탄력밴드 운동이 노인의 신체조성과 체력에 미치는 지속적 효과 (The Effect of Elastic Band Exercise Training and Detraining on Body Composition and Fitness in the Elder)

  • 노화가 진행됨에 따라 근육은 감소 현상(sarcopenia)을 나타낸다. 근육의 감소는 노인의 의료비 증가와 직접적인 관련성은 없어 보이지만, 근육의 감소가 근력의 감소로 연결되어 체력의 약화, 활동성의 감소, 낙상의 증가 등과 같은 독립적인 생활을 감소시킨다. 이는 신체장애로 연결되며, 또다시 당뇨, 비만, 고지혈증, 고혈압 등과 같은 만성퇴행성 질환으로 연결된다. 결국, 근감소증은 사망률을 상승시키는 잠재 위험요인이 된다. 본 연구는 탄력밴드 운동을 통한 근감소증 예방과 관련된 변인인 신체조성 및 체력의 향상된 변화와 더불어 운동 중단에 따른 운동효과의 상쇄(相殺) 정도를 살펴보는데 있다. 본 연구의 피검자는 S시 J구 J노인복지관 운동프로그램에 참가하는 60-70대의 노인 14명으로 선정하였다. 12주 동안의 탄력밴드 운동은 주2회의 빈도로 실시하였다. 측정시기는 12주간의 통제전, 통제후(운동전), 12주간의 운동 후(운동중단 전), 12주간의 운동중단 후로 신체조성 과 체력 변인을 측정하였다. 12주간의 통제전·후 신체조성과 체력 변인에는 통계적인 유의차가 나타나지 않았으나, 12주간의 운동전·후 신체조성의 체중(t=2.978, p=0.001), 체질량지수(t=3.502, p=0.004), 체지방율(t=2.216, p=0.045), 근육량(t=-3.837, p=0.002), 내장지방면적(t=5.186, p<0.001), 허리-엉덩이 둘레비(t=3.045, p=0.009) 모든 변인에서, 체력의 2분 제자리 걷기(t=-6.891 p<0.001), 덤벨들기(t=-4.702, p<0.001), 의자에서 일어섰다 앉기(t=-4.860, p<0.001), 의자앉아 앞으로 굽히기(t=-5.910, p<0.001), 등 뒤에서 손잡기(t=-3.835, p=0.002), 244cm 왕복 걷기(t=7.560, p<0.001)의 모든 변인에서 운동의 효과를 나타내었고, 그 효과가 12주간의 운동중단 후에도 신체조성의 체중(t=2.323, p=0.037), 체질량지수(t=2.503, p=0.026), 근육량(t=-3.137, p=0.008) 변인에서, 체력의 2분 제자리 걷기(t=-6.489 p<0.001), 의자에서 일어섰다 앉기(t=-4.694, p<0.001), 의자앉아 앞으로 굽히기(t=-3.690, p=0.003), 244cm 왕복 걷기(t=7.539, p<0.001)의 변인에서 그 유지가 지속되었다. 노인에게 있어서 탄력밴드 운동은 신체조성 및 체력에 긍정적인 영향을 나타내며, 그 운동의 효과가 12주가 경과되어도 유지되고 있음을 확인할 수 있었다.

Effects of Obesity on Presentation of Breast Cancer, Lymph Node Metastasis and Patient Survival: A Retrospective Review

  • Background: As data on the relation between obesity and lymph node ratio are missing in the literature, we here aimed to assess the impact of obesity on this parameter and other clinicopathological features of breast cancer cases and patient survival. Materials and Methods: Medical data of 646 patients, all referred to two centers in Tehran, Iran, were reviewed. Factors that showed significant association on univariate analysis were entered in a regression model. Kaplan-Meier and Cox-regression were employed for survival analysis. Results: Obesity was correlated with the expression of estrogen and progesterone receptor (p=0.004 and p=0.039, respectively), metastasis to axillary lymph nodes (p=0.017), higher lymph node rate (p<0.001) and larger tumor size (p<0.001). The effect of obesity was stronger in premenopausal women. There was no association between obesity and expression of human epidermal growth factor receptor. Three factors showed independent association with BMI on multivariate analysis; tumor size, estrogen receptor and lymph node ratio. Obesity was predictive of shorter disease-free survival with a hazard ratio of 3.324 (95%CI: 1.225-9.017) after controlling for the above-mentioned variables. Conclusions: The findings of this study support the idea that obese women experience more advanced disease with higher axillary lymph node ratio, and therefore higher stage at the time of diagnosis. Furthermore, obesity was associated with poorer survival independent of lymph node rate.

