ESTIMATING THE NUMBER OF ICU PATIENTS OF COVID-19 BY USING A SIMPLE MATHEMATICAL MODEL

  • Predicting the number of ICU patients holds significant importance, serving as a critical aspect in efficiently allocating resources, ensuring high-quality care for critically ill individuals, and implementing effective public health strategies to mitigate the impact of diseases. This research focuses on estimating ICU patient numbers through the development of a simple mathematical model. Utilizing data on confirmed COVID-19 cases and deaths, this model becomes a valuable tool for predicting and managing ICU resource requirements during the ongoing pandemic. By incorporating historical data on infected individuals and fatalities from previous weeks, we establish a straightforward equation. We found the substantial impact of the delay in infected individuals, particularly those occurring more than five weeks earlier, on the accuracy of ICU predictions. Proactively preparing for potential surges in severe cases becomes feasible by forecasting the demand for intensive care beds, ultimately improving patient outcomes and preventing excessive strain on medical facilities.

전신질환자에서 과도한 감염치아 발치시 스트레스 감소법 : 문헌적 고찰 및 증례보고 (STRESS REDUCTION PROTOCOL FOR PROPER EXTRACTION OF ADVANCED INFECTED TEETH IN MEDICALLY COMPROMISED PATIENTS : REVIEW OF LITERATURE & REPORT OF CASES)

  • Common dental procedures(dental extraction & minor operation) are potentially stress-inducing in many patients, especially medically compromised patients. The body's response to dental stress involves the cardiovascular system(an increase in cardiovascular workload), the respiratory organ and the endocrine system(change in metabolism). To minimize the stress to the medical risk patient, the stress reduction protocol was established. The obtained contents were as follows: (1) Recognize the patient's degree of medical risk (2) Complete medical consultation before dental therapy (3) Schedule the patient's appointment in the morning (4) Monitor and record preoperative and postoperative vital signs (5) Use psychosedation during therapy (6) Use adequate pain control during therapy (7) Short length of appointment : do not exceed the patient's limits of tolerance (8) Follow up with postoperative pain/anxiety control (9) Telephone the higher medical risk patient later on the same day that treatment was given Though the stress reduction protocol above was applied to the dental extraction in medically compromised patients with the advanced infected teeth, the final responsibility for the complications(syncope, bleeding & infection, etc.) in a patient rests with the dentist who ultimately treats him. For the prevention of postextraction complications & poor prognosis, the authors treated the advanced infected teeth with the pulp extirpation, opening drainage through the canal and complete occlusal reduction. The final extraction and wound closure were then done after $1{\sim}2$ weeks. The final prognosis was comfortable without common complications.

The authors used the stress reduction protocol in the care of local anesthesia infected teeth in medically compromised patients. The final prognosis was comfortable without any complications.

  • 이식한 자가혈관이나 인조혈관의 감염은 반복적인 파열 및 폐혈증으로 환자의 생명까지도 위태롭게 하는 심각한 질병이다. 따라서 대퇴동맥을 포함한 서혜부의 감염의 치료에 있어 제일 중요한 원칙은 감염부위의 광범위한 절제와 감염부위와 격리된 동맥재건술이다. 본원에서는 ECMO를 시행했던 환자에서 발생한 대퇴동맥 파열과 주변부의 감염에 대하여, 위의 치료원칙을 제일 잘 반영할 수 있는 수술방법인 폐쇄공 우회술을 성공적으로 시행하였기에 보고하는 바이다.

  • Surgical correction of patent ductus arteriosus is,under most circumstances,highly successful and carries a low mortality. But infected PDA is yet potentially dangerous due to its frequent recurrence and resistant organisms to antibiotics. And,in surgical correction,surgeon may face the possibility of tearing of ductus arteriosus arterial end due to friability and adhesion of its surrounding tissue.This report demonstrats another problem in treatment of infected patent ductus arteriosus.This thirteen years old female patient received susceptible combined antibiotics intravenously from the day of admission and remitted from 4th.week of therapy.This remission state continued for 12days without relapse.But the pulmonary artery ruptured in this remission period.In autopsy,bacteria was not found in ductal vegetation.Also,there was no pulmonary artery aneurysm,Our experience show that in infected PDA,pulmonary artery can rupture spontaneously during remission period without aneurysmal formation.

