  • As we enter the new millennium, there have been dramatic improvements in the care of patients with HIV infection. These have prolonged life and decreased morbidity and mortality. There are fourteen currently available antiretrovirals approved in the United States for the treatment of this infection. The medications, including their pharmacokinetic properties, side effects, and dosing are reviewed. In addition, the current approach to the use of these medicines is discussed.

Acupuncture and blood borne viral infections: a brief summary

  • Blood borne infectious diseases are usually a public concern. The transmissions of many diseases are via blood borne mode. Several activities are related to this transmission such as injection, blood transfusion and transplantation. Also, the acupuncture practice can be a possible route for blood borne infectious disease transmission. In this specific review, the author briefly reviews acupuncture and blood borne viral infections.

Influence of Nurses' COVID-19 Related Stress, Hardiness, and Organizational Citizenship Behavior on Nursing Intention in Infectious Diseases Hospitals (감염병전담병원 간호사의 COVID-19 관련 스트레스, 강인성 및 조직시민행동이 간호의도에 미치는 영향)

  • Purpose: This study aimed to verify the effects of nurses' COVID-19 related stress, hardiness, and organizational citizenship behavior on nursing intention in infectious disease hospitals. Methods: The participants were 182 nurses. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and multiple stepwise regression analysis with the SPSS program. Results: The nursing intention of nurses differed significantly by age (p=.013), marital status (p=.002), clinical career (p=.018), job position (p=.005), and experience of education for infectious disease management (p=.016). A statistically significant positive correlation was observed between nursing intention and COVID-19 related stress(r=.18), and between nursing intention and organizational citizenship behavior(r=.43). COVID-19 related stress, and organizational citizenship behavior explained 21% of the nursing intention. Conclusion: Based on the above results, improve the nursing intention of infectious disease hospital nurses exposed to risk, and it is necessary to manage inadequate stress of infectious diseases and cultivate organizational citizenship behavior.

Four Year Surveillance of the Vector Hard Ticks for SFTS, Ganghwa-do, Republic of Korea

  • The seasonal abundance of hard ticks that transmit severe fever with thrombocytopenia syndrome virus was monitored with a collection trap method every April to November during 2015-2018 and with a flagging method every July and August during 2015-2018 in Ganghwa-do (island) of Incheon Metropolitan City, Republic of Korea. This monitoring was performed in a copse, a short grass field, coniferous forest and broad-leaved forest. A total of 17,457 ticks (8,277 larvae, 4,137 nymphs, 3,389 females, and 1,654 males) of the ixodid ticks comprising 3 species (Haemaphysalis longicornis, H. flava, and Ixodes nipponensis) were collected with collection traps. Of the identified ticks, H. longicornis was the most frequently collected ticks (except larval ticks) (94.26%, 8,653/9,180 ticks (nymphs and adults)), followed by H. flava (5.71%, 524/9,180) and Ix. nipponensis (less than 0.04%, 3/9,180). The ticks collected with collecting traps were pooled and assayed for the presence of SFTS virus with negative results. In addition, for monitoring the prevalence of hard ticks, a total of 7,461 ticks (5,529 larvae, 1,272 nymphs, 469 females, and 191 males) of the ixodid ticks comprising 3 species (H. longicornis, H. flava, and Ix. nipponensis) were collected with flagging method. H. longicornis was the highest collected ticks (except larval ticks) (99.53%, 1,908/1,917 ticks (nymphs and adults)), followed by H. flava (1.15%, 22/1,917).

  • The Serranidae is high-quality fish species with good meat quality and is traded at high price, and is attracting attention in South Korea as a cultured species that creates high added value. However, the high-density fish farming for mass production increases the risk of mass mortality due to infectious diseases, leading to enormous economic losses. Therefore, in order to safely prevent and protect farmed fish from serious infectious diseases, it is necessary to conduct disease monitoring on a regular basis. In this study, Hyporthodus septemfasciatus, Epinephelus moara, and the hybrid longtooth grouper (E. moara ♀×E. lanceolatus ♂) were collected once a month from fish farm of Garorim and Aquabiotech Co., Ltd for a total of six months, from July to December 2023. We investigated infections of five species of bacterial diseases, including Flavobacterium columnare, six species of viral diseases, including LCDV (lymphocystis disease virus), and parasitic pathogens in grouper farms. As the result, Vibrio vulnificus and V. harveyi were detected in H. septemfasciatus in August, in the case of viral diseases, NNV was detected in H. septemfasciatus from July to August using RT-PCR or PCR. Finally, In the case of parasitic diseases, Tricodina sp. was detected in E. moara and the hybrid longtooth grouper from August to December.

