꽃마을한방병원 한방부인과(韓方婦人科)에 불임(不姙)을 주소(主訴)로 내원(來院)한 환자(患者)에 대한 실태분석(實態分析) (Analysis on Infertility Patients in Dept. of Oriental Gynecology of Conmaul Oriental Hospital)

  • Purpose : To investigate clinical-epidemologic aspect of infertility patients in Conmaul Oriental Hospital. Methods : From 2003, 5 to 2004, 4, a total 1223(female 943, male 280) patients was recruited, and their medical records were reviewed retrospectively. Results : The mean age of female was $31.83{\pm}3.63$ yesrs, and $34.08{\pm}3.66$ years in male. The mean BMI of female was $20.83{\pm}2.74$, and $24.36{\pm}3.03$ in male. The most common occupation of female was profession, administration, management.The primary infertility was 52.9% and the secondary infertility 47.1%. The most common past history in female was laparotomy. The mean duration of infertility was $3.32{\pm}2.5$ years and $3.9{\pm}2.63$ years in female and male. 83.6% of secondary infertility women experienced abortion. The incidence of patients with both combinded infertility factors of female and male was the most highest, and the most common single factor was ovulatory. 92.4% of female patients took sterility tests, 71.0% of male patients took semen analysis. 50.8% female took ovulation induction, IUI, IVF before coming. 8.91% of female and 72.14% of male were accompanied by their spouse. 61.2% of female took sterility test with their spouse. Female's duration of treatment was longer than man's. The most reliable source of choices was encouragement of family members, acquaintance. Conclusion : In this study, we presented staus concerend with infertility and the characteristics of patients went to oriental hospital. Furthermore, the study about oriental treatments and the results of that treatments is required.

IVF 시술 실패한 여성 불임환자 1례와 남성 불임환자 1례의 임상보고 (Case Report of Each Example of a Female and a Male Patient Who Were Infertile and Failed in In Vitro Fertilization(IVF) Treatment)

  • Purpose: The purpose of this paper is to report the effect of Korean medicine treatments on a female and a male patient who were infertile and failed to in vitro fertilization(IVF) treatment. Methods: The patients are a 30-year-old female who had been diagnosed with the ovulation dysfunction caused by the depression of ovarian function, and a 33-year-old male who had been diagnosed with sperm defect. The female patient alone underwent Korean medicine treatment after the eighth failure in IVF treatment and the male patient received Korean medicine treatment along with his wife after he failed in the first IVF treatment. The Korean medicine applied include herb-medicine administration, acupuncture, moxibustion, Jwa-Hun, and Du-Han-Jok-Yeol treatment. Results: Through Korean medicine treatment the female patient had succeeded in the IVF treatment and she also recovered completely from allergic rhinitis. The male patient whose wife also received Korean medicine treatment with him got his wife pregnant naturally. Conclusions: This result concludes that Korean medicine treatment is effective in infertile patients. Therefore, there needs to be more of subsequent research on infertile patients that is based on Korean medicine treatment.

Knowledge regarding factors that influence fertility in Thai reproductive-age population living in urban area: A cross-sectional study

  • Objective: To survey knowledge about the factors that influence fertility in a reproductive-age population living in an urban area. Methods: A cross-sectional study was conducted using a questionnaire-based survey among both males and females aged 18-45 years living in the Bangkok metropolitan area. Results: The mean age of the participants was $26.8{\pm}7.2years$ (male, 41.9%; female, 58.1%). Of the participants, 53.1% had an undergraduate degree and 57.1% were single. Only one-fifth of the participants correctly identified the age when fecundity declines in male and female, the definition of infertility, and the period during the menstrual cycle with the highest chance of pregnancy. Approximately three-fourths of the participants correctly identified that cigarette smoking, alcohol consumption, and sexually transmitted infections affect fertility. Conclusion: A considerable knowledge gap about the factors that influence fertility was identified in reproductive-age individuals in an urban area of Thailand. This issue should be urgently addressed by promoting fertility awareness through education, discussions about social perceptions regarding fertility, and reliable sources of knowledge.

사람 성장호르몬 유전자를 발현하는 형질전환생쥐의 불임성 (Infertility of Transgenic Mice Experssing Human Growth Hormone Gene)

  • Many transgenic mice expressing human growth hormone gene were infertile. To investigate the infertility of these transfenic mice, it was looked into the estrus cycle and sexual behaviour and also tested through in vitro fertilization whether the germ cells of these mice normal or not. The infertile female transgenic mice were mated to the fertile males of ICR strain, but in almost all of them the vaginal plugs were not detected and their estrus cycles by vaginal smear were almost irregular which kept up estrus or diestrus stage. Many male transgenic mice did not have the ability of sexual behaviour. Therefore the viability of germ cells in infertile male transgenic mice was investigated by in vitro fertilization, but the sperm were normally fertilized with the eggs and the transgene of parent was passed on to the progeny. These results consequently suggest that the infertility of transgenic mice experssing human growth hormone gene may be due to the physiological activity of human growth hormone, not germ cells.