체중 및 건강행태변화가 혈압 및 콜레스테롤에 미치는 영향 - 철강제조업체 근로자를 대상으로 한 3년 추구연구 - (Effects of change in Obestiy and Life Style Factors on Blood Pressure and Serum Cholesterol - 3-year Follow-up among Workers in a Steel Manufacturing Industry -)

  • Objectives: We investigated the effects of changes in obesity and life style factors, such as cigarette smoking, alcohol drinking, and exercise, on the changes in blood pressure and serum cholesterol among Korean men. Methods: This study included 7,205 healthy male employees in the steel manufacturing industry. Each subject underwent health examination in 1994 and was re-examined in 1997. The study subjects were classified into four categories, according to changes in body mass index (BMI) (loss; stable; mild gain: severe gain), cigarette smoking (quitter; nor-smoker; smoker continued; smoker started), alcohol drinking (quitter; non-drinker; drinker continued; drinker started) and exercise (more exercise; continuous regular exercise; continuous irregular or no exercise; less exercise), respectively. We evaluated the relationship between the categories of change in those independent variables and the changes in blood pressure and serum cholesterol, adjusted for BMI in 1994 and age by analysis of variance. Results: The change in systolic blood pressure was positively associated with the changes in BMI (p<0.001) and drinking (p=0,001), but negatively with smoking (p=0,004), compared to the first category of each independent variables. The systolic blood pressure was significantly less increased in the continuous smoking group than quitter or hon-smoker. The changes in diastolic blood pressure and serum cholesterol appeared to have statistically significant linear relationships only with the change in BMI. The change in exercise showed a marginal significance with diastolic blood pressure (p=0.088). Conclusions: These prospective data emphasize the importance of obesity as a determinant of the changes in blood pressure and serum cholesterol. In addition, the changes in smoking and drinking habits can affect systolic blood pressure.

The Relative Risk Assessment of Leptin for Stroke in Korea

  • Leptin has a close correlation with obesity, which is known to be a major factor for stroke. This study was performed to determine whether serum leptin level would be an independent risk factor for stroke and whether it would change significantly early after stroke. Subjects were selected from those within I month after onset and non-stroke referents at Kyung Hee Oriental Medical Center in Seoul, Korea. We compared leptin and the other characteristics between stroke subjects and referents. Body mass index, hypertension history, presence of drinking and smoking, waist/hip ratio, total cholesterol and triglyceride were recorded. To assess odds ratio of leptin for stroke, we used logistic regression analysis. Leptin was rechecked 2 weeks later and compared with the former value in acute stroke subjects. In this study, serum leptin did not differ significantly between stroke subjects and referents, and its odds ratio was not significant in male (OR=0.52, 95% Cl; 0.13-2.08) and female (OR=1.57, 95% Cl; 0.53-4.67). In acute stroke subjects, leptin did not change significantly 2 weeks later. Hypertension had a significant odds ratio in male (OR=3.39, 95% Cl; 1.02-11.24) and female (OR=12.37, 95% Cl; 3.67-41.65). High waist/hip ratio was only in female (OR=6.70, 95% Cl; 1.73-26.02). In conclusion, leptin was not an independent risk factor for stroke and its serum level did not change significantly early after stroke. Hypertension and waist/hip ratio had significant relative risks.

