Detection of foot-and-mouth disease virus (FMDV) and avian influenza virus (AIV) from animal carcass disposal sites using real-time RT-PCR

  • Foot-and-mouth disease (FMD) and avian influenza (AI) are highly pathogenic viral disease which affects the livestock industry worldwide. Outbreak of these viruses causes great impact in the livestock industry; thus, disease infected animals were immediately disposed. Burial is the commonly used disposal method for deceased animals. However, there is potential for secondary environmental contamination, as well as the risk that infectious agents persisting in the environment due to the limited environmental controls in livestock burial sites during the decomposition of the carcasses. Therefore, this study aimed to investigate the detection of FMD and AI viruses from animal carcass disposal sites using real-time reverse transcription PCR. Soil samples of more than three years post-burial from livestock carcass disposal sites were collected and processed RNA isolation using a commercial extraction kit. The isolated RNA of the samples was used for the detection of FMDV and AIV using qRT-PCR. Based on the qPCR assay result, no viral particle was detected in the soil samples collected from the animal disposal sites. This indicates that 3 years of burial and their carcass disposal method is efficient for the control or at least reduction of spread infections in the surrounding environment.

Noncommunicable Diseases: Current Status of Major Modifiable Risk Factors in Korea

  • A noncommunicable disease (NCD) is a medical condition or disease that is by definition non-infectious and non-transmissible among people. Currently, NCDs are the leading causes of death and disease burden worldwide. The four main types of NCDs, including cardiovascular disease, cancer, chronic lung disease, and diabetes, result in more than 30 million deaths annually. To reduce the burden of NCDs on global health, current public health actions stress the importance of preventing, detecting, and correcting modifiable risk factors; controlling major modifiable risk factors has been shown to effectively reduce NCD mortality. The World Health Organization's World Health Report 2002 identified tobacco use, alcohol consumption, overweight, physical inactivity, high blood pressure, and high cholesterol as the most important risk factors for NCDs. Accordingly, the present report set out to review the prevalence and trends of these modifiable risk factors in the Korean population. Over the past few decades, we observed significant risk factor modifications of improved blood pressure control and decreased smoking rate. However, hypertension and cigarette smoking remained the most contributable factors of NCDs in the Korean population. Moreover, other major modifiable risk factors show no improvement or even worsened. The current status and trends in major modifiable risk factors reinforce the importance of prevention, detection, and treatment of risk factors in reducing the burden of NCDs on individuals and society.

생후 90일 이하의 영아에서 호흡기 바이러스 검출과 관련된 위험인자 (Risk Factors Associated with Respiratory Virus Detection in Infants Younger than 90 Days of Age)

  • 목적: 임상적으로 중증 세균성 감염과 바이러스성 질환의 감별이 어려운 어린 영아에서 호흡기 바이러스를 검출하고 이것과 연관된 임상적 위험인자를 분석하였다. 방법: 2011년 9월부터 2012년 8월까지 생후 90일 이하 영아 중 패혈증을 포함한 감염성 질환이 의심된 227명을 대상으로 비인두 검체를 채취하였으며 임상적 특성에 대한 후향적 연구를 시행하였다. 채취한 검체 내 호흡기 바이러스의 검출은 real-time PCR 검사를 통해 측정되었다. 결과: 총 157명(69.2%)의 영아에서 한 가지 이상의 호흡기 바이러스가 검출되었다. 빈도는 RSV (75명), RV (42명), influenza virus (18명), parainfluenza virus (15명), human metapneumovirus (9명), coronavirus (9명), adenovirus (4명), bocavirus (3명) 순이었다. 이 중 24명(10.6%)에서 세균성 감염을 진단하였다. 기침, 호흡기 질환의 가족력이 있는 경우 혹은 가을/겨울 철에 호흡기 바이러스가 의미있게 높은 빈도로 검출되었으며 로지스틱 회귀분석에서도 같은 경향을 확인하였다. 가을과 겨울에는 세균성 감염 환자보다 그렇지 않은 환자에서 호흡기 바이러스 검출이 유의하게 많은 것을 알 수 있었다. 결론: 호흡기 바이러스는 감염성 질환이 의심되어 입원한 어린 영아의 중요한 병원체이며 그 검출률은 호흡기 증상, 가을/겨울철 발생, 호흡기 증상의 가족력이 있는 환자에서 유의하게 높았다.

