Relationship between the time to positivity of blood culture and mortality according to the site of infection in sepsis

  • Objective: The time to positivity (TTP) of blood culture reflects bacterial load and has been reported to be associated with outcome in bloodstream infections. This study was performed to evaluate the relationship between the TTP of blood culture and the mortality rates associated with sepsis and septic shock according to the site of infection. Methods: We performed a retrospective cohort study on patients with sepsis and septic shock. The rates of blood culture positivity and mortality as well as the relationship between the TTP and 28-day mortality rate were compared among patients with different sites of infection, such as the lungs, abdomen, urogenital tract, and other sites. Results: A total of 2,668 patients were included, and the overall mortality rate was 21.6%. The rates of blood culture positivity and mortality were different among the different infection sites. There was no relationship between the TTP and mortality rates of total, lung, and urogenital infections. Patients with abdominal infections showed a negative correlation between the TTP and 28-day mortality rate. In patients with abdominal infections, a TTP<20 hours was independently associated with 28-day mortality compared with patients with negative blood culture (hazard ratio, 1.73; 95% confidence interval, 1.16-2.58). However, there was no difference in mortality rates of patients with a $TTP{\geq}20$ hours and a negative blood culture. Conclusion: The shorter TTP in patients with abdominal infections in sepsis and septic shock was associated with a higher 28-day mortality rate.

Analysis of Uropathogens of Febrile Urinary Tract Infection in Infant and Relationship with Vesicoureteral Reflux

  • Purpose: This study aimed to investigate the relationship between uropathogens of infants with febrile urinary tract infection (UTI) and vesicoureteral reflux (VUR). Materials and Methods: We analyzed 308 infants hospitalized for febrile UTI between January 2010 and December 2015, and assessed the voiding cystourethrography (VCUG). The medical records, including clinical symptoms, laboratory findings, urinalysis, urine culture tests, ultrasound (US), dimercaptosuccinic acid scan, and VCUG, were retrospectively obtained. The incidences of VUR and high-grade VURs (III, IV, and V) were analyzed in 4 groups categorized by uropathogens and renal US findings. Results: The mean age of 308 infants was $3.29{\pm}2.18months$. The male-to-female ratio was 3.46:1. In urine culture tests, 267 infants (86.69%) showed single bacterial uropathogen; Escherichia coli in 241 infants (78.25%) and non-E. coli uropathogens in 26 infants (8.44%). Multiple distinctive microorganisms were identified as causative uropathogens in 41 infants (13.31%). Abnormal findings of US and VCUG were identified in 216 and 64 patients, respectively. In 308 infants, the incidences of VUR and high-grade VUR were not different among the 4 groups. In 239 male infants, the incidences of high-grade VUR were higher in patients with non-E. coli single or multiple uropathogen and with abnormal US findings (p=0.042). Conclusions: In male infants with non-E. coli uropathogen or multiple uropathogens and with abnormal US findings at febrile UTI, there was an increased chance of finding high-grade VURs on subsequent VCUG tests.

Effect of 2% Chlorhexidine Bathing on the Incidence of Hospital-Acquired Infection and Multidrug-Resistant Organisms in Adult Intensive Care Unit Patients: Systematic Review and Meta-Analysis (2% 클로르헥시딘 침상목욕이 중환자실의 의료관련감염과 다제내성균 감염 발생률에 미치는 효과에 대한 체계적 문헌 고찰 및 메타분석)

