1인 구조자 영아심폐소생술시 세 가지 가슴압박 방법의 가슴압박 효율성 비교 : 해양경찰교육원 신임경찰 교육생 대상으로 (Chest compression efficiency for three methods of single-person rescuer infant cardiopulmonary resuscitation)

  • Purpose: We compared three chest compression methods to find an efficient method for performing infant cardiopulmonary resuscitation (CPR) for single rescuers and improving chest compressions quality. Methods: Thirty new marine police trainees at the Korea Coast Guard Education Institute were tested for five sessions using three methods of single rescuer infant CPR: two-finger chest compression, two-thumb encircling chest compression, and two-finger support chest compression. Results: The depth, accuracy, and number of compressions per minute of resuscitation were analyzed for the above three methods. The depth of two-finger chest compression, two-thumb encircling chest compression, and two-finger support chest compression was 3.53±0.20cm, 4.10±0.13cm, and 4.22±0.15cm, respectively. Accuracy was 23.86±12.59%, 54.11±10.8%, 71.55±18.81%, respectively, while the time for one cycle of 30 chest compression was 16.01±10.5 seconds, 16.45±0.85 seconds, and 16.56±0.91 seconds, respectively. Chest compression interruptions were 6.59±0.78 seconds, 7.17±0.37 seconds, and 6.97±0.35 seconds, respectively. The interruptions were consistent with the range of 5-10 seconds suggested by the American Heart Association. Conclusion: When one rescuer performs CPR for an infant in cardiac arrest, a comparative analysis of three methods showed that two-thumb encircling chest compression is the best for accuracy and efficiency of chest compressions.

Comparison of New Infant Chest Compression Methods: Simulation Study on Randomization Using Manikin

  • In this paper, we propose a the effects of new chest compressions in the implementation of chest compressions during infant cardiopulmonary resuscitation, and to provide basic data for high-quality CPR. On March 12, 2019, the research target used the SPSS 22.0 Version as an experimental study using randomized cross-design of 30 emergency medical services students who completed BLS Health Care-provider. The study also showed significant differences in chest depth and average rate of pressure($34.61{\pm}1.29$, $39.40{\pm}1.08$, <0.001, $105.46{\pm}4.23$, $107{\pm}3.84$, <0.001) depending on chest compressions. There was also a significant difference in the convenience and degree of pain of chest compressions(<0.001). In addition, new chest compressions appeared close to vertical and showed statistically significant differences(p<.001). Based on the results of this study, we can see that the accuracy of the new chest compressions during infant cardiopulmonary resuscitation is increased, and the depth of chest compressions is improved, improving the quality index of chest compressions. However, it will be necessary to further study the use of the new chest compressions to identify the potential for clinical use.

1인 구조자 영아 심폐소생술 가슴압박 방법에 따른 효율성 및 편리성 비교 (Comparing the efficiency and convenience of one-rescuer cardiopulmonary resuscitation chest compression techniques for infants)

  • Purpose: This study aimed to propose an effective one-rescuer infant cardiopulmonary resuscitation (CPR) chest compression technique by comparing the differences in efficacy, convenience, and pain levels between the two thumb-encircling and two finger techniques. Methods: Subjects were randomized to perform either two-thumb-encircling or two-finger technique for 8 minutes each on infant CPR manikins. After the chest compression, a survey was administered to the subjects to measure convenience and pain levels according to compression method. Results: Total compression depth over 8 minutes was significantly deeper for the two-thum-encircling technique ($43.5{\pm}4.8mm$) compared with the two-finger technique ($32.6{\pm}5.4mm$) (p<0.001). In terms of compression depth measured at 1-minute intervals, compression depth with the two-finger technique decreased from $38.3{\pm}4.23mm$ to $29.0{\pm}6.79mm$, whereas compression depth with the two-thumb-encircling technique did not show a significant change (from $43.7{\pm}4.12mm$ to $43.4{\pm}5mm$). The results of the survey indicated that, the majority of subjects found the two-thumb-encircling technique to be the most comfortable technique for compression depth (n=29, 64.4%). The majority of subjects (n=31, 68.9%) answered that the two-finger technique resulted in the most pain. Conclusion: Comparison of efficacy of the two-thumb-encircling and two-finger-techniques in the performance of one-rescuer infant CPR revealed that the two-thumb-encircling technique was more effective in maintaining chest compression depth.