Utility of Peripheral Blood Parameters in Predicting Breast Cancer Risk

  • Purpose: We aimed to study the inflammatory parameters of complete blood count in breast cancer cases. Materials and Methods: This retrospective study covered 178 breast cancer patients and 107 age and body mass index matched healthy women. Complete blood count parameters, neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR) and MPV/platelet were analyzed. Results: The leukocyte, neutrophil and neutrophil/lymphocyte ratio were higher in the patient group (p values 0.001, 0.0001 and 0.0001, respectively) while haemoglobin and hematocrit were higher in the control group (p=0.0001 for both). Logistic regression analysis showed that elevated neutrophils and platelet distribution width (PDW) (OR: 0.627, 95%CI: 0.508-0.774, p=0.001 and OR: 1.191 95%CI: 1.057-1.342 p=0.003) were independent variables for predicting breast cancer. The cut-off value for the neutrophil/lymphocyte ratio was 2.56. Conclusions: According to our study results, neutrophil levels as part of complete blood count may be used as an independent predictor of breast cancer risk.

Vitamin D deficiency is an independent risk factor for cardiovascular disease in Koreans aged ${\geq}50$ years: results from the Korean National Health and Nutrition Examination Survey

  • Vitamin D deficiency is a risk factor for metabolic syndromes. We examined whether vitamin D deficiency altered the prevalence of cardiovascular disease (CVD) in older Koreans. Cross-sectional analysis of data from the Korean National Health and Nutrition Examination Survey IV 2008-2009 was used to examine the association between serum 25-hydroxyvitamin D (25(OH)D) levels and the prevalence of CVD in a representative population-based sample of 5,559 men and women aged ${\geq}50$ years. CVD was defined as angina pectoris, myocardial infarction, or stroke. The prevalence of CVD (7.0%) in the older Korean population was lower than that in the older US population, although average serum 25(OH)D levels were much lower in the Korean population. Additionally, serum 25(OH)D levels did not differ significantly between the CVD and non-CVD groups. However, subjects in the lowest category (< 25 nmol/l) of serum 25(OH)D level had the greatest prevalence of CVD, about two-fold higher than subjects in the highest category (> 75 nmol/l), after adjusting for age, gender, body mass index, education level, residence location, and region. The prevalence of other risk factors for CVD, including higher waist circumference, fasting glucose, low-density lipoprotein (LDL) cholesterol, and triglyceride levels and lower high-density lipoprotein (HDL) cholesterol levels, was also higher among subjects in the lowest category than among those in the highest category. In conclusion, low serum 25(OH)D may be an independent risk factor for CVD in older Koreans.

회전기 및 착지기 분리 구동을 가능케 하는 새로운 무릎 보장구의 기구부 설계 (A Novel Kinematic Design of a Knee Orthosis to Allow Independent Actuations During Swing and Stance Phases)

  • Nowadays many neurological diseases such as stroke and Parkinson diseases are continually increasing. Orthotic devices as well as exoskeletons have been widely developed for supporting movement assistance and therapy of patients. Robotic knee orthosis can compensate stiff-knee gait of the paralyzed limb and can provide patients consistent assistance at wearable environments. With keeping a robotic orthosis wearable, however, it is not easy to develop a compact and safe actuator with fast rotation and high torque for consistent supports of patients during walking. In this paper, we propose a novel kinematic model for a robotic knee orthosis to drive a knee joint with independent actuation during swing and stance phases, which can allow an actuator with fast rotation to control swing motions and an actuator with high torque to control stance motions, respectively. The suggested kinematic model is composed of a hamstring device with a slide-crank mechanism, a quadriceps device with five-bar/six-bar links, and a patella device for knee covering. The quadriceps device operates in five-bar links with 2-dof motions during swing phase and is changed to six-bar links during stance phase by the contact motion to the patella device. The hamstring device operates in a slider-crank mechanism for entire gait cycle. The kinematics and velocity/force relations are analyzed for the quadriceps and hamstring devices. Finally, the adequate actuators for the suggested kinematic model are designed based on normal gait requirements. The suggested kinematic model will allow a robotic knee orthosis to use compact and light actuators with full support during walking.

