구치부 임플란트 고정성 수복물에서의 지대주 나사 풀림 현상과 이에 영향을 미치는 요인 (The incidence of the abutment screw loosening and its affecting factors in posterior implant restorations)

  • 목적: 본 연구의 목적은 구치부 식립 임플란트에서 지대주 나사 풀림의 발생 빈도 및 지대주 나사 풀림에 영향을 주는 다양한 요인들을 후향적 연구를 통하여 평가하는 것이다. 대상 및 방법: 2013년 1월부터 2016년 1월까지 208명의 환자에서 구치부에 식립한 391개의 임플란트를 대상으로 하였다. 모든 수복물은 고정성으로 단일 혹은 연결 크라운, 브릿지로 제작되었으며, 임시 시멘트로 합착하였다. 전체 수복물 중 지대주 나사 풀림의 발생 빈도를 조사하였고, 성별, 보철물의 위치, 대합치, 보철물의 유형, 지대주 연결 유형, 매식체의 지름이 지대주 나사 풀림에 미치는 영향을 평가하였다. 결과: 2 - 5년의 관찰 결과, 총 29개(7.4%)의 임플란트에서 지대주 나사 풀림이 발생하였다. 최종 수복 후 지대주 나사 풀림이 발생하기까지 소요된 기간은 3개월부터 48개월(평균19.5개월)까지 다양하게 나타났으며, 이 중 3개의 임플란트에서 매식체의 파절이 발생되었다. 고려 요인들 중, 임플란트의 식립 위치에 따라 대구치(9.4%)와 소구치(2.6%)에서 통계적으로 유의한 차이를 보였으며(P < .019), 대합치에 따라 자연치(9.9%), 임플란트(1.0%), 가철성 보철물(0%)에서 유의한 차이를 보였다(P < .018). 다른 고려 요인에 따른 지대주 나사 풀림 발생 빈도는 통계적 유의성이 없었다. 결론: 구치부 임플란트 수복물에서 지대주 나사 풀림의 발생 빈도는 7.4%로 나타났다. 소구치보다 대구치에서 유의하게 높은 발생 빈도를 보였으며, 대합치에 따라서는 임플란트, 가철성 보철물과 비교 시 자연치에서 유의하게 높은 발생 빈도를 보였다.

  • 이 연구의 목적은 원추형 내측 연결 임플랜트 시스템에서 하중조건에 따른 지대주의 침하 및 적합도를 평가하는 것이다. 본 연구에서는 내부 원추형 연결방식의 Alloden implant system (Nei Corp. Korea)의 고정체와 2종류(conventional, FDI)의 지대주를 사용하였다. 임상에서 Alloden 임플랜트는 지대주와 고정체 연결시 처음에 손으로 지긋이 눌러 고정시킨 후 mallet을 이용하여 약 3회정도 타격을 가하여 고정한다. 이때 타격시의 정확한 힘을 측정하여 각 실험군에 적용시켰다. 적용 횟수는 손가락으로 누르는 힘을 1회, mallet으로 타격하는 힘을 3회, 저작력으로 가정한 20kg의 힘을 지대주의 침하가 생기지 않을때까지 각각의 표본에 적용하였다. 그 후 각 단계에 대한 지대주의 침하량을 Vernier caliper를 이용하여 측정하였다. 임플랜트는 불포화 폴리에스터(Epovia, Cray Valley Inc. Korea)에 매몰하여 중합시켰고 모든 표본을 절삭한 후 연마하여 주사전자현미경을 통하여 분석, 평가하였다.

