Background: Opioids can present intolerable adverse side-effects to patients who use these analgesics to mitigate chronic pain. In this retrospective analysis, cooled radiofrequency (CRF) denervation was evaluated to provide pain and disability relief and reduce opioid use in patients with sacroiliac joint (SIJ) derived low back pain (LBP). Methods: Twenty-seven patients with pain from SIJ refractory to conservative treatments, and taking opioids chronically (> 3 mo), were included. Numeric rating scale (NRS) and Oswestry disability index (ODI) scores were collected at 1, 6, and 12 months post-procedure. Opioid use between baseline and each follow-up visit was compared for the entire group and for those who experienced successful (pain reduction ≥ 50% of baseline value) or unsuccessful CRF denervation. Results: Severe initial mean pain (NRS score: 7.7 ± 1.0) and disability (ODI score: 50.1 ± 9.0), and median opioid use (morphine equivalent daily dose: 40 ± 37 mg) were significantly reduced up to 12 months post-intervention. CRF denervation was successful in 44.4% of the patients at 12 months. Regardless of procedure success, patients demonstrated similar opioid reductions and changes in opioid use at 12 months. Two patients (7.4%) experienced neuritis following CRF denervation. Conclusions: CRF denervation of the SIJ can safely elicit pain and disability relief, and reduce opioid use, regardless of intervention success. Future studies may support CRF denervation as a dependable therapy to alleviate opioid use in patients with SIJ-derived LBP and show that opioid use measurements can be a surrogate indicator of pain.

No one really doubts that the hottest area of dentistry in 21st century would most likely be 'implant'. With the support of a vast amount of research, implant has been successfully and rapidly absorbed into the field of general dentistry including private practitioners. For teeth with hopeless(or refractory) periodontitis or periapical pathosis, with no hesitation most dentists would think extraction as the sole treatment option followed by prosthodontic replacement possibly including implant. Not many dentists would take Trasplantation/Replantation as another treatment option for a particular condition. Dentistry is often more focused on 'Restoration' than 'Preservation' of natural dentition. 'Biologic Implant' is obviously much closer to the concept of 'Preservation'. Many different types of biologic implant system have been introduced to clinical dentistry so far. Many of those have failed to earn reasonable acknowledgement despite of the clinical success they brought. For some reason biologic implant has rather been alienated for long time. RPI(Ring Pin Implant) is designed to improve the prognosis and success rate of transplanted/replanted teeth. RPI is a Ti-based custom made implant system. It is fabricated either by electric casting or milling process. The major feature RPI gas is the 'ring & hole' structure. The hole should be no less than 1mm diameter to allow bone bridge formation thru it. The ring structure and bone bridge formation creates anti-torque activity, which largely increases the 'initial stability' of the transplanted/replanted teeth. It is also reported that RPI is beneficial in the aspect of resisting root resorption following replantation/transplantation procedure.
This study aims at analysing the process of introduction and the development of the agri-environment policy of the UK and identifying the implication for the Korean government. The UK introduced the AES policy in order to reduce the negative impact of the farming practice on biodiversity and habitats. The initial process was only possible for the enormous research results on the relationship between the farming and environment. Since the UK launched the ESA scheme in 1987, it has extended the designated areas for four stages. Based on the success of the ESA scheme, it started CSS and other schemes. However, these had a modest success and were integrated into the CSS scheme, which from then on became the main measures of the AES in the UK. The whole process of the UK case tells us that the most important work for the start of the AES in Korea would be the necessity and feasibility of the environment protection measures in Korea which could be identified from academic and scientific research on the impact of agricultural practice on the rural environment in Korea.
The purpose of this study is to contribute reference material that provides insight into innovative process management that increases R&D output in commercializing new products. A model of a process from research to commercialization with the cumulative profit and loss curve is put forward and hypotheses related to success and failure are developed at the stages up to product launch. Seventeen large projects that have resulted in successful product launches have been examined from the initial research stage to commercialization. Prefect duration, standardized cumulative R&D expenditures and research resource concentration are analyzed in terms of statistical method and patterns in cumulative profit and loss curves after product sales, as well as the reasons for and other aspects of success/failure are investigated and analyzed. Consequently, valuable information on future management tasks has been obtained such as: (1) project duration differs depending on market sectors, product types and presence/absence of materials research (2) cumulative profit and loss curves can be categorized into four patterns (3) reasons for failure can be divided into technological and market problem categories and (4) these factors have an impact on product sales.