영아 심폐소생술시 지지대를 사용한 두 손가락 가슴 압박의 효과 (The Effect of Two Fingers-Chest Compression Using the Supporting Instrument During Infant Cardiopulmonary Resuscitation)

  • 영아 심폐소생술 1인 수행시 효과적인 가슴 압박 방법을 모색하기 위해 시뮬레이션을 활용하여 두 손가락에 지지대를 사용하기 전 후의 가슴 압박 효과를 측정하였다. 여자 대학생 50명을 대상으로 실험을 실시하고 paired t-test, 카이제곱 검정 및 Fisher's exact test를 이용하여 결과를 분석하였다. 가슴 압박의 깊이는 두 손가락에 지지대를 사용하기 전 $2.50{\pm}0.59cm$에 비해 사용 후 $3.73{\pm}0.33cm$로 유의하게 개선되었다(p < 0.001). 두 손가락 지지대 사용은 대상자의 가슴 압박에 대한 자신감과 효과를 유의하게 증가시켰다(p < 0.001). 결론적으로 영아 심폐소생술 1인 수행시 두 손가락 지지대 사용은 가슴 압박의 깊이와 자신감 그리고 효과를 유의하게 증가시켰다. 따라서 지지대를 이용한 두 손가락 압박법은 영아 심폐소생술 1인 수행시 유용할 것으로 판단된다.

  • 심정지(Cardiac arrest)는 원인과 관계없이 심장의 박동이 정지되어 발생하는 일련의 상태를 말한다. 심정지 발생 시 환자의 생명을 구하기 위한 유일한방법 중 하나는 심폐소생술이며 이 술기를 통하여 순환을 유지 시킬 수 있고, 고품질의 심폐소생술은 환자의 생존률과 신경학적 예후에 영향을 미치기 때문에 매우 중요한 술기이다. 영아 심폐소생술의 경우 두 손가락으로 가슴을 압박하는 방법을 사용한다. 하지만 이 방법은 해부학적으로 손가락의 피로도가 가중되고 수직압박이 힘들어 미국심장협회에서 권장한 가슴압박깊이에 도달하기 힘들 수 있다. 이 연구는 영아 심폐소생술 중 가슴압박 시행 시 새로운 가슴압박법의 효과를 검증하고, 고품질의 심폐소생술을 위한 기초자료를 제공하고자 한다. 연구결과 가슴압박 방법에 따라 가슴의 평균압박깊이 및 평균압박 속도가 유의한 차이가 있었다(p<0.001) 또한 가슴압박의 편리성 및 통증정도에서 유의한 차이를 보였다. 본 연구의 결과를 종합해 볼 때 영아 심폐소생술 중 새로운 가슴압박법 시행 시 정확도가 높아지고, 가슴압박 깊이가 나아져 가슴압박의 질적 지표가 개선되었음을 알 수 있었다.

최초 반응자를 대상으로 한 심폐소생술 교육결과의 분석 (Analysis on the efficacy of CPR training for first responders)

  • This survey is mainly focus on identifying the efficacy of CPR training for 92 first responders who participated in the first aid and emergency rescue program dated on November 22 and November 24, 2000. First responders are trained and evaluated using AHA guideline for 4 hours. Data are analyzed by Percentage, Mean, T-test, F-test using SPSS program. The results for this study are as follows: 1. An average score for CPR evaluation is high as 1.71 of total score 2.0. 2. Between adult CPR and infant CPR, an average score for adult CPR is higher than infant CPR. The highest item of adult CPR and infant CPR is checking for unresponse(1.95 and 1.93), the lowest items are checking for circulation and chest compression position in the adult CPR and location of chest compression in the infant CPR 3. There are statistically significant differences in the evaluation score of CPR according to age, education level, occupation, job experience and emergency accident situation in the past.