맞춤형 치유 지대주를 이용한 임플란트 수복 증례 (Clinical case of implant restoration using customized healing abutment)

  • 최근 임플란트 수복 영역에서 심미성이 강조되고 있다. 그러나 수술 후 초기 연조직 회복 과정에서 기성 치유 지대주를 사용하는 것은 연조직 형태 및 치경부 출현각을 형성하기 어려운 문제점들을 가지고 있다. 이러한 한계점을 극복하기 위하여 본 증례에서는 맞춤형 치유 지대주를 제작하여 사용하였으며, 맞춤형 치유 지대주의 형태는 원형이 아닌 최종 맞춤형 지대주 형태로 제작하여 임플란트 수복 시 심미성의 향상을 관찰 하였다. 본 증례는 임플란트 수술 후 맞춤형 치유 지대주를 이용하여 연조직의 치유를 도모함으로써 이후, 최종 맞춤형 지대주 장착 시 연조직의 변위를 줄여보다 심미적이고 기능적으로 좋은 결과를 얻었기에 보고하고자 한다.

손상 없이 영구 접착 보철물을 제거할 수 있는 cementation type 임플랜트 지대주 개발에 관한 연구 (A STUDY ON THE COMPLETE RETRIEVAL SYSTEM OF THE CEMENTATION TYPE IMPLANT ABUTMENT)

  • Purpose: This study was peformed to investigate the retrievability of the cemented crown from the cementation type implant abutment. Material and method: The cementation type implant abutments (NEOBIOTECH implant abutment regular, 3 degree taper, 10mm length, 4mm diameter, Ti grade III, machined surface. Hwasung, Kyunggi-do) and cemented crowns were divided into 3 groups, depending on their hole angles formed in the crowns for their retrievability. The abutments and crowns were luted with 4 kinds of cements and separation test using metal wedge was executed with Instron 4465 Universal Testing Machine and the maximum impact force of the modified crown ejector was measured. Results and conclusion : 1. All of the cementation type implant abutments and cemented crowns were separated with relatively small force by metal wedge. 2. The retrieving force was minimum when the metal wedge was applied perpendicular to the axis of abutment. 3. The force for retrieving crowns from abutments was maximum in resin cement group, and reduced in orders of zinc phosphate cement, glass ionomer cement and zinc oxide eugenol cement. 4. The maximum force obtained by the crown ejector was higher than the retrieval force in ZOE and GI cement and lower than that in ZPC and resin cement. 5. If it has similar conditions clinically, the cemented crowns luted with 2 types of cements (ZOE, GI cement) can be safely retrieved from the cementation type implant abutments by the modified crown ejector.

Biomechanical investigation of maxillary implant-supported full-arch prostheses produced with different framework materials: a finite elements study

  • PURPOSE. Four and six implant-supported fixed full-arch prostheses with various framework materials were assessed under different loading conditions. MATERIALS AND METHODS. In the edentulous maxilla, the implants were positioned in a configuration of four to six implant modalities. CoCr, Ti, ZrO2, and PEEK materials were used to produce the prosthetic structure. Using finite element stress analysis, the first molar was subjected to a 200 N axial and 45° oblique force. Stresses were measured on the bone, implants, abutment screw, abutment, and prosthetic screw. The Von Mises, maximum, and minimum principal stress values were calculated and compared. RESULTS. The maximum and minimum principal stresses in bone were determined as CoCr < ZrO2 < Ti < PEEK. The Von Mises stresses on the implant, implant screw, abutment, and prosthetic screws were determined as CoCr < ZrO2 < Ti < PEEK. The highest Von Mises stress was 9584.4 Mpa in PEEK material on the prosthetic screw under 4 implant-oblique loading. The highest maximum principal stress value in bone was found to be 120.89 Mpa, for PEEK in 4 implant-oblique loading. CONCLUSION. For four and six implant-supported structures, and depending on the loading condition, the system accumulated different stresses. The distribution of stress was reduced in materials with a high elastic modulus. When choosing materials for implant-supported fixed prostheses, it is essential to consider both the number of implants and the mechanical and physical attributes of the framework material.