Background/Aims: Endoscopic vacuum therapy (EVT) can heal a variety of defects within the gastrointestinal (GI) tract via applying negative pressure, which reduces the defect size, aspirates the infected fluid, and promotes granulation tissue. Here we present our experience with EVT as it relates to both spontaneous and iatrogenic upper GI tract perforations, leaks, and fistulas. Methods: This retrospective study was conducted at four large hospital centers. All patients who underwent EVT between June 2018 and March 2021 were included. Data on multiple variables were collected, including demographics, defect size and location, number and intervals of EVT exchanges, technical success, and hospital length of stay. Student t-test and the chi-squared test were used to analyze the data. Results: Twenty patients underwent EVT. The most common defect cause was spontaneous esophageal perforation (50%). The most common defect location was the distal esophagus (55%). The success rate was 80%. Seven patients were treated with EVT as the primary closure method. The mean number of exchanges was five with a mean interval of 4.3 days between exchanges. The mean length of hospital stay was 55.8 days. Conclusions: EVT is a safe and effective initial management option for esophageal leaks and perforations.
The development of the Surrey Research Park by the University of Surrey is an addition to a number of existing strategies to collaborate with industry that it has developed over its 120 year history. The potential to undertake this development was based on owning a substantial land holding that the University acquired when the Borough Council for the town of Guildford invited the University to relocate from Battersea in London to its new location in 1966. Initial plans for the Park in 1979 were accelerated in 1981 in response to plans by the then government to reduce funding for Higher Education in the UK. Beyond a broad master plan for the site that was based on topography and access to the site the plans that were developed were based on a survey of 100 companies that were deemed to be in the target market for the site and a review of the other 7 science parks that were being developed in the UK in 1981. The findings from this proved to be important in developing the master plan for the site. Another important influence on the project was the objectives that were defined for the 3 stakeholders in the project of the University, the tenant companies and the planning authority relate to economic development, a competitive advantage of tenant companies, knowledge transfer, the profile for the University and the capacity to generate income proved to be a valuable framework on which to develop a master plan. These details were underpinned by five objectives which served the three stakeholders in the site. Those for the University included commercial potential, knowledge transfer and image and reputation; those for the town primarily related to economic development and the plan was to help tenants gain a competitive advantage by locating on the site. In addition a number of success indicators were defined for the project against which to measure performance and have remained as a useful set of parameters on which to base the assessment of the performance of the site. In combination with these indicators a further analysis deals with the success factors that are considered as important in influencing performance. The paper sets details the history of the park and covers the success indicators and factors and reviews these in the context of the original objectives for the site.

Objective : Although a transradial angiography is accepted as the gold standard for cardiovascular procedures, cerebral angiography has been performed via transfemoral approach in most institutions. The purpose of this study is to present our experience concerning the feasibility, efficacy, and safety of a transradial approach to cerebral angiography as an alternative to a transfemoral approach. Methods : Between February 2007 and October 2009, a total of 1,240 cerebral angiographies were performed via a transradial approach in a single center. The right radial approach was used as an initial access route. The procedure continued only after the ulnar artery was proven to provide satisfactory collateral perfusion according to two tests (a modified Allen's test and forearm angiography). Results : The procedural success rate was 94.8% with a mean duration of 28 minutes. All supra-aortic vessels were successfully catheterized with a success rate of 100%. The success rates of selective catheterization to the right vertebral artery, right internal carotid artery, left internal carotid artery, and left vertebral artery were 96.1%, 98.6%, 82.6% and 52.2%, respectively. The procedure was performed more than twice in 73 patients (5.9%), including up to 4 times in 2 patients. The radial artery occlusion was found in 4 patients (5.4%) on follow-up cerebral angiography, but no ischemic symptoms were observed in any of the cases. Conclusion : This study suggests that cerebral angiography using a transradial approach can be performed with minimal risk of morbidity. In particular, this procedure might be useful for follow-up angiographies and place less stress on patients.