  • Purpose: This systematic review and meta-analysis analyzed the effects of 2% chlorhexidine bathing on the incidence of hospital-acquired infection (HAI) and multidrug-resistant organisms (MDRO) in adult intensive care units. Methods: PubMed, CINAHL, Cochrane library, and RISS database were systematically searched, and 12 randomized studies were included in the analysis. Comprehensive Meta-Analysis version 3.0 was used to calculate the effect size using the odds ratio (OR) and a 95% confidence interval (CI). Subgroup analysis was performed according to the specific infection and intervention types. Results: In general, 2% chlorhexidine bathing has a significant effect on the incidence of HAI (OR, 0.59; 95% CI, 0.40~0.86) and MDRO (OR, 0.52; 95% CI, 0.34~0.79). Subgroup analyses show 2% chlorhexidine bathing is effective in bloodstream infections (OR, 0.51; 95% CI, 0.39~0.66) but not for urinary tract infections, ventilator-associated pneumonia infections, and Clostridium difficile infections. Moreover, 2% chlorhexidine bathing alone or its combination with other interventions has a significant effect on the incidence of HAI and MDRO (OR, 0.59; 95% CI, 0.38~0.92). Conclusion: This meta-analysis reveals that 2% chlorhexidine bathing significantly reduces the incidence of HAI and MDRO in intensive care units. The effect of 2% chlorhexidine bathing on pediatric patients or patients at general wards should be further assessed as a cost-effective intervention for infection control.

The effect of Panax notoginseng saponins on oxidative stress induced by PCV2 infection in immune cells: in vitro and in vivo studies

  • Background: Panax notoginseng saponins (PNS) are bioactive substances extracted from P. notoginseng that are widely used to treat cardiovascular and cerebrovascular diseases and interstitial diseases. PNS have the functions of scavenging free radicals, anti-inflammation, improving blood supply for tissue and so on. Objectives: The aim of this study was to investigate the effects of PNS on the oxidative stress of immune cells induced by porcine circovirus 2 (PCV2) infection in vitro and in vivo. Methods: Using an oxidative stress model of PCV2 infection in a porcine lung cell line (3D4/2 cells) and mice, the levels of nitric oxide (NO), reactive oxygen species (ROS), total glutathione (T-GSH), reduced glutathione (GSH), and oxidized glutathione (GSSG) and the activities of xanthine oxidase (XOD), myeloperoxidase (MPO) and inducible nitric oxide synthetase (iNOS) were determined to evaluate the regulatory effects of PNS on oxidative stress. Results: PNS treatment significantly reduced the levels of NO and ROS, the content of GSSG and the activities of XOD, MPO, and iNOS (p < 0.05), while significantly increasing GSH and the ratio of GSH/GSSG in infected 3D4/2 cells (p < 0.05).Similarly, in the in vivo study, PNS treatment significantly decreased the level of ROS in spleen lymphocytes of infected mice (p < 0.05), increased the levels of GSH and T-GSH (p < 0.05), significantly decreased the GSSG level (p < 0.05), and decreased the activities of XOD, MPO, and iNOS. Conclusions: PNS could regulate the oxidative stress of immune cells induced by PCV2 infection in vitro and in vivo.

Comparison of Virus Elimination Methods for Disease-free Seedlings of the Apple Dwarfing Rootstock (사과 왜성대목 무독묘 생산을 위한 바이러스 제거 방법 비교)

  • Apple (Malus domestica) is one of the most economically important fruits in Korea. But virus infection has decreased sustainable production of apple and caused the serious problems such as yield loss and poor fruit quality. Virus or viroid infection including Apple chlorotic leaf spot virus (ACLSV), Apple stem pitting virus (ASPV), Apple stem grooving virus (ASGV), Apple mosaic virus (ApMV) and Apple scar skin viroid (ASSVd) has been also reported in Korea. In many cases, apple is infected with virus and viroid with no specific symptoms, the damage caused by the virus are unaware significantly. In our research, we tried to eliminate viruses in the rootstock for the disease-free seedlings of the apple dwarfing rootstock M.9 and M.26. The method of virus elimination was meristem culture, heat($37^{\circ}C$, 6weeks) treatment and chemistry($Ribavirin^{(R)}$) treatment. The analytical methods commonly used for the detection of virus is Enzyme-linked Immuno-Sorbent Assay(ELlSA) and Reverse Transcription-polymerase Chain Reaction(RT-PCR). RT-PCR method was more 30% sensitive than ELISA method. Efficiency of method eliminate virus appeared meristem method > heat treatment > chemistry treatment. The higher acquisition rate of disease-free seedlings is 30~40% on meristem treatment. In meristem treatment, the apple dwarfing rootstock M.9 gained infection ratio of ACLSV, ASPV and ASGV were 45%, 60% and 50% respectively. In the apple dwarfing rootstock M.26, infection ratio of ACLSV, ASPV and ASGV were 40%, 55%, 55%, respectively. Based on our results, it was found that most effective method of disease-free seedlings apple dwarfing rootstocks was by meristem treatment than heat method and chemistry treatment.

