Introduction : The sensitivity of tumor cells to radiotherapy is a critical determinant of local control and potential cure in advanced head and neck squamous cell carcinoma(HNSCC). The emergence of radioresistant tumor cells is an obstacle to cancer therapy. Most radioresistant cells have a higher proportion of cells in the Sphase of the cell cycle and a lower apoptotic fraction than radiosensitive cells. HSV replication is increased in cells that have higher S-phase fractions. NV1066 is an oncolytic herpes simplex virus type-1 mutant. We hypothesized that NV1066 replication and cytotoxicity are increased in radioresistant cells. The purpose of this study is to evaluate the antitumor efficacy of NV1066 to treat radioresistant HNSCC. Methods : Radioresistant cells were selected by treating five HNSCC cell lines with repeated conventional fractionated doses of radiation(2Gy/day), using a Cs-137 irradiator, up to a cumulative dose of 70Gy. Clonogenic cell survival and S-phase fractions were compared between radioresistant and parental radiosensitive cells. The two cell populations were then treated with NV1066 to examine viral replication, by the viral plaque assay and viral cytotoxicity. Results : Fractionated irradiation resulted in the selection of radioresistant cells. Radioresistant cells had a higher S-phase fraction(42.9%) compared to parental cells(26.2%). NV1066 replication in radioresistant cells was 7.4 times higher than in parental cells(p<0.01). Treatment with NV1066 resulted in increased cytotoxicity of 24.5% in radioresistant cells compared to parental cells(p<0.05). Conclusion : NV1066 showed increased viral replication and cytotoxicity in radioresistant HNSCC cell lines. These findings suggest a potential clinical application for this oncolytic viral therapy as treatment for radioresistant head and neck cancers.
This study was performed to investigate the effect of suboccipital stretch on the head and neck posture and the electromyographic(EMG) activity of some cervical muscles. For this study, 39 patients with temporomandibular disorders(TMD) and 34 dental students without any signs and symptoms in the masticatory system were selected as the patients group and as the normal group, respectively. Head position by goniometer CROM$^{(R)}$(Performance attainment, St. Paul, USA), EMG activity by BioEMG$^{(R)}$(Bioresearch Inc., Milwaukee, USA), and craniocervical posture by cephaloradiography were observed in both natural head posture(NHP) and head posture with suboccipital stretch(tuck posture) abtained from slight posteroinferior finger pressure on the chin. Variables measured on the cephaloradiograph were SN angle, atlas angle, CVT angle, occiput-atlas and atlas-axis distance, and pharyngeal width. The data obtained were analysed by SPSS windows program and the results of this study were as follows : 1. In the sagittal plane, degree of anterior rotation of the head by suboccipital stretch was 6.3 in the patients group, and 6.2 in the normal group, respectively. So there was no significant difference between the two groups for degree of anterior rotation, but the position of the head in the patients group were more posteriorly extended than in the normal group in both NHP and tuck posture. 2. EMG activity of the stemocleidomastoideus in the patients group, and that of the upper trapezius and the sternocleidomastoideus in the normal group were increased by suboccipital stretch. The range of EMG activity, however, in these cervical muscles were 1.6 -2.3)u.V. 3. Cephalometric variables such as SN angle, atlas angle, CVT angle, occiput-atlas and atlas-axis distance except pharyngeal width were generally increased by suboccipital stretch. There was some difference, however, in results between the two groups. Atlas angle was not changed in the patients group whereas CVT angle was not changed in the normal group. 4. The distance from subocciput to spinous process of axis was significantly increased as much as 3.0mm in the patients group, and 3.7mm in the normal group by suboccipital stretch.