The Study of X Chromosome Inactivation Mechanism in Klinefelter's Syndrome by cDNA Microarray Experiment

  • To investigate the XIST gene expression and its effect in a Klinefelter's patient, we used Klinefelter's syndrome (XXY) patient with azoospermia and also used a normal male (XY) and a normal female (XX) as the control, We were performed cytogenetic analysis, Y chromosomal microdeletion assay (Yq), semi-quantitative RT-PCR, and the Northern blot for Klinefelter's syndrome (KS) patient, a female and a male control, We extracted total RNA from the KS patient, and from the normal cells of the female and male control subjects using the RNA prep kit (Qiagen), cDNA microarray contained 218 human X chromosome-specific genes was fabricated. Each total RNA was reverse transcribed to the first strand cDNA and was labeled with Cy-3 and Cy-5 fluorescein, The microarray was scanned by ScanArray 4000XL system. XIST transcripts were detected from the Klinefelters patient and the female by RT-PCR and Northern blot analysis, but not from the normal male, In the cDNA microarray experiment, we found 24 genes and 14 genes are highly expressed in KS more than the normal male and females, respectively. We concluded that highly expressed genes in KS may be a resulted of the abnormal X inactivation mechanism.

Sperm chromatin structure assay results in Nigerian men with unexplained infertility

  • Objective: Several publications have established a relationship between sperm DNA damage and male factor infertility, based on data from America, Europe, and Asia. This study aimed to compare the extent of sperm DNA damage in sperm samples from Nigerian men with unexplained infertility and in sperm samples from a fertile group composed of sperm donors who had successfully impregnated a female partner naturally or through assisted conception. Methods: A total of 404 men underwent male fertility evaluation at Androcare Laboratories and Cryobank participated in this study. Semen analysis and a sperm chromatin structure assay (SCSA) were performed on all subjects. Results: The men in the unexplained infertility group were slightly older than the men in the fertile sperm group ($36{\pm}10$ years vs. $32{\pm}6$ years, p=0.051). No significant difference was observed between the two groups in semen analysis parameters ($p{\geq}0.05$). Men in the unexplained infertility group with normal semen parameters had a significantly higher DNA fragmentation index (DFI) than men in the fertile sperm group ($27.5%{\pm}7.0%$ vs. $14.1%{\pm}5.3%$, p<0.05). In the unexplained infertility group, 63% of the men had a DFI greater than 20%, compared to 4% in the fertile sperm group. In the unexplained infertility group, 15.2% of the subjects had a DFI greater than 30%, compared to 1% in the fertile sperm group. Conclusion: Our study showed that the SCSA may be a more reliable predictor of fertility potential than traditional semen analysis in cases of unexplained infertility.

Polymerase Chain Reaction을 이용한 성의 감별 (Sex Determination by Polymerase Chain Reaction)

  • Sex determination in genomic DNA from human blood leucocytes was performed by amplification of human Y chromosome-specific DNA sequences using PCR technique. A clear DNA fragment(154 nucleotides long) was appeared only in the male genomic DNA, but no specific band was observed in the case of female genomic DNA and negative control. To know the sensitivity of this method, the amplification reaction was performed in genomic DNA diluted to 2pg equivalent to the amonut present in the single human cell, and clear band also observed. The PCR amplification was so succesfully performed in the single leucocyte separated from human blood using micromanipulator that this techniqe is assumed to be applied to single blstomere before embryo transfer.

  • We demonstrated that the normal pregnancy and delivery by round spermatid injection (ROSI) into oocytes was achieved from nonobstructive azoospermia patient. In this case, the normal fertilization rate was 50%. All of the two pronuclear stage embryos cleaved and were transferred to the patient's uterus. A singleton pregnancy was achieved and resulted in the birth of normal female infant. This resuJt show that intracytoplasraic injection of round spermatid seems to be new treatment of nonobstmctive azoospermia male infertility. Further research is needed to evaluate the required culture conditions to induce progression of the round spermatid into a more elongated stage.

Estrogen Receptor Alpha Agonist Propyl Pyrazole Triol Causes Alterations of the Morphology and Function of the Mouse Male Reproductive System

  • Known as a female hormone, estrogen, performs important functions, and the activities of the hormone are mediated via the estrogen receptor. The principal objective of the present study was to assess the effects of a estrogen receptor agonist in male reproductive organs. In this study, the estrogen receptor alpha agonist, PPT, was injected subcutaneously into adult male mice. The effects of PPT on the murine reproductive system were histologically assessed at 3,5, and 8 weeks after treatment. In the treatment group, reductions were observed in the weight of the body, testis and epididymis. Microscopic examination revealed a reduction in seminiferous tubular diameter in the testis, and epithelial cell height in the epididymis during the experiment. 8 weeks after treatment, spermatogenesis was not detected, nor was the lumen of the seminiferous tubules. In the fertility test, 1 week after PPT injection, the fertilizing ability of males was decreased, and on the 2nd and 3rd weeks, complete infertility was observed. In conclusion, the injection of high concentrations of PPT into adult males induced physiological changes, including infertility, and also induced morphological changes, including a reduction in the height of epithelial cells within the reproductive system.