Imported Malaria in United Arab Emirates: Evaluation of a New DNA Extraction Technique Using Nested PCR

  • Local malaria transmission in the United Arab Emirates (UAE) came to an end in 1997. Nevertheless, UAE has been subjected to substantial importation of malaria cases from abroad, concerning both UAE nationals and immigrants from malarious countries with a total number of 2,119 cases in 2007. To evaluate a new DNA extraction technique using nested PCR, blood samples were collected from 132 individuals who presented to Infectious Diseases Department in Rashid Hospital, Dubai, and Central Department of Malaria Control with fever and persistent headache. Giemsa-stained blood films and ELISA test for malaria antibodies were carried out for detection of Plasmodium infection. Plasmodium infections were identified with the genus-specific primer set and species differentiation using nested PCR. A rapid procedure for diagnosis of malaria infections directly from dried blood spots using for the first time DNA extract from FTA Elute cards was evaluated in contrast to extraction techniques using FTA classic cards and rapid boiling technique. Our new simple technique for DNA extraction using FTA Elute cards was very sensitive giving a sensitivity of 100% compared to 94% using FTA classic cards and 62% in the rapid boiling technique. No complex preparation of blood samples was required prior to the amplification. The production cost of DNA isolation in our PCR assay was much less incomparable to that of other DNA extraction protocols. The nested PCR detected plasmodial infection and could differentiate P. falciparum from P. vivax, and also detected the mixed infection.

  • 구제역과 같은 가축 전염병은 강한 전염력으로 인해 막대한 경제적 손실을 초래하고 있다. 가축의 전염성 질병은 조기 감지가 매우 중요하나, 개별 농가에서 조기 진단이 어렵고, 수의사나 사료 공급 차량 등의 농장간 이동을 통해서 전파되는 사례도 빈번하다. 본 연구에서는 발병 초기단계에서 유비쿼터스 기반의 정보통신기술을 이용하여 가축의 체온을 자동으로 모니터링 하여 실시간 전송함으로써 수의사의 농장방문 없이 빠른 진단이 가능하고 추가 확산을 방지하는 기술을 연구하였다. 자동으로 수집된 정보와 연계된 원격진료시스템을 활용하여 농장단위, 지역단위, 국가단위에서 가축전염병을 커뮤니티 매핑 기법을 이용하여 체계적으로 관리하는 기술을 제시하였다. 이 과정에서 사용자 참여형 공간정보기술 적용을 위한 각 단계별, 주체별 커뮤니티 매핑 항목을 도출하였다.

광주광역시동물보호소 입양 대상 유기견의 호흡기 질병 실태 조사 (A survey of respiratory pathogens in dogs for adoption in Gwangju metropolitan city animal shelter, South Korea)

  • Canine infectious respiratory disease (CIRD), also known as infectious tracheobronchitis or kennel cough occurs in a multiple-dog environment such as a shelter. In this study, we were collected 300 of nasal swab samples from dogs and 145 of environmental samples from a shelter to investigate respiratory pathogens of dogs in the Gwangju metropolitan city animal shelter from February to October, 2019. Bacteria cultures for isolation of Bordetella (B.) bronchiseptica and polymerase chain reaction (PCR) tests were performed for detection of eleven canine respiratory pathogens, namely Mycoplasma (M.) cynos, canine distemper virus (CDV), canine influenza virus (CIV), canine parainfluenza virus (CPIV), canine respiratory coronavirus (CRCoV), alpha-coronavirus (CCoV), canine pneumovirus (CnPnV), canine hepacivirus (CHeV), canine adenovirus type 2 (CAdV-2), canine herpesvirus-1 (CHV-1) and canine bocavirus (CBoV). Among 300 nasal swab samples, 148 samples (49.3%) were positive for at least one pathogens. CHV-1 was the most common pathogen, found in 95/300 (31.7%) samples. Subsequently, M. cynos (22.0%), B. bronchiseptica (2.3%), CPIV (2.0%), CBoV (1.7%), CCoV (0.7%) were detected. The detection rates of M. cynos and CHV-1 according to the duration of stay in the shelter were statistically significant. Among environmental samples, M. cynos, CCoV, CBoV and CHV-1 were detected in 45/145 (31.0%). These results indicated the need for disease control and prevention systems in the shelter.

PCR-based detection of feline vector-borne pathogens in Daejeon

  • This study was conducted to investigate the detection of various vector-borne pathogens in such cats. A total of 48 stray cats collected in Daejeon were included in this study. The total positive rate of hemotropic mycoplasmas and Babesia spp. was 25% and 4%, respectively. It is recommended that species-level classification of hemotropic mycoplasmas and Babesia spp. is needed and that a large-scale prevalence study of infectious agents in all the regions of South Korea be conducted.

