  • Purpose: This study was done to develop a predictive model for quality of life of elderly residents in long-term care facilities (LTCF). The study was based on Brofenbrenner's ecological system theory and a literature review. Methods: Data were collected using a convenience sample of 205 elderly residents in 2 nursing homes located in D city and 1 nursing home located in K province. The exogenous variables were individual factors, family support, and facility environmental factors. The endogenous variables were self-esteem, accommodation adaptation and quality of life of elderly residents in LTCF. Collected data were analyzed through structural equation modeling using AMOS 20.0. Results: Eleven of the twelve hypotheses were supported, but the hypothesis that facility environment factors effect self-esteem was not supported. Quality of life of elderly residents in LTCF was explained first by facility environmental factors, followed by self-esteem, individual factors, accommodation adaptation, and family support with an explanatory power of 83.0%. Conclusion: To improve the quality of life of elderly residents in LTCF, the service and environment preparation provided by facilities is important, and it is necessary to provide emotional counseling to improve the self-esteem of these elders.

The main themes of successful aging derived by interview were as follows: 1) fulfill responsibilities of raising children as parents, 2) establish a stable life to overcome difficulties, 3) accept the body being different from the past, and 4) live in harmony with people around them. • Purpose: The rapid change to an aging society generates an increase in the incidence of chronic diseases. Many chronic patients have been facing their illnesses without enough preparation. In order to solve these problems, we designed and tested a public healthcare service based on ubiquitous technology. Method: Telemedicine has emerged as new medical care system of chronic disease. However, public potential of its technology is difficult to know under current traditional health care system. In this work, we developed a Personal Digital Access (PDA) phone based healthcare system by trained visiting nurses for elderly patients. A field test was performed by SeongBuk Public Health Center in Seoul, Republic of Korea (ROK). Result: Surveys were generated to assess the effects of this system compared to conventional public health system. Findings of trials demonstrate that healthcare coordination enhanced by PDA phone technology is satisfactory to the patients and nurses compared to previous one because prompt responses arouse their recognition of health. Conclusion: Ubiquitous healthcare system applied to public health service proved to be efficient and time-saving to monitor and control the chronic illnesses of large population.

  • The purpose of this research is to analyze the overall problems at the moment of October 2008, and then to find the improvements of home-help services of the Long-Term Care Insurance(LTCI), which has been revealed many problems since it was released in July 2008. The research uses the literature survey which analyzes 2nd-hand materials studied by other people already, and survey research was executed from active social workers in the area of LTCI. Based on the policy analysis framework of Gilbert and Specht, all the data are analyzed in the scopes of client·benefit(service)·finance·transferring system. This research has found the problems in each scope of home-help services of the LTCI. Firstly, the client system has some problems in mismatching between registered and service clients, estimating client number, and judging service levels. Secondly, the service system reveals deficiency in professionality of social workers, service quality lowering by loose qualification criteria on workers, non-reasonable limitation of service time available, and the same fare system applied to visiting-help service in spite of different levels. Thirdly, in financing system, clients need to pay additional money to get extra services such as meal, hair cutting, bathing etc., due to government financial support stopped, some organizations have to reduce services and replace full-time workers to part-time ones, which makes the service quality worse. Lastly, in the transferring system, the management system for service quality is not well prepared. There are too much competion because of allowing too many home-help service organizations and care worker academies. The suggestions that this research has found to improve the policy are as follows. ① It is desirable to make the registered clients the service ones as many as possible in the long term perspective. ② The LTCI organization requires more workers and higher professionality. ③ Many elderly people who are not eligible now require connection system to be more served. ④ Management system and service manual for care worker are to be developed. ⑤ Laws related to the service contents and process should be modified, the proportion of client charge needs to adjust. ⑥ Home-help service organization licensed by the LTCI needs to be financially supported publicly. ⑦ Monitoring system to home-help service organization needs to be strengthened. ⑧ Evaluation tools to home-help service organization and workers is required. ⑨ Specification to open the home-help service organization needs to be more strict.

Method: Non-equivalent control group pretest-posttest design was employed. Data were collected from April 2007 to January 2009. Among a total of fifty subjects, 25 subjects were belonged to the case management group and the others were belonged to the general management group. Results: 1) In experimental group, significant increasing rate of number of people within normal blood pressure by time, but no significant increasing rate of number of people within normal blood neutral fat, total cholesterol, HDL, & LDL. 2) In experimental group, the score of knowledge about disease and healthy life practice were increased on 8 week period but decreased on follow up period. Number of people within normal blood HDL and mean score of healthy life practice were significantly higher than control group on follow up period. Conclusion: Case management service was effective on controlling blood pressure but partly effective on blood lipid, knowledge about disease, and healthy life practice in hypertensive patients. Further study is needed to conduct a periodic re-evaluation of the effect of case management and to verify an optimal interval to provide the case management service.