  • Deep neck infection (DNI) is a potentially serious pathology that can lead to serious complications and high rate of mortality. Since DNI affects the cervical spaces, which can compromise airway, aggressive antibiotic administration and prompt surgical drainage are essential for recovery. Although most causes of DNI are known to be dental infections, developmental cysts such as dentigerous cysts are relatively few causes. In this case, we report a rare patient with severe deep neck space infection caused by infected third molar with dentigerous cyst.

Detection of Enteropathogens in Human Immunodeficiency Virus and Non-Human Immunodeficiency Virus-Infected Children with Acute Diarrhea in an Indonesian Tertiary Hospital Using Multiplex Real-Time Polymerase Chain Reaction

  • Purpose: Diarrhea is one of the leading causes of mortality in children living in developing countries. The etiology of acute diarrhea in each healthcare center varies depending on place, time, and population. This study aimed to identify pathogen patterns in human immunodeficiency virus (HIV)-infected and non-HIV children suffering from acute diarrhea, using multiplex real time reverse transcriptase polymerase chain reaction (RT-PCR), in an Indonesian tertiary hospital. Methods: This cross-sectional study was conducted at Dr. Cipto Mangunkusumo National Hospital from March 2019 to April 2020. Results: The study showed that multiplex RT-PCR results were positive in 58.9% of the specimens, with more positive results in HIV-infected children than in non-HIV-infected children (70% vs. 54.7%). Altogether 72 enteropathogens were detected from all specimens. Enteropathogens in non-HIV children with acute diarrhea consisted of bacteria (70.6%) and viruses (29.4%) with a predominance of enteroaggregative Escherichia coli (25.4%), followed by Campylobacter spp. (11.8%), enteropathogenic E. coli (9.8%), Norovirus GII (7.8%), and Clostridium difficile (7.8%). Enteropathogens in HIV-infected children consisted of viruses (57.1%), bacteria (28.6%), and parasites (14.3%) comprising Norovirus GII (24%), Cryptosporidium spp. (14.3%), Campylobacter spp. (14.3%), Norovirus GI (14.3%), and Astrovirus (14.3%). Cryptosporidium spp. was the only parasite found in this study and was found only in HIV-infected children. In non-HIV children with acute diarrhea, most pathogens were invasive bacteria, while in HIV-infected children, more viral and parasite infections occurred, primarily caused by opportunistic pathogens. Conclusion: The pattern of enteropathogens can help clinicians determine further examinations and appropriate empirical antimicrobial therapy for the patient.

  • 심박동기는 동기능 부전 증후군에서부터 완전 방실차단까지 서맥의 치료에 가장 보편적으로 사용되는 중재적 치료다. 일반적으로 경정맥을 통한 심내 심박동기를 많이 사용하고 있으나 감염 및 혈전증, 박동기의 기능부전, 부정맥, 심근천공, 삼첨판 폐쇄부전증 등의 합병증이 발생할 수 있다. 심박동기의 감염은 전신감염으로 이행되어 환자 상태가 빠르게 악화될 수 있으므로 감염으로 의심되면 제거하는 것이 원칙이다. 추나 올가미 등을 이용한 방법등 비수술적인 방법외에도 체외순환을 이용한 적극적인 제거도 환자 상태에 따라 고려해야 할 것이다. 감염된 심박동기를 체외순환을 이용하여 제거한 두 개의 증례를 보고하는 바이다.

  • Prosthetic valve endocarditis with aortic root abscess is a serious condition requiring urgent surgical intervention. We present a case caused by an infected Bentall mechanical valve conduit after cardiac surgery in a patient who was referred for a suprasternal pulsatile mass. The patient also had 1 episode of sentinel haemorrhage.

Non-infected and Infected Bronchogenic Cyst: The Correlation of Image Findings with Cyst Content

  • We hereby report a case on bronchogenic cyst which is initially non-infected, then becomes infected after bronchoscopic ultrasound (US)-guided transesophageal fine-needle aspiration (FNA). The non-infected bronchogenic cyst appears to be filled with relatively echogenic materials on US, and the aspirate is a whitish jelly-like fluid. Upon contrast-enhanced MRI of the infected bronchogenic cyst, a T1-weighted image shows low signal intensity and a T2-weighted image shows high signal intensity, with no enhancements of the cyst contents, but enhancements of the thickened cystic wall. The patient then undergo video-assisted thoracic surgery 14 days after the FNA. The cystic mass is known to be completely removed, and the aspirate is yellowish and purulent. To understand the image findings that pertain to the gross appearance of the cyst contents will help to diagnose bronchogenic cysts in the future.