Serotype Variations of Agglutinogen and Fimbriae in the Korean Isolates of Bordetella pertussis (국내 Bordetella pertussis 분리균주에서 Agglutinogen과 Fimbriae 혈청형 변이 분석)

  • Bordetella pertussis is pathogenic bacteria causing pertussis, a infectious respiratory disease for the infants. The incidence rate of pertussis was significantly decreased after introduction of vaccine. However, increased pertussis cases are recently reported in several countries with high vaccine coverage. One of the inferred reasons is genotype or serotype variation between circulating strains and vaccine strains. Therefore, it is required to confirm the variation status of the isolates by genotype or serotype analysis and the possibility of pertussis outbreak in Korea should be estimated. For this, the serotype variations of the isolates from $1999\sim2006$ were investigated in agglutinogen and fimbriae. As the result, the most frequent serotype in the isolated strains was agglutinogen 1 and fimbriae 2 serotypes. Moreover, serotype transition from vaccine serotypes to non-vaccine serotypes was observed. Especially, the transition pattern of agglutinogen serotype was directed to increase a different type (agg 1) from the vaccine type (agg 1,2). However, in case of fimbriae, the same type (fim 2) with vaccine strain was increased. These results were also observed in other countries with increasing incidence of pertussis. For more predictable results to know increasing possibility of pertussis incidence in Korea, the studies on genetic variations of antigenic determinant genes and prevalence of antibody titer in normal population should be performed in the further.

A Delphi Study on National Public Vaccine Research and Development Policy in Korea (델파이기법을 활용한 국가 공공(公共)백신 연구개발 정책수립 기초조사)

  • Vaccination is the most powerful and useful preparation against infectious diseases. However, developing vaccines costs a lot and requires extensive long-term efforts. Therefore, the government should research and develop vaccines with a national-level policy. To greatly enhance the success rate of vaccine development, the policy should be set up considering priorities such as the current status of domestic research, the importance for public health, the urgency of research. The Delphi technique was utilized to draft this survey, through a brainstorming stage, then two inquiries, and finally the final panel meeting where unresolved items were discussed, to draw the conclusion. Among the results, firstly, the highest ranked item on centralized fields for vaccine development by the Ministry of Health was 'self-sufficiency of vaccines.' Secondly, 'emerging infectious disease' was most highly ranked in prioritized fields of vaccine development and research. Thirdly, for the vaccine that needs to be improved and developed further by the government to improve its efficacy and safety, BCG (Bacille de Calmette) for tuberculosis was ranked the highest on both types (intradermal and subcutaneous injection) from National Immunization Programme (NIP) and non-NIP. As for the high risk pathogens, 'anthrax' and 'smallpox' were first and second, consecutively. Lastly, 'development and control of vaccine candidates' was ranked the highest for the area in need for technique development in order to improve domestic vaccine's research level. The results of this study will be put to good use as basic data for the national vaccine research and development (R&D) policy of the country. This study was first step and more studies should be carried out for the final decision of the national vaccine R&D priority.

  • Fever of unknown origin (FUO) has been a convenient term used to classify patients who warrant a particular systemic approach to diagnostic evaluation and management. The greatest clinical concern in evaluating FUO is identifying patients whose fever has a serious or life-threatening cause when a delay in diagnosis could jeopardize successful intervention. Thorough history and complete physical examination are critical to uncover the etiologic diagnosis. Most cases of FUO in children are caused by atypical presentations of common diseases rather than by typical manifestations of rare disorders. Selection of diagnostic tests and speed of investigation should be guided by a knowledge of the disease severity, patient age, epidemiologic and geographic information, and any positive findings from a detailed history and physical examination. The three most common causes of FUO in children are infectious diseases, connective tissue diseases, and malignancy. In general, the prognosis of FUO in children is better than that of adults. Although the outcome is dependent on the primary disease process, fever abates spontaneously in most cases in whom the cause of fever remains unclear.