Studies on Seed Transmission of Alternaria spp. in Three Cruciferous Vegetable Crops (십자화과(十字花科) 채소(菜蔬) 검은무늬병균(病菌)(Alternaria spp.)의 종자전염(種子傳染)에 관(關)한 연구(硏究))

  • Sixty one commercial seed samples of three cruciferous vegetable crops were examined for seed-borne Alternaria spp. by deep freezing blotter method. Alternaria raphani, A. brassicicola and A. brassicae were detected from 7, 6 and 1 seed samples of chinese cabbage out of 20 with average infection ratio of 3.8, 1.8 and 0.5, respectively. A. raphani and A. brassicae were detected from 11 and 2 seed samples of radish out of 38 with average infection ratio of 7.2 and 1.0, respectively. A. brassicicola was detected from 3 samples of cabbage seed with average infection ratio of 21.8. Most of the Alternaria spp. detected were recovered from the seed coat and caused seed rot and seedling infections. A. brassicicola was recovered from the embryos of radish, chinese cabbage and cabbage in low infections and A. raphani was recovered from the embryos of radish. In inoculation experiments, A. brassicicola, A. brassicae and A. raphani produced black leaf spots on old leaves of three creciferous vegetable crops and the spots gradually increased in size appearing gray to light brown lesions which later coalesce and caused defoliation.

A Study on the Change of the IEL Number in the Intestine of Rats Infected with Echinostoma hotense (Echinostoma hortense를 실험 감염시킨 흰쥐 장상피에서 림프구 (IEL)증감에 관한 연구)

  • The relationship between the intestinal histopathology and number and position of intraepithelial lymphocytes (IEL) was observed chronologically in the small intestine of rats experimentally infected with Echinostoma hortense. Sprague-Dawley rats were orally infected with 200 metacercariae obtained from Misgurnus anguillicaudatus. The rats each were sacrificed on the week 1, 2, 4, 6, 8 post-infection (PI) and samples of the intestine in the part of duodenum and jejunum were taken. The samples were stained with Hematoxylin-eosin and Giemsa. The intestinal histopathology was the severest after the week 1 PI and characterized by villous atrophy, crypt hyperplasia and decrease of villus/crypt(v/c) ratio, which continued until the week 8 Pl. The number of IEL dramatically decreased during the week 1 PI, but increased gradually thereafter with a slight decrease on the week 8 PI. In control rats, the great majority of the IEL were located at the basal region of the epithelium. During the early stage of infection, however, we found a considerable proportion of IEL to moved to the intermediate or apical regions of the epithelium. From the above results, it is sugested that the change of IEL number and position during the course of E. hortense infection should be closely related to the progression and recovery of the intestinal histopathology.

Seroepidemiologic Study of Hepatitis B Virus (HBV) and Hepatitis C Virus (HCV) Infection among Koreans in Rural Area, Korea (농촌지역의 B형, C형 간염에 관한 혈청역학적 연구)

