구강악안면외과 환자의 전신 마취에 있어서 흡입 마취와 정맥 마취의 차이에 대한 임상적 고찰 (A Clinical Study about Comparison of Inhalation Anesthesia and Intravenous Anesthesia in Oral and Maxillofacial Patients)

  • Intravenous anesthesia was compared with inhalation anesthesia in 20 patients of oral and maxillofacial surgery. The patients were randomly assigned to two treatment groups so that 20 patients were injected ketamine and propofol and 20 patients were administered enflurane. The hemodynamic responses of patients and recovery profile of the two groups were compared. Intravenous anesthesia group awoke significantly faster than inhalation anesthesia after operation. Time to full recovery in intravenous anesthesia group was significantly shorter than that of inhalation anesthesia group. We conclude that intravenous anesthesia is a practical technique for oral and maxillofacial surgery patients and intravenous anesthesia may be more preferable because of the significant shortness of recovery time.

  • Intravenous anesthesia was compared with inhalation anesthesia in 20 patients of oral and maxillofacial surgery. The patients were randomly assigned to study in two treatment groups. 20 patients were injected ketamine and propofol. 20 patients were administered Enflurane. The respond of patients consciousness and general recovery condition of the two groups were compared. Intravenous anesthesia group were awake significantly faster without complications such as nause, vomiting, and agitation after operation than inhalation anesthesia group. Full recovery time of intravenous anesthesia group was significantly 3 times less than inhalation anesthesia group. The authors conclude that intravenous anesthesia is a practical technique for oral and maxillofacial surgery patients undergoing and may be preferable to intravenous anesthesia because of the significantly short of recovery time without complications.

Desflurane Inhalation Provide Cardiovascular Stability During Intubation but Prevention of Rocuronium Injection Pain

  • Background: Desflurane has very short induction time because its physical characteristics. But its pungent odor and tendency to irritate the upper airway make it unsuitable for induction of anesthesia. This study was performed to determine what time is prefer to start the desflurane inhalation. Methods: Forty adults (17-45 years) were enrolled in a randomized, double-blind study. Twenty start desflurane inhalation just after loss of consciousness, and the others received desflurane after intubation. We monitored vital signs, BIS, desflurane concentration, rocuronium injection pain response, and airway irritation signs. Results: The demographic data were not different two groups. Early inhalation group showed more stable cardiovascular response than that of late inhalation group. But rocuronium injection pain response and airway irritation sings were not different between two groups. Conclusions: Early inhalation of desflurane (6 vol%) just after loss of consciousness attenuates cardiovascular responses during intubation.

충남장애인구강진료센터에서 시행된 외래전신마취와 치과치료에 대한 조사연구 (A Survey of the Outpatient General Anesthesia and Dental Treatment in Chungnam Dental Clinic for the Disabled)

  • Background: Dental treatment under outpatient general anesthesia is an option for disabled person, children and dentally disabled person who have the difficulty of cooperating voluntary with dental care. The aim of this study was to access the patients who had dental management under general anesthesia at Chungnam dental clinic for the disabled. Methods: This survey is consisted of 114 severely disabled and dentally disabled person who undergone dental treatment under general anesthesia from January to June 2011. Data were collected by individual dental and anesthesia records. Results: Of the 114 patients studies, the 45 patients was the dentally disabled and the 69 patients was the severely disabled. Rate of dental treatment among male patients was higher than among female patients. The dentally disabled was an similar age distribution. In the severly disabled, above 13-year-old was higher than below 12-year-old. The most popular induction method of general anesthesia was sevoflurane inhalation after induced sedative effect of nitrous oxide inhalation. The severly disabled who had definitely negative behavior was applied inhalation of high concentration sevoflurane. Conclusions: Chungnam Dental clinic for the disabled provide dental management for the disabled and the dentally disabled.

복강경하 담낭절제술환자에서의 마취방법에 따른 수술 후 오심과 구토의 비교 (The Comparison of the Effects of Two Anaesthetic Techniques on Postoperative Nausea and Vomiting in Patients Undergoing Laparoscopic Cholecystectomy)

  • Purpose: Postoperative nausea and vomiting(PONV) is a common problem after general anesthesia. The aim of this prospective, double-blind randomized study was to compare the effect of Propofol-Remifentanil vs. Sevoflurane inhalational anesthetics on PONV after laparoscopic cholecystectomy. Methods: Forty patients (ASA physical status 1, 2) scheduled for elective surgery participated in the study. Twenty of them received total intravenous anesthesia (TIVA group) with Propofol-Remifentanil, and the rest were given Sevoflurane inhalational anesthetics (inhalation group). The TIVA group was induced with Propofol 5mcg/ml and Remifentanil 3~4mcg/ml. The anesthesia was maintained with the continuous infusion of Propofol 2~3mcg/ml and Remifentanil 2~3mcg/ml IV. The inhalation group was induced with Pentotal Sodium 5mg/kg and 3~4mcg/kg/hr IV Remifentanil. Maintenance was obtained with 1.5~2.0 vol% Sevoflurane. Results: The subjects in TIVA group reported less PONV than those in Sevoflurane inhalation anesthesia group. Conclusion: Propofol-Remifentanil anesthesia (TIVA group) was considered a satisfactory anesthetic technique in reducing PONV in patients with laparoscopic cholecystectomy.

