  • This study was done to report the improvement of second case report form(CRF) and standard operating procedure(SOP) of Tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke. We were in charge of developing case report form(CRF) and educating the investigators. In the process of this project, we needed to develop standard operating procedure(SOP) for this CRF. So we made Tentative Korean Standard Differentiation of the Symptoms and Signs for Stroke and tried clinical application at Department of Oriental Internal Medicine of Wonkwang University and Daejeon University in 2005. And in this pilot study we can find out some problems and need to improve it. We strengthen the incision and exclusion criteria of CRF We canceled the Chief complains entry for efficiency. We reflected the decision of Stroke standard committee. We reduced the differentiation index of CRF to promote efficiency and accuracy. We rearranged the order of the differentiation index to promote rationality and practicality. We regulated detail item belonging to Differentiation index. We used a colloquialism in question. We inserted flow chart in SOP. We inserted picture of diagnostic index.

H형 기관지 식도루에 대한 임상 경험 (Clinical Experience of H-type Tracheoesophageal Fistula)

  • Tracheoesophageal fistula without esophageal atresia (H-type TEF) is a congenital anomaly that is characterized by a fistula between the posterior wall of the trachea and the anterior wall of the esophagus, not accompanied by esophageal atresia. The purpose of this study is to investigate the clinical characteristics, diagnostic time, the side of cervical approach and short term result after surgery by searching medical records of patients treated for H-type TEF. The search was done at University of Ulsan, Department of Pediatric Surgery of Asan Medical Center, and the total number of patients from May 1989 to December 2010 was 9 with M:F ratio of 1:2. The median gestational age was $39^{+6}$ ($32^{+6}{\sim}41^{+0}$) wks. Seven out of nine patients were born at term and the other two were born premature. The clinical presentation was aspiration pneumonia, difficulty in feeding, chronic cough, vomiting, abdominal distension and growth retardation. The symptoms presented right after birth. The diagnosis was made with esophagography and the median time of diagnosis was 52 days of life. The majority of surgical corrections were performed within two weeks of diagnosis (median; 15d, range; 1d - 6m). Six patients had associated anomalies, and cardiac anomalies were most common. The cervical approach was utilized in all cases (right 2, left 7). Transient vocal cord palsy and minor esophageal leakage complicated two cases. Although the diagnosis of H-type TEF was difficult and often delayed, we had a good short term result. The left cervical approach was preferred.

The analgesic efficacy of a single injection of ultrasound-guided retrolaminar paravertebral block for breast surgery: a prospective, randomized, double-blinded study

  • Background: The thoracic paravertebral block is an effective analgesic technique for postoperative pain management after breast surgery. The ultrasound-guided retrolaminar block (RLB) is a safer alternative to conventional paravertebral block. Thus, we assessed the analgesic efficacy of ultrasound-guided RLB for postoperative pain management after breast surgery. Methods: Patients requiring breast surgery were randomly allocated to group C (retrolaminar injection with saline) and group R (RLB with local anesthetic mixture). The RLB was performed at the level of T3 with local anesthetic mixture (0.75% ropivacaine 20 mL + 2% lidocaine 10 mL) under general anesthesia before the skin incision. The primary outcome was cumulative morphine consumption using intravenous patient-controlled analgesia (IV-PCA) at 24 hour postoperatively. The secondary outcomes were the visual analogue scale (VAS) scores at 1, 6, 24, and 48 hour postoperatively and the occurrence of adverse events and patient satisfaction after the surgery. Results: Forty-six patients were included, 24 in group C and 22 in group R. The cumulative morphine consumption using IV-PCA did not differ between the two groups (P = 0.631). The intraoperative use of remifentanil was higher in group C than in group R (P = 0.025). The resting and coughing VAS scores at 1 hour postoperatively were higher in group R than in group C (P = 0.011, P = 0.004). The incidence of adverse events and patient satisfaction was not significantly different between the two groups. Conclusions: A single injection of ultrasound-guided RLB did not reduce postoperative analgesic requirements following breast surgery.

  • 저자들은 2례의 제2새열낭종을 경험하였다. 증례 1은 26세의 여자에서 3년 전부터 서서히 크기가 증가하다가 1년 전 임신과 함께 현저히 크기가 증가한 병력이 있으며, 낭종은 우측 악하부와 흉쇄유돌근 전내측에 위치한 무통성의 파동을 보이는 $7.5{\times}5{\times}4cm$ 크기의 연성 종물로서 내외측으로 누공 형성은 없었다. 증례 2는 56세 남자에서 2년전 악하간극 농양으로 진단하고 절개 및 배농을 시행한 병력이 있으며, 낭종은 좌측 악하부와 흉쇄유돌근 전내측에 위치한 무통성의 파동을 보이는 $5.5{\times}4{\times}3cm$ 크기의 연성 종물로서 역시 내외측으로 누공 형성은 보이지 않았다. 조직학적으로 중층편평상피로 이장되어 있었고 상피 하방에서는 림프양 조직이 관찰되었으며, 완전절제술을 시행한 다음 술후 $3{\sim}4$년이 경과된 현재까지 재발이나 다른 합병증 없이 양호한 경과를 보이고 있다.