Purpose: Hrd1 has recently emerged as a critical regulator of B-cells in autoimmune diseases. However, its role in the pathogenesis of chronic rhinosinusitis with nasal polyps (CRSwNP) remains largely unexplored. This study aimed to examine Hrd1 expression and B-cell accumulation and their possible roles in CRSwNP. Methods: Quantitative real-time polymerase chain reaction, immunohistochemistry, enzyme-linked immunosorbent assay and Western blotting were used to assess gene and protein expression in nasal tissue extracts. Cells isolated from nasal tissues and peripheral blood mononuclear cells were characterized by flow cytometry. Local antibody production was measured in tissue extracts with a Bio-Plex assay. Additionally, changes in Hrd1 expression in response to specific inflammatory stimuli were measured in cultured dispersed polyp cells. Results: Nasal polyps (NPs) from patients with eosinophilic CRSwNP (ECRS) had increased levels of Hrd1, B-cells and plasma cells compared with NPs from patients with non-eosinophilic CRSwNP (non-ECRS) or other control subjects (P < 0.05). The average Hrd1 levels in B-cells in NPs from ECRS patients were significantly higher than those from non-ECRS patients and control subjects (P < 0.05). NPs also contained significantly increased levels of several antibody isotypes compared with normal controls (P < 0.05). Interestingly, Hrd1 expression in cultured polyp cells from ECRS patients, but not non-ECRS patients, was significantly increased by interleukin-$1{\beta}$, lipopolysaccharide and Poly(I:C) stimulation, and inhibited by dexamethasone treatment (P < 0.05). Conclusions: Differential Hrd1 expression and B-cell accumulation between the ECRS and non-ECRS subsets suggests that they can exhibit distinct pathogenic mechanisms and play important roles in NP.
Background and Objectives: Adenoid cystic carcinoma of salivary glands is characterized by insidious growth over many years, local recurrences, and distant metastasis and classified to three distinct histologic subtypes: tubular, cribriform, and solid. The solid type is known to have the worst prognosis. However, histopathologic heterogeneity is observed in tumors from the same patient. We have attempted to elucidate the genotypic differences, characterized by polysomies of chromosome 17, in adenoid cystic carcinoma according to the phenotypic histopathologic heterogeneity. Materials and Methods: Fluorescence in situ hybridization was performed on formalin-fixed paraffin blocks from seven patients with head and neck adenoid cystic carcinoma, using the centromeric $\alpha$-satellite probe of chromosome 17 to detect nuclei exhibiting polysomy. The difference in polysomeric chromosome expression in cribriform, tubular, solid type and type I, II, III according to the Szanto classification was analyzed. Results: Polysomy of chromosome 17 was found in 15.28% of the cribriform type, in 15.68% of the tubular type, and in 18.87% of the solid type. The proportion of polysomy was statistically higher in the solid type than in the cribriform type(p<0.05), and the proportion of polysomy increased progressively from type 1 to type 3, but this trend was statistically insignificant(p>0.05). Conclusion: We suggest that there may be genetic variations in tumor from the same patient depending on the histopathologic heterogenetiy in adenoid cystic carcinomas.
Anesthetized dogs were tilted from horizontal to the upright and head down position. Tilting to the upright position was followed by an increase in heart rate. In the head down position a decreased heart rate was obtained. The arterial blood pressure was decreased in the upright position and was decreased markedly in the head down position. The central venous pressure was decreased in the upright position and was markedly decreased down to the negative pressure in the head down position. The respiratory rate was slightly increased in the upright position comparing to that in the horizontal position. No remarkable changes were noted in the head down position. From the above results the following factors were discussed The decreased arterial blood pressure during the upright position was supposed to be the secondary effect from the diminished venous return that was suggested by the decreased central venous pressure. The decreased arterial blood pressure in the head down position was also supposed as the above reason as the diminished central venous pressure during the tilt. In addition the cardioinhibitory effects originated from the baroreceptors might have been operated during head down tilting. In the heart rate there was slight tachycardia in the upright position this was assumed as the abolished cardioinhibitory impulses from the baroreceptor in the upright position. On the contrary, despite of the decrease of arterial blood pressure in the head down position as well as in the upright, the bradycardia have been appeared. This was suggestive of cardioinhibitory impulses from the baroreceptors which was stretched during head down tilting. From the above findings there is a possibility of continous cardioinhibitory responses during head down tilting for this kind of the short period of 10 minutes which was chosed in this study.
This paper proposes a method which visualizes MRI head data in 3 dimensions with direct volume rendering. Though surface rendering is usually used for MRI data visualization, it has some limits of displaying little speckles because it loses the information of the speckles in the surfaces while acquiring the information. Direct volume rendering has ability of displaying little speckles, but it doesn't treat MRI data because of the data features of MRI. In this paper, we try to visualize MRI head data in 3 dimensions as follows. First, we separate the brain region from the head region of MRI head data, next increase the pixel level of the brain region, then combine the brain region with the increased pixel level and the head region without brain region, last visualizes the combined MRI head data with direct volume rendering. We segment the brain region from head region based on histogram threshold, morphology operations and snakes algorithm. The proposed segmentation method shows 91~95% similarity with a hand segmentation. The method rather clearly visualizes the organs of the head in 3 dimensions.