호흡마취 유도제로서 alfaxalone의 후향적 평가: 150례 (Retrospective Evaluation of Alfaxalone as an Induction Agent of Inhalation Anesthesia: 150 Cases)

  • This study was performed to evaluate the clinical efficacy of alfaxalone for induction of inhalation anesthesia in small animal practice. Patient data were collected according to anesthetic records (136 dogs and 14 cats) presented to the Veterinary Medical Teaching Hospital of Seoul National University for surgeries and diagnostic imaging from July 2013 to March 2014. Anesthetic results included signalment, American Society of Anesthesiologists (ASA) grade, premedicated drugs, procedures, induction quality, and recovery after anesthesia. One hundred fifty anesthetic events were classified according to the ASA grade. Three patients were ASA grade I, 52 patients grade II, 86 patients grade III, and 9 patients grade IV, respectively. The most common premedication was midazolam and hydromorphone combination (n = 59, 39.3%) follow by acepromazine and hydromorphone combination (n = 22, 14.7%). The majority of anesthesia procedures were diagnostic imaging (n = 33, 22.0%) and ophthalmic surgeries (n = 31, 20.7%), followed by soft tissue surgeries (n = 27, 18.0%), and orthopedic surgeries (n = 20, 13.3%). Intravenous alfaxalone provided smooth induction for inhalation anesthesia in almost cases, but transient apnea and twitching/paddling were observed after induction and during recovery, respectively. In addition, alfaxalone did not show pain response during intravenous administration. Alfaxalone showed smooth induction of inhalation anesthesia in dogs and cats with mild to severe systemic disease (ASA 2-4). Alfaxalone was considered as an acceptable induction agent for patients with higher risk in small animal practice.

  • Day stay anesthesia should include: rapid smooth onset; rapid recovery without residual side effects; absence of adverse effects (N/V); and providing postoperative analgesia. General anesthesia with multi-modalities (inhalation, intravenous and local anesthesia) may be preferable in day stay surgery. Future studies on new drugs and techniques for day stay anesthesia need comparing the increased coat of newer treatments with the potential financial savings resulting from earlier hospital discharge, reduced supplemental drugs, and earlier return to work.

  • Myringotomy and ventilation tube insertion are widely performed in pediatric patients with chronic otitis media. This procedure is performed under general anesthesia or sedation with local anesthesia infiltration in pediatric patients. In this case report, we report a case of transient facial paralysis in a pediatric patient who underwent myringotomy and ventilation tube insertion using sevoflurane inhalation and four-quadrant blocks with lidocaine.

행동조절장애 환자에서 치과치료를 위한 외래마취의 분석 (The Analysis of Outpatient Anesthesia for Dental Treatment in Handicapped Patients with Behavior Disorder)

  • Background: The aim of this study was to analyze outpatient anesthesia for dental treatment in handicapped patients with behavior disorder in order to use data for carrying out better and safe anesthetic management. Methods: The data were drawn from the 100 patients with behavior disorder who visited CNUDH dental clinic for disabled based on anesthesia record to investigate patient's systemic condition, cooperative level, anesthesia method according to patients cooperation, and side effects after recovery time. Results: Mental retardation (58%) is the most reason to choose general anesthesia. The methods of induction according to cooperative level are intravenous propofol injection in 22 cases and inhalation of sevoflurane in 78 cases. Induction time of anesthesia were within 10 seconds in cases of propofol induction and average $48.8{\pm}18.5$ seconds in cases of inhalation induction. The time spent on dental treatment was average $3.2{\pm}1.1$ hours. After the end of treatment, average time to move from unit chair to recovery bed, to recliner, and to discharge from hospital are $10.4{\pm}5.1$, $36.9{\pm}17.1$ and $72.4{\pm}16.0$ minutes, respectively. During recovery, there are nausea with 9%, vomiting with 4%, dizziness with 2%, finger injury with 1%. Conclusions: This study showed our successful anesthetic outcomes without any severe side effects or complications. Through this study, it will be used for safe anesthetic management as useful reference data.

  • General anesthesia has been developed in dentistry as a type of management technique to sedate patients who may be uncontrollable or require medical consideration. However, emergency treatment for controlling pain before general anesthesia could be required due to the time for medical evaluation for general anesthesia. There is a greater possibility of developing complications under extended waiting period. A disabled patient who needs dental treatment under general anesthesia underwent pretreatment with intramuscular injection of midazolam and $N_2O-O_2$ inhalation sedation in these cases report.
