Glioblastoma Misdiagnosed as Brain Abscess after Surgical Evacuation of Spontaneous Basal Ganglia Hemorrhage (기저핵부 뇌실질내 출혈에 대한 수술후 뇌농양으로 오인한 다형성 교아세포종)

  • A 51-year-old woman presented with sudden severe headache, vomiting, and right hemiparesis at first admission. Computed tomography(CT) scans revealed an hemorrhagic density at left basal ganglia. Preoperative cerebral angiography showed no vascular lesion. Under the diagnosis of hypertensive intracerebral hemorrhage(ICH), total extirpation of hematoma was done. The postoperative neurological condition improved gradually and discharged without any neurological sequelae. Two months later, she revisited with headache, vomiting and progressive right hemiparesis. CT scans at second admission showed an irregular rim enhanced mass with central low density with surrounding edema at the initial bleeding area. Repeated craniotomy was performed and the mass was partially removed. The histopathological diagnosis of the specimen was confirmed as glioblastoma. The authors report a glioblastoma, which occurred at initial ICH site and regarded as a brain abscess with literature review.

Clinical Observation on 2 Case of Guillain-$Barr{\'{e}}$ Syndrome (Guillain-$Barr{\'{e}}$ 증후군 환자의 치험 2례)

  • Objectives : The purpose of this case is to report the improvement after the acupuncture therapy, and herbal medicine about two patients with Guillain-$Barr{\'{e}}$ Syndrome. Methods : We treated the patients with acupuncture, moxibustion and herbal medication. Results : We have experienced two case of Guillain-$Barr{\'{e}}$ Syndrome. One case improved significantly through treatment, but one case had lingering symptoms.. Conclusion : In Guillain-Barre Syndrome, the initial diagnosis is important, and through a collaboration of Western and Korean medicine, We were able to achieve meaningful treatment results.

  • ㆍPurpose: This study was performed to investigate the initial conservative treatment for TMD patients using careful counselling and medication prospectively. ㆍMaterials and Methods: Careful counselling and medication were performed in 51 TMD patients and 27 patients had follow-up check 2 months or more. Diagnosis of TMD was based on medical history and, physical and radiographic examination. TMD included masticatory disorder, internal derangement, degenerative joint disease, inflammatory joint disorder. and problems resulting from extrinsic trauma. All patients had chief complaints of TMJ pain, mouth-opening limitation. joint noise, and/or referred pain. We counselled and explained to the patient about the pathogenesis, etiologic factors, diagnosis and treatment plan for abut 10 minutes. We prescribed nonsteroidal anti-inflammatorv analgesic(Somalgen) and amitriptyline 10mg per day for 2 weeks. We informed the patient of the attention sheet and taught self-exercise of jaw. The patient were assessed by answering the questionnaire of subjective evaluation of TMD & maxillofacial pain. Questionnaire of an activity limitation. Questionnaire of a jaw function, and Questionnaire for the evaluation of TMD. ㆍResults: In questionnaire for the evaluation of TMD, 88.5% of 26 patients answered that the treatment was efficacious. 71.4% of 21 patients answered no problem in everyday life. There were significant differences between pretreatment and final follow-up in the evaluation of the subjective pain in the following sections: opening widely, chewing, resting, morning, masticatory muscle, and temporal portion(SAS program, paired T-test, P = 0.05). ㆍConclusions: Considerate counselling and proper medication could be significantly effectve in the initial treatment of TMD.

Megaoesophagus Seconday to Myasthenia Gravis in Three Dogs (중증근무력증에 의한 개의 거대식도증 3례)

  • Three dogs (An 8 years-old intact female Poodle, a 7 years-old intact male Schunauzer, and an 8 yearsold Golden Retriever) were presented due to acute vomiting, dyspnea, and generalized weakness. Megaesophagus was confirmed through radiographic examination in all 3 dogs. Relative oesophageal diameter (ROD) was measured and results of ROD measurements showed the possibility of megaesophagus secondary to myasthenia gravis in three dogs. Thus we performed anticholinesterase test as screening test for myasthenia gravis. In all three dogs, esophageal diameter was reduced after neostigmine methylsulfate administration. For definite diagnosis of acquired myasthenia gravis, serum acetylcholine receptor antibody titer was measured, but definite diagnosis was confirmed only in one case. However, based on history, radiographic findings, anticholinesterase test, ROD measurement, other two cases were still suspected as megaesophagus secondary to myasthenia gravis. Treatment with pyridostigmine bromide was initiated in all dogs, and improvement of esophageal diameter was shown in all dogs. One dog was successfully managed for 15 months after initial treatment and, is still alive, but other two dogs were died shortly after initial treatment, because of severe aspiration pneumonia.