  • The bilateral sagittal split ramus osteotomy (BSSRO) is preferred method of surgical correction for mandibular prognathism, retrognathism and asymmetry. This technique performed from primarily an intraoral incision to avoid a scar. After forward movement of the distal segment of the mandible, healing of bone by primary or secondary intention is easily accomplished through large areas of cancellous bony overlap. When rigid fixation is used for the BSSRO, it is possible to open the mouth during the immediate post-operative period because it promotes the healing process. Although this surgical procedure has been well-documented, the incidence of postoperative trigeminal neurosensory disorder in the region of the inferior alveolar nerve and the mental nerve remains one of the major complication. However, evaluation of objective methods for sensory recovery patterns is insufficient although most patients find their sensory return. Neurometer electrodiagnostic device performs automated neuroselective sensory nerve conduction threshold evaluation by determining current perception threshold (CPT) measures. The purpose of this study was to evaluate the sensory recovery patterns of inferior alveolar and mental nerve over time. Nerve examination with a neurometer was performed in 30 patients undergoing the BSSRO at pre-operative, post-operative 1-, 2-, 4- week, and 2-, 3-, 4-, 5-, 6- month follow-up visits after the osteotomy to compare the differences of nerve injury and recovery patterns after the BSSRO with or without genioplasty and sensory recovery patterns associated with the kind of nerve fiber.

Medical Thoracoscopy in Pleural Disease: Experience from a One-Center Study

  • Background: Medical thoracoscopy (MT) is a minimally invasive, endoscopic procedure for exploration of the pleural cavity under conscious sedation and local anesthesia. MT has been performed at the Seoul National University Hospital since February 2014. This paper summarizes the findings and outcomes of MT cases at this hospital. Methods: Patients who had undergone MT were enrolled in the study. MT was performed by pulmonologists, using both rigid and semi-rigid thoracoscopes. During the procedure, patients were under conscious sedation with fentanyl and midazolam. Medical records were reviewed for clinical data. Results: From February 2014 to January 2016, 50 procedures (47 cases) were performed (diagnostic MT, 26 cases; therapeutic MT, 24 cases). The median age of patients was 66 years (59-73 years), and 38 patients (80.9%) were male. The median procedure duration from initial incision to insertion of the chest tube was 37 minutes. The median doses of fentanyl and midazolam were $50{\mu}g$ and 5 mg, respectively. All procedures were performed without unexpected events. Of the 26 cases of pleural disease with an unknown cause, 19 were successfully diagnosed using MT. Additionally, diagnostic MT provided clinically useful information in the other six patients. Therapeutic MT was very effective for treatment of malignant pleural effusion or empyema. The median number of days with chest tube drainage was 6 (3 days for diagnostic MT and 8 days for therapeutic MT). Conclusion: MT is a useful and necessary procedure for both diagnosis and treatment of pleural diseases.

Chest Wall Tuberculosis: Clinical Features and Treatment Outcomes

  • Background: A diagnosis and treatment of chest wall tuberculosis (CWTB) is both difficult and controversial. The aim of this study was to collect information on the optimal treatment for CWTB. Methods: The clinical features, radiographic findings, and treatment outcomes of 26 patients, who underwent surgery and were diagnosed histopathologically, were retrospectively analyzed. Results: The most common presenting symptom was a palpable mass found in 24 patients (92.3%). In all patients, CT revealed a soft tissue mass that was accompanied by a central low density, with or without peripheral rim enhancement. The sensitivity and specificity of the bone scintigram for bone involvement were 87.5% and 100%, respectively. CWTB was diagnosed preoperatively by aspiration cytology and smear for acid-fast bacilli in five out of 11 patients. Twenty-three patients (88.5%) underwent a radical excision and three underwent incision/drainage or an incisional biopsy. The duration of antituberculous medication was 7.5${\pm}$3.98 months with a follow-up period of 28.2${\pm}$26.74 months. Among the 20 patients who completed their treatment, nine received chemotherapy for six months or less and 11 received chemotherapy for nine months or more. Two patients had a recurrence four and seven months after starting their medication. Conclusion: A 6 month regimen may be appropriate for CWTB patients who have undergone a complete excision.

안위대책 간호가 수술 후 회복에 미치는 영향 (Effects of Comfort Nursing Measures on Postoperative Recovery of Patient)