In head lettuce seedlings, NaCl stress was treated with hydroponic culture containing 0, 50, or with 100 mM NaCl in the seedling grown up to two leaf stages. Our focus was on the effect of NaCl on fresh and dry weights, antioxidant materials, and inorganic ion level. Fresh and dry weights of head lettuce seedlings increased with the increase in salinity while the optimal growth occured at 50 mM NaCl. The chlorophyll a (Chl a), total Chl and Chl a/b ratio increased 6 days after treatment with 100 mM NaCI. However, the Chl b content decreased. Total glutathione increased only in the root of head lettuce seedlings, whereas significant increase of total arcorbate content was observed in both shoot and root after the treatment with 100 mM NaCl. In addition, the NaCl treatment resulted in the decreased level of spermidine content, and a increased spermine content. Furthermore, $Na^+$ content in shoot and root increased significantly while $K^+$, $Ca^{2+}$, and $Mg^{2+}$ content decreased. The alteration of inorganic ion level after treatment with NaCl caused the reduction of $K^+/Na^+$, $Ca^{2+}/Na^+$, and $Mg^{2+}/Na^+$ ratio with the increase of NaCl concentration. Taken together, these findings indicate that the treatment of NaCl causes the induction of oxidative stress, and results in the alteration of metabolic mechanism in head lettuce seedlings.
Head and neck cancer is one of the leading causes of deaths worldwide. Two genes GSTM1 and GSTT1 involved in phase II of carcinogen detoxification have been frequently studied in the literature. Their null genotypes are thought to be associated with increased head and neck cancer risk. However, the published reviews are not up to date and many important papers have been skipped. The current literature review was restricted to the null genotypes of the GSTM1 and GSTT1 genes with special emphasis on the genotypic status. We found that the size of study sample varied greatly and the oral cavity cancer was more influenced by GSTM1 and GSTT1 gene deletions. With respect to ethnicity Asians are more prone to head and neck cancers with these null genotypes as compared to Europeans and Americans. The current review showed significant associations (OR=9.0, 95%CI; 1.4-9.5; OR=3.7, 95%CI; 1.4-9.5) of GSTM1 and GSTT1 null genotypes with head and neck cancers. Review confirms the data of previous reviews that GSTM1 and GSTT1 gene polymorphisms may be risk factors for cancer initiation.
An air chamber is designed to keep the pressure from exceeding a predetermined value, or to prevent low pressures and column separation. Therefore, it can be used to protect against rapid transients in a pipe system following abrupt pump stoppage. In this research, an air chamber was applied to a hypothetical pipe system to analyze attenuation effect of pressure head for different air volumes, locations, chamber areas, coefficients of orifice loss and polytropic exponents. With an increase of air volume, the maximum pressure head at pump site is decreased and the minimum pressure head is increased. For different locations and areas of the chamber, the attenuation effects do not show much difference. Also, as the orifice loss coefficient increases, the maximum pressure head is decreased. For different polytropic exponents, isothermal process shows lower maximum pressure head than that of the adiabatic process.
Objectives : Well differentiated thyroid carcinoma(WDTC) has relatively good prognosis. But recurrence is associated with increased morbidity and mortality, and always results in reoperation. So it is important to evaluate the risk factors relative to the recurrence. The aim of this study is to evaluate the risk factors of recurrence. Materials and Method : Two hundred seventeen patients who had undergone thyroidectomy and proved as WDTC from January 1999 to December 2004 was included in the study. We reviewed patient-related and tumorrelated factors respectively and analyzed the correlation with tumor recurrence. Results : Fourteen patients from two hundreds seventeen had recurred. Recurrence rate was 6.5%, and average interval of recurrence was 28 month. Male, age over 45, multiple mass, and advanced TMN stage patient group show higher recurrence rate, but no statistical significance. However, the recurrence rate of lymph node metastasis, tumor size>1.5cm, and extracapsular invasion group were statistically high. Conclusion : Lymph node metastasis, tumor size and extracapsular invasion are significant risk factors related to the recurrence of WDTC.
