.78 min. At 90 min post-administration, 30% of the injected pCK-VEGF was found at the site of injection, where it persisted for up to 8 hours. Less than 1.6% of the injected pCK-VEGF plasmid DNA was detected in highly vascularized tissues such as the lung, kidney; and liver at 90 min post-administration, but the plasmid was undetectable at later time points. These results suggested that intramuscularly administrated pCK-VEGF persisted for longer periods of time in muscles than in other tissues and that direct intra-muscular injection of pCK-VEGF might be useful for local therapeutic angiogenesis.