  • For the purpose to clarify the effects of nursing, intervention with comfort measures which promote rest, exercise and sleep on the patient′s rehabilitation, this study was carried out on 119 postoperative patients at St. Mary′s Hospital, the National Medical Center and Seoul Red Cross Hospital during the 9 months period from March 1971 to November 1971. In this study one experimental nursing approach was utilized; an emphasis on interpersonal techniques along with physical care-comfort measures. A daily evening care including support and instruction was given to facilitate interaction of nursing to the experimental group by the investigator. For the control group, routine hospital nursing care was performed. The nursing observation was followed for 4 days postoperatively and recorded in check list. The results of the findings were as follows. 1. 3.5% of control group and 32.3% of experimental group got out of bed within 24 hours postoperatively. 38.6% of control group got out of bed within 72 hours postoperatively where only 16.1% of the experimental group did (x$^2$= 19.865, p<0.005). Interaction in nursing is, in turn, significantly more effective than the usual routine care in improving rate of healing. 2. The irritations and tension that may interfere patient′s sleep and rest at night can be reduced to a minimum if nursing environment is better controlled with planned nursing care for individual patient. Various treatments which tend to give patient discomfort may preferably be performed before 6 p.m. if not absolutely indicated. 3. During 4 days of observation the patients without administration of sedatives and analgesics postoperatively were 25.9% in the experimental group where as 10.5% in the control group. The frequency of administration of sedatives and analgesics in average was 1.4 in the experimental group, and 2.0 in the control group. This indicates that not all postoperative discomforts expressed by the patients should be regarded as incision pain, and those discomforts could be relieved to a certain extent by nursing interventions effectively. 4. There were significant differences between the responses to nursing care given in the experimental group and 33% of the control group in average through 4 days of observation responded "good". 3.6% of the experimental group and 17. 1% of the control group responded "poor" in this study. It was recommended that the study be replicated in a more defined and controlled manner. Some alternative areas for investigation were suggested.

흡연이 치주 판막술후 치유에 미치는 영향 (The effect of smoking on the healing response following flap debridement surgery)

  • The ultimate goal of periodontal therapy is the regeneration of periodontal tissue which has been lost due to destructive periodontal disease. Various periodontal procedures have been used throughout the years in an attempt to reestablish attachment of periodontal tissues to root surfaces affected by periodontitis. Flap debridement surgery has been demonstrated to be a successful procedure in gaining the probing attachment level and reducing probing depth. A tendency towards impaired wound healing following periodontal procedures in smokers has been clinically documented. But, previous clinical studies on healing response in smokers are based on a retrospective design. The purpose of this study was to evaluate the treatment outcome following flap debridement surgery in smokers compared to nonsmokers. 25 patients with moderate to advanced periodontitis were included for study. Among these patients, 13 patients were smokers, and 12 patients were nonsmokers. Mucoperiosteal flap was raised with the sulcular incision. No antibiotic treatment was administered postsurgery. The patients was recalled at monthly intervals during a period of 6 months following the surgery. The patients were received supragingival scaling and oral hygiene reinforcement. All the recordings, including modified O' Leary plaque control record, bleeding on probing, probing pocket depth, probing attachment level,were recorded, presurgery and 6 months postsurgery. The changes of all the recordings at 6 months after flap debridement surgery revealed the following results: 1. PI on all the dentitions and surgical sites showed no statistical significance between smokers and nonsmokers at presurgery. But, smokers demonstrated a significantly lower % of PI than nonsmokers at 6 months postsurgery. 2. Smokers demonstrated a greater % of BOP sites than nonsmokers on the surgical sites and all the dentitions, presurgery and 6 months postsurgery. But, there was no statistical significance between two groups. 3. Smokers exhibited significantly less reduction of probing depth in the 3 mm or less probing pocket depth(PPD) group, 6mm or more PPD group and total PPD group when compared to nonsmokers at 6 months postsurgery. 4. Smokers exhibited significantly less gain of probing attachment level(PAL) in the 3mm or less PPD group, 6 mm or more PPD group and total PPD group when compared to nonsmokers at 6 months postsurgery.

구강 악안면 영역의 생체 흡수성 고정판 사용에 관한 임상 연구 (A CLINICAL STUDY OF BIODEGRADABLE PLATES AND SCREWS IN ORAL AND MAXILLOFACIAL SURGERY)

  • Metallic bone plates and screws have been commonly used in oral and maxillofacial surgery for internal fixation. However, there are several disadvantages such as atrophy of cortical bone inherent to excessive rigid fixation systems, growth disturbance in growing individual, allergy reaction, interference with radiographic imaging, palpability, thermal sensitibity and the need for subsequent removal. To overcome these disadvantages and avoid additional surgery of removal of plates and screws, there have been many studies of biodegradable plates and screws. But, It also has complication such as foreign body reactions. We have undertaken a clinical and retrospective study on 140 patients in Dept. of Oral and Maxillofacial Surgery, Inha University Hospital from February 2006 to March 2009. The purpose of this study is to report the clinical cases and review of the literatures with biodegradable plates and screws. And we concluded following results. 1. 6 cases(3.4%) of the 177 operation sites(140 patients) experienced complications. 1 case(0.6%) was a failure of initial fixation, 1 case(0.6%) was a postoperative infection, 4 cases(2.3%) were inflammations or foreign body reaction. 2. Postoperative infections, inflammations and foreign body reactions were completely recovered with incision and drainage, supporative care with antibiotic coverage and removal of biodegradable plates. 3. Biodegradable plates and screws provide acceptable rigidity and stability clinically. But, long-term observation is required for the tissue reactions around the biodegradable plates and screws because of long resorption periods of the biodegradable materials.