국내 기생충 질환의 현황 및 전망

  • The current status and future prospects of parasitic infections in Korea is briefly reviewed. Soil-transmitted helminth infections including ascariasis, trichuriasis, and hookworm infections decreased remarkably. owing to the national control activities excuted by the Korea Association of Health Promotion(formerly Korea Association of parasite Eradication) using mass heath education. Important recent trends include reemergence of vivax malaria since 1993, persistence of food-borne trematode infections including clonorchiasis and intestinal trematode infections, increased detection of zoonotic parasitosis, close-up of infection with opportunistic parasites including cryptosporidiosis, toxoplasmosis, and pneumosytosis, increase of imported tropical infectious disease, appearance of new parasitic disease such as gymnophalloidiasis, and increase of accidental infections with free-living amoebae. These trends represent greatly changed overall patterns of parasitic infections in Korea.

신선 농산물내 식중독균 검출 방법의 비교 분석 (Comparative Analysis of Detection Methods for Food-borne Pathogens in Fresh-cut Agricultural Materials)

  • 건강에 대한 관심 증대와 1인 가구 증가라는 사회구조적인 변화로 이용하기 편리한 농산물에 대한 소비가 증가하고 있다. 대부분의 신선 농산물은 가열하지 않고 섭취하는 경우가 많기 때문에 식품 매개 병원체에 쉽게 노출될 수 있어 세계적으로 과채류가 원인인 식중독 사고의 보고가 증가하고 있다. 이에 본 연구에서는 신선 농산물의 미생물학적 품질을 평가하고 식중독균 검출 방법을 비교 분석하고자 하였다. 신선 농산물 중 채소류 129건을 구입하여 배양기반 방법으로 식중독균을 분석한 결과, non-pathogenic Escherichia coli (3.9%), Bacillus cereus (31.8%), Clostridium perfringens (5.4%), Yersinia enterocolitica (0.8%), enterohemorrhagic E. coli (0.8%)가 검출되었다. 이러한 식중독균의 분석에는 증균 배양과정이 중요하게 작용을 하며 균주의 순수 분리 및 확인 동정에까지 상대적으로 많은 시간과 노력이 요구된다. 따라서 증균 배양의 과정 없이 식중독균을 신속하게 검출 할 수 있는 PCR-DGGE를 수행하여 배양 기반의 분석법과 비교하였다. 비병원성 대장균은 배양 기반 방법에서 검출되지 않았음에도 PCR-DGGE에서는 검출된 경우가 2건이 있었다. 본 연구에서 사용한 대장균 정량 분석방법은 시료를 10배 희석한 후 배양하는 과정에서 시료의 손실 가능성과 검출 한계가 높은 단점으로 PCR-DGGE가 균종의 확인에 더욱 용이할 것으로 보였다. 저위해성 식중독균은 배양 기반 방법보다 PCR-DGGE에서 검출 한계가 높은 것으로 보였다. 고위해성 식중독균은 배양 기반 방법보다 PCR-DGGE (10 CFU/g)에서 검출 한계가 낮아 균종 확인과 검출에 용이하다고 판단되었고 이를 통해 채소류에서 고위해성 식중독균의 잠재적 위험성을 확인하였다. 본 연구의 결과는 신선 농산물의 미생물 위해 평가와 기준 설정을 위한 기초 자료로 활용될 수 있으며 신선 농산물 관련 식중독균 검출 방법의 개선과 식중독 발생 예방에 기여할 것으로 기대한다.

Social Distancing and Public Health Guidelines at Workplaces in Korea: Responses to Coronavirus Disease-19

  • Background: In the absence of a vaccine or treatment, the most pragmatic strategies against an infectious disease pandemic are extensive early detection testing and social distancing. This study aimed to summarize public and workplace responses to Coronavirus Disease-19 (COVID-19) and show how the Korean system has operated during the COVID-19 pandemic. Method: Daily briefings from the Korean Center for Disease Control and the Central Disaster Management Headquarters were assembled from January 20 to May 15, 2020. Results: By May 15, 2020, 11,018 COVID-19 cases were identified, of which 15.7% occurred in workplaces such as health-care facilities, call centers, sports clubs, coin karaoke, and nightlife destinations. When the first confirmed case was diagnosed, the Korean Center for Disease Control and Central Disaster Management Headquarters responded quickly, emphasizing early detection with numerous tests and a social distancing policy. This slowed the spread of infection without intensive containment, shut down, or mitigation interventions. After entering the public health blue alert level, a business continuity plan was distributed. After entering the orange level, the Ministry of Employment and Labor developed workplace guidelines for COVID-19 consisting of social distancing, flexible working schedules, early identification of workers with suspected infections, and disinfection of workplaces. Owing to the intensive workplace social distancing policy, workplaces remained safe with only small sporadic group infections. Conclusion: The workplace social distancing policy with timely implementation of specific guidelines was a key to preventing a large outbreak of COVID-19 in Korean workplaces. However, sporadic incidents of COVID-19 are still ongoing, and risk assessment in vulnerable workplaces should be continued.
