Background The use of acellular dermal matrix (ADM) in implant-based immediate breast reconstruction has been increasing. The current ADMs available for breast reconstruction are offered as aseptic or sterile. No published studies have compared aseptic and sterile ADM in implant-based immediate breast reconstruction. The authors performed a retrospective study to evaluate the outcomes of aseptic versus sterile ADM in implant-based immediate breast reconstruction. Methods Implant-based immediate breast reconstructions with ADM conducted between April 2013 and January 2016 were included. The patients were divided into 2 groups: the aseptic ADM (AlloDerm) group and the sterile ADM (MegaDerm) group. Archived records were reviewed for demographic data and postoperative complication types and frequencies. The complications included were infection, flap necrosis, capsular contracture, seroma, hematoma, and explantation for any cause. Results Twenty patients were reconstructed with aseptic ADM, and 68 patients with sterile ADM. Rates of infection (15.0% vs. 10.3%), flap necrosis (5.0% vs. 7.4%), capsular contracture (20.0% vs. 14.7%), seroma (10.0% vs. 14.7%), hematoma (0% vs. 1.5%), and explantation (10.0% vs. 8.8%) were not significantly different in the 2 groups. Conclusions Sterile ADM did not provide better results regarding infectious complications than aseptic ADM in implant-based immediate breast reconstruction.

Kawasaki disease (KD) is a systemic vasculitis that occurs predominantly in infants and young children. The etiology of KD is unknown and coronary heart disease is a major complication of KD. Acute scrotum is a rare complication of acute KD, and not as well recognized as other manifestations of the disease. We report a 2-month-old boy with acute scrotum in the acute phase of KD. He was treated with intravenous immunoglobulin (total 2 g/kg) and aspirin (50 mg/kg/day). The treatment was effective in resolving his fever and other clinical symptoms, but 2 days after starting treatment he experienced scrotal swelling. Scrotal ultrasound and transillumination were used in the diagnosis of acute scrotum. After 2 months, a follow-up testicular ultrasound revealed a remission of the acute scrotum. Subsequently, he has been followed up for KD.
Peritonitis is a serious problem in patients undergoing peritoneal dialysis. Rarely pneumatosis intestinalis can occur as a complication of this infectious process. Pneumatosis intestinalis is a potential life threatening condition with a challenging management. The mortality of peritoneal dialysis patients with pneumalosis intestinalis secondary to mesenteric ischemia is almost 100%. We describe a rare case of pneumatosis intestinalis in a peritoneal dialysis patient who developed Staphylococcus aureus peritonitis which was initially treated with appropriate antibiotics. Since initial response to therapy was not achieved, an abdominal computerized tomography was done which revealed a pneumatosis intestinalis. A laparotomy was performed and small bowel necrosis was seen. A segmental resection with ileostomy, jejunostomy was done. Though surgical treatment was performed, the patient died in 2 weeks after admission. Pneumocystitis intestinalis in peritoneal dialysis peritonitis is a uncommon complication which requires prompt evaluation to rule out mesenteric ischemia as it carries a high mortality and its management will be surgical.
Purpose: Intra-abdominal infection is a common postoperative complication of laparoscopic pylorus-preserving gastrectomies (PPGs). Many studies have reported that intra-abdominal infectious complications after gastrectomy adversely affect patient survival outcomes. To prevent gastric fluid leakage into the abdominal cavity, we developed a novel anastomosis method in which the stomach lumen is not opened (termed the non-opened clean end-to-end anastomosis method [NoCEAM]) and evaluated its feasibility. Materials and Methods: Subsequent to lymphadenectomy, the oral and anal resection lines were sutured using an intraoperative endoscope. After closing the stomach circumferentially with clips, the specimen was rolled outward like a "donut." We resected the specimen circumferentially using a linear stapler, and anastomosis was completed simultaneously. We examined the feasibility of this procedure ex vivo, using three porcine stomachs, and in vivo, using one pig. Subsequently, we applied the procedure to 13 consecutive patients with middle-third early gastric cancer utilizing laparotomic, laparoscopic, and robotic PPG. Results: NoCEAM was completed in all porcine models and human cases. In the human cases, the mean operation time (±standard deviation) was 279±51 minutes, and mean blood loss volume was 22±45 mL. The mean number of linear staples used was 5.06±0.76. None of the patients had complications, and all were discharged on the eighth postoperative. The serum total protein, serum albumin, and hemoglobin levels did not change significantly after surgery. Conclusions: NoCEAM is feasible and safe for performing totally laparoscopic or robotic PPG. It may reduce postoperative complications, such as intra-abdominal infections.