가슴압박 방법에 따른 소아 심폐소생술의 효율성 비교 (A comparative analysis of the efficacy of tiny child cardiopulmonary resuscitation (CPR) by chest compression methods)

  • Purpose: This study aimed to compare the effectiveness of one-hand compression method (one-hand) and two-thumb compression methods (two-thumb) using a 3-year-old pediatric mannequin. Methods: Participants in the experiment were randomly assigned to 21 people with one hand and 21 people with two thumbs, who had completed a basic life support training course. The mannequin used a few junior QCPR manikis (Laerdal, Norway), based on an age of 3. The study followed the guidelines for two-rescuer pediatric CPR, performing chest compressions and ventilations in a 15:2 ratio for 5 cycles, 10 minutes. The chest compression number, depth, speed rate, and recoil rate were measured after 1, 3, and 5 cycles, respectively. Data were analyzed using SPSS 23.0. Results: Comparing the efficiency of chest compressions performed during 1, 3, and 5 cycles, the depth of chest compressions was 40.84±3.10 mm for the two thumbs and 51.48±4.79 mm for the one-hand, which was significantly different (p<.001). According to pediatric CPR guidelines, the frequency corresponding to the compression depth range of 40.00-50.00 mm was statistically higher at 57.1% (12 people) of two thumb than at 23.8% (5 people) of one hand (p<.001). Conclusion: While the current guidelines recommend one- or two-thumb encircling hand compressions for larger children, the use of one- or two-handed wrapped thumb compressions should be considered for smaller children, depending on the rescuer's judgment, when distinguishing between a child and an infant is difficult.

  • The study is designed to develop an educational CD-Title for Basic Life Support. The study is conducted from July to December, 2002, Based on the course of program development suggested by Dick and Cray. the study followed the planning, development, education and evaluation of a program. The developed CD-Title consists of 8 parts. 1. Adult Foreign-Body Airway Obstruction include assessment responsiveness, activating EMS system, position victim, opening a airway, checking for breathing, 2 rescue breathing, reopening the airway, 2 rescue breathing, finger sweep, 5 abdominal thrusts, unresponsiveness adult Foreign-Body Airway Obstruction video. 6. Infant Foreign-Body Airway Obstruction consist of, causes, assessment responsiveness, position victim, opening the airway, checking for breathing, 2 rescue breathing, reopening the airway, 2 rescue breathing, checking foreign-body object in oral cavity of victim, 5 back blow, 5 chest thsusts, activating EMS system, video. 7. 32 exercise consist of 8 Adult CPR, 12 Child & Infant CPR, 5 Adult Foreign-Body Airway Obstruction, 7 Infant Foreign-Body Airway Obstruction. Comparing the Effectiveness Between Typical Infant CPR method and Over-head CPR method : A Study of the Single-Person Rescuer Simulation Using a Manikin

  • 본 연구는 1인 구조자 영아 심폐소생술 방법에 따른 효율성을 알아보기 위한 연구이다. 일반인 51명을 대상으로 일반적인 두 손가락을 이용한 방법과 새로운 방법인 머리 위 양손 감싼 두 엄지 가슴압박 방법으로 심폐소생술을 비교하였다. 두 심폐소생술 방법을 비교하기 위하여 SPSS 22.0을 이용하였다. 연구결과 머리 위 양손 감싼 두 엄지 가슴압박법이 평균 가슴압박의 깊이(39.38±1.07 mm)가 유의하게 깊게 나타났고(p<0.001), 인공호흡의 용이성(p<0.001), 방법의 편리성( p<0.001), 손가락 통증 정도(p<0.001)도 유의한 차이를 보였다. 그리고 심폐소생술 방법의 선호도는 80.4%(41명)가 머리 위 양손 감싼 두 엄지 가슴압박 방법을 이용한 심폐소생술을 선호하였다. 이 연구는 마네킨을 이용한 가상연구이기 때문에 향후 질 높은 심폐소생술 방법을 현장에 적용하기 위해 추가적인 연구가 필요할 것이다.

  • Insertion of an epicardial pacemaker is a useful treatment for pediatric patients with an abnormal heart rhythm. However, there are limitations and concerns when implanting epicardial pacemakers in infants and neonates due to their small body size. We report a patient who experienced rare complications after implantation of a permanent pacemaker.