Since it was first reported in 1997, somatic cell cloning has been demonstrated in several other mammalian species. On the mouse, it can be cloned from embryonic stem (ES) cells, fetus-derived cells, and adult-derived cells, both male and female. While cloning efficiencies range from 0 to 20%, rates of just 1-2% are typical (i.e. one or two live offspring per one hundred initial embryos). Recently, abnormalities in mice cloned from somatic cells have been reported, such as abnormal gene expression in embryo (Boiani et al., 2001, Bortvin et al., 2003), abnormal placenta (Wakayama and Yanagimachi 1999), obesity (Tamashiro et ai, 2000, 2002) or early death (Ogonuki et al., 2002). Such abnormalities notwithstanding, success in generating cloned offspring has opened new avenues of investigation and provides a valuable tool that basic research scientists have employed to study complex processes such as genomic reprogramming, imprinting and embryonic development. On the other hand, mouse ES cell lines can also be generated from adult somatic cells via nuclear transfer. These 'ntES cells' are capable of differentiation into an extensive variety of cell types in vitro, as well assperm and oocytes in vivo. Interestingly, the establish rate of ntES cell line from cloned blastocyst is much higher than the success rate of cloned mouse. It is also possible to make cloned mice from ntES cell nuclei as donor, but this serial nuclear transfer method could not improved the cloning efficiency. Might be ntES cell has both character between ES cell and somatic cell. A number of potential agricultural and clinical applications are also are being explored, including the reproductive cloning of farm animals and therapeutic cloning for human cell, tissue, and organ replacement. Objective : This paper presents data from a retrospective study of endoscopic third ventriculostomy (ETV) in patients with shunt malfunction and proposes a simple and reasonable post-operative protocol that can detect ETV failure. Methods : We enrolled 19 consecutive hydrocephalus patients (11 male and 8 female) who were treated with ETV between April 2001 and July 2010 after failure of previously placed shunts. We evaluated for correlations between the success rate of ETV and the following parameters : age at the time of surgery, etiology of hydrocephalus, number of shunt revisions, interval between the initial diagnosis of hydrocephalus or the last shunt placement and ETV, and the indwelling time of external ventricular drainage. Results : At the time of ETV after shunt failure, 14 of the 19 patients were in the pediatric age group and 5 were adults, with ages ranging from 14 months to 42 years (median age, 12 years). The patients had initially been diagnosed with hydrocephalus between the ages of 1 month 24 days and 32 years (median age, 6 years 3 months). The etiology of hydrocephalus was neoplasm in 7 patients; infection in 5; malformation, such as aqueductal stenosis or megacisterna magna in 3; trauma in 1; and unknown in 3. The overall success rate during the median follow-up duration of 1.4 years (9 days to 8.7 years) after secondary ETV was 68.4%. None of the possible contributing factors for successful ETV, including age (p=0.97) and the etiology of hydrocephalus (p=0.79), were statistically correlated with outcomes in our series. Conclusion: The use of ETV in patients with shunt malfunction resulted in shunt independence in 68.4% of cases. Age, etiology of hydrocephalus, and other contributing factors were not statistically correlated with ETV success. External ventricular drainage management during the immediate post-ETV period is a good means of detecting ETV failure.
The effects of low salinity (fertilization success and larval survival) on the limpet Cellana grata were studied at early stages of development using the marine bioassay technique. It was shown that, under normal conditions for development from fertilization to the post veliger stage, the salinity must be not less than 20.0~35.0 psu. However, the fertilization rate and larval survival of C. grata was obviously reduced at 5.0 psu and 10.0 psu, respectively. Mass mortality was estimated to occur at <20.0 psu (48-h $EC_{50}=19.54psu$) and the survival rate of normal veliger larvae decreased with experimental time during exposure. No observed effective concentration (NOEC) and lowest observed effect concentration (LOEC) of post veliger were estimated at 30.0 psu and 25.0 psu, respectively, during 48-h exposure. The tolerance limits of the test species to salinity revealed various concentration ranges of salinity, which may reflect the physiology and ecology of the initial development stages of C. grata. These results demonstrate that reduced salinity is detrimental to the reproductive success and larval survival of C. grata, and if salinity is lowered by natural or anthropogenic sources during spawning, this would lead to decreased reproductive success and larval settlement.