From August 1979 to August 1995, 73 consecutive patients with various pulmonary diseases underwent pneumonectomy Underlying diseases were lung cancer(53 cases), pulmonary tuberculosis(10 cases), bronchiectasis(4 cases) and others(6 cases). Operative mortality and complication rate for 73 patients and respiratory capacity for 53 patients at postoperative 6 months were measured, and statistically analysed for the influencing factors. The influencing factors on prognosis included age, sex, pathologic finding (benign or malignant), associated diseases, preoperative pulmonary function test and operation time. The statistically significant factors for operative mortality were preoperative MW(% prep)(P=0.013) and operation time(P=0.009). The factors influencing operative complication was infectious disease (P=0. 015), and for respiratory capacity a postoperative 6 months, preoperative FVC(%. prod) (PED.0018), FEVI(%. prod)(P=0.0024), and MW(%. prod) (P=0.004)) were statistically significant factors. The preoperative FVC(%. tyred), FEVI(% . prod) and MW(%. prod) should be measured exactly. We conclude that preoperative lung function, cardiovascular and nutritional status, postoperative care and infection prevention were important factors to decrease the operative mortality and complication as well as to increase respiratory capacity.
Also called necrobacillosis or postanginal sepsis, Lemierre's syndrome (LS) is an uncommon but potentially lethal complication of odontogenic infection. A 27-year-old male diagnosed with Ludwig's angina was transferred from a local hospital due to continuous fever and chills after incision and drainage under general anesthesia. The swelling of both submental and submandibular area subsided, but the fever and chills persisted. While generalized malaise improved, sepsis developed together with the deterioration of liver function. The chest computed tomography scan revealed multiple cavitations throughout both lungs, which were diagnosed as septic pulmonary embolism. After consulting the department of infectious diseases, the patient was treated with intravenous antibiotics focusing on vancomycin and additional antibiotics. After 3 weeks of treatment, the patient recovered completely. Despite its decreased mortality, dentists are not familiar with LS, and it is difficult to diagnose correctly. In this paper, we report a case and present a review of literature.
Sinusitis has been reported as a complication of sinus lift surgery. Obstruction of the sinus outflow tract by mucosal edema and particulate graft material may result in sinusitis. Two main surgical procedures have been proposed for the treatment of associated infectious complications; inferior meatal antrostomy and functional endoscopic sinus surgery through transnasal approach. We performed superior lateral wall antrostomy through introral approach in patient suffering from the sinusitis after sinus floor augmentation and implant installation. This procedure permitted easier access to the maxillary sinus for treat sinusitis caused by sinus lifting.

Mycoplasma pneumoniae infection is usually confined to the respiratory tract but it can cause a variety of extrapulmonary manifestations such as rashes, myalgia, hemolytic anemia, cerebral infarction, transverse myelitis, cerebellar ataxia, Guillain-Barre syndrome and meningoencephalitis. Neurologic complications of Mycoplasma pneumonia have been rarely reported until now. Cerebral infarction as a complication of mycoplasma infection in children has been very rarely reported. In our case, in a young girl with M. pneumoniae infection, a cerebral infarct resulted in persistent and significant neurological dysfunction. We report a 11-year-old girl with cerebral infarction associated with clinical and serologic evidence of Mycoplasma infection.
Hemolytic anemia due to cold agglutinin disease is a known complication of Mycoplasma pneumoniae infection but is rarely observed, particularly in children. A case of Mycoplasma pneumonia complicated with hemolytic anemia is presented. A 7 year-old girl was adimitted because of fever, cough, sputum and pale appearance. Chest X-ray showed pneumonic consolidation of Rt. upper lobe, lingular division. Laboratory studies disclosed the following values : Hb 5.3g/dL, Hct 11.1%, reticulocyte 2.9%, indirect Coombs test negative, direct Coombs test(monovalent) Anti-C3d positive, Anti-IgG negative, Anti-IgM negative, cold agglutinin titer 1 : 256, mycoplasma antibody titer 1 : 640, total bilirubin 1.0mg/dL. Initial PBS before wanning showed agglutination of red blood cells. The diagnosis of cold agglutinin hemolytic anemia complicating mycoplasma pneumonia was made. And treatment with roxithromycin, prednisolone and avoiding cold exposure was initiated, and complete recovery ensued. We report a case of cold agglutinin hemolytic anemia complicating mycoplasma pneumonia in children.

Bacille Calmette-$Gu\acute{e}rin$ (BCG) lymphadenitis is the most common complication of BCG vaccination. It commonly occurs in infants aged <6 months involving ipsilateral axillary lymph nodes. We described BCG lymphadenitis in a 22-month-old boy presenting swelling of left supraclavicular lymph node that was confirmed by real-time polymerase chain reaction (PCR) and the multiplex PCR targeting the region of difference (RD).