  • To estimate the prevalence of hepatitis B virus (HBV) and hepatitis C virus (HCV) infection and to determine associated risk factors, a population-based seroepidemiologic study was carried out. In 1993, a health examination survey of the population was carried out in rural area known to have a high incidence of liver cancer. The study population were those who volunteered to participate in a health survey over 10 years of age. Examinees were interviewed by specially trained staffs. Sera from 1,033 study subjects were tested for hepatitis B surface antigen (HBsAg) by .everse passive hemagglutinin (RPHA) estimation and for hepatitis C virus antibody (anti-HCV) by 2nd generation passive hemagglutinin (PHA) estimation. The age and sex standardized prevalence of HBsAg was 6.3% which was similar to national average, but that of anti-HCV was 5.1% which was 4 to 5 times higher than that of blood donors or other health examinees in Korea. In a multivariate analysis, transfusion history, surgical operative history, and acupuncture history were not associated with HBsAg positivity. In contrast, acupuncture history (adjusted odds ratio[OR]=2.2 : 95% Confidence interval[CI] 1.0-4.7) and surgical operative history(adjusted OR=2.0 : 95% CI 1.0-4.1) were associated with anti-HCV positivity. The present study suggest that there is an highly endemic area of HCV infection in Korea and probably this endemicity is associated with a parenteral source of HCV infection other than blood transfusion.

Pathologic condition of the stranded Manila clam Ruditapes philippinarum by storm-originated sediment erosion in spring 2007 in Incheon Bay off the west coast of Korea (2007년 봄 인천만 갯벌에 있어 폭풍으로 인한 갯벌의 침식작용에 의해 대기 중으로 노출된 바지락 (Ruditapes philippinarum)의 조직병리학적 특성에 관한 연구)

  • In Incheon bay, mass mortalities of Manila clam associated with winter storms have been reported. In the present study we have monitored pathologic condition of the clams stranded on the tidal flats by the winter storms occurred in late March to early April in 2007. The field surveyed indicated that mortality of the Manila clam in the study areas ranged 10-15%. Condition index, a ratio of tissue weight to the shell weight, of the stranded clams was significantly lower than the non-stranded normal clams collected from the same locations (p < 0.05), indicating that the stranded clams were comparatively in poor physiological condition. Perkinsus olseni, the protozoan parasite was observed most of clams used in the analysis and the infection prevalence ranged 77-90%. The infection intensity of P. olseni determined using Ray's fluid thioglycollate medium (RFTM) cultivation and the 2M NaOH digestion assay indicated that the clams collected during late March and early April in 2007 involved 67,182-1,124,727 P. olseni cells/g tissue. The infection intensity of clams from Gung-Pyeung was significantly higher than the intensities observed from Dae-Bu and Young-Heung (p < 0.05). No clear correlation was found between the infection intensities of P. olseni in the non-stranded normal clams and the stranded clams. The stranded Manila clams were also infected with trematode parasite with the prevalence ranged 5 (Young-Heung) to 12.5% (Dae-Bu). The trematode-infected clams exhibited castrated follicles in the gonad, a typical sign of trematode infection. It was believed that mass mortality of Manila clam observed in this study was associated with the poor physiological condition as indicated by CI, although impacts of the parasite infection cannot be ruled out.

Weather Effect and Response of Promoted Rice Varieties on Fusarium Infection in Paddy Field (벼 붉은곰팡이병 감염에 대한 기상조건의 영향과 장려품종의 반응)

  • Fusarium infection rate of the paddy rice grain after harvest seemed to be influenced by the average temperature from late July (before heading) to the end of September (during ripening). In case of 2010 and 2013 in which average temperature of the same period was similar, Fusarium infection was related to cumulative precipitation, cumulative precipitation days, and precipitation durations over two days. The distribution ratio of Fusarium species complex isolated from paddy rice grains after harvest was 57% in 2010 and 45% in 2013 for Fusarium graminearum species complex (FGSC), 35% and 50% for Fusarium incarnatum-equiseti species complex, and 8% and 5% for Fusarium fujikuroi species complex (FFSC). The distribution ratios of FGSC and FFSC were higher in 2010 than 2013. Among the total 26 promoted rice varieties, the 'Mihyang' showed resistant response against the natural infection with Fusarium species belonging to FGSC and the varieties of 'Nampyeong', 'Hi-ami'and 'Younghojinmi' showed resistant response against the natural infection with overall Fusarium pathogens. Majority of the promoted rice varieties could not be classified for resistance or susceptibility. These results are valuable as basic data to determine the resistance and susceptibility of rice variety against Fusarium spp. infection in the field.