Effect of cement washout on loosening of abutment screws and vice versa in screw- and cement- retained implant-supported dental prosthesis

  • PURPOSE. The purpose of this study was to examine the abutment screw stability of screw- and cement-retained implant-supported dental prosthesis (SCP) after simulated cement washout as well as the stability of SCP cements after complete loosening of abutment screws. MATERIALS AND METHODS. Thirty-six titanium CAD/CAM-made implant prostheses were fabricated on two implants placed in the resin models. Each prosthesis is a two-unit SCP: one screw-retained and the other cemented. After evaluating the passive fit of each prosthesis, all implant prostheses were randomly divided into 3 groups: screwed and cemented SCP (Control), screwed and non-cemented SCP (Group 1), unscrewed and cemented SCP (Group 2). Each prosthesis in Control and Group 1 was screwed and/or cemented, and the preloading reverse torque value (RTV) was evaluated. SCP in Group 2 was screwed and cemented, and then unscrewed (RTV=0) after the cement was set. After cyclic loading was applied, the postloading RTV was measured. RTV loss and decementation ratios were calculated for statistical analysis. RESULTS. There was no significant difference in RTV loss ratio between Control and Group 1 (P=.16). No decemented prosthesis was found among Control and Group 2. CONCLUSION. Within the limits of this in vitro study, the stabilities of SCP abutment screws and cement were not significantly changed after simulated cement washout or screw loosening.

임플란트 고정체와 지대주 연결 형태의 차이에 따른 유지 나사 안정성에 대한 연구 (A VITRO STUDY OF RETAINED SCREW STABILITY BY VARIOUS CONNECTION DESIGNS BETWEEN FIXTURE AND ABUTMENT IN IMPLANT DENTISTRY)

  • Statement of problem : Since the concept of osseointegrated dental implant by $Br{\aa}nemark$ et al was first applied to mandibular full edentulous patients. Recently it is considerated the first treatment option on missing teeth. A common problem associated with dental implant restorations is loosening of screws that retain the prosthesis to the abutment and the abutment to the implant fixture. Purpose : This study is to examine the influence on screw loosening of implant-abutment designs. Material and methods : External hex, cone screw, beveled hex, cam cylinder, cylinder hex by means of evaluating the loosening torques, with respect to a range of tightening torques after repeated loading. Result : 1. Cone screw, beveled hex groups are the highest initial tightening rate and cylinder hex, external hex groups are the lowest initial tightening rate (p < 0.05). 2. Cone screw groups are the highest after repeated loading tightening rate and cylinder hex groups are lowest after repeated loading tightening rate(p < 0.05). 3. Cone screw groups have the highest initial stability and anal stability. 4. All groups are decreased tightening rate after repeated loading.

기성 지대주와 맞춤형 CAD-CAM 지대주의 적합 및 나사 안정성 비교 (A comparative study on the fit and screw joint stability of ready-made abutment and CAD-CAM custom-made abutment)

  • 연구 목적: 본 연구에서는 기성 지대주와 맞춤형CAD-CAM 지대주의 적합성 및 나사 사이의 안정성 대해 알아보고자 한다. 연구 재료 및 방법: 오스템 임플란트 시스템을 이용하였다. 동일회사에서 제작된 기성 지대주(Transfer abutment, Osstem Implant Co. Ltd, Busan, Korea) 및 맞춤형CAD-CAM 지대주(CustomFit abutment, Osstem Implant Co. Ltd, Busan, Korea)와 국내 외주 제작된 맞춤형CAD-CAM 지대주(Myplant, Raphabio Co., Seoul, Korea)를 5개씩 선택, 제작하였으며 나사는 각 회사에서 제공되는 것을 사용하였다. 제조사의 지시에 따라 고정체와 지대주를 30 Ncm으로 조인 후 초기 풀림 토크를 3회 반복 측정한 후 통계 분석하였다(${\alpha}=.05$). 고정체-지대주 연결체를 에폭시 레진에 매몰 후 습식 절삭 및 연마하여 계면 사이의 적합성을 FE-SEM으로 관찰하였다. 결과: 지대주의 초기 풀림 토크 값은 각각 기성 지대주(Transfer abutment)에서 $26.0{\pm}0.30Ncm$, 동일 회사에서 제작된 맞춤형CAD-CAM 지대주(CustomFit abutment)에서 $26.3{\pm}0.32Ncm$, 외주로 제작된 맞춤형CAD-CAM 지대주(Myplant abutment)에서 $24.7{\pm}0.67Ncm$였다. 국내 외주 제작된 맞춤형CAD-CAM 지대주에서 유의적으로 낮은 초기 풀림 토크 값을 보였으며(P=.027), 고정체-지대주 계면에서 변연 간극이 관찰되었다. 결론: 본 실험의 한계 내에서 국내 외주 제작된 맞춤형CAD-CAM 지대주는 동일 회사에서 제작된 기성 지대주나 맞춤형CAD-CAM 지대주에 비해 더 낮은 나사 안정성 및 적합성을 보인다고 볼 수 있다.

Influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with a lingual slot

  • PURPOSE. Cement-retained implant prostheses can lack proper retrievability during repair, and residual cement can cause peri-implantitis. The purpose of this in vitro study was to evaluate the influence of abutment height and convergence angle on the retrievability of cement-retained implant prostheses with lingual slots, known as retrievable cement-type slots (RCS). MATERIALS AND METHODS. We fabricated six types of titanium abutments (10 of each type) with two different heights (4 mm and 6 mm), three different convergence angles ($8^{\circ}$, $10^{\circ}$, and $12^{\circ}$), a sloped shoulder margin (0.6 mm depth), a rectangular shape ($6mm{\times}6.5mm$) with rounded edges, and a rectangular ledge ($2mm{\times}1mm$) for the RCS. One monolithic zirconia crown was fabricated for each abutment using a dental computer-aided design/computer-aided manufacturing system. The abutments and crowns were permanently cemented together with dual-curing resin cement, followed by 24 hours in demineralized water at room temperature. Using a custom-made device with a slot driver and torque gauge, we recorded the torque ($N{\cdot}cm$) required to remove the crowns. Statistical analysis was conducted using multiple regression analysis and Mann-Whitney U tests (${\alpha}=.05$). RESULTS. Removal torques significantly decreased as convergence angles increased. Multiple regression analysis showed no significant interaction between the abutment height and the convergence angle (Durbin-Watson ratio: 2.186). CONCLUSION. Within the limitations of this in vitro study, we suggest that the retrievability of cement-retained implant prostheses with RCS can be maintained by adjusting the abutment height and convergence angle, even when they are permanently cemented together.

A novel retentive type of dental implant prosthesis: marginal fitness of the cementless double crown type implant prosthesis evaluated by bacterial penetration and viability

  • PURPOSE. This study aims to compare the marginal fitness of two types of implant-supported fixed dental prosthesis, i.e., cementless fixation (CL.F) system and cement-retained type. MATERIALS AND METHODS. In each group, ten specimens were assessed. Each specimen comprised implant lab analog, titanium abutment fabricated with a 2-degree tapered axial wall, and zirconia crown. The crown of the CL.F system was retained by frictional force between abutment and relined composite resin. In the cement-retained type, zinc oxide eugenol cement was used to set crown and abutment. All specimens were sterilized with ethylene oxide, immersed in Prevotella intermedia culture in a 50 mL tube, and incubated with rotation. After 48 h, the specimens were washed thoroughly before separating the crown and abutment. The bacteria that penetrated into the crown-abutment interface were collected by washing with 500 µL of sterile saline. The bacterial cell number was quantified using the agar plate count technique. The BacTiter-Glo Microbial Cell Viability Assay Kit was used to measure bacterial adenosine triphosphate (ATP)-bioluminescence, which reflects the bacterial viability. The t-test was performed, and the significance level was set at 5%. RESULTS. The number of penetrating bacterial cells assessed by colony-forming units was approximately 33% lower in the CL.F system than in the cement-retained type (P<.05). ATP-bioluminescence was approximately 41% lower in the CL.F system than in the cement-retained type (P<.05). CONCLUSION. The CL.F system is more resistant to bacterial penetration into the abutment-crown interface than the cement-retained type, thereby indicating a precise marginal fit.