  • Lemierre's syndrome is rare disease characterized by anaerobic sepsis, internal jugular vein thrombosis, septic emboli that resulted from head and neck infection. Lemierre's syndrome has significant morbidity, so immediate, accurate diagnosis and treatment is needed. It is necessary to perform contrast-enhanced computed tomography (CT) for diagnosis. Systemic antibiotics is recommended, and surgical interventions, anticoagulation may beis considered for treatment. We report misdiagnosed case as a simple deep neck infection on initial ultrasonography with simultaneous abscess aspiration but finally diagnosed and treated internal jugular vein thrombophlebitis (Lemierre's syndrome) on CT scan. We report a case of a 45-year-old patient, who was diagnosed with a simple deep neck infection and treated with simultaneous abscess aspiration, but finally diagnosed and treated internal jugular vein thrombophlebitis (Lemierre's syndrome) on CT scan.

A Clinical Analysis on 82 Cases of Unilateral Vocal Cord Paralysis (편측 성대마비 82례에 대한 임상 분석)

  • Background: Unilateral vocal cord paralysis(UVCP) by recurrent laryngeal nerve injury is one of the common laryngological diseases. Objectives : We attempted to study the clinical feature and the causes of UVCP and also tried to investigate what is to be the initial approach for determining the causes of unknown-origin UVCP Materials and methods The charts of 82 patients with the diagnosis of UVCP were reviewed. The records were analyzed for patient's gender and age, the status of paralysed vocal cord, the crucial tests for the diagnosis, and the etiologies. Results : forty-nine(59.8%) male and 33(40.2%) female patients were included in this study. The age group of sixth decade was most commonly involved. Most of the cases showed paramedian position of palsy, and the left side(59.8%) was more frequently attacked. The most common cause of UVCP in this series was the unknown origin, followed by the surgical trauma and neoplasms. The etiologies of the six(12.5%) unknown-cause cases were found with the further evaluation, with the most useful test being a CT scan. Conclusion: Chest X-ray, esophagography, and CT scan should be included in the mandatory initial investigation of patients with unknown-origin UVCP

Tongue Image Segmentation via Thresholding and Gray Projection

  • Tongue diagnosis is one of the most important diagnostic methods in Traditional Chinese Medicine (TCM). Tongue image segmentation aims to extract the image object (i.e., tongue body), which plays a key role in the process of manufacturing an automated tongue diagnosis system. It is still challenging, because there exists the personal diversity in tongue appearances such as size, shape, and color. This paper proposes an innovative segmentation method that uses image thresholding, gray projection and active contour model (ACM). Specifically, an initial object region is first extracted by performing image thresholding in HSI (i.e., Hue Saturation Intensity) color space, and subsequent morphological operations. Then, a gray projection technique is used to determine the upper bound of the tongue body root for refining the initial object region. Finally, the contour of the refined object region is smoothed by ACM. Experimental results on a dataset composed of 100 color tongue images showed that the proposed method obtained more accurate segmentation results than other available state-of-the-art methods.

Diagnosis and management of toxic shock syndrome after breast reconstructive procedures with silicone implants

  • Breast implant insertion is one of the most commonly used methods for breast reconstruction after total mastectomy. However, infection is a common postoperative complication of implant insertion. In most cases, these infections can be managed with antibiotics and supportive therapy. However, severe septic conditions, such as toxic shock syndrome (TSS), can sometimes occur. TSS is an extremely rare but life-threatening complication, for which early diagnosis and proper management play a crucial role in determining patients' outcome. Although only 16 cases of TSS after breast implant insertion have been reported in the literature, most of those cases involved a serious clinical course. The reason for the seriousness of the clinical course of TSS in these cases is that the initial impression and presentation of these patients are nonspecific, and patients can easily be misdiagnosed as having a simple upper respiratory infection, causing the underlying condition to be neglected. Herein, we present two patients who were diagnosed with TSS after receiving breast reconstruction surgery via a silicone implant after total mastectomy. Both patients were misdiagnosed at the initial examination since they showed no local infectious signs on the postoperative wound.