산업체 근로자의 비만도 따른 척추안정화근력 차이 분석 (Analysis of Differences Between Spinal Stabilization Muscle Strength According to BMI for Industrial Workers)

  • 본 연구는 대기업에 근무하는 산업체 근로자를 대상으로 비만도에 따른 척추안정화근력의 차이를 분석하는데 목적을 두었다. 편의표본 추출방법(Convenience Sampling Method)에 의하여 50명을 표본을 추출하여, 체질량지수(BMI) $25kg/m^2$ 이상을 비만군(25명), 이하를 정상군(25명)으로 분류하여 독립표본 t검정(Independent t-test)으로 분석하였다. 집단간 척추안정화근력의 측정결과 정상군의 평균이 비만군에 비해 모두 높았으며, Sagittal Plane에서는 $0^{\circ}$ Forward에서 유의한 차이를 보였고, Coronal plane에서는 $90^{\circ}$ Left, $90^{\circ}$ Right에서 유의한 차이를 보였으며, Diagonal Plane에서는 $45^{\circ}$ Left Tilt, $45^{\circ}$ Right Tilt에 유의한 차이를 보였다. BMI 분류에 따른 근육량과 골격근량의 분석결과에서는 비만군이 정상군에 비해 모두 높았으며 통계적으로 유의한 차이를 보였다. 비만군이 정상군에 비해 근육량은 많지만 척추안정화근력은 약하다는 것을 감안하여 Sagittal Plane, Coronal plane, Diagonal Plane에서의 비만인을 위한 척추안정화 강화 프로그램의 개발이 요구된다.

신발 사이즈 호칭 개선을 위한 발치수 성장 추이 탐색 (Research on Growing Progress of Foot Size to Improve Appellation of Shoe Sizes)

  • This study aims to categorize age groups for shoes manufacturing including shoes size and boot tree development for Koreans. In order to carry out the research, the characteristics of different parts of foot in relation to size are analyzed according to age and sex, making use of human body measurement database. As a result of statistical analysis, it was found that the measurement items such as foot length, metatarsal tibiale, foot breadth and instep circumference of a person reached the range of standard deviation and average numerical figures of an adult group (18-24) from the age of 14 in men's case, and from 12 for women's case, which indicates the time of completion of their foot growth. Based on these findings where males of 14 years old and females of 12 are within similar measurement range to adults, it is necessary to categorize the age groups for shoe sizing system into the following four groups: males 13 years old or under, males 14 years old or over, females 11 years or under, and females 12 years or over. The proportional measurement produced by dividing each measurement item of foot parts by foot length was compared in an independent samples t-test, and there were meaningful differences according to different foot shapes of the two age groups of males 13 or under - males 14 or over, and females 11 or under - females 12 or over. Also, the independent samples t-test for the age groups of males 13 or under - females 11 or under, and males 14 or over - female 12 or over, showed similar meaningful differences, which indicates the differences in foot shapes of male and female during the period of growth.

Wound Complications after Laparotomy for Endometrial Cancer

  • This study was conducted to determine the incidence of wound complications after laparotomy for endometrial cancer and significant predictors of risks. Medical records of patients with endometrial cancer undergoing laparotomy for surgical staging at Srinagarind Hospital, Khon Kaen University between January 2007 and December 2013 were reviewed. Intravenous antibiotic prophylaxis was routinely given 30 minutes before surgery. The primary endpoint was wound complications (including seroma, hematoma, separation, or infection) requiring additional medical and/or surgical management within 4 weeks of laparotomy. During the study period, 357 patients with complete medical records were reviewed. The mean age was 56.9 years. Wound complications were observed in 28 patients (7.84%, 95% CI, 5.27% to 11.14%). Body mass index (BMI) ${\geq}30kg/m^2$, diabetes mellitus (DM), and prior abdominal surgery were observed as significant independent factors predicting an increased risk of wound complications with adjusted odds ratios (95% CIs) of 2.96 (1.23-7.16), 2.43 (1.06-5.54), and 3.05 (1.03-8.98), respectively. In conclusion, the incidence of wound complications after laparotomy for endometrial cancer was 7.8%. Significant independent predictors of risk included BMI, DM and prior abdominal surgery.