  • In 60 dairy cows with inactive ovaries, follicular cyst, luteal cyst, persistent corpus luteum and silent heat as diagnosed by rectal palpation, and those that had not resumed ovarian cycles until 60 days postpartum, progesterone concentrations for differential diagnosis of reproductive disorders were measured and were compared in matched plasma, skim milk and milk fat samples at 10 days interval. The incidence rate of reproductive disorders were as follows; inactive ovaries 20(33.3%), silent heat 11(18.3%), follicular cyst 7(11.7%), luteal cyst 7(11.7%), persistent corpus luteum 7(11.7%), pyometra 4(6.7%), vaginitis 2(3.3%), cystic corpus luteum 1(1.7%), and endometritis 1(1.7%), respectively. Cows having a progesterone concentration in plasma and skim milk < 1.0 ng/ml, and in milk fat < 80.0 ng/ml were considered to have inactive ovaries or follicular cyst. Those with concentrations in plasma and skim milk ${\geq}1.0ng/ml$, and in milk fat ${\geq}80.0ng/ml$ were regarded as the cases of luteal cyst or persistent corpus luteum. Progesterone concentrations in above cows did not differ significantly between the time of initial determination and the 10 days after initial determination. But progesterone concentrations in cows with silent heat did differ significantly between the time of initial determination and the 10 days after initial determination(P<0.05). The accuracy of rectal palpation for making a differential diagnosis of ovarian dysfunction, as defined on basis of progesterone concentrations, were as follows; follicular cyst 55.6%, luteal cyst 50.0%, inactive ovaries 90.5% and persistent corpus luteum 60.0%, respectively. It may be concluded that progesterone determinations at 10 days interval is practical as an aid to diagnosing ovarian dysfunction, particularly follicular cyst and luteal cyst.

Real-World Application of Artificial Intelligence for Detecting Pathologic Gastric Atypia and Neoplastic Lesions

  • Purpose: Results of initial endoscopic biopsy of gastric lesions often differ from those of the final pathological diagnosis. We evaluated whether an artificial intelligence-based gastric lesion detection and diagnostic system, ENdoscopy as AI-powered Device Computer Aided Diagnosis for Gastroscopy (ENAD CAD-G), could reduce this discrepancy. Materials and Methods: We retrospectively collected 24,948 endoscopic images of early gastric cancers (EGCs), dysplasia, and benign lesions from 9,892 patients who underwent esophagogastroduodenoscopy between 2011 and 2021. The diagnostic performance of ENAD CAD-G was evaluated using the following real-world datasets: patients referred from community clinics with initial biopsy results of atypia (n=154), participants who underwent endoscopic resection for neoplasms (Internal video set, n=140), and participants who underwent endoscopy for screening or suspicion of gastric neoplasm referred from community clinics (External video set, n=296). Results: ENAD CAD-G classified the referred gastric lesions of atypia into EGC (accuracy, 82.47%; 95% confidence interval [CI], 76.46%-88.47%), dysplasia (88.31%; 83.24%-93.39%), and benign lesions (83.12%; 77.20%-89.03%). In the Internal video set, ENAD CAD-G identified dysplasia and EGC with diagnostic accuracies of 88.57% (95% CI, 83.30%-93.84%) and 91.43% (86.79%-96.07%), respectively, compared with an accuracy of 60.71% (52.62%-68.80%) for the initial biopsy results (P<0.001). In the External video set, ENAD CAD-G classified EGC, dysplasia, and benign lesions with diagnostic accuracies of 87.50% (83.73%-91.27%), 90.54% (87.21%-93.87%), and 88.85% (85.27%-92.44%), respectively. Conclusions: ENAD CAD-G is superior to initial biopsy for the detection and diagnosis of gastric lesions that require endoscopic resection. ENAD CAD-G can assist community endoscopists in identifying gastric lesions that require endoscopic resection.
